m.rtality is a well-established patient-im.ortant outcom. in critical care *** contrast,m.rbidity is less uniform.y reported(given the m.riad of critical care illnesses and com.lications of each)but m.y have a com.on end-im.act on a patient’s functional capacity and health-related quality-of-life(HRQoL).Survival with a poor quality-of-life m.y not be acceptable depending on individual patient values and ***,as m.rtality decreases within critical care,it becom.s increasingly im.ortant to m.asure intensive care unit(ICU)survivor *** m.asurem.nts with a preference-based scoring algorithm.can be converted into health utilities on a scale anchored at 0(representing death)and 1(representing full health).They can be com.ined with survival to calculate quality-adjusted life-years(QALY),which are one of the m.st widely used m.thods of com.ining m.rbidity and m.rtality into a com.osite *** QALYs have been use for health-technology assessm.nt decision-m.king,an em.rging and novel role would be to inform.clinical decision-m.king for patients,fam.lies and healthcare providers about what expected HRQoL m.y be during and after ICU *** care random.zed control trials(RCTs)have not routinely m.asured or reported HRQoL(until m.re recently),likely due to incapacity of som. patients to participate in patient-reported outcom. *** differences in HRQoL m.asurem.nt tools can lead to non-com.arable *** this end,we propose the validation of a gold-standard HRQoL tool in critical care,specifically the *** com.ined health-utility and m.rtality(disaggregated)and QALYs(aggregated)can be reported,with disaggregation allowing for determ.nation of which com.onents are the m.in drivers of the QALY *** use of HRQoL,health-utility,and QALYs in critical care RCTs has the potential to:(1)Increase the likelihood of finding im.ortant effects if they exist;(2)im.rove research efficiency;and(3)help inform.optim.l m

AIm.To assess the association of inter-ethnic vs intra-ethnic m.rriage with severity of coronary artery disease(CAD) in m.n undergoing *** We conducted a prospective m.lticenter,m.lti-ethnic,cross sectional observational study at five hospitals in Saudi Arabia and the United Arab Em.rates,in which we used logistic regression analysis with and without adjustm.nt for baseline differences. RESULTS Data were collected for 1068 enrolled patients undergoing coronary angiography for clinical indications during the period of April 1^(st),2013 to m.rch 30^(th),2014. Ethnicities of spouses were available only for m.le patients. Of those enrolled,687 were m.rried m.n and constituted the cohort for the present analysis. Intra-ethnic m.rriages were reported in 70% and inter-ethnic m.rriages in 30%. After adjusting for baseline differences,interethnic m.rriage was associated with lower odds of having significant CAD [adjusted odds ratio 0.52(95%CI:0.33,0.81)] or m.lti-vessel disease(m.D) [adjusted odds ratio 0.57(95%CI:0.37,0.86)]. The adjusted association with left m.in disease showed a sim.lar trend,but was not statistically significant [adjusted odds ratio 0.74(95%CI:0.41,1.32)]. The association between interethnic m.rriage and the presence of significant CAD and m.D was not m.dified by num.er of concurrent wives(P interaction > 0.05 for both).CONCLUSION Am.ng m.rried m.n undergoing coronary angiography,inter-ethnic,as com.ared to intra-ethnic,m.rriage is associated with lower odds of significant CAD and m.D.

m.niaturized optical benches process free-space light propagating in-plane with respect to the substrate and have a large variety of applications,including the coupling of light through an optical *** coupling efficiency is usually obtained using assem.led m.cro-optical parts,which considerably increase the system.cost and integration *** this work,we report a high coupling efficiency,m.nolithically integrated silicon m.crom.rror with controlled three-dim.nsional(3D)curvature that is capable of m.nipulating optical beam. propagating in the plane of the silicon *** on our theoretical m.deling,a spherical m.crom.rror with a m.croscale radius of curvature as sm.ll as twice the Gaussian beam.Rayleigh range provides a 100%coupling efficiency over a relatively long optical path *** dim.nsionless param.ters facilitates the elucidation of the role of key design param.ters,including the m.rror’s radii of curvature,independent of the wavelength.A m.crom.chining m.thod is presented for fabricating the 3D m.crom.rror using fluorinated gas *** m.asured coupling efficiency was greater than 50%over a 200-m. optical path,com.ared to less than 10%afforded by a conventional flat m.crom.rror,which was in good agreem.nt with the *** the 3D m.crom.rror,an optical cavity was form.d with a round-trip diffraction loss of less than 0.4%,resulting in one order of m.gnitude enhancem.nt in the m.asured quality factor.A nearly 100%coupling was also estim.ted when m.tching the sagittal and tangential radii of curvature of the presented m.crom.rror’s *** reported class of 3D m.crom.rrors m.y be an advantageous replacem.nt for the optical lenses usually assem.led in silicon photonics and optical benches by transform.ng them.into real 3D m.nolithic system. while achieving wideband high coupling efficiency over subm.llim.ter distances.
