Comparison of breast-conserving surgery and mastectomy in early breast cancer using observational data revisited:a propensity score-matched analysis

Recent observational studies showed that breast-conserving surgery(BCS) resulted in superior survival compared to mastectomy in breast cancer patients. This study compared the clinical outcomes of BCS and mastectomy using propensity score(PS)matching analysis, which had advantages over conventional methods in reducing bias. Nonmetastatic breast cancer patients who underwent BCS and mastectomy were matched 1:1 based on their PS. We used the Kaplan-Meier method and Cox-regression model to estimate the treatment effects. A total of 2,866 patients with a median follow-up time of 67 months were included in the original study population. Although the mastectomy cohort(N=1,219) had more advanced disease compared to the BCS cohort(N=1,647), LRFS was similar between the two groups(93.8% vs. 92.4%, P>0.05). BCS(vs. mastectomy) was associated with improved DFS(73.8% vs. 58.7%, P0.05) or CSS(87.5% vs. 84.9%, P>0.05). We found that PS methods reduce bias when estimating treatment effects using observational data. BCS and mastectomy show equivalent outcomes in nonmetastatic breast cancer patients.

本研究旨在鉴定和分析锦州某猪场病死猪脏器中分离出的一株病原菌,以期为该细菌病的临床诊治提供科学依据。通过LB琼脂培养进行革兰氏染色,BP特殊培养基培养、药物敏感性试验、16S rDNA测序及小鼠致病性试验,并制作病理组织切片进行观察。由革兰氏染色镜检结果可知该菌为革兰氏阳性球菌;BP培养皿可见黑色菌落生长及菌落周边透明圈;药敏试验表明该菌对恩诺沙星、氧氟沙星、阿米卡星和卡那霉素非常敏感;经16S rDNA测序,该菌和金黄色葡萄球菌16S rDNA片段相似性高;镜检观察病理切片,可见各组织脏器有明显病理损害。该猪场致病菌经分离鉴定为金黄色葡萄球菌,对部分抗生素产生耐药,小鼠攻毒试验具有一定的致病性。

Central venous port placement in advanced breast cancer patients:comparison of the anatomic- landmark and ultrasound-guided techniques

Objective： The aim of this study was to compare the anatomic-landmark and ultrasound-guided techniques in the placement of an internal jugular vein port in patients with advanced breast cancer. Methods： Between March 2010 and October 2010, 60 patients with advanced breast cancer underwent central venous port placement for the delivery of chemotherapy, preferably through the internal jugular vein. Patients were randomly assigned to either the anatomic-landmark or the ultrasound-guided group. Failure on first attempt, number of attempts until successful catheterization, time to successful placement, the accordance of the two placement approaches, and the demographics of each patient were recorded. Results： The consistency of the direction of two lines drawn using the anatomic-landmark and ultrasound-guided techniques or of the diameter of the internal jugular vein as determined by the two approaches was 85% （51/60）. The rate of successful place- ment at first attempt was higher in the ultrasound group than in the anatomic-landmark group （P 〈 0.05）. A greater number of attempts and longer time to successful port placement were needed in the latter （P 〈 0.05）. Conclusion： The findings of this study indicate that, in the placement of an internal jugular vein port, the ultrasound （US）-guJded technique has several advantages over the anatomic-landmark technique.

Extending Structures for Gel'fand-Dorfman Bialgebras

Gel'fand-Dorfman bialgebra,which is both a Lie algebra and a Novikov algebra with some compatibility condition,appeared in the study of Hamiltonian pairs in completely integrable *** also emerged in the description of a class special Lie conformal algebras called quadratic Lie conformal *** this paper,we investigate the extending structures problem for Gel'fand-Dorfman bialgebras,which is equivalent to some extending structures problem of quadratic Lie conformal ***,given a Gel'fand-Dorfman bialgebra(A,o,[.,.]),this problem is how to describe and classify all Gel'fand-Dorfman bialgebra structures on a vector space E(A⊂E)such that(A,o,[.,.])is a subalgebra of E up to an isomorphism whose restriction on A is the identity *** by the theories of extending structures for Lie algebras and Novikov algebras,we construct an object gH2(V,A)to answer the extending structures problem by introducing the notion of a unified product for Gel'fand-Dorfman bialgebras,where V is a complement of A in *** particular,we investigate the special case when dim(V)=1 in detail.

研究了同步定位与地图创建(SLAM)中的数据关联问题。针对环境特征数未知时,数据关联的误关联率增加,导致SLAM的定位精度偏低的问题,提出了高斯混合概率假设密度SLAM算法。首先采用UFastSLAM解决SLAM中的粒子退化和耗尽问题,其次针对地图特征数未知的情况,将UFastSLAM算法中的数据关联问题转换成有限集统计理论跟踪算法的高斯混合问题,利用高斯混合概率假设密度(Gaussian Mixture Probability Hypothesis Density,GMPHD)算法解决UFastSLAM中数据关联问题。仿真实验结果表明本文提出的GMPHD-UFastSLAM算法在地图特征个数未知的情况下,数据关联准确率和定位精度都得到了提高。

Facile synthesis of transition metal complexes with five coplanar metal-carbon σ-bonds

Carbon is one of the most important elements in chemistry, forming a vast number of organic compounds. It can also form a variety of organometallic compounds with a metal–carbon bond. Metal alkyl, carbene and carbyne complexes are classical examples of organometallic compounds. A particularly interesting class of organometallic compounds are all-carbon metallacycles formed by combination of a metal and a hydrocarbon chain serving as a chelating agent with only carbon atoms as the donor atoms. Many interesting monocyclic metallacycles made of a chelating bidentate hydrocarbon chain are known, for example, metallacyclobutadienes, metallacyclopentadienes and metallabenzenes[1].
