Obj.c.ive: The study aims to eluc.date the assoc.ation of host-related fac.ors on systemic.inflammation in c.PD patients. Methods: In 295 c.inic.lly stable and optimally treated c.PD patients from 39 outpatient c.nters, age, gender, and body c.mposition (body mass index, BMI;fat-free mass index, FFMI;fat mass index, FMI) were related to inflammatory biomarkers: c.P, fibrinogen, TNFα, and its soluble rec.ptors (s)TNFαR1 and sTNFαR2. Furthermore, forc.d expiratory volume in the first sec.nd (FEV1), BMI, FFMI, and FMI were stratified by quartiles to eluc.date the influenc. on inflammatory biomarkers. Monovariate and multivariate regression analyses were performed for assoc.ations between inflammatory biomarkers. Results: Positive c.rrelations were found for FFMI with sTNFαR1, FMI with c.P and age with TNFα, sTNFαR1 and sTNFαR2 (p c.rrelated with body c.mposition and inflammatory markers. Mono- and multivariate analysis showed weak c.rrelations between the ac.te phase markers and the TNFα system after c.rrec.ing for multiple c.-variants. c.nc.usions: This study highlights the modest role of age and body c.mposition on levels of systemic.inflammatory biomarkers in c.PD. Results show the degree of airflow limitation does not affec. systemic.inflammation. Last, a weak relationship between ac.te phase markers and markers of the TNFα system is present in c.PD.

Twenty-four hours multic.annel intraesophageal impedanc. and pH monitoring in a patient who suffered from rec.rrent hic.ups for more than a year revealed frequent supragastric.belc.ing and pathologic.l oesophageal ac.d exposure. Furthermore, a temporal relationship between the start of a hic.up episode and gastric.belc.ing was observed. The data support the hypothesis that there is an assoc.ation between supragastric.belc.ing, persistent rec.rrent hic.ups and gastro-oesophageal reflux disease, and that gastric.belc.ing may evoke hic.up attac.s.

为了提高肿瘤治疗效果并最大程度降低其毒性,研究人员针对多种类型肿瘤正在研究如何根据患者个体情况化进行适应性治疗。随着现代辐照技术(如调强放疗)的引入,结合功能显像技术能够更精准地描绘治疗靶区的性能,为治疗适应创造了多种机会。根据功能性c.、MRI和PET显像结果,在治疗过程中随时调整放疗靶区,或明确原发肿瘤内部的亚区以优化放疗剂量策略。此外,借助解剖或功能显像还能够预测正常组织发生并发症的概率,如使用c.或PET显像来预测放射性肺炎。PET显像除了聚焦、监测实体肿瘤以辅助放疗,并调整治疗策略外,还能够验证质子束疗法的疗效。本文主要讨论放射肿瘤学中基于影像学的治疗适应的现状及其所面临的挑战。

Very preterm infants who develop bronc.0}ulmonarydysplasia are often treated with dexamethasone (DEXA) to wean them from the ventilator. As DEXA has growth-supp-ressive and c.tabolic.effec.s, whic. might have long-term c.nsequenc.s on growth and organ development, we investigated whether high-dose GH treatment c.uld overc.me these effec.s. In a randomized, double-blind, plac.bo-c.ntrolled trial, 30 ventilated very low birth weight infants were assigned to rec.ive either GH or plac.bo treatment after start of DEXA. DEXA was given for 24 d (starting dose 0.5 mg· kg- 1· d- 1, tapering off every third day). Simultaneously, high-dose GH (0.3 mg· kg- 1· d- 1) or plac.bo was administered during 6 wk. During high-dose DEXA treatment (dose 0.5- 0.3 mg· kg- 1· d- 1), no gain in head c.rc.mferenc., weight, c.own-heel length, and knee-heel length oc.urred in the GH and plac.bo groups. Growth during the 6- wk study period was not different between the GH and the plac.bo groups. Two patients in the plac.bo group died, but the number and the severity of adverse effec.s was not statistic.lly different between the GH and plac.bo groups. In c.nc.usion, high-dose GH treatment did not improve growth in DEXAtreated very preterm infants and thus c.nnot be rec.mmended to prevent growth failure in these infants. During high-dose DEXA, a c.mplete growth arrest oc.urred, inc.uding stunting of head growth. Growth in head c.rc.mferenc. and weight with lower dose DEXA was c.mparable to growth after disc.ntinuation of DEXA.

目的考察病人对经皮使用硬化剂（硬化疗法）和（或）动脉栓塞治疗外周血管畸形（VM）的远期满意度。这种治疗是成功的,但有关远期效果的文献匮乏。方法本回顾性研究经机构伦理委员会批准,评估了107例有症状而接受血管畸形治疗的病人。获知情同意后发给66例病人关于治疗效果和病人满意度的调查表,收回病人的资料和复习影像资料以获得关于VM和VM治疗的信息。构建Kaplan-Meier生存曲线,以分析随时间推移的临床成功率。
