BACKGROUND Vancomycin flushing syndrome(VFS),also known as red man syndrome,is an allergic reaction to *** typically presents as a rash on the face,neck,and upper torso after intravenous administration of *** is blamed on rapid intravenous infusion of vancomycin during management and rarely happens after local use.A review of the literature showed that in the last 23 years,4 such cases have been ***,we add another case of VFS developed after slow local absorption of vancomycin in cement *** SUmmARY A 44-year-old male with a known case of hypertension,no history of allergies to medications,and a history of chronic osteomyelitis of the right tibia with discharging sinus over the anterolateral aspect of the *** pus culture grew Staphylococcus aureus,which was sensitive to clindamycin and *** patient underwent irrigation and debridement with the placement of vancomycin cement beads made from 4 g of vancomycin powder and 40 g of polymethyl *** hours postoperatively,the patient developed a pruritic,erythematous,macular rash predominantly on his face,neck,chest,and lower extremities and to a lesser extent his upper extremities.A diagnosis of VFS was made and was successfully treated with cetirizine(10 mg,oral)and methylprednisolone sodium succinate(125 mg,intravenous).The patient continued to have itching with a facial rash for 12 h with gradual improvement.A decision was made to not remove the beads as the patient continued to ***,the rash disappeared after 96 h with no further *** VFS can occur not only after rapid intravenous injection of vancomycin but also with local release,as in our *** orthopaedic surgeons routinely use vancomycin with polymethyl methacrylate in chronic osteomyelitis and revision arthroplasty,they should be aware of such a complication occurring.

Radium isotopes can be analyzed by different analytical methods based on gamma spectrometric measurements or alpha spectrometry. An improved method was developed to determine radium isotopes from water using gamma spectrometry after radiochemical separation. The Radium was selectively extracted from acidified samples using co-precipitation procedure with iron hydroxide and followed by precipitation of radium as radium sulphate Ba(Ra)SO4. The precipitate Ba(Ra)SO4 was filtered through the millipore filter paper, dried and weighed to calculate chemical yield. 226Ra and 228Ra activities were measured using low-background gamma spectrometry in water samples. Radium was pre-concentrated from environmental samples by co-precipitation with BaSO4. The amounts of 226Ra and 228Ra on the sample were obtained by gamma-ray spectrometry for the 351 keVγ-ray from 214Pb and for the 911 keV γ-ray from 228Ac, both in radioactive equilibrium with precursors, respectively. The accuracy, selectivity, traceability, applicability and minimum Detectable Activity (mDA) of the technique were discussed. Also, the effect of physical and chemical characteristics of the water samples such as TDS, pH, soluble species, sulphate and bicarbonate that effect on the radium determination were taking into consideration. The method has been validated with a certified reference material supplied by the International Atomic Energy Agency and reliable results were obtained. The radiochemical yields for radium were 70% - 90% and recovery was 97% and 80% for 226Ra and 228Ra, respectively.

AIm： To assess the objective and subjective results of a two-stage procedure for management of keratoconus： clear lensectomy with aspherical intraocular lens（IOL） implantation followed by wave front-guided photorefractive keratotomy（WFG-PRK）.mETHODS： This prospective interventional non-randomized study included patients aged 35 years old or more with grade I and II stable keratoconus, a clear visual axis, minimal corneal thickness（mCT） 420 μm or more and average keratometric reading（K） less than 54 diopter（D）. Refraction of all selected eyes should be-8.00 D sphere or more with less than-6.00 D cylinder and could be corrected two lines or more with spectacles or contact lenses. All studied eyes underwent a two-stage approach treatment： first refractive lens exchange and aspherical IOL implantation followed, after at least 3 mo, by WFGPRK. Pre and postoperative complete ophthalmological examination were performed. Topographical, visual and aberrometric results were recorded and evaluated during 6 mo follow up period. moreover, patient satisfaction and other subjective outcomes were also ***： The 13 eyes of 11 patients diagnosed with stable keratoconus and aged from 39 to 49 y（42.4±6.2 y） were enrolled in the study. At baseline, 8 eyes had grade I and 5 eyes had grade II keratoconus. The manifest sphere was-10.3±4.2 D（ranged from-8.0 to-14.0 D） and the manifest cylinder was-4.2±1.2 D（ranged from-1.75 to-5.50 D）. After the two-stage procedure, sphere and cylinder reduced significantly to-0.43±0.22 D and-1.3±0.72 D respectively（P〈0.001）. There was also a highly significant improvement in the mean uncorrected distance visual acuity（UDVA） from logmAR 1.41±0.49 preoperatively to 0.51±0.16 postoperatively（P〈0.001） and the mean corrected distance visual acuity（CDVA） from 0.76±0.24 preoperatively to 0.49±0.13 after the operation（P〈0.001）. AIm: To evaluate the effect of scleral encircling bands on the development and progression of diabetic retinopathy(DR) in diabetic ***: The medical records of diabetic patients who underwent unilateral retinal detachment(RD) surgery using scleral buckle and encircling band were reviewed retrospectively. Both eyes of patients were included in the study: one eye in each patient had a scleral buckle with encircling band(the operated eye) and the other one is the non-operated eye. The demographic characters, duration of diabetes and period between surgery and the last recall visit were retrieved from each patient. All the cases underwent fundus photo and fluorescein angiography(when indicated) to confirm the DR ***: Totally 25 patients fulfilled the inclusion and the exclusion criteria were become eligible for the study. A total of 50 eyes of 25 patients were enrolled in this analysis. The mean period of time passed from surgery with encircling band and the last reassessment visit was 12.5±2 y. Even though DR could develop in the operated eyes, it was at a less degree of severity compared to the non-operated eyes of same patients(P=0.027).CONCLUSION: Scleral encircling bands have protective effects against the development and progression of DR.

