Male Breast Cancer:10-Year Experience at Mansoura University Hospital in Egypt

Objective Male breast cancer （MBC） is a rare disease representing less than 1% of all malignancies. The objective of the study is to report clinicopathological characteristics, treatment patterns, and outcomes of MBC in Mansoura University Hospital, Egypt. Methods This retrospective study focused on male breast cancer patients during 10 years （2000-2009）. The studied variables were data regarding general characteristics of patients, treatment modalities and survival. Results The series included 37 patients （0.8% of all breast cancer）. The median age was 57.7 years （range： 26-86 years）. The main clinical complaint was a mass beneath the areola ill 94.5% of the cases. Most patients had a locally advanced disease. 94.5% of tumors were invasive duct carcinomas. The treatment was essentially surgery in 91.8%, followed by adjuvant radiotherapy （in 89.2%）, hormonal therapy （in 56.7%） and chemotherapy （in 91.8%）. Follow-up period ranged from 6-115 months. Local recurrence occurred in 4 cases and metastasis in 11 cases. The 2-year and 5-year overall survival （OS） rates were 81.6% and 60.5%, respectively. The 2-year and 5-year disease-free survival （DFS） rates were 68.4%, and 52.6%, respectively. OS was not significantly affected by any of the studied parameters. Factors influencing DFS were： T stage （P=0.05）, positive lymph nodes （P=0.043）, metastasis （P=0.004）, and chemotherapy （1＇=0.046）. Conclusions MBC is a rare disease and often diagnosed at a locally advanced stage. The management of male and female breast carcinoma is identical. Future research for better understanding of this disease is needed to improve the management and prognosis of male breast cancer patients.

In this paper,the mathematical model describing the third-grade non-Newtonian blood flow suspended with nanoparticles through porous arteries is exactly *** present physical model was solved in the research literature via the optimal homotopy analysis method and the collocation method,where the obtained solution was compared with the numerical fourth-order Runge-Kutta ***,the present paper only introduces a new approach to obtain the exact solution of the concerned system and implements such exact solution as a reference to validate the published approximate *** remarks on the previously published results are observed and discussed in detail through tables and *** view of the present calculations,the obtained results in the literature by Ghasemi et al.[Ghasemi,Hatami,Sarokolaie et al.(2015)]may need ***,it is found that the obtained approximate results in the relevant literature agree with the current exact ones up to only two or three decimal places,at ***,the present approach along with the obtained results reflexes the effectiveness and efficiency of our analysis when compared with the corresponding study in the ***,the present results can be directly invested for similar future problems of the same constructions.

Anthropometric assessment of a Middle Eastern group of autistic children

Phosphoric Acid Pretreatment and Saccharification of Paper Sludge as a Renewable Material for Cellulosic Fibers

Recycling of paper sludge waste is crucial for establishing a sustainable green *** waste contains valuable sugars that can be converted into important chemicals such as ethanol,poly hydroxybutyrate,and lactic ***,the main challenge in obtaining sugars in high yield from paper sludge is the high crystallinity of cellulose,which hinders *** address this,pretreatment using phosphoric acid was optimized using response surface methodology to facilitate cellulose hydrolysis with minimal energy and *** created prediction model using the response surface method considered factors such as acid concentration(ranging from 60%to 85%),consistency(ranging from 4%to 10%),temperature(ranging from 25℃to 80℃),and time(ranging from 0.5 to 4 h).The results revealed that the model’s significant factors affecting the yield were acid concentration,reaction time,temperature,and the product of acid concentration and temperature,while the model’s significant factors affecting the crystallinity were the consistency,the temperature,and their *** results showed that the optimum conditions for pretreatment were using an acid concentration of 64%,temperature of 25℃,consistency of 10%,and time of 30 *** hydrolysis of the conditionally pretreated paper sludge resulted in a weight loss of 42%,compared to only 18%weight loss in non-pretreated paper ***,the optimized conditions led to low levels of furfurals and acetic acid,which are undesirable by-products that can interfere with sugar *** total sugar obtained under the optimized conditions was 0.43 g glucose/g sample(10.46 g/L),while the contents of methyl furfural,furfural,and acetic acid were 21.65,235.7,and 4.57 mg/L,*** study demonstrates the potential of phosphoric acid for pretreatment and hydrolysis of paper sludge,enabling efficient saccharification and the production of sugars with minimal undesired by-products.

Assessment of hepatic fibrosis in pediatric cases with hepatitis C virus in Egypt

AIM: To assess hepatic fibrosis and factors associated with its progression in children with HCV infection. METHODS: At the Hepatology Unit,Cairo University Children's Hospital,a single liver biopsy was performed to 43 children with HCV infection after an informed consent between 1998-2004. Their mean age at liver biopsy was 8.67 ± 4.3 years. RESULTS: Among the 43 patients' biopsies,12 (27.9%) were having no fibrosis,20 (46.5%) mild fibrosis and 11 (25.6%) moderate to severe fibrosis. The median time for development of fibrosis was estimated to be 5.5 years. Developing fibrosis was significantly associated with shorter duration from first detected ALT elevation to biopsy (12 mo vs 1.2 mo,P = 0.015) and having higher levels of direct serum bilirubin (0.3 mg/dL vs 0.5 mg/dL,P = 0.048). No association was found between fibrosis stage and the presence of co-morbid conditions (P = 0.33). CONCLUSION: Hepatic fibrosis was present in 72.1% of children with HCV infection. The development of fibrosis was associated with higher levels of direct serum bilirubin. There was no significant association between fibrosis and age,duration of infection,risk factors,co-morbid conditions and most biochemical parameters.

