Coinfection with hepatitis C virus and schistosomiasis:Fibrosis and treatment response

AIM:To assess whether schistosomiasis coinfection with chronic hepatitis C virus (HCV) influences hepatic fibrosis and pegylated-interferon/ribavirin (PEG-IFN/ RIB) therapy response. METHODS:This study was designed as a retrospective analysis of 3596 chronic HCV patients enrolled in the Egyptian National Program for HCV treatment with PEG-IFN/RIB. All patients underwent liver biopsy and anti-schistosomal antibodies testing prior to HCV treatment. The serology results were used to categorize the patients into group A (positive schistosomal serology) or group B (negative schistosomal serology). Patients in group A were given oral antischistosomal treatment(praziquantel, single dose) at four weeks prior to PEG-IFN/RIB. All patients received a 48-wk course of PEG-IFN (PEG-IFNα2a or PEG-IFNα2b)/RIB therapy. Clinical and laboratory follow-up examinations were carried out for 24 wk after cessation of therapy (to week 72). Correlations of positive schistosomal serology with fibrosis and treatment response were assessed by multiple regression analysis. RESULTS:Schistosomal antibody was positive in 27.3% of patients (15.9% females and 84.1% males). The patients in group A were older (P = 0.008) and had a higher proportion of males (P = 0.002) than the patients in group B. There was no significant association between fibrosis stage and positive schistosomal serology (P = 0.703). Early virological response was achieved in significantly more patients in group B than in group A (89.4% vs 86.5%, P = 0.015). However, significantly more patients in group A experienced breakthrough at week 24 than patients in group B (36.3% vs 32.3%, P = 0.024). End of treatment response was achieved in more patients in group B than in group A (62.0% vs 59.1%) but the difference did not reach statistical significance (P = 0.108). Circulating micro RNA, mi R-122 and mi R-221 signature in Egyptian patients with chronic hepatitis C related hepatocellular carcinoma

AIM: To explore the potential usefulness of serum miR-122 and miR-221 as non-invasive diagnostic markers of hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).METHODS: This prospective study was conducted on 90 adult patients of both sex with HCV-related chronic liver disease and chronic hepatitis C related HCC. In addition to the 10 healthy control individuals, patients were stratified into; interferon-na?ve chronic hepatitis C(CH)(n = 30), post-hepatitis C compensated cirrhosis(LC)(n = 30) and treatment-naive HCC(n = 30). All patients and controls underwent full clinical assessment and laboratory investigations in addition to the evaluation of the level of serum miR NA expression by ***: There was a significant fold change in serum mi RNA expression in the different patient groups when compared to normal controls; mi R-122 showed significant fold increasing in both CH and HCC and significant fold decrease in LC. On the other hand, mi R-221 showed significant fold elevation in both CH and LC groups and significant fold decrease in HCC group(P = 0.01). Comparing fold changes in miR NAs in HCC group vs non HCC group(CH and Cirrhosis), there was non-significant fold elevation in miR-122(P = 0.21) and significant fold decreasing in miR-221 in HCC vs non-HCC(P = 0.03). ROC curve analysis for miR-221 yielded 87% sensitivity and 40% specificity for the differentiation of HCC patients from non-HCC at a cutoff 1.82. CONCLUSION: Serum miR-221 has a strong potential to serve as one of the novel non-invasive biomarkers of HCC.

