mortality is a well-established patient-important outcome in critical care *** contrast,morbidity is less uniformly reported(given the myriad of critical care illnesses and complications of each)but may have a common end-impact on a patient’s functional capacity and health-related quality-of-life(HRQoL).Survival with a poor quality-of-life may not be acceptable depending on individual patient values and ***,as mortality decreases within critical care,it becomes increasingly important to measure intensive care unit(ICU)survivor *** measurements with a preference-based scoring algorithm can be converted into health utilities on a scale anchored at 0(representing death)and 1(representing full health).They can be combined with survival to calculate quality-adjusted life-years(QALY),which are one of the most widely used methods of combining morbidity and mortality into a composite *** QALYs have been use for health-technology assessment decision-making,an emerging and novel role would be to inform clinical decision-making for patients,families and healthcare providers about what expected HRQoL may be during and after ICU *** care randomized control trials(RCTs)have not routinely measured or reported HRQoL(until more recently),likely due to incapacity of some patients to participate in patient-reported outcome *** differences in HRQoL measurement tools can lead to non-comparable *** this end,we propose the validation of a gold-standard HRQoL tool in critical care,specifically the *** combined health-utility and mortality(disaggregated)and QALYs(aggregated)can be reported,with disaggregation allowing for determination of which components are the main drivers of the QALY *** use of HRQoL,health-utility,and QALYs in critical care RCTs has the potential to:(1)Increase the likelihood of finding important effects if they exist;(2)improve research efficiency;and(3)help inform optimal m

AIm To assess the association of inter-ethnic vs intra-ethnic marriage with severity of coronary artery disease(CAD) in men undergoing *** We conducted a prospective multicenter,multi-ethnic,cross sectional observational study at five hospitals in Saudi Arabia and the United Arab Emirates,in which we used logistic regression analysis with and without adjustment for baseline differences. RESULTS Data were collected for 1068 enrolled patients undergoing coronary angiography for clinical indications during the period of April 1^(st),2013 to march 30^(th),2014. Ethnicities of spouses were available only for male patients. Of those enrolled,687 were married men and constituted the cohort for the present analysis. Intra-ethnic marriages were reported in 70% and inter-ethnic marriages in 30%. After adjusting for baseline differences,interethnic marriage was associated with lower odds of having significant CAD [adjusted odds ratio 0.52(95%CI:0.33,0.81)] or multi-vessel disease(mVD) [adjusted odds ratio 0.57(95%CI:0.37,0.86)]. The adjusted association with left main disease showed a similar trend,but was not statistically significant [adjusted odds ratio 0.74(95%CI:0.41,1.32)]. The association between interethnic marriage and the presence of significant CAD and mVD was not modified by number of concurrent wives(P interaction > 0.05 for both).CONCLUSION Among married men undergoing coronary angiography,inter-ethnic,as compared to intra-ethnic,marriage is associated with lower odds of significant CAD and mVD.

Silicon micromirrors with three-dimensional curvature enabling lensless efficient coupling of free-space light

miniaturized optical benches process free-space light propagating in-plane with respect to the substrate and have a large variety of applications,including the coupling of light through an optical *** coupling efficiency is usually obtained using assembled micro-optical parts,which considerably increase the system cost and integration *** this work,we report a high coupling efficiency,monolithically integrated silicon micromirror with controlled three-dimensional(3D)curvature that is capable of manipulating optical beams propagating in the plane of the silicon *** on our theoretical modeling,a spherical micromirror with a microscale radius of curvature as small as twice the Gaussian beam Rayleigh range provides a 100%coupling efficiency over a relatively long optical path *** dimensionless parameters facilitates the elucidation of the role of key design parameters,including the mirror’s radii of curvature,independent of the wavelength.A micromachining method is presented for fabricating the 3D micromirror using fluorinated gas *** measured coupling efficiency was greater than 50%over a 200-mm optical path,compared to less than 10%afforded by a conventional flat micromirror,which was in good agreement with the *** the 3D micromirror,an optical cavity was formed with a round-trip diffraction loss of less than 0.4%,resulting in one order of magnitude enhancement in the measured quality factor.A nearly 100%coupling was also estimated when matching the sagittal and tangential radii of curvature of the presented micromirror’s *** reported class of 3D micromirrors may be an advantageous replacement for the optical lenses usually assembled in silicon photonics and optical benches by transforming them into real 3D monolithic systems while achieving wideband high coupling efficiency over submillimeter distances.

The mediterranean Sea is home to an interesting biodiversity. The current cumulative and descriptive study aims to enumerate some relatively large marine mammals, reptiles, and fishes that have been spotted, caught, by-caught, or stranded on the coast of the Gaza Strip, Palestine, which extends about 42 km along the mediterranean Sea. This 20-year study from 2003 to 2022 relied much on frequent field visits, meetings, and discussions with stakeholders, following local media sites and social media pages, and photography. The study recorded at least 26 giants or relatively large marine organisms on the mediterranean coast of the Gaza Strip, Palestine. Certainly, the coming years may bring other marine organisms of relatively large sizes. marine mammals included three species of cetaceans with the Fin Whale (Balaenoptera physalus) being the largest mammal and even the largest animal ever recorded in this study. marine reptiles included three species of sea turtles, the largest of which is the Leatherback Sea Turtle (Dermochelys coriacea), which is in fact the world’s largest sea turtle. The bony fishes were represented by ten species, the largest of which was the Ocean Sunfish (mola mola), which is the largest bony fish in the world. The cartilaginous fishes included ten species with the largest specimens encountered were the Shortfin Shark (Isurus oxyrinchus), Bluntnose Sixgill Shark (Hexanchus griseus), Scalloped Hammerhead Shark (Sphyrna lewini), and Giant Devil Ray (mobula mobular). Seabirds were not included in the study. The Alexandria Pompano (Alectis alexandrinus) and the Silver-Cheeked Toadfish (Lagocephalus sceleratus), which are bony fishes, appear to be the relatively smallest marine organisms recorded here. In conclusion, the role of the various parties must be coordinated to ensure the sustainability of human activities and their compatibility with the task of conserving local marine biota, including the gigantic or relatively large ones.
