BACKGROUND:Low central venous pressure(CVP) affects hemodynamic stability and tissue *** prospective study aimed to evaluate the optimal CVP during partial hepatectomy for hepatocellular carcinoma(HCC).METHODS:Ninety-seven patients who underwent partial hepatectomy for HCC had their CVP controlled at a level of 0 to 5 mmHg during hepatic parenchymal *** systolic blood pressure(SBP) was maintained,if possible,at 90 mmHg or *** B surface antigen was positive in 90 patients(92.8%) and cirrhosis in 84 patients(86.6%).Pringle maneuver was used routinely in these patients with clamp/unclamp cycles of 15/5 *** average clamp time was 21.4±8.0 *** patients were divided into 5 groups based on the CVP:group A:0-1 mmHg;B:1.1-2 mmHg;C:2.1-3 mmHg;D:3.1-4 mmHg and E:4.1-5 *** blood loss per transection area during hepatic parenchymal transection and the arterial blood gas before and after liver transection were ***:With active fluid load,a constant SBP ≥90 mmHg which was considered as optimal was maintained in 18.6% in group A(95% CI:10.8%-26.3%);39.2% in group B(95% CI:29.5%-48.9%);72.2% in group C(95% CI:63.2%-81.1%);89.7% in group D(95% CI:83.6%-95.7%);and 100% in group E(95% CI:100%-100%).The blood loss per transection area during hepatic parenchymal transection decreased with a decrease in *** to groups D and E,blood loss in groups A,B and C was significantly less(analysis of variance test,Phe baseline,the blood oxygenation decreased significantly when the CVP was *** excess and HCO 3-in groups A and B were significantly decreased compared with those in groups C,D and E(Phepatectomy for HCC.

In this study,focusing on the geometry characteristics of spherical expanding flame,the turbulent premixed flames of natural gas/air mixtures were investigated in a fan-stirred turbulent *** effects of initial temperature（T=300-400 K）,initial pressure（P=0.1-0.3 MPa）,turbulence intensity（u'=1.0-2.7 m/s）,oxygen volumetric percentage(φ（O2）=15%-21%) and carbon dioxide volumetric percentage(φ（CO2）=0-20%)were delved *** flame profile under the Cartesian coordinate system was derived from the schlieren images taken by the high-speed ***,from both macroscopic and microscopic perspectives,the influence of experimental conditions on the flame geometry characteristics was explored through flame front extraction,wavelet decomposition and network *** results demonstrate that for significant flame wrinkling,changes in species concentrations and turbulence intensity have more pronounced effects on the flame wrinkling *** wrinkling of the flame front maintains a certain degree of similarity,as evidenced by the locally concentrated distribution of the angles of the maximum fluctuation *** disturbance energy under large-scale（D6-D8） disturbances exhibits relatively high values with a similar trend,exerting a significant impact on the geometry characteristics of the flame *** peaks of correlation degree are scattered either with the decomposition scale or the development of flame radius,indicating no linear correlation between different detail ***,the probability distribution of node degrees in key wrinkled regions exhibits different trends with that of large-scale wrinkling and disturbance energy,especially with changes in initial *** occurs because the number of key wrinkles varies based on the perturbation's strength or the region's ***,an increase in the fluctuation frequency of the flame's local radius may not necessarily lead to an increase in the number of key folded regions.

Endoplasmic reticulum(ER) viscosity has emerged as a new potential biomarker for hepatic fibrosis(HF). herein, we report the first ER-targeting viscosity-responsive near-infrared(NIR) fluorescent probe HBT-PP for investigations of HF *** twisted intramolecular charge transfer(TICT) and the excited state intramolecular proton transfer(ESIPT) are combined to endow HBT-PP with the selective response towards viscosity(～62-fold) and a super large stokes shift(～320 nm). HBT-PP mainly targets organelle ER and is able to image the drug-or alcohol-induced viscosity elevation in living cells. Significantly,HBT-PP was successfully applied to visualize the gender difference in HF and the protective role of estrogen in restraining HF progression. Of note, after 10 min of stain with HBT-PP, HF lesions showed a stage-based response, indicating the potential of this probe in fast and early diagnosis of HF. These results clearly demonstrated the promising application of HBT-PP as a new tool for exploring ER viscosity-involved physiological and pathological processes.

