Isolated sacral injuries: Postoperative length of stay, complications, and readmission

AIM: to investigate inpatient length of stay(LOS), complication rates, and readmission rates for sacral fracture patients based on operative ***: All patients who presented to a large tertiary care center with isolated sacral fractures in an 11-year period were included in a retrospective chart review. Operative approach(open reduction internal fixation vs percutaneous) was noted, as well as age, gender, race, and American Society of Anesthesiologists' score. Complications included infection, nonunion and malunion, deep venous thrombosis, and hardware problems; 90-d readmissions were broken down into infection, surgical revision of the sacral fracture, and medical complications. LOS was collected for the initial admission and readmission visits if applicable. Fisher's exact and non-parametric t-tests(Mann-Whitney U tests) were employed to compare LOS, complications, and readmissions between open and percutaneous ***: Ninety-four patients with isolated sacral fractures were identified: 31(30.4%) who underwentopen reduction and internal fixation(ORIF) vs 63(67.0%) who underwent percutaneous fixation. there was a significant difference in LOS based on operative approach: 9.1 d for ORIF patients vs 6.1 d for percutaneous patients(P = 0.043), amounting to a difference in cost of $13590. ten patients in the study developed complications, with no significant difference in complication rates or reasons for complications between the two groups(19.4% for ORIF patients vs 6.3% for percutaneous patients). Eight patients were readmitted, with no significant difference in readmission rates or reasons for readmission between the two groups(9.5% percutaneous vs 6.5% ORIF).CONCLUSION: there is a significant difference in LOS based on operative approach for sacral fracture patients. Given similar complications and readmission rates, we recommend a percutaneous approach.

Developing a new agenda for increased food and climate security

In many countries, political and environmental pressures are currently combining to generate a perfect storm of circumstances that is reducing food availability, increasing food costs and thereby reducing the availability of food to many. the UK is currently considering new national food and land management policies, and attention is also being given to legislation to address diet-related health issues. Many now argue for a revolution in UK farming practices to reduce their impact on the natural environment. the UK is not alone in facing these and other challenges. Both the contribution of agriculture to greenhouse gas(GHG) emissions and the effects of climate change on food production are issues receiving worldwide *** agricultural practices can result in greater C capture, reduced GHG emissions, enhanced soil quality and enhanced biodiversity. However, it is questioned if such farming systems will be productive enough to feed a growing population with the food required for social and health benefits. to fully exploit the impact of new plant science in farmer fields, it is imperative to effectively link science to farming practices and conduct a broader conversation around the food revolution with social scientists and with the general public.

Grow-light smart monitoring system leveraging lightweight deep learning for plant disease classification

this work focuses on a novel lightweight machine learning approach to the task of plant disease classification,posing as a core component of a larger grow-light smart monitoring *** the extent of our knowledge,this work is the first to implement lightweight convolutional neural network architectures leveraging down-scaled versions of inception blocks,residual connections,and dense residual connections applied without pre-training to the PlantVillage *** novel contributions of this work include the proposal of a smart monitor-ing framework outline;responsible for detection and classification of ailments via the devised lightweight net-works as well as interfacing with LED grow-light fixtures to optimize environmental parameters and lighting control for the growth of plants in a greenhouse *** adaptation of dense residual connections achieved the best balance of minimizing model parameters and maximizing performance metrics with accuracy,precision,recall,and F1-scores of 96.75%,97.62%,97.59%,and 97.58%respectively,while consisting of only 228,479 model *** results are further compared against various full-scale state-of-the-art model architectures trained on the PlantVillage dataset,of which the proposed down-scaled lightweight models were capable of performing equally to,if not better than many large-scale counterparts with drastically less com-putational requirements.

Nitrate reduction capacity of the oral microbiota is impaired in periodontitis:potential implications for systemic nitric oxide availability

the reduction of nitrate to nitrite by the oral microbiota has been proposed to be important for oral health and results in nitric oxide formation that can improve cardiometabolic conditions. Studies of bacterial composition in subgingival plaque suggest that nitrate-reducing bacteria are associated with periodontal health, but the impact of periodontitis on nitrate-reducing capacity(NRC)and, therefore, nitric oxide availability has not been evaluated. the current study aimed to evaluate how periodontitis affects the NRC of the oral microbiota. First, 16S rRNA sequencing data from five different countries were analyzed, revealing that nitratereducing bacteria were significantly lower in subgingival plaque of periodontitis patients compared with healthy individuals(P tasets with n = 20–82 samples per dataset). Secondly, subgingival plaque, saliva, and plasma samples were obtained from 42 periodontitis patients before and after periodontal treatment. the oral NRC was determined in vitro by incubating saliva with 8 mmol/L nitrate(a concentration found in saliva after nitrate-rich vegetable intake) and compared with the NRC of 15healthy individuals. Salivary NRC was found to be diminished in periodontal patients before treatment(P t recovered to healthy levels 90 days post-treatment. Additionally, the subgingival levels of nitrate-reducing bacteria increased after treatment and correlated negatively with periodontitis-associated bacteria(P t effect of periodontal treatment on the baseline saliva and plasma nitrate and nitrite levels was found, indicating that differences in the NRC may only be revealed after nitrate intake. Our results suggest that an impaired NRC in periodontitis could limit dietary nitrate-derived nitric oxide levels, and the effect on systemic health should be explored in future studies.

