Vancomycin flushing syndrome in orthopaedic practice:A case report

BACkGROUND Vancomycin flushing syndrome(VFS),also known as red man syndrome,is an allergic reaction to *** typically presents as a rash on the face,neck,and upper torso after intravenous administration of *** is blamed on rapid intravenous infusion of vancomycin during management and rarely happens after local use.A review of the literature showed that in the last 23 years,4 such cases have been ***,we add another case of VFS developed after slow local absorption of vancomycin in cement *** SUMMARY A 44-year-old male with a known case of hypertension,no history of allergies to medications,and a history of chronic osteomyelitis of the right tibia with discharging sinus over the anterolateral aspect of the *** pus culture grew Staphylococcus aureus,which was sensitive to clindamycin and *** patient underwent irrigation and debridement with the placement of vancomycin cement beads made from 4 g of vancomycin powder and 40 g of polymethyl *** hours postoperatively,the patient developed a pruritic,erythematous,macular rash predominantly on his face,neck,chest,and lower extremities and to a lesser extent his upper extremities.A diagnosis of VFS was made and was successfully treated with cetirizine(10 mg,oral)and methylprednisolone sodium succinate(125 mg,intravenous).The patient continued to have itching with a facial rash for 12 h with gradual improvement.A decision was made to not remove the beads as the patient continued to ***,the rash disappeared after 96 h with no further *** VFS can occur not only after rapid intravenous injection of vancomycin but also with local release,as in our *** orthopaedic surgeons routinely use vancomycin with polymethyl methacrylate in chronic osteomyelitis and revision arthroplasty,they should be aware of such a complication occurring.

Background:To meet the future challenges of infectious diseases and limit the spread of multidrug resistant microorganisms,a better understanding of published studies in the field of infectious diseases is *** objective of this study was to analyze the quantity and quality of research activity in the field of infectious diseases in Arab countries and compare it with that in non-Arab ***:Documents published in Arab countries within the research category of“infectious diseases”were extracted and analyzed using the Web of Science *** data analyzed represent research productivity during the time interval between 1900–***:Worldwide,the total number of documents published in the field of infectious diseases up to 2012 was 227,188.A total of 2,408 documents in the field of infectious diseases were published in Arab countries,which represents 1.06%of worldwide research *** output from Arab countries in the field of infectious diseases was low for ***,approximately a five-fold increase was observed in the past *** countries ranked 56th to 218th on the standard competition ranking(SCR)in worldwide publications in the field of infectious ***,with a total publication of 464(19.27%)documents ranked first among Arab countries,while kuwait University was the most productive institution with a total of 158(6.56%)*** citation per document published in Arab countries was 13.25 and the h-index was ***(230;9.55%),malaria(223;9.26%),and hepatitis(189;7.8%)were the top three infectious diseases studied as according to the retrieved ***:The present data reveals that some Arab countries contribute significantly to the field of infectious ***,Arab countries need to work harder to bridge the gap in this *** with non-Arab countries in the Middle East,research output from Arab countries was high,but more efforts are needed to enhance the quality o

Isolation of aerobic bacteria from ticks infested sheep in Iraq

Objective:To highlight the presence of aerobic bacteria in live ticks infested sheep,in Diyala Governorate,***:One hundred and thirty adult alive ticks were picked up from sheep which were reared in different farms in Diyala Governorate,Iraq,during the period from November 2012 to May *** were classified in the Natural History Museum in *** were dissected aseptically for extraction of the salivary gland and *** removed tissue from each organ was inoculated in buffer peptone water(1%)and incubated for 2 h at 37 °C,to maintain weak and/or injured bacterial cells,then transmitted to nutrient broth incubated at 37 °C for 18 *** was done on three solid bacteriological media(nutrient,blood and McConkey agars),and then incubated at37 ℃ for 24 *** identification was performed by using multiple biochemical tests and API-20 *** were analyzed by using Statistical Analysis System version 9.1,***-square test was used for comparison at significant level of P ≤ ***:Two species of ticks were identified[Rhipicephalus(Boophilus) annulatus and Hyalomma turanicum].High bacterial isolation rate was observed(483 isolates).A significant high isolation rate was recorded from Rhipicephalus annulatus(63.14%).Six bacterial species were identified[Escherichia coli(28.36%),Pseudomonas aeruginosa(18.01%),Bacillus cereus(14.69%),Staphylococcus aureus(13.66%),Citrobacter freundii(13.04%),and Enterobacter species(12.21%)].also the high bacterial isolation rates were recorded in the temperate months(November,March and April);these coincided with high reproductive performance of ***:The high isolation rate of aerobic pathogens from ticks might reflect the active contribution of this arthropod in environmental contamination and increase the probability of transmitting bacterial pathogens to their hosts.

Determination of Voriconazole in Human Plasma by Liquid Chromatography Tandem Mass Spectrometry: Application in Therapeutic Drug Monitoring

A sensitive, accurate and robust Liquid Chromatography Tandem Mass Spectrometry method has been developed and validated to measure voriconazole trough levels in human plasma. The plasma samples were mixed with fluconazole as an Internal Standard and directed to protein precipitation and drug extraction. An aliquot of 1 μl was injected into the chromatographic system and separated by the Acquity BEH C18 column at a flow rate of 0.30 ml/min in a gradient mobile phase consisting of acetonitrile, Ultrapure water (UPW), methanol and formic acid. Voriconazole was detected by a Triple Quadrupole Detector (TQD) operating on Multiple Reaction Monitoring (MRM) and a positive ion mode Electrospray ionization (ESI) Q1 mass: 350.1 m/z, Q3 mass: 281.1 m/z. Method linearity of the calibration curve (0.10 - 8.00 μg/ml) indicated a correlation coefficient r ≥ 0.99. The intra and inter-assay accuracy was within 85% - 115% and the intra and inter-assay precision was ≤5.76%. Voriconazole recovery percentage was between 97.69 - 119.62%. The method was successively applied in routine voriconazole TDM.

