Isolation and Identification of Multi-Drug Resistant Strains of non-Lactose Fermenting Bacteria from Clinical Isolates

Purpose: We studied the drug resistance of different microbes from clinical isolates. The morphological characteristics of bacteria were observed through culture characteristics and by carrying out gram staining techniques while the biochemical characteristics of bacteria were carried out by biochemical test. Methods: A total of 324 samples were collected from suspected patients visiting different hospitals at district Peshawar. For morphological identification, samples of clinical isolates were analyzed by blood agar, MacConkey agar and Eosine Methylene Blue, identified by gram staining and characterized by different biochemical tests. Antibiotic Sensitivity test by Modified Kirby-Bauer Disc diffusion method was used to test the in-vitro susceptibility of the identified isolates to different antibiotics such as Ceftazidime, Ceftazidime, Ceftriaxone, Cefepime and Imipenem. Results: These resistant non-lactose fermenting gram negative bacteria were isolated from samples of pus/wound (33.30%, n = 108/324), blood (33.30%, n = 108/324), urine (23.30%, n = 75/324) and from ascetic/pleural fluids (10.20%, n = 33/324). The study revealed that the percentage of non-fermenting bacterial infection was higher in females (53%) as compared to males (47%) along with higher infection observed in the age group of 11 - 30 years. Pseudomonas aeroginosa showed high resistance against Cefepime (88.80%), followed by Cefoperazone (55.50%), Ceftazidime (48.10%), Ceftriaxone (33.30%). Imipenem was active with low resistance (7.40%). More resistance was seen in Morganella morganii against Imipenem (66.70%) followed by Cefoperazone (55.50%), Ceftriaxone (55.50%). Cefepime showed low resistance (11%). Multi-drug resistant Proteus mirabillis was highly resistance to Ceftriaxone (74.07%), followed by Cefepime (59.20%), Cefoperazone (44.40%) and low resistance for Imipenem (25.90%). 背景灌注指数是一种无创性的来自干血氧饱和度监测的反映外周循环灌注的指标。本研究评估了灌注指数做为探索成人丙泊酚麻醉过程中血管内注入包含15μg肾上腺素的硬膜外试验剂量药物后变化指标的有效性；同时将它与常规评估标准-心率（HR）（如果升高≥10bpm则为阳性）及收缩压（SBP）（如果升高≥15mmHg则为阳性）进行了可靠性比较。方法40例全静脉麻醉、择期行普外科手术的患者随机静脉注射3ml含5μg/ml肾上腺素浓度为15mg／ml的利多卡因或3ml生理盐水（每组n=20）。监测注射后5分钟的心率、收缩压和灌注指数（PI）。结果注射试验剂量在39±15秒后平均最大灌注指数降低65％±13％。心率和血压的最大增量分别为49±25秒后19±8bpm和102±34秒后17±7mmHg。用灌注指数作为血管内注射的评估指标（如果灌注指数较注射前降低≥10％则为阳性），其敏感性、特异性、阳性预期值和阴性预期值是100％（95％可信区间CI=83％-100％）。相对地，心率和血压指标的敏感性分别为95％（CI=76％-99％）和90％（CI=70％-97％）。结论灌注指数是评价成年丙泊酚静脉麻醉患者血管内注射硬膜外试验剂量药物后常规血流动力学变化的可靠的选用指标。

背景双腔气管导管（DLT）的准确放置对于其最佳功能的发挥至关重要。我们利用可回缩隆突钩对标准的左侧DLT盲置法进行改良。本研究旨在明确这种改良法与传统方法相比，是否能够提高首次插管精确定位的成功率。方法对40例拟行左侧DLT的择期全麻手术患者进行了前瞻性随机对照研究。记录使用传统方法（传统方法组，n=20）或者改良法（隆突钩组，n=20）出现位置不正的发生率以及最终放置正确所需要的时间。可回缩隆突钩是经过改良的橡胶弹性探条。采用纤维支气管镜（FOB）证实导管放置位置正确。结果首次放置DLT后，单纯的临床评估发现传统方法组与隆突钩组分别有3例（共20例，15％）和1例（共20例，5％）导管位置不正。随后FOB发现隆突钩组导管位置不正的概率（20例中出现2例，10％）显著低于传统方法组（20例中出现9例，45％）（P=0．03）。两组首次放置DLTs的时间相似（P=0．63），而FOB确认与调整所需的时间在隆突钩组（88．9±35．7秒）显著短于传统方法组（152．3±40．3秒）（P〈0．0001）。结论可回缩隆突钩的使用提高了DLT准确放置的成功率并且缩短了DLT位置调整时间。此外，该方法的安全性与可靠性仍需更大量的研究进行证实。