Use of Basal Serum Testosterone Level as Predictor for Poor Ovarian Response in Women with Unexplained Infertility Undergoing In VitroFertilization Cycle: Prospective Study

Background: Delayed pregnancy in women and marked increase in the numbers of older women who fail to respond to ovarian stimulation had been a significant issue. This study aims to assess the value of basal serum testosterone level as a predictor of ovarian response for induction of ovulation in women with unexplained infertility undergoing IVF (in vitro fertilization) cycle. Patients and methods: A prospective study was conducted in Ain Shams University maternity hospital Infertility Center during a period of time from October 2016 to June 2017. This study recruited 89 women. On day 2 or 3 of a spontaneous menstrual cycle of the included women within 3 months before fresh IVF cycle, basal hormonal (FSH, LH, estradiol, total testosterone) concentrations, AFC (antral follicle count) were performed. Using the Long-protocol for induction of ovulation, serial monitoring of ovarian response was assessed by transvaginal ultrasound. When the expected ovarian response was reached (at least three oocytes ≥ 17 mm), we gave trigger dose of HCG. Ultrasound guided oocyte aspiration was performed 34 - 36 hours later. Two to three days after oocyte aspiration, we transferred the embryos according to the patient’s age and the condition of embryos available. Biochemical pregnancy was considered if serum B-hCG test was positive at day 14 from embryo transfer, where all the data were correlated with serum testosterone level and ovarian response as 1 ry outcome. Results: There were significant positive correlations between testosterone and LH, Prolactin, AFC, Number of oocytes & Number of Embryos (0.014, 0.032, 0.023, 0.004, 0.033, p 0.001 respectively). Poor responders versus good responders as regards testosterone level (0.81 ± 0.47 versus 1.08 ± 0.45) Fertilized & pregnant cases had significantly higher testosterone than non-fertilized & non pregnant had (1.20 ± 0.45, 0.92 ± 0.47 p value 0.035, 0.021 respectively). AIm: To evaluate the effect of prophylactic administration of nepafenac in prevention of macular edema occurring in diabetic patients after phacoemulsification and to investigate the correlation between optical coherence tomography(OCT) foveal thickness and multifocal electroretinogram(mF-ERG) ***: The study included two groups. Group 1 included 50 diabetic patients with senile cataract(50 eyes, 30 females, 20 males, aged 55±7 y) received nepafenac 0.1% eye drop. Group 2 included another 50 diabetic patients with senile cataract(50 eyes, 22 female, 28 males, aged 53.8±8 y) did not receive nepafenac. All patients were followed up for 3 mo postoperatively. OCT and mF-ERG were done preoperative and at 1 wk, 1, 2 and 3 ***: The mean foveal thickness was statistically significantly lower in Group 1. Five eyes in Group 2 developed clinical cystoid macular oedema(CmO)(10%), and no patients in Group 1 developed central macular thickening more than 50 μm. There were insignificant differences in mF-ERG amplitudes and latencies between the two groups except in the five eyes that developed CmO, there statistically significant reduction of mF-ERG amplitude with increase in foveal thickness. CONCLUSION: Perioperative nepafenac reduces the incidence of CmO following uncomplicated phacoemulsification significantly. Nepafenac has no side effects.