Preparation and characterization of (CeO2)x-(Fe2O3)1-x nanocomposites:reduction kinetics and hydrogen storage

A series of nanosized CeO2-Fe2O3 mixed-oxide nanocomposites with different Ce4+/Fe3+molar ratios were successfully prepared by a co-precipitation *** surface area increased with Fe2O3 content increasing up to 60 wt%in the ***,with further increase in Fe2O3 content,the surface area began to *** reduction processes of the CeO2-Fe2O3 nanocomposites were studied in a hydrogen atmosphere at 300-600℃.The reduction rates increased by increasing both the temperature and Fe2O3 content in the *** microstructure of the reduced composites at 500℃illustrated the presence of a considerable number of macro-and *** activation energy values were calculated which were in the range of 3.56-5.37 kJ mol-1 at the initial stages(up to 35%reduction)and 5.21-10.2 kJ·mol-1 at the final stages(up to 80%reduction)of *** rate-controlling mechanisms in both the initial and final reduction stages were determined,and the initial reaction stage was controlled by combined gaseous diffusion and interfacial chemical reaction mechanisms for all the composites except for pure CeO2,which was controlled by a chemical reaction *** final reaction stage was controlled by a gaseous diffusion mechanism for some composites,while for the others it was controlled by combined gaseous diffusion and interfacial chemical reaction *** hydrogen sorption properties of the nanocomposites were studied by pressure composition isotherms using a volumetric *** storage in the nanocomposites increased by increasing the temperature because of the formation of oxygen vacancies which enhance atomic H adsorption and function as strong adsorption sites forming more metal hydride covalent bonds.

Learning Patterns from COVID-19 Instances

Coronavirus disease,which resulted from the SARS-CoV-2 virus,has spread worldwide since early 2020 and has been declared a pandemic by the World Health Organization(WHO).Coronavirus disease is also termed *** affects the human respiratory system and thus can be traced and tracked from the Chest X-Ray ***,Chest X-Ray alone may play a vital role in identifying COVID-19 *** this paper,we propose a Machine Learning(ML)approach that utilizes the X-Ray images to classify the healthy and affected patients based on the patterns found in these *** article also explores traditional,and Deep Learning(DL)approaches for COVID-19 patterns from Chest X-Ray images to predict,analyze,and further understand this *** experimental evaluation of the proposed approach achieves 97.5% detection performance using the DL model for COVID-19 versus normal *** contrast,for COVID-19 versus Pneumonia Virus scenario,we achieve 94.5% accurate *** extensive evaluation in the experimental section guides and helps in the selection of an appropriate model for similar ***,the approach can be used for medical usages and is particularly pertinent in detecting COVID-19 positive patients using X-Ray images alone.

Study the Effect of Polymeric Quenching on Wear Resistance of Eutectic Modified aluminium – Silicon alloy

Polymer quenchants are being used increasingly in the heat treating industry. One reason for this increase is the continual engineering advancements that facilitate their replacement for quench oils and water. The physical properties of a polymer quench bath directly affect the cooling rate of a quenched part. These properties include the type of quenchant, concentration, and agitation. These parameters must be controlled to optimize the quenching process in terms of alloy microstructure, properties, and performance. Such data is scarce for cast aluminum alloys in the literatures and a quantitative measurement of the effects from individual process parameter is not available. In this study a master alloy of modified eutectic (al-12%Si) was prepared and used. The specimens were quenched and heat treated in distilled water, food oil, machine oil and (0.2-1.4) wt% concentrations of polyvinylalcohol (PVA). The specimens are characterized for x-ray diffraction, microstructure, hardness, and wear rate. The results had shown that the use of (0.6) wt% PVA revealed a maximum hardness with a slight decrease in the wear rate, and a maximum decrease in the wear rate was achieved when (1.2) wt % PVA was used compared to the reference cast alloy, also results had revealed that the use of polymer quenchants affect positively well in the decrease in the wear rate of the eutectic al-Si alloy over the traditional quenching media.

Energy Consumption in the Municipal Water Supply Sector in the kingdom of Bahrain

Water-energy nexus is an emerging issue that receives considerable attention in the world in general and in the Gulf Cooperation Council (GCC) countries in particular. The GCC countries depend mainly on energy generated from fossil fuels to produce drinking water. Yet, the amount of water-related energy use in Bahrain remains unexplored. This study aims to quantify the amount of energy used in the water supply cycle for the first time in Bahrain using quantitative methods. A bottom-up approach for data collection was adopted where data for the three main stages of the water supply in Bahrain: water production, water transmission, and water distribution were collected. Results show that the water production stage consumes about 97% of the total energy consumption in the water supply sector, followed by water transmission (2.9%) and water distribution (0.1%). Comparisons conducted with best practices in the world show that water desalination plants in Bahrain consume relatively high amounts of energy to produce water based on the desalination technology used. This study calls for focusing on the production stage in achieving energy efficiency since it is the largest consumer and where losses are occurring based on the benchmarking. This study also recommends investigating the share of electricity and thermal energy consumed in the water supply cycle in Bahrain in addition to the wastewater treatment sector. This is imperative to provide a holistic overview of the water-related energy use in Bahrain.