Assessment of hepatic fibrosis in pediatric cases with hepatitis C virus in Egypt

AIM: To assess hepatic fibrosis and factors associated with its progression in children with HCV infection. METHODS: At the Hepatology Unit,Cairo University Children's Hospital,a single liver biopsy was performed to 43 children with HCV infection after an informed consent between 1998-2004. Their mean age at liver biopsy was 8.67 ± 4.3 years. RESULTS: Among the 43 patients' biopsies,12 (27.9%) were having no fibrosis,20 (46.5%) mild fibrosis and 11 (25.6%) moderate to severe fibrosis. The median time for development of fibrosis was estimated to be 5.5 years. Developing fibrosis was significantly associated with shorter duration from first detected ALT elevation to biopsy (12 mo vs 1.2 mo,P = 0.015) and having higher levels of direct serum bilirubin (0.3 mg/dL vs 0.5 mg/dL,P = 0.048). No association was found between fibrosis stage and the presence of co-morbid conditions (P = 0.33). CONCLUSION: Hepatic fibrosis was present in 72.1% of children with HCV infection. The development of fibrosis was associated with higher levels of direct serum bilirubin. There was no significant association between fibrosis and age,duration of infection,risk factors,co-morbid conditions and most biochemical parameters.

Predictors of Complete Early Virological Response to Pegylated Interferon and Ribavirin in Egyptian Patients with Chronic Hepatitis C Genotype 4

We aim to determine the baseline factors associated with partial and cEVR by analyzing the data of 1861 Egyptian patients treated for 12 weeks with a course of Peg-IFN plus RBV. Base line data of 1861 Egyptian patients with chronic hepatitis C coming at Cairo-Fatemic Hospital for HCV treatment were studied including full clinical, Ultrasonographic examination, laboratory evaluation and liver biopsy. The most significant variables in relation to complete early virological response were low Hb level (13 gm/dl) with p 0.01, the stage of fibrosis p value eline before initiating treatment is mandatory to predict which patient will be more expected to achieve a cEVR and thus reducing the side-effects and healthcare costs associated with interferon therapies.

HIV Prevalence among HCV Egyptian Infected Patients and Its Impact on the Result of HCV Treatment

Background and Aim of the Study: HCV infection is the most common co-infection in HIV patients so we aimed to determine the prevalence of HIV infection in chronic HCV patients and its impact on chronic HCV patients treatment response. Patients and Methods: A retrospective study performed on 1852 chronic HCV patients subjected to anti HCV treatment with alpha 2a, alpha 2b or standard interferon and Ribavirin and tested and confirmed for HIV co infection by elISA twice. Upon HIV testing, two groups were generated, Group 1: 1840 HCV patients, positive for HCV RNA, and Group 2: 12 HIV positive patients and positive also for HCV. Informed consents were obtained from patients. Proper hematological biochemical investigations and other causes of hepatitis rather than HCV were carried out and excluded. Results: The prevalence of HIV among HCV infected Egyptian patients was 0.64%. We found a male gender predominance;the hematological and biochemical parameters were similar in both groups with mild elevations in liver enzymes in group II. High rates of failure to treatment (77.8%) with lower SVR (22.2%) were in group II compared to group I (59.9%) as SVR was 22.1% in group II vs. 34.1% in group I, however with no statistical significance. Conclusion: Despite the lower prevalence of HIV in Egyptian patients with HCV infection, it still affects their response to treatment .Therefore;we must screen HIV in all HCV patients and recommend its test to routine investigations before starting HCV therapy.

BACKGROUND Picky eating is a commonly observed behavior among children globally,negatively impacting their physical and mental *** common characteristics distinguish peaky eaters,including food selectivity,food neophobia,and food avoidance,there is no clear definition to assess this *** to the unavailability of data regarding picky eating,it wasn’t easy to estimate its *** To develop a regional protocol to help healthcare professionals identify and manage mild and moderate picky eating *** A virtual roundtable discussion was held in April 2021 to gather the opinions of seven pediatricians and two pediatric dietitians from eight Middle Eastern countries who had great experience in the management of picky *** discussion covered different topics,including clearly defining mild and moderate picky eating,identifying the role of diet fortification in these cases,and the possibility of developing a systematic approach to diet *** The panel identified picky eating as consuming an inadequate amount and variety of foods by rejecting familiar and unfamiliar *** of the time,moderate picky eating cases had micronutrient deficiencies with over-or undernutrition;the mild cases only showed inadequate food consumption and/or poor diet *** attention to the organic red flags like growth faltering and development delay and behavioral red flags,including food fixation and anticipatory gagging,will help healthcare professionals evaluate the picky eaters and the caregivers to care for their *** dietary supplementation and commercial food fortification play an important role in picky eating,they were no benefit in the Middle *** The panel agreed that food fortification through a food-first approach and oral nutritional supplements would be the best for Middle Eastern *** recommendations would facilitate identifying and managing picky-eating children in the Middle East.

