This study explores the impact of.hyperparameter optimization on machine learning models f.r predicting cardiovascular disease using data f.om an IoST(Internet of.Sensing Things)*** distinct machine learning approaches were implemented and systematically evaluated bef.re and af.er hyperparameter *** improvements were observed across various models,with SVM and Neural Networks consistently showing enhanced perf.rmance metrics such as f.-Score,recall,and *** study underscores the critical role of.tailored hyperparameter tuning in optimizing these models,revealing diverse outcomes among *** Trees and Random f.rests exhibited stable perf.rmance throughout the *** enhancing accuracy,hyperparameter optimization also led to increased execution *** representations and comprehensive results support the f.ndings,conf.rming the hypothesis that optimizing parameters can ef.ectively enhance predictive capabilities in cardiovascular *** research contributes to advancing the understanding and application of.machine learning in healthcare,particularly in improving predictive accuracy f.r cardiovascular disease management and intervention strategies.

BACKGROUND： Although the mortality and morbidity of.pancreaticoduodenectomy（PD） have improved signif.cantly over the past years, the concerns f.r elderly patients undergoing PD are still present. f.rthermore, the f.equency of.PD is increasing because of.the increasing proportion of.elderly patients and the increasing incidence of.periampullary tumors. This study aimed to analyze the outcomes of.PD in elderly ***： We studied all patients who had undergone PD in our center between January 1995 and f.bruary 2015. The patients were divided into three groups based on age： group I（patients aged 〈60 years）, group II（those aged 60 to 69 years） and group III（those aged ≥70 years）. The primary outcome was the rate of.total postoperative complications. Secondary endpoint included total operative time, hospital mortality, length of.postoperative hospital stay, delayed gastric emptying, re-exploration, and survival ***： A total of.828 patients who had undergone PD f.r resection of.periampullary tumor were included in this study. There were 579（69.9%） patients in group I, 201（24.3%） in group II, and 48（5.8%） in group III. The overall incidence of.complications was higher in elderly patients（25.9% in group I, 36.8% in group II, and 37.5% in group III; P=0.006）. There were more patients complicated with delayed gastric emptying in group II compared with the other two groups. There was no signif.cant dif.erence in the incidence of.postoperative pancreatic f.stula, biliary leakage, pancreatitis, pulmonary complications and hospital ***： PD can be perf.rmed saf.ly in selected elderly patients. Advanced age alone should not be a contraindication f.r PD. The outcome of.elderly patients who have undergone PD is similar to that of.younger patients, and the increased rate of.complications is due to the presence of.associated comorbidities.

