History of major bleeding predicts risk of clinical outcome of patients with atrial fibrillation: results from the COOL-AF registry

Objective To compare clinical outcomes between patients with and without history of major bleeding according to types of antithrombotic medications in patients with non-valvular atrial fibrillation(NVAF). Methods We conducted a multicenter registry of patients with NVAF during 2014 to 2017 in Thailand. The following data were collected: demographic data, type of NVAF, medical illness, components of CHA2DS2-VASc and HAS-BLED scores, history of bleeding and severity, investigations, and antithrombotic medications. Clinical outcomes were death, bleeding, and ischemic stroke/transient ischemic attack(TIA). Results There were a total of 3218 patients. The average age was 67.3 ± 11.3 years, and 58.3% were men. Sixty-nine patients(2.14%) had a history of major bleeding. Antithrombotic use was, as follows: 2126 patients(75.3%) received oral anticoagulant(OAC) alone, 555(17.2%) received antiplatelet alone, 298(9.3%) received both, and 239(7.4%) received neither. During follow-up, 9.9% had major adverse outcomes, including death(5.9%), ischemic stroke/TIA(2.5%), and major bleeding(4.0%). There were no significant differences in the types of antithrombotic medications between patients with and without history of major bleeding. Multivariate analysis revealed old age, low body mass index, hypertension, diabetes, heart failure, and history of major bleeding to be independently associated with major adverse outcome. Adverse events significantly increased in patients with OAC plus antiplatelet. Conclusions History of major bleeding was identified as a factor that significantly affects clinical outcome. Inappropriate use of OAC plus antiplatelet should be avoided. Special caution should be made in this high-risk patients.

＜正＞ Polycyclic aromatic hydrocarbons （PAHs） are bioactivated to reactive metabolites which can bind covalently to DNA and subsequently initiate mutation and carcino-genesis. The purpose of this study was to measure level of urinary 1 - hydroxypyrene, a metabolite of PAHs, in subjects exposed to automobile exhaust fumes compared to non - exposed subjects. A urine sample was collected from individual subject after the end of working day and quantitated for 1 - hydroxypyrene and creatinine by HPLC and spectrophotometric method, respectively. The results showed that average urinary 1 - hydroxypyrene level in exposed subjects was significantly higher than non - exposed subjects, P = 0.000. The ratio of urinary 1 - hydroxypyrene / mol creatinine level, of the exposed subjects was significantly higher than that of the non-exposed subjects, P = 0.002. Thus, automobile exhaust fume exposed subjects have a higher risk to be exposed to PAHs than the non- exposed subjects. Urinary 1 - hydroxypyrene can be used as an index for an exposed of PAHs which are originated from automobile exhaust fume and other sources as well.

《太子须大拏经》跟随佛教传到世界的各个地方,其中也包括中国和泰国。通过两国译者的翻译与编写,使得须大拏本生故事在两国传流。为了了解中泰故事之间的相似点与区别点,了解在不同语言版本下,中泰两国所接受的内容是否相似,以及了解其差异的价值。本文选择了该故事不同的中泰版本进行比较,以及找出中泰故事内容的相似点和区别点,通过区别点了解其差异的价值。另外,本文还将两国关于须大拏太子故事的壁画进行了比较,揭示两国人民对该故事的理解。本文内容分为四个章节:一、《太子须大拏经》中泰故事及内容情节对比;二、中泰故事中布施思想的描述比较分析;三、中泰故事的主要人物以及其行为比较分析;四、中泰须大拏太子故事壁画内容比较分析。通过研究之后发现,不同的中泰版本的本生故事在主要内容基本上相似。这反映在关于释迦牟尼佛陀前生须大拏太子进行修檀波罗蜜的内容情节上,在中泰故事的内容情节相似,区别点不大。中泰故事之间的差异大都来自三个方面的原因,如:一、中泰版本来自不同原文,并且原文内容不一致;二、不同的故事均吸收了当地文化;三、译者或编者的能力技巧差异。中泰故事内容情节之间的差异,让两国读者对该故事的了解不同。由于该故事主要推崇的是布施思想,在中文版本中除了极力宣扬布施思想,还夹杂了中国的传统思想在里面,如:孝顺。在泰文版本中只是单独强调布施为非常高上的功德,没有任何功德任何东西能比得上布施,而且泰文版本的故事还会让读者对人物的背景有着更深的了解。另外,该故事的壁画还让我们更加了解两国人民对太子须大拏故事的看法以及哪些他们所重视的部分,例如:中国人重视布施情节,而泰国人比较重视动情的情节。