Late Ordovician Mass Extinction: Earth, fire and ice

the Late Ordovician Mass Extinction was the earliest of the ‘big’ five extinction events and the earliest to affect the trajectory of metazoan life. two phases have been identified near the start of the Hirnantian period and in the middle. It was a massive taxonomic extinction, a weak phylogenetic extinction and a relatively benign ecological extinction. A rapid cooling, triggering a major ice age that reduced the temperature of surface waters, prompted a drop in sea level of some 100 m and introduced toxic bottom waters onto the shelves. these symptoms of more fundamental planetary processes have been associated with a range of factors with an underlying driver identified as volcanicity. Volcanic eruptions, and other products, may have extended back in time to at least the Sandbian and early Katian, suggesting the extinctions were more protracted and influential than hitherto documented.

Preparation of microparticles and nanoparticles using membrane-assisted dispersion,micromixing,and evaporation processes

Synthetic microporous membranes are increasingly used for energy-efficient and controlled production of micro-and nanoparticles and micro-and nanoemulsions with tuneable morphology and physico-chemical properties through various micromixing,emulsification,and evaporation *** emul-sification processes,the membrane pores are used for dispersed phase injection and size-controlled generation of droplets and droplet-templated *** micromixing processes,membrane is utilised as a micromixer for mixing two miscible liquids,usually solvent and antisolvent-rich solutions,which leads to the creation of supersaturation and subsequent nanoprecipitation or *** mem-brane evaporation processes,membrane is used to prevent phase dispersion while allowing efficient molecular diffusion of solvent and/or antisolvent vapour through gas-filled *** dispersion processes can be operated continuously by decoupling shear stress on the membrane surface from cross flow using tube insets,flow pulsations,swirling flow,membrane oscillations or membrane *** generation and solidification can be performed continuously in a single pass by connecting membrane module with a downstream *** dispersion processes can be used for pro-duction of nanoparticles such as nanovesicles(liposomes,micelles,ethosomes,and niosomes),nanogels,polymeric,lipid and metallic nanoparticles,and *** main advantages of membrane-assisted particle generation are in low energy consumption,controlled geometry and hydrodynamic conditions at the microscale level,flexible throughput due to modular and scalable design of membrane devices,and a wide choice of available microporous membranes with various wall porosities,wetta-bilities,pore sizes,and pore morphologies to suit different applications.

the choice of a type of combustion technology to be used for heat or power generation depends on economic,technical,operational and fuel availability *** benefits associated with the evolving market driven by the fluidised bed combustion(FBC)technology cannot be overlooked especially when gauged at 65 GWth of worldwide installed capacity alongside added benefits of handling fuel variation,low pollutant emissions and high combustion *** or biomass waste will continue to have a vital role to play in the future FBC technology-based power *** often contains high levels of inorganic species that can form sticky agglomerates posing a significant risk to boiler operation resulting in unscheduled *** added complexity of the behaviour of the fuel and bed material mix highlights the requirement for simulation models to identify agglomeration to help improve the overall performance and reliability of FBC *** resolve this problem,this research devised a simulation strategy for the detection of agglomeration using the Eulerian–Eulerian *** developed modelling strategy is validated with the experimental data available in literature for two-dimensional simplified geometry of a pilot-scale fluidised bed *** model results were found promising and robust to predict bed defluidisation times and other parameters consistent with the experimental data.

He has served on the California, texas and Massachusetts Professional Associations, as well as the National Association. He has tendered 20 years of service to the field through academic medicine, earning a master’s degree in medical education to assist in developing Chinese medical doctoral programs and institutional review boards. He earned an OM.D. in 1988, a DAOM in 2002, and a Ph.D. focused on pulse diagnosis as clinical epistemology in 2009. He has published five books, including tCM Case Studies Dermatology, Li Shi-zhen Pulse Studies: An Illustrated Guide, Neoclassical Pulse Diagnosis, Cycles in Medical Astrology, and transformation: treating trauma with Acupuncture and Herbs.