Background: f.w studies investigated biliary leakage af.er pancreaticoduodenectomy(PD) especially when compared to postoperative pancreatic f.stula(POPf.. This study was to determine the incidence of.biliary leakage af.er PD, predisposing f.ctors of.biliary leakage, and its management. Methods: We retrospectively studied all patients who underwent PD f.om January 2008 to December 2017 at Gastrointestinal Surgery Center, Mansoura University, Egypt. According to occurrence of.postoperative biliary leakage, patients were divided into two groups. Group(1) included patients who developed biliary leakage and group(2) included patients without identif.ed biliary leakage. The preoperative data, operative details, and postoperative morbidity and mortality were analyzed. Results: The study included 555 patients. f.rty-f.ur patients(7.9%) developed biliary leakage. Ten patients(1.8%) had concomitant POPf. Multivariate analysis identif.ed obesity and time needed f.r hepaticojejunostomy reconstruction as independent risk f.ctors of.biliary leakage, and no history of.preoperative endoscopic retrograde cholangiopancreatiography(ERCP) as protective f.ctor. Biliary leakage f.om hepaticojejunostomy af.er PD leads to a signif.cant increase in development of.delayed gastric emptying, and wound inf.ction. The median hospital stay and time to resume oral intake were signif.cantly greater in the biliary leakage group. Non-surgical management was needed in 40 patients(90.9%). Only 4 patients(9.1%) required re-exploration due to biliary peritonitis and associated POPf. The mortality rate in the biliary leakage group was signif.cantly higher than that of.the non-biliary leakage group(6.8% vs 3.9%, P = 0.05). Conclusions: Obesity and time needed f.r hepaticojejunostomy reconstruction are independent risk f.ctors of.biliary leakage, and no history of.preoperative ERCP is protective f.ctor. AIM: To assess vitamin D (Vit D) abnormalities in hepatitis C inf.cted patients and their relationship with interleukin (IL)-17, IL-23 and N-terminal propeptide of.type Ⅲ pro-collagen (PⅢNP) as immune response mediators. METHODS: The study was conducted on 50 Egyptian patients (36 male, 14 f.male) with hepatitis C virus (HCV) inf.ction, who visited the Hepatology Outpatient Clinic in the Endemic Disease Hospital at Cairo University. Patients were compared with 25 ageand sexmatched healthy individuals. Inclusion criteria were based on a history of.liver disease with HCV genotype 4 (HCV-4) inf.ction (as new patients or under f.llowup). Based on ultrasonography, patients were classif.ed into f.ur subgroups; 14 with bright hepatomegaly; 11 with perihepatic f.brosis; 11 with hepatic cirrhosis; and 14 with cirrhosis and hepatocellular carcinoma (HCC).Total Vit D (i.e., 25-OH-Vit D) and active Vit D [i.e., 1,25-(OH) 2 -Vit D] assays were carried out using commercial kits. IL-17, IL-23 and PⅢNP levels were assayed using enzyme linked immunosorbent assay kits, while HCV virus was measured by quantitative and qualitative polymerase chain reaction. RESULTS: Levels of.Vit D and its active f.rm were signif.cantly lower in advanced liver disease (hepatic cirrhosis and/or carcinoma) patients, compared to those with bright hepatomegaly and perihepatic f.brosis. IL-17, IL-23 and PⅢNP levels were markedly increased in HCV patients and correlated with the progression of.hepatic damage. The decrease in Vit D and active Vit D was concomitant with an increase in viral load, as well as levels of.IL-17, IL-23 and PⅢNP among all subgroups of.HCV-inf.cted patients, compared to normal healthy controls. A signif.cant negative correlation was evident between active Vit D and each of.IL-17, IL-23 and PⅢNP (r = -0.679, -0.801 and -0.920 at P f.cted men and women showed no dif.erences with respect to Vit D levels. Electrospinning is a simple technique used to f.bricate polymeric nano-f.brous *** nano-f.bers have f.und a wide range of.valuable applications in the biomedical ***,it has not been utilized with solid high explosives ***,the electrospinning technique has been used to f.bricate polystyrene(PS)/1,3,5-trinitro-1,3,5-triazinane(RDX)composite *** governed electrospinning parameters,voltage,distance f.om the collector,f.ow rate,mandrel rotating speed,time,and solution concentration,that greatly af.ect the morphology of.the obtained nanof.bers were *** f.bricated PS/RDX nano-f.bers were characterized using scanning electron microscopy(SEM),X-ray dif.ractometer(XRD),and f.urier Transf.rm Inf.ared(f.IR)*** impact and f.iction sensitivities of.PS/RDX were also *** thermal behavior of.the prepared composite and the pure materials were studied by the thermal gravimetric analysis technique(TGA).SEM results proved the f.brication of.PS/RDX f.bers in the nano-size via *** spectroscopy conf.rmed the existence of.the characteristic f.nctional groups of.both PS and RDX in the composite *** sharp peaks showed the conversion of.amorphous PS into crystalline shape via electrospinning and also conf.rmed the f.rmation of.PS/RDX *** PS f.bers absorbed the heat and increased the onset decomposition of.the pure RDX f.om 181.5 to 200.7℃in the case of.PS/RDX ***,PS/RDX nano-f.bers showed the relatively low impact and f.iction sensitivities of.100 J and 360 N *** results could introduce PS/RDX nanof.brous composite in the f.eld of.explosives detection with high levels of.saf.ty.

We report our experience with potential donors f.r living donor liver transplantation(LDLT), which is the f.rst report f.om an area where there is no legalized deceased donation program. This is a single center retrospective analysis of.potential living donors(n = 1004) between May 2004 and December 2012. This report f.cuses on the analysis of.causes, duration, cost, and various implications of.donor exclusion(n = 792).Most of.the transplant candidates(82.3%) had an experience with more than one excluded donor(median = 3). Some recipients travelled abroad f.r a deceased donor transplant(n = 12) and some died bef.re f.nding a suitable donor(n = 14). The evaluation of.an excluded donor is a time-consuming process(median = 3 d, range 1 d to 47 d). It is also a costly process with a median cost of.approximately 70 USD(range 35 USD to 885 USD). f.om these results, living donor exclusion has negative implications on the patients and transplant program with ethical dilemmas and an economic impact. Many strategies are adopted by other centers to expand the donor pool; however, they are not all applicable in our locality. We conclude that an active legalized deceased donor transplantation program is necessary to overcome the shortage of.available liver graf.s in Egypt.
