Efficacy and Safety of Chinese Herbal Medicines Combined with Chemical Drugs for Alzheimer's Disease:A Systematic Review and Meta-Analysis

Objective:To evaluate the efficacy and safety of Chinese herbal medicines(CHMs)combined with chemical drugs for the treatment of Alzheimer's disease(AD).Materials and Methods:Databases were searched to retrieve randomized controlled trials(RCTs)of CHMs combined with tacrine,galantamine,rivastigmine,donepezil,or memantine,following strict inclusion and exclusion *** research papers published in English,Chinese,and Japanese were *** primary outcome was the mini-mental state examination(MMSE)score and the secondary outcomes were AD assessment scale cognitive subscale(ADAS-Cog)score and safety ***-analysis was carried out using Rev Man 5.3 and subgroup analysis was conducted to identify the sources of ***:A total of 15 RCTs with 1386 participants were included in this *** donepezil was used in the retrieved ***-analyses showed that the combination of CHMs with donepezil led to significant improvement in the MMSE score(Z=9.45;P<0.00001;weighted mean difference[WMD]=2.68,95%confidence interval[CI]:2.12-3.24)and a significant decrease in the ADAS-Cog score(Z=5.53;P<0.00001;WMD=-3.79;95%CI:-5.13--2.44).Safety evaluation demonstrated that these combination treatments relieved adverse events such as insomnia(risk ratio[RR]=0.20,95%CI:0.06-0.68;P=0.01)and hive(RR=0.10;95%CI:0.01-0.73;P=0.02).Conclusions:The combination of CHMs with a chemical drug like donepezil led to significant improvements in patient cognition as well as a better safety profile when compared to the application of a chemical drug alone.

Prognostic value of ECOG performance status and Gleason score in the survival of castration-resistant prostate cancer:a systematic review

Eastern Cooperative Oncology Group(ECOG)performance status and Gleason score are commonly investigated factors for overall survival(OS)in men with castration-resistant prostate cancer(CRPC).However,there is a lack of consistency regarding their prognostic or predictive value for ***,we performed this meta-analysis to assess the associations of ECOG performance status and Gleason score with OS in CRPC patients and compare the two markers in patients under different treatment regimens or with different chemotherapy histories.A systematic literature review of monotherapy studies in CRPC patients was conducted in the PubMed database until May *** data from 8247 patients in 34 studies,including clinical trials and real-world data,were included in our *** these,twenty studies reported multivariate results and were included in our main *** patients with higher ECOG performance statuses(≥2)had a significantly increased mortality risk than those with lower ECOG performance statuses(<2),hazard ratio(HR):2.10,95%confidence interval(CI):1.68-2.62,and P<*** synthesized HR of OS stratified by Gleason score was 1.01,with a 95%CI of 0.62-1.67(Gleason score≥8 vs<8).Subgroup analysis showed that there was no significant difference in pooled HRs for patients administered taxane chemotherapy(docetaxel and cabazitaxel)and androgen-targeting therapy(abiraterone acetate and enzalutamide)or for patients with different chemotherapy *** performance status was identified as a significant prognostic factor in CRPC patients,while Gleason score showed a weak prognostic value for OS based on the available data in our meta-analysis.

AtCLH2, a Typical but Possibly Distinctive Chlorophyllase Gene in Arabidopsis

Chlorophyllase （EC 3.***.1.14） is involved in the first step of chlorophyll degradation. Isolation of chlorophyllase genes greatly facilitates characterization of chlorophyllase properties and elucidation of molecular regulation of their in vivo activities. There are two chlorophyllase genes, AtCLH1 and AtCLH2, in Arabidopsis thaliana. The in vivo roles of AtCLH1 have been reported previously. However, few studies have been carried out on AtCLH2. Here, we show that purified recombinant Chlase2, encoded by AtCLH2, exhibits in vitro chlorophyllase activity. Interestingly, ＂activation＂ of in vitro activity of the recombinant Chlase2 required higher concentrations of a detergent or a polar solvent. To determine its activity in vivo, the expression of AtCLH2 was inhibited by RNA interference. RNAi plants showed decreased contents of chlorophyllide without a substantial change in the total amount of the extractable chlorophyll and consequently presented lower chlorophyllide to chlorophyll ratios in their leaves. In addition, the two AtCLHs exhibited differential expression patterns. Our results suggest that AtCLH2 might play a distinctive role in chlorophyll catabolism in vivo.

