背景：开放复位内固定术后，在内固定物存在的情况下治疗伤口深部感染极为棘手，关于治疗方案的使用，文献报道甚少。本研究旨在探讨骨折内固定术后6周内发生感染后保留内固定物直至骨性愈合的可能性。方法：通过回顾3个I级创伤中心以往的病历和创伤登记表，共纳入121例新鲜骨折患者，这些患者术中细菌培养均为阳性，内固定术后6周内共123处创口发生感染。统计保留内固定物并获骨折愈合者的比率，评价预测其治疗成败的指标。结果：86例患者（87处骨折；71％）经手术清创、保留内固定物和敏感抗生素治疗获得骨折愈合。治疗失败的有关因素包括开放性骨折（p=0．03）和髓内钉固定（p=0．01）。其他非明显相关但有相关倾向的变量包括吸烟、假单胞菌感染以及累及股骨、胫骨、踝部或足部的骨折。结论：骨折内固定术后深部感染者可通过手术清创、敏感抗生素治疗及保留内固定物成功获得骨折愈合。这些结果可通过基于危险基因、特定菌种和有关的内固定种类对患者进行选择而获得改善。

Beta-lactam antibiotic resistance among Escherichia coli isolates from patients with urinary tract infections in Vietnam:A systematic review and meta-analysis

Objective:to investigate the evolving patterns of antimicrobial resistance against beta-lactam antibiotics in Escherichia coli isolates from urinary tract infection patients in Vietnam,covering the period from 2005 to ***:29 Descriptive studies published between 2010 and 2022 in English and Vietnamese were included in the *** on resistance rates to beta-lactam antibiotics,including cephalosporins and carbapenems,were extracted and *** pooled resistance rates were calculated using random-effects *** trends in resistance were assessed using linear regression ***:Among the beta-lactam antibiotics studied,Escherichia coli exhibited varying levels of resistance,with cephalosporins showing higher resistance rates compared to *** pooled resistance rates were 66%for cefotaxime,65%for ceftriaxone,54%for ceftazidime,and 56%for *** contrast,carbapenems demonstrated lower resistance rates,with weighted pooled resistance rates ranging from 3%to 4%for meropenem,ertapenem,and *** rates were also observed for amoxicillin/clavulanate(35%),ticarcillin/clavulanate(26%),and piperacillin/tazobactam(12%).Significant annual increases in resistance were noted for imipenem(0.56%,Pthis study highlights the critical issue of antimicrobial resistance in urinary tract infections in Vietnam and emphasizes the importance of prudent antibiotic use and the regular monitoring of resistance *** insights are useful for guiding healthcare professionals in optimizing treatment strategies and for policymakers in formulating evidence-based clinical guidelines to combat antibiotic resistance effectively.

Bacterial colonization of orthopedic implants is one of the leading causes of failure and clinical complexities for load-bearing metallic implants. topical or systemic administration of antibiotics may not offer the most efficient defense against colonization, especially in the case of secondary infection, leading to surgical removal of implants and in some cases even limbs. In this study, laser powder bed fusion was implemented to fabricate ti3Al2V alloy by a 1:1 weight mixture of Cpti and ti6Al4V powders. ti-tantalum(ta)–Copper(Cu) alloys were further analyzed by the addition of ta and Cu into the ti3Al2V custom alloy. the biological,mechanical, and tribo-biocorrosion properties of ti3Al2V alloy were evaluated. A 10 wt.% ta(10ta) and 3 wt.% Cu(3Cu) were added to the ti3Al2V alloy to enhance biocompatibility and impart inherent bacterial resistance. Additively manufactured implants were investigated for resistance against Pseudomonas aeruginosa and Staphylococcus aureus strains of bacteria for up to 48 h. A 3 wt.% Cu addition to ti3Al2V displayed improved antibacterial efficacy, i.e.78%–86% with respect to Cpti. Mechanical properties for ti3Al2V–10ta–3Cu alloy were evaluated, demonstrating excellent fatigue resistance, exceptional shear strength, and improved tribological and tribo-biocorrosion characteristics when compared to ti6Al4V. In vivo studies using a rat distal femur model revealed improved early-stage osseointegration for alloys with10 wt.% ta addition compared to Cpti and ti6Al4V. the 3 wt.% Cu-added compositions displayed biocompatibility and no adverse infammatory response in vivo. Our results establish the ti3Al2V–10ta–3Cu alloy’s synergistic effect on improving both in vivo biocompatibility and microbial resistance for the next generation of load-bearing metallic implants.

In this work,we propose a stateless blockchain called CompactChain,which compacts the entire state of the UtXO(Unspent transaction Output)based blockchain systems into two RSA *** first accumulator is called transaction Output(tXO)commitment which represents the tXO *** second one is called Spent transaction Output(StXO)commitment which represents the StXO *** this work,we discuss three algorithms:(i)to update the tXO and StXO commitments by the *** miner also provides the proofs for the correctness of the updated commitments;(ii)to prove the transaction’s validity by providing a membership witness in tXO commitment and non-membership witness against StXO commitment for a coin being spent by a user;(iii)to update the witness for the coin that is not yet spent;the experimental results evaluate the performance of the CompactChain in terms of time taken by a miner to update the commitments and time taken by a validator to verify the commitments and validate the *** compare the performance of CompactChain with the existing state-of-the-art works on stateless *** shows a reduction in commitments update complexity and transaction witness size which inturn reduces the mempool size and propagation latency without compromising the system throughput(transactions per second(tPS)).

In Asia, the rice crop sustains millions of people. However, growing demand for this crop needs to be met while simultaneously reducing its water consumption to cope with the effects of climate change. Lowland cropping systems are the most common and productive but have particularly high water requirements. High-yielding rice genotypes adapted to drier environments(such as rainfed or aerobic rice ecosystems) are needed to increase the water use efficiency of cropping. Identifying these genotypes requires fast and more accurate selection methods. It is hypothesized that applying a new quantitative selection method(the score index selection method), can usefully compare rice yield responses over different years and stress intensities to select genotypes more rapidly and efficiently. Applying the score index to previously published rice yield data for 39 genotypes grown in no-stress and two stress environments, identified three genotypes(ARB 8, IR55419-04 and ARB 7) with higher and stable yield under moderate to severe stress conditions. these genotypes are postulated to be better adapted to stress environment such as upland and aerobic environments. Importantly, the score index selection method offers improved precision than the conventional breeding selection method in identifying genotypes that are well-suited to a range of stress levels within the target environment.