Assessment of Land Erosion and Sediment Accumulation Caused by Runoff after a Flash-Flooding Storm Using Topographic Profiles and Spectral Indices

This research deals with the characterization of areas associated with flash floods and erosion caused by severe rainfall storm and sediment transport and accumulation using topographic attributes and profiles, spectral indices (SI), and principal component analysis (PCA). To achieve our objectives, topographic attributes and profiles were retrieved from ASTER-V2 DEM. PCA and nine SI were derived from two Landsat-OLI images acquired before and after the flood-storm. The images data were atmospherically corrected, sensor radiometric drift calibrated, and geometric and topographic distortions rectified. For validation purposes, the acquired photos during the flood-storm, lithological and geological maps were used. The analysis of approximately 100 colour composite combinations in the RGB system permitted the selection of two combinations due to their potential for characterizing soil erosion classes and sediment accumulation. The first considers the “Intensity, nDWI and nMDI”, while the second associates form index (FI), brightness index (BI) and nDWI. These two combinations provide very good separating power between different levels of soil erosion and degradation. Moreover, the derived erosion risk and sediment accumulation map based on the selected spectral indices segmentation and topographic attributes and profiles illustrated the tendency of water accumulation in the landscape, and highlighted areas prone to both fast moving and pooling water. In addition, it demonstrated that the rainfall, the topographic morphology and the lithology are the major contributing factors for flash flooding, catastrophic inundation, and erosion risk in the study area. The runoff-water power delivers vulnerable topsoil and contributes strongly to the erosion process, and then transports soil material and sediment to the plain areas through waterpower and gravity. In our medical practice, in particular obstetrics, it is difficult to change certain consolidated dogmas, but the necessity and the current situation of our obstetrical exercise pushed us to find new technical supports, to make the exercise of our specialty as stripped of pitfalls as possible. Our work is summarized in a prospective comparative study, aimed at evaluating the existence or not of a difference between the administration of oxytocin just before the hysterotomy in a cesarean section and its administration after fetal extraction. We used a set of criteria to include patients in our study. This study took place over a period of one year (2020) in the Mother and Child regional center, in Meknes Imperial city, Morocco, involving a total number of 364 patients. With a group A comprising 176 patients 48% (176/364) who received oxytocin just before the hysterotomy and a group B of 188 patients 52% (188/364) who received it classically after fetal extraction. The difference was very significant as detailed in the article. Conclusion: the very convincing results of our study and the difference between the two groups, allowed us to demonstrate the effectiveness of our process and to endorse its use in our routine practice, with the perspective of conducting a prospective randomized study on a larger series.

Pseudotumoral Abdominal Tuberculosis in Immunocompetent Adults: Report of Three Cases and Review of the Literature

Purpose: To highlight diagnostic challenges of pseudotumoral abdominal tuberculosis. Materials and methods: Three cases of pseudotumoral abdominal tuberculosis were compiled in our department between 2014 and 2015. They were aged 34 years, 42 years and 61 years respectively. They were immunocompetent and had no personal or family history of tuberculosis. Clinical presentations were non-specific, represented by abdominal pain and weight loss in the three patients and chronic diarrhea in one patient. Abdominal ultrasound and computed tomography CT were performed in all patients. Abdominal MRI was performed in one case as well as a colonoscopy. Results: Retroperitoneal tuberculosis and colic tuberculosis were noted in the 1st and 2nd case. Macro-nodular and biliary hepatic tuberculosis was retained in the third case. The diagnosis was based upon histopathology in the 3 cases showing tubercular granuloma with caseation. Anti-tuberculosis therapy was prescribed for 6 months. Evolution was favorable in all patients. Conclusion: Tuberculosis continues to present diagnostic difficulties, particularly in its pseudo-tumoral form, even in endemic tuberculosis countries like ours.