Objective:To determine the prevalent pathogens responsible for spontaneous bacterial peritonitis(SBP) and their sensitivity pattern,to test the efficiency of different culture techniques in microbial isolation,and to study the diagnostic predictors of such ***:One hundred eight SBP episodes from 92 adult patients were compared to 88 cirrhotic ascites patients cross - matched with age and sex without *** fluid was subjected to cytological,biochemical examination and culture on both conventional and blood culture bottles at the bedside for bacterial identification and antimicrobial susceptibility ***:The prevalence of SBP was 25.02%.Logistic regression analysis revealed that;previous SBP episode,low ascitic fluid protein levels, high serum creatinine and low serum albumin levels were the independent significant predictors of *** forty - five per cent of SBP episodes were detected by conventional culture compared to 73.15%by modified technique with a significant *** - negative bacteria were the cause of SBP in 46(58.23%) culture positive *** coli and Staphylococcus aureus were the most commonly detected organisms. 目的熟悉直肠骶骨筋膜形态及走行,对于在全直肠系膜切除术(TmE)中,保证直肠系膜完整性及保护自主神经盆丛至关重要,但目前尚缺乏对直肠骶骨筋膜的全面完整描述。本文通过高清腹腔镜或机器人TmE中的临床观察和尸体标本解剖,对直肠骶骨筋膜形态及走行进行观察总结,并讨论该区域的最佳游离路径。方法采用描述性病例系列研究方法,回顾性分析2018年1—12月期间就诊于福建医科大学附属协和医院结直肠外科的127例直肠癌患者的临床病理资料和手术录像,及同期科室数码数据库的20例TmE术后直肠标本的高清照片,同时纳入来源于福建医科大学解剖学教研室的28例人体尸体标本,观察直肠骶骨筋膜形态和移行情况。结果(1)总结手术录像提示,127例患者均可观察到直肠骶骨筋膜从后方呈水平弧形附着于直肠固有筋膜,形成融合筋膜。融合区域无法直接分离,如不离断,则容易破坏直肠固有筋膜。离断后,沿着直肠后方直肠骶骨筋膜附着缘向右侧观察,见该水平以下右侧间隙下半部仍为融合筋膜,该融合筋膜在侧方上半部重新分开为直肠固有筋膜与腹下神经前筋膜,该分开处的腹下神经前筋膜为侧方间隙分离时的刚性障碍。沿着其分开处逐步切断该筋膜,可见由右骶(S)2~S4发出的盆丛被灰白色腹下神经前筋膜覆盖,观察到该筋膜与Denonvilliers筋膜前间隙已被切断的Denonvilliers筋膜前叶相延续;而内侧的直肠固有筋膜仍完整。右侧直肠骶骨筋膜附着缘从后上向前下斜行走行。左侧直肠骶骨筋膜形态与右侧对称。(2)观察28例半骨盆尸体标本发现,于S4椎体下缘水平,腹下神经前筋膜向前与直肠固有筋膜融合成直肠骶骨筋膜。向直肠右侧间隙进行分离,逐步切断直肠骶骨筋膜的右侧附着缘,见直肠骶骨筋膜附着缘从后上走行至前下,呈斜行走向。附着缘向头侧移行为腹下神经前筋膜,向前移行为Denonvilliers筋膜前叶。盆丛在前侧方发出多支细小直肠支,呈束状穿过腹下神经前筋膜和Denonvilliers筋膜前叶的相互移行区,支配直肠。(3)对20例TmE标本进行观察,见后方直肠骶骨筋膜附着缘围绕着直肠系膜呈弧形,两侧附着缘呈斜行。后方和两侧附着缘后下方的直肠系膜被直肠骶骨筋膜包绕,附着缘水平的前上方直肠系膜被直肠固有筋膜包绕。结论结合直肠骶骨筋膜形态学特点,术中应于S4椎体水平在直肠后方弧形切断直肠骶骨筋膜,从直肠后间隙进入肛提肌上间隙;进行两侧间隙分离前应先行直肠前方间隙的分离,倒"U"型切断Denonvilliers筋膜前叶,沿着Denonvilliers筋膜后间隙从上向下分离侧前方间隙,最后切断直肠骶骨筋膜的两侧附着缘,方可保证直肠侧方筋膜的完整,且并不损伤盆丛分支与神经血管束。

Recent economic growth and development have considerably raised energy consumption over the *** load prediction approaches become essential for effective planning,decision-making,and contract evaluation of the power *** order to achieve effective forecasting outcomes with minimumcomputation time,this study develops an improved whale optimization with deep learning enabled load prediction(IWO-DLELP)scheme for energy storage systems(ESS)in smart grid *** major intention of the IWO-DLELP technique is to effectually forecast the electric load in SG environment for designing proficient *** proposed IWO-DLELP model initially undergoes pre-processing in two stages namely min-max normalization and feature ***,partition clustering approach is applied for the decomposition of data into distinct clusters with respect to distance and objective ***,IWO with bidirectional gated recurrent unit(BiGRU)model is applied for the prediction of load and the hyperparameters are tuned by the use of IWO *** experiment analysis reported the enhanced results of the IWO-DLELP model over the recent methods interms of distinct evaluation measures.