Valproic acid's effects on visual acuity in retinitis pigmentosa: a systemic review and Meta-analysis

AIM: To gain a better understanding of the overall efficacy of valproic acid(VPA) treatment for retinitis pigmentosa(RP). METHODS: Publications in PubMed, EMBASE, Cochrane Library, Web of Science and *** were searched for clinical trials of patients with RP assigned to treatment with VPA. Patients' pre-and post-treatment visual field(VF) and best-corrected visual acuity(BCVA) scores were extracted and compared to assess changes. RESULTS: A total of 78 reports were retrieved and 6 studies involving 116 patients were included in the *** combined results showed a significant decrease in logarithm of minimal angle of resolution(logMAR) scores,calculated using baseline and post-treatment BCVA(P<0.00001, mean difference=-0.05, 95%CI:-0.05,-0.04,I2=36%) scores, which means there was considerable improvement in visual acuity. Meanwhile, more BCVA changes were observed in short-term(≤6 mo) treatment studies(P<0.00001, mean difference=-0.05, 95%CI:-0.05,-0.04, I2=38%), studies conducted in Asia(P<0.00001,mean difference=-0.05, 95%CI:-0.05,-0.04, I2=4%), studies with a sample size of 30 or fewer patients(P<0.00001,mean difference=-0.05, 95%CI:-0.05,-0.04, I2=38%) and prospective studies(P<0.00001, mean difference=-0.05,95%CI:-0.05,-0.04, I2=0%). However, VPA's effect on VF was inconsistent across studies(P=0.75, mean difference=-22.76, 95%CI:-160.56, 115.05, I2=68%). CONCLUSION: This Meta-analysis reveals that most RP patients who were treated with VPA showed improvement in BCVA. However, its effect on VF remains *** may be a promising treatment for RP.

Spontaneous Vesicle Formation in Mixed Aqueous Solution of Poly-tailed Cationic and Anionic Surfactants

Spontaneous vesicles from the mixed aqueous solution of poly-tailed cationic surfactant PTA and anionic surfactant AOT are firstly obtained. Vesicle formation and characterizations are demonstrated by negative-staining TEM and dynamic light scattering. A monodisperse vesicle system is obtained with a polydispersity of 0.082. Ultrasonication can promote the vesicle formation. Mechanism of vesicle formation is discussed from the viewpoint of molecular interaction.

Solid-state lithium metal batteries are one of the most promising options for next-generation batteries pursuing high-energy density and ***,the inevitable volatilization of lithium compounds during sintering leads to low relative density and low ionic conductivity of solid-state ***,the dynamic lithium-compensation mechanism is proposed to facilitate the densification of Ta-substituted garnet-type electrolyte(Li_(6.5)La_(3)Zr_(1.5)Ta_(0.5)O_(12)(LLZT))through the reversible manipulating of Li_(2)O ***_(2)ZrO_(3)is used as mother powder additive,which reacts with Li_(2)O in sintering atmosphere and forms Li_(6)Zr_(2)O_(7).Li_(2)ZrO_(3)/Li_(6)Zr_(2)O_(7)buffer pair manipulates the sintering Li_(2)O atmosphere,which is vital for LLZT,within the Li_(2)O partial pressure range corresponding to Li_(2)ZrO_(3)and Li_(6)Zr_(2)O_(7).Furthermore,the reversibility mechanism of buffer pair for Li_(2)O absorption and release is *** obtained LLZT exhibits a relative density of over 96%and an ionic conductivity exceeding 7×10^(−4)S·cm^(−1)with no abnormal grain *** symmetric cell demonstrates an excellent lithium dendrite suppressing ability(stable cycling at a current density of 0.3 mA·cm^(−2)for over 1000 h).Such dynamic lithium-compensation strategy has been successfully applied in atmosphere manipulation of LLZT sintering process,which reduces the dependence of LLZT on the Li_(2)O atmosphere,making it conducive to large-scale preparation of electrolyte ceramics.

The lifetime of Si-based environmental barrier coatings(EBCs)is constrained by thermally grown SiO_(2)oxidelayer(SiO_(2)-TGO),which can cause premature cracking and *** address this issue,a new approach for surfacemodification using aluminum is *** oxidation performance was examined in a 50 vol%H_(2)O-50 vol%O_(2)environment at 1350℃for up to 300 *** results indicate that a dense ytterbium aluminum garnet(YbAG)layer wasformed after modification,decreasing the porosity by 80%.Due to the elimination of fast diffusion channels and the lowoxygen permeability of YbAG,aluminum modification significantly reduced the growth rate of SiO_(2)-TGO by nearly twoorders of ***,its thickness decreased by more than 70%after 300 h of exposure.A diffusioncontrolled oxidation mechanism indicates that the modified dense surface is equivalent to an initial SiO_(2)layer with a specificthickness,causing a shift in the oxidation time and increasing the oxidation *** research provides valuableinsights for designing Si-based EBC with improved lifetimes.
