Clinical and Genetic Study of Friedreich’s Ataxia and Ataxia with Vitamin E Deficiency in 44 Moroccan Families

Introduction: Friedreich ataxia (FRDA) is a multi-system autosomal-recessive disease, the most common one of the genetically inherited ataxias. FRDA occurs as a consequence of mutations in the frataxin gene, with an expansion of a GAA trinucleotide. Ataxia with vitamin E deficiency (AVED) is characterized clinically by neurological symptoms with often striking resemblance to those of Friedreich’s ataxia (FA) but serum concentrations of vitamin E are low. Aim of study: To study clinical and genetic features of the Friedreich’s ataxia and AVED patients in 44 Moroccan families. Patients and Methods: Retrospective series of 72 Moroccan patients displaying Friedreich’s ataxia syndrome was recruited over a period of 22 years (1987-2009). All patients had a clinical and ophtalmological examinations, 30 patients underwent electromyography, and CT scan was performed in 29 patients. GAA repeats in the frataxin gene and the 744 del A mutation α-TTP gene were performed in all patients. Results: 17 patients (24% of cases) had the 744 del A mutation in the α-TTP gene responsible of ataxia with vitamin E deficiency (AVED) phenotype. 55 patients ?(76% of cases) had GAA expanded allele in the first intron of the frataxin gene. Phenotype-genotype correlation revealed a high frequency of head titubation, decreased visual acuity and slower disease progression in AVED than in Friedreich’s ataxia phenotype (p Our study represents a large series which highlight the clinical and genetic differences between AVED and Friedreich’s ataxia. AVED patients have a better prognosis after alpha-tocopherol treatment.

Objective:This study aimed to examine the psychometric properties of the Conditions for Workplace Effectiveness Questionnaire-Ⅱ-Arabic Version(CWEQ-Ⅱ-AV),which measures structural empowerment among *** enhance the usability and credibility of the CWEQ-Ⅱ among researchers within Arabicspeaking countries,a valid and reliable Arabic version of the measure is ***:A cross-sectional research design was *** December 2018 to June 2019,275 nurses working in 4 hospitals participated in the *** was assessed by examining internal consistency and split-half reliability.A confirmatory factor analysis was performed to evaluate the factor structure of the CWEQ-Ⅱ-***:The psychometric properties of the CWEQ-Ⅱ-AV were excellent regarding the six-factor model(opportunity,information,resources,support,formal power,and informal power).The results showed the following fit indices meet the criteria set a priori:comparative fit index(CFI)=0.96,root mean square of error approximation(RMSEA)=0.06,andХ^(2)/df=***'s a coefficient was 0.95 for the total questionnaire and ranged between 0.83 and 0.89 for the individual *** split-half reliability was 0.91 for the total questionnaire and ranged from 0.83 to 0.87 for individual ***:This study provides evidence that CWEQ-Ⅱ-AV is both a reliable and valid measure of structural empowerment among Arab nurses.

Impact of Thyroglobulin on Survival and Prognosis of Differentiated Thyroid Cancer

