Optimal approach for total endoscopic discectomy and its effect on lumbar and leg function in patients with disc herniation

BACKGROUND Disc herniation(DH)is a fragment of the disc nucleus that is pushed out of the annulus into the spinal canal due to a tear or rupture in the *** is a common cause of lumbar and leg *** advancements have been made to determine the cause of DH and to ensure accurate diagnosis,imaging,and treatment of this *** endoscopic discectomy is an alternative surgical technique that is less *** To study the optimal approach for a total endoscopic discectomy and its influence on lumbar and leg function in DH *** This prospective study enrolled 120 patients with lumbar DH who were treated in our hospital from February 2018 to January *** patients were randomly divided into the following two groups:The observation group,comprising 62 patients who underwent surgery using the interlaminar approach,and the control group,comprising 58 patients who were operated through the foramina *** treatment effects,perioperative indicators,functional recovery,pain,and quality of life were compared between the two *** The treatment effect in the observation group(93.55%)was significantly better than that in the control group(77.59%).There was no difference in the operative time and intraoperative blood loss amount between the two groups(P>0.05).The hospitalization time of the observation group(4.34±1.33 d)was significantly shorter than that of the control group(5.38±1.57 days)(Pboth groups after treatment,but the scores were lower in the observation group than in the control *** visual analog scale scores of the lower back and legs of the two groups were significantly reduced after treatment,but scores were lower in the observation group(2.18±0.88 in the lower back and 1.42±0.50 in the leg)than in the control group(3.53±0.50 in the lower back and 2.21±0.52 in the leg).A short form of the Arthritis

A review of the use of pteridophytes for treating human ailments

The aim of this review was to explore the pharmacological activity of early tracheophytes （pteridophytes） as an alternative medicine for treating human ailments. As the first vascular plants, pteridophytes （aka, ferns and fern allies） are an ancient lineage, and human beings have been exploring and using taxa from this lineage for over 2000 years because of their beneficial properties. We have documented the medicinal uses of pteddophytes belonging to thirty different families. The lycophyte Selaginella sp. was shown in earlier studies to have multiple pharmacological activity, such as antioxidant, anti-inflammatory, anti-cancer, antidiabetic, antiviral, antimicrobial, and anti-Alzheimer properties. Among all the pteridophytes examined, taxa from the Pteridaceae, Polypodiaceae, and Adiantaceae ex- hibited significant medicinal activity. Based on our review, many pteridophytes have properties that could be used in alternative medicine for treatment of various human illnesses. Biotechnological tools can be used to preserve and even improve their bioactive molecules for the preparation of medicines against illness. Even though several studies have reported medicinal uses of ferns, the possible bioacUve compounds of several pteridophytes have not been identified. Furthermore, their optimal dosage level and treatment strategies still need to be determined. Finally, the future direc- tion of pteridophyte research is discussed.

The potential mechanism and clinical application value of remote ischemic conditioning in stroke

Some studies have confirmed the neuroprotective effect of remote ischemic conditioning against stroke. Although numerous animal researches have shown that the neuroprotective effect of remote ischemic conditioning may be related to neuroinflammation, cellular immunity, apoptosis, and autophagy, the exact underlying molecular mechanisms are unclear. This review summarizes the current status of different types of remote ischemic conditioning methods in animal and clinical studies and analyzes their commonalities and differences in neuroprotective mechanisms and signaling pathways. Remote ischemic conditioning has emerged as a potential therapeutic approach for improving stroke-induced brain injury owing to its simplicity, non-invasiveness, safety, and patient tolerability. Different forms of remote ischemic conditioning exhibit distinct intervention patterns, timing, and application range. Mechanistically, remote ischemic conditioning can exert neuroprotective effects by activating the Notch1/phosphatidylinositol 3-kinase/Akt signaling pathway, improving cerebral perfusion, suppressing neuroinflammation, inhibiting cell apoptosis, activating autophagy, and promoting neural regeneration. While remote ischemic conditioning has shown potential in improving stroke outcomes, its full clinical translation has not yet been achieved.

