The regulatory role of the Micro-RNAs (miRNAs) in the messenger RNAs (mRNAs) gene expression is well understood by the biologists since some decades, even though the delving into specific aspects is in progress. Clustering is a cornerstone in bioinformatics research, offering a potent computational tool for analyzing diverse types of data encountered in genomics and related fields. MiRNA clustering plays a pivotal role in deciphering the intricate regulatory roles of miRNAs in biological systems. It uncovers novel biomarkers for disease diagnosis and prognosis and advances our understanding of gene regulatory networks and pathways implicated in health and disease, as well as drug discovery. Namely, we have implemented clustering procedure to find interrelations among miRNAs within clusters, and their relations to diseases. Deep clustering (DC) algorithms signify a departure from traditional clustering methods towards more sophisticated techniques, that can uncover intricate patterns and relationships within gene expression data. Deep learning (DL) models have shown remarkable success in various domains, and their application in genomics, especially for tasks like clustering, holding immense promise. The deep convolutional clustering procedure used is different from other traditional methods, demonstrating unbiased clustering results. In the paper, we implement the procedure on a Multiple Myeloma miRNA dataset publicly available on GEO platform, as a template of a cancer instance analysis, and hazard some biological issues.

A Rare Entity of Accelerated Chronic Lymphocytic Leukemia: A Report of Two Cases and Review of Literature

Background: Accelerated-chronic lymphocytic leukemia (A-CLL) is a rare disease entity as it represents less than 1% of all reported cases of chronic lymphoid leukemia (CLL). Moreover, it is most likely an under diagnosed entity due to its rarity and the non-standardized practice of lymph node biopsy in CLL. Purpose: The aims of our work are to establish the diagnosis of A-CLL and to study the prognosis and treatment of this rare entity. Method: here, we report the clinical presentation and the follow up of two cases of A-CLL. Results: Distinguishing Richter transformation (RT) from A-CLL is important as it may result in a major change in disease management. The prognosis of A-CLL is intermediate between CLL and RT. The prognosis is mainly poor due to a predominance of poor prognostic markers including an increasing number of p53-positive cases. Conclusion: To this date, no prospective study has been led to define the best treatment for A-CLL. The shorter survival of A-CLL when compared to typical CLL implies the need of a more aggressive treatment.

Severe hemorrhagic colitis in a patient with chronic myeloid leukemia in the blastic phase after dasatinib use

Dasatinib is a second-line tyrosine kinase inhibitor used in patients with imatinib resistant or intolerant chronic myeloid leukemia (CML) and Philadelphia chromosomepositive acute leukemia. Gastrointestinal bleeding may occur in up to 7% of patients using dasatinib, although, severe dasatinib-related acute colitis had rarely been reported. Here, we present the case of a 36-year-old female who progressed to acute myeloid leukemia after fourteen months of receiving imatinib for CML in the chronic phase and was treated with a dasatinib-containing chemotherapy regimen. On day 34 of treatment, the patient developed moderate abdominal pain and bloody diarrhea with mucous. Analyses of stool specimens were negative for parasites, Clostridium difficile , and other pathogenic bacteria. The cytomegalovirus pp65 antigen was negative in her blood leukocytes. A colonoscopy revealed acute colitis, and a mucosal biopsy showed nonspecific colitis. The patient was treated with broad-spectrum antibiotics, bowel rest and hydration, and dasatinib treatment was stopped. Her bloody diarrhea improved within 72 h. After confirming cytological remission, the patient received initial course of consolidation, and dasatinib treatment was reinstated. However, hemorrhagic colitis recurred. After discontinuing dasatinib, herhemorrhagic colitis drastically improved and did not recur following the administration of nilotinib. The characteristics of our patient suggest that dasatinib treatment can lead to hemorrhagic colitis, which typically resolves after discontinuation of the drug.

Epidemiology of Helicobacter pylori Infection and Related Gastric Pathologies in Moroccan Population

Helicobacter pylori （H. pylori） colonize the gastric mucosa of 50% of the world＇s population. This infection is closely associated with the development of peptic ulcer disease and gastric cancer. The present study was conducted to assess the prevalence of H, pylori infection in Moroccan population and the risk to develop gastric precancerous lesions and gastric cancer. Totally, 298 patients were enrolled, 68 of asymptomatic subjects and 230 of patients with gastric diseases. Histological examination was effected to diagnostic gastric lesions and to detect H. pylori. ELISA was used to determine H. pylori status of patients. The prevalence of H. pylori within asymptomatic and symptomatic subjects was observed higher. A significant relationship was detected between H. pylori infection and the risk of gastric diseases （p-value 〈 0.0001）. A meaningful association between chronic gastritis increasing and age was observed （p-value = 0.03）. The risk to develop gastric cancer among infected patients was observed elevated with rate of 9%. Our results showed a high prevalence of H. pylori in both asymptomatic and gastric diseases patients. We noticed that chronic gastric infection increases with age. We remarked also that the risk to develop gastric cancer among infected patients was elevated in our population.

Fetoscopy Role in Fetal Surgery and Fetal Medicine

Aim: We attempted to illustrate our experience of fetoscopic therapy in a tertial institute, whereby to highlight the present Egyptian situation regarding this issue. We described fetoscopic therapy undergone at al-Hussein Hospital, al-Azhar University, Cairo, Egypt, from Dec-2012 to Sep-2019. Presentation of cases: Sixteen women with 17 fetuses were included in the analysis of this study with a mean age of 25.31 ± 6.15 years;four with positive marriage to a near relevant, and eight were primigravida. The fetus of twelve women had lower urinary tract obstruction (LUTO), two fetal anemia, one hydrothorax, and one twin-to-twin transfusion syndrome (TTTS). The mean time for fetoscopy was gestational week 24.13 ± 3.77. We did therapeutic drainage to amniotic fluid in hydrothorax and LUTO, therapeutic septostomy to the TTTS case, and blood transfusion in cases of fetal anemia. During the operation, the procedure succeeded in 14 out of 16 women. Overall, nine of the 16 women experienced some complications;five with rupture of membranes, four with preterm labor, and three with intrauterine fetal death (IUFD). Conclusion: Fetoscopy can manage LUTO and well-selected cases of TTTS, hydrothorax, and fetal anemia. The procedure is promising. Fetoscopic therapy has just begun in Egypt, and this report may contribute to the future advance of fetoscopic therapy in Egypt.

