Perioperative outcomes of transvaginal specimen extraction laparoscopic total gastrectomy and conventional laparoscopicassisted total gastrectomy

BACKGROUND Natural orifice specimen extraction surgery(NOSES)has emerged as a promising alternative compared to conventional laparoscopic-assisted total gastrectomy(LATG)for treating gastric cancer(GC).However,evidence regarding the efficacy and safety of NOSES for GC surgery is limited.This study aimed to compare the safety and feasibility,in addition to postoperative complications of NOSES and LATG.METHODS Dual circular staplers were used in Roux-en-Y digestive tract reconstruction for transvaginal specimen extraction LATG,and its outcomes were compared with LATG in a cohort of 51 GC patients with tumor size≤5 cm.The study was conducted from May 2018 to September 2020,and patients were categorized into the NOSES group(n=22)and LATG group(n=29).Perioperative parameters were compared and analyzed,including patient and tumor characteristics,postoperative outcomes,and anastomosis-related complications,postoperative hospital stay,the length of abdominal incision,difference in tumor type,postoperative complications,and postoperative survival.RESULTS Postoperative exhaust time,operation duration,mean postoperative hospital stay,length of abdominal incision,number of specific staplers used,and Brief Illness Perception Questionnaire score were significant in both groups(P<0.01).In the NOSES group,the postoperative time to first flatus,mean postoperative hospital stay,and length of abdominal incision were significantly shorter than those in the LATG group.Patients in the NOSES group had faster postoperative recovery,and achieved abdominal minimally invasive incision that met aesthetic requirements.There were no significant differences in gender,age,tumor type,postoperative complications,and postoperative survival between the two groups.CONCLUSION The application of dual circular staplers in Roux-en-Y digestive tract reconstruction combined with NOSES gastrectomy is safe and convenient.This approach offers better short-term outcomes compared to LATG,while long-term survival rates are comparable to those of conventional la

In recent years,natural orifice specimen extraction surgery(NOSES),a novel minimally invasive surgical technique,has become a focus in the surgical field,and has been initially applied in gastric surgery in many national medical centers worldwide.In addition,this new surgical technique was launched in major hospitals in China.With an increasing number of patients who have accepted this new surgical technique,NOSES has provided new prospects for the treatment of gastric cancer(GC),which may achieve a better outcome for both patients and surgeons.More and more experts and scholars from different countries and regions are currently paying close attention to NOSES for the treatment of GC.However,there are only a few reports of its use in GC.This review focuses on the research progress in NOSES for radical gastrectomy in recent years.We also discuss the challenges and prospects of NOSES in clinical practice.

Let u={u(t,x),t∈[0,T],x∈R}be a solution to a stochastic heat equation driven by a space-time white noise.We study that the realized power variation of the process u with respect to the time,properly normalized,has Gaussian asymptotic distributions.In particular,we study the realized power variation of the process u with respect to the time converges weakly to Brownian motion.

A LIL and Limit Distributions for Trimmed Sums of Random Vectors Attracted to Operator Semi-stable Laws

Let θ∈^d be a unit vector and let X, X1, X2,…… be a sequence of i.i.d. Xd-valued random vectors attracted to operator semi-stable laws. For each integer n ≥1, let X1,≤……≤ Xn,n denote the order statistics of X1, X2,..., Xn according to priority of index, namely ｜（X1,nθ）｜≥…≥ [（Xn,n,θ）1, where （., .） is an inner product on Rd. For all integers r ≥ 0, define by （r）Sn =∑n-r i=1Xi,n the trimmed sum. In this paper we investigate a law of the iterated logarithm and limit distributions for trimmed sums （r）Sn. Our results give information about the maximal growth rate of sample paths for partial sums of X when r extreme terms are excluded. A stochastically compactness of （r）Sn is obtained.

A Strong Approximation Theorem for Quasi-associated Sequences

By combining the Csorgo-Révész quantile transform methods and the Skorohod-Strassen martingale embedding theorem, we prove a strong approximation theorem for quasi-associated random variables with mean zero and finite （2 ＋ δ）th moment under polynomial decay rate. As a consequence, the decay rate for a strong approximation theorem of associated sequences of Yu （1996） is weakened.

Study on the driving factors and regulation mode for coal production capacity

Coal production capacity regulation is a complex system involving economic growth,structural optimization,high-efficiency mining,and environmental protection.Based on its driving factors,this paper forms four regulation modes representing different control orientations,establishes a system dynamics model,and predicts the regulation effects of single-factor and combined control mode.The result shows:(1) Except for the mechanization degree and recovery rate,the other nine individual production capacity control policies are all conducive to reducing coal production capacity and restraining the excessive growth of coal production capacity.(2) The effect of combined regulation mode on slowing down the growth of coal demand,regulating the excessive growth of coal production capacity and new capacity investment are obviously better than that of single policy.(3) The combined control modes have obvious differences in the suppression effect on coal production capacity:transformational development mode > technology-driven mode > structural optimization mode > efficiency improvement mode.Therefore,in the process of achieving optimal regulation of coal production capacity,attention should be paid to the preferential use of transformational development and technology-driven mode.At the same time,the comprehensive use of regulation and control methods should also be considered to improve the regulation effect and the regulation efficiency of coal production capacity.

This paper studies the global and local properties of the trajectories of Gaussian random fields with stationary increments and proves sufficient conditions for Strassen's functional laws of the iterated logarithm at zero and infinity respectively.The sets of limit points of those Gaussian random fields are obtained.The main results are applied to fractional Riesz-Bessel processes and the sets of limit points of this field are obtained.

The Finite-time Ruin Probability of a Discrete-time Risk Model with Subexponential and Dependent Insurance and Financial Risks

Consider a discrete-time risk model with insurance and financial risks in a stochastic economic environment. Assume that the insurance and financial risks form a sequence of independent and identically distributed random vectors with a generic random vector following a wide type of dependence structure. An asymptotic formula for the finite-time ruin probability with subexponential insurance risks is derived. In doing so, the subexponentiality of the product of two dependent random variables is investigated simultaneously.