本研究工作的目的是制备和表征环氧化天然橡胶-g-壳聚糖(ENR-g-CTS)/聚乙烯醇(PVA)复合材料纳米纤维。用静电纺丝技术制备了ENR-g-CTS/PVA复合材料纳米纤维,并用SEM、TEM和AFM方法观察ENR-g-CTS/PVA复合材料纳米纤维的微观形态。PVA纳米纤维和ENR-g-CTS/PVA复合材料纳米纤维的平均直径分别为约250和144 nm。与纯环氧化天然胶(ENR)相比,ENR-g-CTS在自然环境条件下的分解效果更好。此外,透过由ENR-g-CTS/PVA复合材料制备的载体膜,CSC释放20小时后的最大累积释放量达到18%。

Dengue viral infection(DVI)is one of the world’s most significant viral infections *** of the patients have been asymptomatic,with relatively benign clinical manifestations and ***,a small number of patients have progressed to severe dengue diseases,including hemorrhage,multi-organ impairment,and increased vascular leakage causing hypovolemic shock,which can cause cardiovascular collapse and *** lines of evidence have demonstrated that DVI could also cause cardiac dysfunction,arrhythmias,and severe *** treatment for dengue hemorrhagic fever(DHF)patients remains symptomatic and supportive,with close monitoring of hemodynamic *** contributory role of cardiac dysfunction in DHF patients has potentially critical implications on the *** review will address the current knowledge of cardiac involvement in DHF patients and the management strategy to reduce the fatality outcome.

Transient polycythemia during diabetic ketoacidosis triggered by dengue fever in a middle-aged woman

Either dengue fever or diabetic ketoacidosis can cause the high hemoglobin concentration due to intravascular volume *** both entities were found together as in our case,the hemoglobin concentration could access the strikingly high level until either of them could not contribute *** was a Thai woman with 46 years of age presenting with fever,fatigue and severe sore throat for a few *** physical examination showed she had mild dyspnea and oral thrush,the temperature was 36.4 degree Celsius,and her pulse rate was 102/*** blood tests showed:Hb 18.3 g%,WBC 9,850/mm^(3),atypical lymphocyte 10%,platelet 16,000/mm^(3),MCV 89.2 fl,Dengue NS1 antigen-positive,dengue IgG-positive,IgM-weakly positive,KOH preparation of curd from buccal mucosa-positive for budding yeasts and pseudo-hyphae,blood sugar 442 mg%,triglyceride 578 mg%,Na 122.8 mEq/L,K 6.28 mEq/L,Cl 90.6 mEq/L,CO_(2)9.2 mEq/L,blood ketone 53.6 mmol/L,lactate 4.5 mmol/L,pH 7.257,pCO_(2)27.4 mmHg,HCO_(3)11.9 mmo/L,BUN 22.9 mg%,creatinine 0.64 mg%,AST 375 U/L,ALT 224 U/L,alkaline phosphatase 151 U/*** was diagnosed as having diabetes with diabetic ketoacidosis,dengue fever and *** was immediately treated with intravenous fluid therapy to correct the metabolic acidosis,electrolyte imbalance and dehydration,insulin for hyperglycemia,clotrimazole for oral thrush and platelet *** high hemoglobin level was dramatically lowered until became normal within one day of therapy whereas she also recovered from DKA although the platelet was progressively lowered every *** suggested that transient polycythemia was solely contributed by DKA that was triggered by dengue fever hence DKA from dengue fever should be added in the list of unusual causes of transient polycythemia.

AIM:To examined the efficacy and safety of treatment with boceprevir,PEGylated-interferon and ribavirin(PR)in hepatitis C virus genotype 1(HCVGT1) PR treatmentfailures in ***:The Boceprevir Named-Patient Program provided boceprevir to HCVGT1 PR *** physicians were invited to contribute data from their patients:baseline characteristics,ontreatment responses,sustained virological response at week 12(SVR12),and safety were collected and *** analysis was performed to determine predictors of ***:150 patients were enrolled from Australia,Malaysia,Singapore and Thailand(Asians = 86,Caucasians = 63).Overall SVR12 was 61%(Asians= 59.3%,Caucasians = 63.5%).SVR12 was higher in relapsers(78%) compared with non-responders(34%).On-treatment responses predicted SVR,with undetectable HCVRNA at week 4,8 and 12 leading to SVR12 s of 100%,87%,and 82%respectively,and detectable HCVRNA at week 4,8 and 12,leading to SVR12 s of 58%,22%and 6%*** patients were similar to Caucasian patients with regards to on-treatment *** with cirrhosis(n= 69) also behaved in the same manner with regards to on-treatment *** with the IL28 B CC genotype(80%) had higher SVRs than those with the CT/TT(56%) genotype(P = 0.010).Multivariate analysis showed that TW8 and TW12 responses were independent predictors of *** adverse events occurred in 18.6%:sepsis(2%),decompensation(2.7%) and blood transfusion(14%).Discontinuations occurred in 30.7%,with 18.6%fulfilling stopping ***:Boceprevir can be used successfully in PR treatment failures with a SVR12 > 80%if they have good on-treatment responses;however,discontinuations occurred in 30%because of virological failure or adverse events.