Introduction: This study aimed to evaluate, discuss and illustrate the role of fetoscopy diagnostically and therapeutically. Material and Methods: This study was conducted in private center in under the supervision of the professors of Azhar University, Egypt from Dec-2012 to Mar-2017. Women with confirmed fetal congenital malformations, and willing to do fetoscopy were recruited. Fetoscopy was attempted in all cases to treat the underlying fetal conditions. Follow up was made until delivery. Results: Twenty patients with 22 fetuses were included in this study with different congenital anomalies. Therapeutic drainage or coagulation was made in all cases. In cases of lower urinary tract obstruction, fetoscopy confirmed pre-suspected urethral atresia and changed the diagnosis from complete PUV to urethral atresia in some cases. The procedure succeeded in all cases. However, the on-going success rate was 17 (77.3%) of fetuses. The overall number of live birth was 11 fetuses (50%). Overall, 15 (75%) of the 20 patients experienced some complication withpreterm labor the most prevalent 15 (75%). Conclusion: Fetoscopy can diagnose and differentiate between posterior urethral valve (PUV) and atresia, and manage well-selected cases of twin to twin transfusion syndrome (TTTS), and have a reasonable survival rate in lower urinary tract obstruction (LUTO).

Glutaredoxins(Grxs) constitute a superfamily of proteins executing diverse biological *** cerevisiae glutaredoxin Grx6 not only serves as a glutathione(GSH)-dependent oxidoreductase and a GSH S-transferase,but also an essential [2Fe-2S]-binding *** we report the dimeric structure of Grx6 C-terminal domain(holo-Grx6C),bridged by one [2Fe-2S] cluster coordinated by the active-site Cys136 and two external GSH *** comparison combined with multiple-sequence alignment demonstrated that holo-Grx6 C is similar to the [2Fe-2S] cluster incorporated dithiol Grxs,which share a highly conserved [2Fe-2S] cluster binding pattern and dimeric conformation,distinct from the previously identified [2Fe-2S] cluster ligated monothiol Grxs.

目的介绍美国临床药学领域进展情况,为我国临床药学学科发展提供借鉴参考。方法通过Pubmed、中国知网等工具检索介绍美国临床药学教育、毕业后培训以及临床药学实践相关文献,结合美国专家学术报告内容,了解美国临床药学学科进展情况。结果经过60年发展,美国临床药学教育、毕业后培训及临床药学实践已形成较完善体系。Pharm.D已成为药师从业的唯一准入学位。美国药学教育越来越重视对学生能力的培养,注重药学实践的实际需求。完成PGY1项目培训已被作为从事临床药学实践的基本要求,完成PGY2项目培训并取得专业资格证书已逐渐成为在特殊专业领域工作的必需要求。药学实践不断实现专科化、标准化。临床药学研究主要针对临床药学服务及相关问题,而不是进行基础性药学科学研究。临床药学研究促进了临床药学学科发展和临床药学服务的有效实施。通过药学专业委员会(BPS)的专业资格认证保证了临床药师的专业胜任力。结论美国临床药学学科体系经过60年发展已趋于完善,相关经验值得我们学习和借鉴。

Thoracoscopic internal mammary lymph nodes dissection: a staging tool for internal mammary lymph nodes in breast cancer

Objective： The aim of our study was to investigate the feasibility and safety of thoracoscopic internal mammary lymphadenectomy as a method to refine and thereby improve nodal staging in breast cancer. Methods： During the period from June 2004 to May 2007, 50 patients with operable breast cancer underwent modified radical mastectomy （MRM） or breast conserving surgery （BCS）, followed by thoracoscopic internal mammary lymphadenectomy, using 3 ports through the skin incision of the MRM or the BCS. Metal clips were used to mark precise site of lymphadenectomy. Results： of total number of 50 patients, the mean age of patients was 44 years （range, 27-60 years）. 40 （80%） had medio-central tumor, 10 （20%） had lateral tumor. 35 （70%） had clinically involved axillary nodes. 16 out of 50 patients received neo-adjuvant CTH. 44 patients underwent MRM and 6 patients underwent BCS. No intra-operative complications occurred. Atelectasis was the only postoperative complication that was encountered, which occurred in 12 cases, and was treated conservatively. The average chest drainage period was 1.2 day （range, 1-2 days）. The total number of IMN metastasis was 18 patients （36%）. The risk of IMN metastasis was higher; in younger patients （P = 0.03）, in medio-central tumors （P = 0.03）, in bigger tumors （P = 0.05）, with heavier metastasis of axillary LNs （P = 0.001）. But a correlation with the histological pattern of the lry tumor didn＇t exist （P = 1）. Knowing the IMN status helped in proper staging of patients, 7 patients showed evident stage migration after adding the IMN analysis to that of primary tumor and axillary LN. During the follow up period （the median, 22 months; range, 7 to 42 months）, no patient had pleural dissemination or port-site metastasis. Conclusion： Thoracoscopic IMN lymphadenectomy is a safe procedure, which can be done serious additional complications or cosmetic compromise. And allow proper nodal staging, which allow proper treatment planning.