Proper assessment of risk factors contributes to the principle management of differentiated thyroid carcinoma post operatively. Aim of the study: to investigate the effect of Thyroglobulin (Tg) levels on prognosis together with other risk factors for Differentiated Thyroid Cancer (DTC). Patients and methods: Medical records of all patients with DTC presented to Clinical Oncology and Nuclear Medicine Department referred from Diabetes & Endocrine unit (Internal Medicine Hospital) and Surgery Department Mansoura University from 2011-2016 were retrospectively reviewed. Patients with distant metastasis or who lost follow-up were excluded. So data of 220 patients were analyzed. Data collected included pre-surgical assessment, also surgical interference either total or near total thyroidectomy with or without lymph node neck dissection were reviewed. Different prognostic factors that affect progression free survival (PFS) include age, umorsize, ymph node status, ex, multifocality, capsular infiltration, vascular invasion and Tg level were evaluated through multivariate analysis. Results: Most of the patients included were <45 years (69.1%). Incidence of the disease was higher in female (80%) with papillary type predominance (80.9%). About 59.5% of cases presented with tumor size ≤ 2 cm and multifocality was reported in 13.6%. While 30% had lymph node metastasis, 11% had vascular invasion. Capsular infiltration was observed in 15% and most of them showed Tg level ≤ 10 ng/ml (68.2%). About 70% received ablative radioiodine. The 5-year Progression Free Survival (PFS) was 85%. On multivariate analysis of variable prognostic factors on PFS, we found that tumor size, age, lymph node status, capsular infiltration, Tg level and vascular invasion significantly affected PFS (P = 0.01, 0.005, 0.004, 0.005, 0.02, 0.003) respectively. 语言是文化的载体,是不同文化交流与传播的基石,在“一带一路”倡议中发挥着无比重要的作用。近几千年来,汉语言和汉文化凭借其源远流长的历史、博大精深的内涵与风格迥异的形式,影响着周边国家,向全世界展示了其独特的魅力与强大的传播态势。在新的时代背景下,汉语言文化在与世界各国交流中的作用日显突出,已经引起了全世界多国的重视,成为国家总体软实力中不可缺少的力量。作为一名来自也门的留学生,我深深感受到了中也两国间的友好协作日益加深,因而产生了写作本文的强烈愿望。本文首先对其他国家在“一带一路”背景下汉语言文化传播的研究和也门汉语言文化传播的研究进行了分析。其次,对也门汉语言文化传播的主要模式进行了概述,回顾了也门汉语言文化传播的历史,并分析了也门汉语言文化传播的主要模式。接下来,了解了也门高校开展汉语及文化教学的最新动态,调查了在也门教授汉语的相关院校的运行现状,探讨了“一带一路”背景下中也两国在政治和经济上的交流对汉语言文化传播的影响。然后分析了当前也门汉语言文化传播的情况、也门汉语言文化传播存在的问题及挑战。最后针对也门汉语言文化传播中发现的问题寻找出相对完善的解决办法并提出建设性的建议。希望能为中也两国关系重新定位、主动适应“一带一路”倡议建设提供借鉴和参考,为更深层次、高质量地推动汉语言文化在也门的传播、制定中也相关文化交流政策提供建议与参考。

Primary Lymphoma of the Larynx: A Case Report and Literature Review

Background: Primary non-hodgkin lymphoma of the larynx is a rare entity accounting for 1% of laryngeal tumors. Fewer than 100 cases have been reported in the literature. Case presentation: A 52-year-old man, presented with a two-month history of dysphonia. Nasofibroscopy showed left submucosal bulging of the laryngeal supraglottic floor. Cervical computed tomography showed extensive left supraglottic mass, with partial lysis of the thyroid cartilage. Direct laryngoscopy showed left submucosal bulging of the supraglottic floor. Transmucosal biopsies were performed. Histological study concluded to the presence of a diffuse large-cell B lymphoma. The patient was treated with chemotherapy followed by external radiotherapy. Evolution was favourable. Conclusion: Although primary laryngeal non-hodgkin lymphoma is rare, it should be considered in the differential diagnosis of a mass in the neck region, especially in the supraglottic area. Direct laryngoscopy with biopsy makes the diagnosis. Treatment and prognosis depend on the stage and the grade of the lymphoma.

Study of Angiotensin Converting Enzyme Gene Polymorphism in Egyptian Type 2 Diabetes Mellitus with Diabetic Kidney Disease

Objective: Diabetic kidney disease DKD (Diabetic nephropathy DN) is considered one of the chronic micro vascular complications of diabetes mellitus and considered the commonest cause leading to chronic renal failure and chronic renal dialysis. Genetic susceptibility has been implicated in DKD. The angiotensin converting enzyme (ACE) is one of the key roles in the renin angiotensin system cascade by converting angiotensin I to angiotensin II which plays a key role in regulation of blood pressure as well as electrolytes and fluid balance. This study addressed the association of (ACE) gene polymorphisms with DN in Egyptian (T2DM) patients. Methods: Our research comprised of 75 cases of T2DM with diabetic kidney disease, 100 cases of T2DM without DKD and 94 healthy volunteers. Different genotypes of ACE gene were determined by SSP-PCR analysis. Results: Gene polymorphism of ACE (DD, ID, II) in diabetic patient with DKD is 44%, 52%, 4% respectively and for T2DM individuals without DKD is 23%, 72%, 5% respectively. (DD) had significant higher frequencies in T2DM patients with DKD compared to those without DKD (p < 0.005) and (ID) had significant higher frequencies in T2DM without DKD (p < 0.0001). These results indicated that there is an association between ACE gene polymorphisms and susceptibility of diabetic patients to be affected by diabetic kidney disease. Conclusion: From our results, we can conclude that genotype of ACE in Egypt DD is the genotype of cases diabetic kidney disease. So the presence of D allele has a significant relation with diabetic kidney disease. Our data confirm the role of ACE in its relationship with diabetic kidney disease in Egyptian type 2 diabetic patients.

