Quantitative hepatitis B surface antigen in predicting recurrence of hepatitis B-related hepatocellular carcinoma after liver transplantation

Aim:Recurrence of hepatocellular carcinoma (HCC) after liver transplantation (LT) for chronic hepatitis B (CHB) can be associated with reappearance of hepatitis B surface antigen (HBsAg). The current study determined the significance of HBsAg qualitatively and quantitatively using a highly sensitive assay in recurrent HCC after transplantation. Methods:Consecutive patients with HBV-related HCC with LT were included. Oral nucleos(t)ide analogues without hepatitis B immune globulin were used as hepatitis B virus (HBV) prophylaxis. Quantitative HBsAg levels were performed at time of transplant, at 1 month, 3 and 6 months post transplant using a highly sensitive (hs)-HBsAg assay. Results:One hundred and fourteen patients were included, with a median follow-up of 80 months, with 24 cases of HCC recurrence, and a cumulative rate of 20.7% at 5 years. There was significant correlation between time of tumor recurrence and time of HBsAg reappearance (r = 0.551,P = 0.027). Early HCC recurrence was associated with higher median level of hs-HBsAg at the time of transplant (72.85vs. 69.70 IU/mL,P = 0.018). Using a hs-HBsAg cut-off level of 0.0005 IU/mL, patients with levels above this threshold at 3 and 6 months were associated with higher rate of early HCC recurrence (28.6%vs. 3.0% and 26.9%vs. 2.9% respectively, bothP =0.0006). There was no significant difference in HCC recurrence between positive and negative HBsAg using the conventional qualitative HBsAg assay. Conclusion:Serum hs-HBsAg levels of≥ 0.0005 IU/mL at 3 to 6 months after LT is associated with higher rates of early HCC recurrence, and may be useful as an early tumor marker.

Periodic Tail Motion Linked to wing Motion Affects the Longitudinal Stability of Ornithopter Flight

During slow level flight of a pigeon, a caudal muscle involved in tail movement, the levator caudae pars vertebralis, is activated at a particular phase with the pectoralis wing muscle. Inspired by mechanisms for the control of stability in flying animals, especially the role of the tail in avian flight, we investigated how periodic tail motion linked to motion of the wings affects the longitudinal stability of ornithopter flight. This was achieved by using an integrative ornithopter flight simulator that included aeroelastic behaviour of the flexible wings and tail. Trim flight trajectories of the simulated omithopter model were calculated by time integration of the nonlinear equations of a flexible multi-body dynamics coupled with a semi-empirical flapping-wing and tail aerodynamic models. The unique trim flight characteristics of ornithopter, Limit-Cycle Oscillation, were found under the sets of wingbeat frequency and tail elevation angle, and the appropriate phase angle of tail motion was determined by parameter studies minimizing the amplitude of the oscillations. The numerical simulation results show that tail actuation synchronized with wing motion suppresses the oscillation of body pitch angle over a wide range of wingbeat frequencies.

Objective: This paper mainly discusses and summarises the potential issues of pollution caused by the Mineral elements, Mercury, Lead and Arsenic, its possible impacts on the human beings and the suggested solutions. Methods: This paper is prepared by reviewing the latest academic literatures. Result: First, this article discusses two aspects including the effects of Mercury, Lead and Arsenic on the Chinese herbal medicine and the potential issues of causing the environmental pollution. And then further study its toxicity effects and the side impacts on the human bodies in order to realize the actual circumstances people are encountering nowadays. This paper will also the corresponding its treatment method of reviews. Hope this will provide a valuable reference. Conclusion: Theses issues caused by the Mineral elements are prominent nowadays, thus the ongoing researches on the impacts of pollution and the possible solutions are regarded as highly valued in order to conserve the natural environment and meanwhile safeguard the well beings of people and the future offspring.

An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in Hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and Hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in Hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Validated model for prediction of recurrent hepatocellular carcinoma after liver transplantation in Asian population

BACKGROUND Liver transplantation(LT) is regarded as the best treatment for both primary and recurrent hepatocellular carcinoma(HCC). Post-transplant HCC recurrence rate is relatively low but significant, ranging from 10%-30% according to different series. When recurrence happens, it is usually extrahepatic and associated with poor prognosis. A predictive model that allows patient stratification according to recurrence risk can help to individualize post-transplant surveillance protocol and guidance of the use of anti-tumor immunosuppressive *** To develop a scoring system to predict HCC recurrence after LT in an Asian *** Consecutive patients having LT for HCC from 1995 to 2016 at our hospital were recruited. They were randomized into the training set and the validation set in a60:40 ratio. Multivariable Cox regression model was used to identity factors associated with HCC recurrence. A risk score was assigned to each factor according to the odds ratio. Accuracy of the score was assessed by the area under the receiver operating characteristic *** In total, 330 patients were eligible for analysis(183 in training and 147 invalidation). Recurrent HCC developed in 14.2% of them. The median follow-up duration was 65.6 mo. The 5-year disease-free and overall survival rates were78% and 80%, respectively. On multivariate analysis, alpha-fetoprotein > 400 ng/mL [P = 0.012, hazard ratio(HR) 2.92], sum of maximum tumor size and number(P = 0.013, HR 1.15), and salvage LT(P = 0.033, HR 2.08) were found to be independent factors for disease-free survival. A risk score was calculated for each patient with good discriminatory power(c-stat 0.748 and 0.85, respectively,in the training and validation sets). Gamma is a kernel programming language with an elegant chemical reaction metaphor in whichprograms are described in terms of multiset rewriting. Gamma formalism allows one to describe analgorithm without introducing artificial sequentiality and leads to the derivation of a parallel solution to agiven problem naturally. However, the difficulty of incorporating control strategies makes Gamma not onlyhard for one to define any sophisticated approaches but also impossible to reach a decent level of efficiencyin any direct implementation. Recently, a higherorder multiset programming paradigm, named higher--order Gamma, is introduced by Metayer to alleviate these problems. In this paper, we investigate the possibility of implementing higherorder Gamma on Maspar, a massively data parallel computer. The results showthat a program written in higher--order Gamma can be transformed naturally toward an efficientimplementation on a real parallel machine.