Male Breast Cancer:10-Year Experience at Mansoura University Hospital in Egypt

Objective Male breast cancer （MBC） is a rare disease representing less than 1% of all malignancies. The objective of the study is to report clinicopathological characteristics, treatment patterns, and outcomes of MBC in Mansoura University Hospital, Egypt. Methods This retrospective study focused on male breast cancer patients during 10 years （2000-2009）. The studied variables were data regarding general characteristics of patients, treatment modalities and survival. Results The series included 37 patients （0.8% of all breast cancer）. The median age was 57.7 years （range： 26-86 years）. The main clinical complaint was a mass beneath the areola ill 94.5% of the cases. Most patients had a locally advanced disease. 94.5% of tumors were invasive duct carcinomas. The treatment was essentially surgery in 91.8%, followed by adjuvant radiotherapy （in 89.2%）, hormonal therapy （in 56.7%） and chemotherapy （in 91.8%）. Follow-up period ranged from 6-115 months. Local recurrence occurred in 4 cases and metastasis in 11 cases. The 2-year and 5-year overall survival （OS） rates were 81.6% and 60.5%, respectively. The 2-year and 5-year disease-free survival （DFS） rates were 68.4%, and 52.6%, respectively. OS was not significantly affected by any of the studied parameters. Factors influencing DFS were： T stage （P=0.05）, positive lymph nodes （P=0.043）, metastasis （P=0.004）, and chemotherapy （1＇=0.046）. Conclusions MBC is a rare disease and often diagnosed at a locally advanced stage. The management of male and female breast carcinoma is identical. Future research for better understanding of this disease is needed to improve the management and prognosis of male breast cancer patients.

Anthropometric assessment of a Middle Eastern group of autistic children

Phosphoric Acid Pretreatment and Saccharification of Paper Sludge as a Renewable Material for Cellulosic Fibers

Recycling of paper sludge waste is crucial for establishing a sustainable green *** waste contains valuable sugars that can be converted into important chemicals such as ethanol,poly hydroxybutyrate,and lactic ***,the main challenge in obtaining sugars in high yield from paper sludge is the high crystallinity of cellulose,which hinders *** address this,pretreatment using phosphoric acid was optimized using response surface methodology to facilitate cellulose hydrolysis with minimal energy and *** created prediction model using the response surface method considered factors such as acid concentration(ranging from 60%to 85%),consistency(ranging from 4%to 10%),temperature(ranging from 25℃to 80℃),and time(ranging from 0.5 to 4 h).The results revealed that the model’s significant factors affecting the yield were acid concentration,reaction time,temperature,and the product of acid concentration and temperature,while the model’s significant factors affecting the crystallinity were the consistency,the temperature,and their *** results showed that the optimum conditions for pretreatment were using an acid concentration of 64%,temperature of 25℃,consistency of 10%,and time of 30 *** hydrolysis of the conditionally pretreated paper sludge resulted in a weight loss of 42%,compared to only 18%weight loss in non-pretreated paper ***,the optimized conditions led to low levels of furfurals and acetic acid,which are undesirable by-products that can interfere with sugar *** total sugar obtained under the optimized conditions was 0.43 g glucose/g sample(10.46 g/L),while the contents of methyl furfural,furfural,and acetic acid were 21.65,235.7,and 4.57 mg/L,*** study demonstrates the potential of phosphoric acid for pretreatment and hydrolysis of paper sludge,enabling efficient saccharification and the production of sugars with minimal undesired by-products.