Single versus combination therapy in acute tocolysis: A prospective randomized controlled trial

This is a prospective controlled randomized trial conducted in 92 women with singleton pregnancies in preterm labor. The tocolytic efficacy and safety of combination atosiban and nifedipine was compared with that of the single agent, atosiban. Both lines of intervention was administered for 48 hours. Progression of labor was assessed by the frequency of uterine contractions and cervical changes. For statistical purpose, intent-to-treat (ITT) analysis was used throughout. Efficacy, as determined by the proportion of women in each group who did not deliver after therapy initiation, was comparable with no significant differences, at 48 hrs (91.5% vs. 91.1%) and at 7 days (90.7% vs. 85.7%) for the atosiban and the combination groups respectively. Safety was assessed by the numbers of adverse events. Maternal side effects were reported more in the combination group (34% vs. 64%;P = 0.006). Perinatal outcomes were similar between the groups. We conclude that the addition of nifedipine did not substantially improve the clinical outcomes beyond that were achieved with atosiban alone. Moreover, it has increased maternal side effects. Future research could focus on combination of other tocolytics.

Vitamin E is one of the most important lipid-soluble antioxidants. It is essential for the neurological function but its role in the central nervous system has not fully been elucidated. It is known that tocopherol acts in protecting cell membranes from oxidative damage and it can act as an anti-in?ammatory agent, which may also be neuroprotective, as well as regulating speci?c enzymes. There is growing evidence that oxidative stress plays a key role in the pathophysiology of several neurodegenerative disorders. These diseases are defined by the progressive loss of speci?c neuronal cell populations and are associated with protein aggregates. We reviewed some aspects related to the role of antioxidant properties of Vitamin E in preventing and/or curing neurodegenerative disorders such as the Alzheimer’s disease, Parkinson’s disease, amyotrophic lateral sclerosis, ataxia, tardive dyskinesia and Huntington’s disease.

Effect of Using Baker＇s Yeast and Exogenous Digestive Enzymes as Growth Promoters on Growth, Feed Utilization and Hematological Indices of Nile tilapia, Oreochromis niloticus Fingerlings

The present study was conducted to evaluate the effect of baker yeast, Saccharomyces cerevisiae （SC） and exogenous digestive enzymes （pepsin, papain and a-amylase, EDE） dietary supplementation on growth performance, feed utilization and hematological indices of Nile tilapia, Oreochromis niloticus fingerlings. A total of 630 Nile tilapia fingerlings with an average body weight of 26.4 ± 0.2 g were divided in the seven experimental net-pen treatments （three replicates each）. The experiment was conducted for 119 days. Seven isonitrogenous （26.50%） digestible protein and isocaloric （13.40 MJ kgl） digestible energy experimental diets were formulated. The control diet had no SC and EDE added. Diets 2-3 each contained SC at levels of 2 and 4 g 100 g diet-t, respectively, while diets 4-5 each contained EDE at levels of （0.64, 1.28, 0.16） and （1.28, 2.56, 0.32） g 100 gdiefI of pepsin, papain and a-amylase, respectively. Diet 6 contained mixture of SC and EDE at levels of 1 g yeast and 0.32, 0.64, 0.08 g of pepsin, papain and a-amylase, respectively 100 gdiet1 and diet D7 contained 2 g yeast and 0.64, 1.28, 0.16 g of pepsin, papain and a-amylase, respectively 100 g dietl. Growth performance and feed utilization efficiency of Nile tilapia were significantly （P 〈 0.05） higher in all treatments receiving SC and/or EDE supplemented-diets than the control diet which suggests that the addition of SC and EDE enhanced the growth performance. Red blood cells counts, hematocrit and hemoglobin were significantly （P 〈 0.05） highest in all treatments receiving mixture of SC and EDE supplemented-diets （D6 ＋ D7）. The same trend was observed for total plasma protein and total plasma globulin levels. The results of present study suggested that Nile tilapia fingerlings fed diets containing the mixture of I g yeast, SC and 0.32, 0.64, 0.08 g of pepsin, papain and a-amylase, respectively 100 gdiet^-1, for 119 days had enhanced growth performance, diet utilization efficiency and hematological ind