Coinfection with hepatitis C virus and schistosomiasis:Fibrosis and treatment response

AIM:To assess whether schistosomiasis coinfection with chronic hepatitis C virus (HCV) influences hepatic fibrosis and pegylated-interferon/ribavirin (PEG-IFN/ RIB) therapy response. METHODS:This study was designed as a retrospective analysis of 3596 chronic HCV patients enrolled in the Egyptian National Program for HCV treatment with PEG-IFN/RIB. All patients underwent liver biopsy and anti-schistosomal antibodies testing prior to HCV treatment. The serology results were used to categorize the patients into group A (positive schistosomal serology) or group B (negative schistosomal serology). Patients in group A were given oral antischistosomal treatment(praziquantel, single dose) at four weeks prior to PEG-IFN/RIB. All patients received a 48-wk course of PEG-IFN (PEG-IFNα2a or PEG-IFNα2b)/RIB therapy. Clinical and laboratory follow-up examinations were carried out for 24 wk after cessation of therapy (to week 72). Correlations of positive schistosomal serology with fibrosis and treatment response were assessed by multiple regression analysis. RESULTS:Schistosomal antibody was positive in 27.3% of patients (15.9% females and 84.1% males). The patients in group A were older (P = 0.008) and had a higher proportion of males (P = 0.002) than the patients in group B. There was no significant association between fibrosis stage and positive schistosomal serology (P = 0.703). Early virological response was achieved in significantly more patients in group B than in group A (89.4% vs 86.5%, P = 0.015). However, significantly more patients in group A experienced breakthrough at week 24 than patients in group B (36.3% vs 32.3%, P = 0.024). End of treatment response was achieved in more patients in group B than in group A (62.0% vs 59.1%) but the difference did not reach statistical significance (P = 0.108). Glassy behaviour of random field Ising spins on Bethe lattice in external magnetic field

The thermodynamics and the phase diagram of random field Ising model （RFIM） on Bethe lattice are studied by using a replica trick. This lattice is placed in an external magnetic field （B）. A Gaussian distribution of random field （hi） with zero mean and variance （hi2 = H2RF is considered. The free-energy （F）, the magnetization （M） and the order parameter （q） are investigated for several values of coordination number （z）. The phase diagram shows several interesting behaviours and presents tricritical point at critical temperature Tc = J/k and when HRF = 0 for finite z. The free-energy （F） values increase as T increases for different intensities of random field （HRE） and finite z. The internal energy （U） has a similar behaviour to that obtained from the Monte Carlo simulations. The ground state of magnetization decreases as the intensity of random field HRF increases, The ferromagnetic （FM） paramagnetic （PM） phase boundary is clearly observed only when z →∞. While FM PM-spin glass （SG） phase boundaries are present for finite z. The magnetic susceptibility （X） shows a sharp cusp at Tc in a small random field for finite z and rounded different peaks on increasing HRF.

Glassy phase and thermodynamics for random field Ising model on spherical lattice in magnetic field

Phase diagram and thermodynamic parameters of the random field Ising model （RFIM） on spherical lattice are studied by using mean field theory. This lattice is placed in an external magnetic field （B）. The random field （hi） is assumed to be Gaussian distributed with zero mean and a variance （hi2） = HRF2. The free energy （F）, the magnetization （M） and the order parameter （q） are calculated. The ferromagnetic （FM） spin-glass （SG） phase transition is clearly observed. The critical temperature （Tc） is computed under a critical intensity of random field HRF = V/2/πJ. The phase transition from FM to paramagnetic （PM） occurs at TC = J/k in the absence of magnetic field. The critical temperature decreases as HRF increases in the phase boundary of FM-to-SG. The magnetic susceptibility （X） shows a sharp cusp at TC and the specific heat （C） has a singularity in small random field. The internal energy （U） has a similar behaviour to that obtained from the Monte Carlo simulation.

Impact of lifestyle interventions on pathogenesis of nonalcoholic fatty liver disease

This editorial builds on the article titled“Establishment and validation of an adherence prediction system for lifestyle interventions in non-alcoholic fatty liver disease”by Zeng et *** carried out a critical examination of nonalcoholic fatty liver disease(NAFLD)pathogenesis and how lifestyle interventions could facilitate disease resolution,particularly highlighting that non-alcoholic steatohepatitis(NASH)is a severe form of *** discussion details that weight loss is a pivotal factor in disease outcomes:A 3%-5%reduction is enough for resolution in 50%of non-obese individuals,while a 7%-10%reduction achieves similar benefits in obese individuals,as demonstrated by magnetic resonance ***,the editorial underscores that such lifestyle changes are instrumental not only in resolving NAFLD but also in reversing hepatic steatosis and *** insights,derived from the research,emphasize the critical role of personalized lifestyle modifications in halting the progression of NAFLD to NASH and even reversing fibrosis,thus offering a template for effective patient management.