Moment Redistribution Effect of the Continuous Glass Fiber Reinforced Polymer-Concrete Composite Slabs Based on Static Loading Experiment

This study aimed to investigate the moment redistribution in continuous glass fiber reinforced polymer(GFRP)-concrete composite slabs caused by concrete cracking and steel bar yielding in the negative bending moment *** experimental bending moment redistribution test was conducted on continuous GFRP-concrete composite slabs,and a calculation method based on the conjugate beam method was *** composite slabs were formed by combining GFRP profiles with a concrete layer and supported on steel beams to create two-span continuous composite slab *** methods,epoxy resin bonding,and stud connection,were used to connect the composite slabs with the steel *** experimental findings showed that the specimen connected with epoxy resin exhibited two moments redistribution phenomena during the loading process:concrete cracking and steel bar yielding at the internal *** contrast,the composite slab connected with steel beams by studs exhibited only one-moment redistribution phenomenon throughout the loading *** the concrete at the internal support cracked,the bending moment decreased in the internal support section and increased in the midspan *** the steel bars yielded,the bending moment further decreased in the internal support section and increased in the mid-span *** GFRP profiles do not experience cracking,there was no significant decrease in the bending moment of the mid-span *** test specimens experienced compressive failure of concrete at the mid-span *** results showed good agreement between the calculated and experimental values of bending moments in the mid-span section and internal support *** proposed model can effectively predict the moment redistribution behavior of continuous GFRP-concrete composite slabs.

Theory predicts that the degree of testes asymmetry should be positively correlated with male body condition only when species in which there is an overall directional testis asymmetry. We tested if the above prediction from theoretical models can be applied to Rhacophorus omeimontis,a species where females mate with more than one *** results showed a non-significant difference in testes mass between left and right testes,and the treefrog did not exhibit directional testis asymmetry,but fluctuating testis ***,we also tested the difference in body size and mass,relative testis mass,testis asymmetry and sperm size among initially paired,joiner and unpaired *** found that body size and mass,testis mass,testis asymmetry and sperm length did not differ significantly among initially paired,joiner and unpaired *** mass showed a positive relationship with soma mass,but there were non-significant correlations between fluctuating testis asymmetry and sperm length and between testis mass and sperm length.

Effects of aggregate size distribution and carbon nanotubes on the mechanical properties of cemented gangue backfill samples under true triaxial compression

The mechanical behavior of cemented gangue backfill materials(CGBMs)is closely related to particle size distribution(PSD)of aggregates and properties of cementitious ***,the true triaxial compression tests,CT scanning,SEM,and EDS tests were conducted on cemented gangue backfill samples(CGBSs)with various carbon nanotube concentrations(P_(CNT))that satisfied fractal theory for the PSD of *** mechanical properties,energy dissipations,and failure mechanisms of the CGBSs under true triaxial compression were systematically *** results indicate that appropriate carbon nanotubes(CNTs)effectively enhance the mechanical properties and energy dissipations of CGBSs through micropore filling and microcrack bridging,and the optimal effect appears at P_(CNT)of 0.08wt%.Taking PSD fractal dimension(D)of 2.500 as an example,compared to that of CGBS without CNT,the peak strength(σ_(p)),axial peak strain(ε_(1,p)),elastic strain energy(Ue),and dissipated energy(U_(d))increased by 12.76%,29.60%,19.05%,and90.39%,***,excessive CNTs can reduce the mechanical properties of CGBSs due to CNT agglomeration,manifesting a decrease inρ_(p),ε_(1,p),and the volumetric strain increment(Δε_(v))when P_(CNT)increases from 0.08wt%to 0.12wt%.Moreover,the addition of CNTs improved the integrity of CGBS after macroscopic failure,and crack extension in CGBSs appeared in two modes:detour and pass through the ***σ_(p)and U_(d)firstly increase and then decrease with increasing D,and porosity shows the opposite ***ε_(1,p)andΔε_(v)are negatively correlated with D,and CGBS with D=2.150 has the maximum deformation parameters(ε_(1,p)=0.05079,Δε_(v)=0.01990)due to the frictional slip effect caused by coarse *** increasing D,the failure modes of CGBSs are sequentially manifested as oblique shear failure,"Y-shaped"shear failure,and conjugate shear failure.