Porous organic cages(POCs)with permanent porosity and excellent host–guest property hold great potentials in regulating ion transport behavior,yet their feasibility as solid-state electrolytes has never been testified in a practical ***,we design and fabricate a quasi-solid-state electrolyte(QSSE)based on a POC to enable the stable operation of Li-metal batteries(LMBs).Benefiting from the ordered channels and cavity-induced anion-trapping effect of POC,the resulting POC-based QSSE exhibits a high Li+transference number of 0.67 and a high ionic conductivity of 1.25×10^(−4) S cm^(−1) with a low activation energy of 0.17 *** allow for homogeneous Li deposition and highly reversible Li plating/stripping for over 2000 *** a proof of concept,the LMB assembled with POC-based QSSE demonstrates extremely stable cycling performance with 85%capacity retention after 1000 ***,our work demonstrates the practical applicability of POC as SSEs for LMBs and could be extended to other energy-storage systems,such as Na and K batteries.

As a class of functional crystalline porous materials, metal-organic frameworks(MOFs) gained rapid development in the past three decades and a large number of MOFs with ordered structures, high surface areas, and functionalized channels have been investigated. MOFs and MOF-derived/composite materials show great potential in many application fields. In this review, we discussed the main applications of MOFs and MOF-derived/composite materials in small molecule storage, separation, luminescence, sensing, multitype catalysis, and energy storage. In addition, challenges and problems in the future research of MOFsrelated fields are also discussed.

目的探讨甘油三酯（TG）／高密度脂蛋白胆固醇（HDL-C）比值与急性缺血性脑卒中（AIS）患者预后的关系。方法收集1006例首发AIS住院患者的临床资料。所有患者在入院36h内分别测定血浆TG、总胆固醇（TC）、HDL-C和低密度脂蛋白胆固醇（LDL-C）的水平。3个月后根据改良Rankin评分量表（mRS）分为两种不同的预后：mRS 0-1预后良好，6即死亡。结果较高水平TG、非高密度脂蛋白胆固醇（Non-HDL-C）和TG／HDL-C与AIS患者预后密切相关：预后良好的OR值分别为1．39、1．89和2．34，死亡的OR值分别为0．59、0．29和0．26。TG／HDL-C≥0．87是临床预后良好[ROC曲线下面积（AUC）为0．596，敏感度73．3％，特异度42．7％]和非死亡（AUC0．674，敏感度67．8％，特异度60．6％）的最佳区分因子。与TG／HDL-C／〉0．87比较，TG／HDL-C〈0．87时AIS患者死亡风险增加2．94倍。结论较低的TG／HDL-C比值与AIS患者3个月后死亡和预后不良独立相关。

目的系统评价持续静脉-静脉血液滤过(continuous veno-venous hemoi ltration,CVVH)为主要方式治疗高脂血症性胰腺炎的有效性和安全性。方法计算机检索h e Cochrane Library(2014第2期)、PubMed、EMbase、CBM、CNKI和WanFang Data,查找有关CVVH治疗高脂血症性胰腺炎的随机对照试验(RCT),检索时限截至2014年2月12日。由2位评价员根据纳入与排除标准独立筛选文献、提取资料并评价质量后,采用RevMan 5.2软件进行Meta分析。结果最终纳入7个研究,共360例患者,其中CVVH组183例,常规综合治疗组177例。Meta分析结果显示:与常规治疗比较,CVVH能显著降低患者治疗后血脂水平[WMD=–4.63,95%CI(–5.98,–3.27),Phe II评分[WMD=–3.34,95%CI(–5.12,–1.56),P=0.000?2]明显低于常规治疗组。结论与常规治疗相比,CVVH治疗高脂血症性胰腺炎更加有效。但受纳入研究数量和质量所限,上述结论尚需今后开展多中心、大样本的高质量RCT加以验证。

目的探讨中药在MDR-AB重症肺炎治疗中的作用。方法前瞻性方便收集2014年4月—2015年4月期间,福建中医药大学附属人民医院重症医学科收治的60例MDR-AB肺炎的病例,随机分为序贯组和对照组,各30例。两组早期均选用敏感抗生素治疗,序贯组根据辩证选方联合、序贯中药治疗2周。观察体温、外周血白细胞、PCT、痰培养情况、胸片转阴时间、呼吸机辅助时间、抗生素使用强度等指标。结果序贯组30例,成功26例,失败4例,平均序贯时机在第(5.23±1.76)天。成功病例的炎症指标较对照组改善显著,机械通气时间缩短,抗生素使用强度明显下降,差异有统计学意义(P<0.05)。结论抗生素联合序贯中药治疗MDR-AB重症肺炎的方法切实可行,效果显著,关键是辨证用药的准确性和序贯的时机把握。
