AIM:To study if impaired renal function is associated with increased risk of peri-infarct heart failure (HF) in patients with preserved ejection fraction (EF).METHODS:Patients with occluded infarct-related arteries (IRAs) between 1 to 28 d after myocardial infarction (MI) were grouped into chronic kidney disease (CKD) stages based on estimated glomerular filtration rate (eGFR).Rates of early post-MI HF were compared among eGFR *** regression was used to explore independent predictors of ***:Reduced eGFR was present in 71.1% of 2160 patients,with significant renal impairment (eGFR was higher with worsening renal function:15.5%,17.8% and 29.4% in patients with CKD stages 1,2 and 3 or 4,respectively (P was again higher with worsening renal function among patients with preserved EF:10.1%,13.6% and 23.6% (P was not significant among patients with depressed EF:27.1%,26.2% and 37.9% (P= 0.071).Moreover,eGFR was an independent correlate of HF in patients with preserved EF (P=0.003) but not in patients with depressed EF (P=0.181).CONCLUSION:A significant proportion of post-MI patients with occluded IRAs have impaired renal *** renal function was associated with an increased rate of early post-MI HF,the association being strongest in patients with preserved *** findings have implications for management of peri-infarct HF.

在中国,由社会组织提起环境公益诉讼的重要性和范围均在逐步发展中。原《民事诉讼法》( 1991 年) 第 108 条有关原告诉讼资格的条件规定成为限制环境公益诉讼发展的障碍。通过对中国 2005 -2011 年间围绕上述原告资格限制的立法和司法实践探索的归纳,并比较分析美国环境公共利益团体的发展历史及其通过司法诉讼实现减少污染、保护健康生态系统的目的,我们可以发现,美国《宪法》第 iii 条"案例或争论"要求和最高法院有关环境公益诉讼 NGO 原告资格的众多案例在许多方面与中国经验相"并行",但是中国的制度存在一些自身的社会特性因素。在未来中国环境公益诉讼的立法和司法实践中,我们应准确解读全国人大常委会起草《民事诉讼法》( 2012 年修正)第55 条时不断扩大"有关组织"范围的趋势; 应还原《民事诉讼法》(2012 年修正) 第 55 条于其诉讼程序法属性中解释"有关组织",而非实体法意义上界定主体的类型; 并应以培育社会组织为中国社会做出贡献的能力为目的,斟酌减少对社会组织的限制。
