Results of Open Surgical Treatment of Humeral Paddle Fractures about 63 Cases

Fractures of the humeral paddle, common to young adults, are most often complex, linked to violence and an increase in road accidents. The objective of our work is to evaluate our functional results, in the medium term, correlated with a review of the literature. This is a retrospective study of 63 patients, carried out in the traumatology-orthopedics department 1 of the IBN el JAZZAR hospital in KAIROUAN, over a period of 7 years from January 2015 to December 2021. The average age of patients was 39 years (17 - 68 years). Predominantly male. The etiologies are dominated by falls and accidents on public roads. Fractures are classified according to the Müller and Allgöwer classification where type C is found in 51% of cases. All our patients undergo an olecranon osteotomy in 71% of cases. Osteosynthesis using a Lecestre plate combined with screwing or plugging is used in 84% of cases. The evolution is marked by complications observed in eight patients (16%), including two cases of sepsis, four cases of elbow stiffness (8%), one case of joint callus and one case of pseudarthrosis. Our results are evaluated according to the Mayo elbow Performance Score, they are excellent and good in 71% of cases, average in 18% of cases and poor in 11% of cases. Fractures of the humeral paddle are fractures with a satisfactory functional prognosis, requiring ad integrum anatomical restoration and solid osteosynthesis allowing early rehabilitation of the elbow. One case of joint callus and one case of pseudarthrosis.

Tibial pilon fractures are rare injuries most often associated with soft tissue injuries, occurring during high-energy trauma. Surgical treatment represents the therapy of choice and must ensure anatomical reconstruction of the joint surface while respecting the surrounding tissues. The evolution of our results showed a majority of good functional clinical radio results by the open treatment which showed its superiority compared to the closed treatment and the combined treatment.

Surgical Treatment of Anterior Shoulder Instabilities Using the Latarjet Technique (about 40 Cases)

Purpose: Surgical treatment of chronic anterior shoulder instability is a difficult therapeutic challenge for contact athletes. The aim of our study is to evaluate the clinical and radiological results of 40 cases of chronic anterior shoulder instability treated in our institution by Latarjet technique. Methods: We report our experience with the intervention of Latarjet modified by the analysis of a retrospective study of 40 cases. They were all young athletic men, with an average age of 29 years with a predominance of the dominant shoulder. Results: Eighty percent had bone lesions, the first dislocation being traumatic (plating). One patient had a recurrence of his instability due to a new trauma and 67% resumed sport within an average period of eight months, some of whom still had functional discomfort in sports practice (9 patients). After an average follow-up of 75 months, 93% of patients were satisfied with their intervention. Only two mechanical complications were observed. Conclusion: Chronic anterior shoulder instability is pathology of young and active subjects. The preregulenoid coracoid block according to Latarjet represents the therapeutic method of choice in the treatment of chronic anterior instabilities of the shoulder, especially in young and athletic subjects. The result of this intervention remains good despite the complications that can occur such as pseudarthrosis, osteoarthritis, lysis or mobility of the screw.

Clinicobiochemical investigations of gangrenous mastitis in does:immunological responses and oxidative stress biomarkers

A total of 50 does were used to determine selected hematological and biochemical parameters with special references to oxidative stress markers, acute phase protein profiles, and proinflammatory cytokines in healthy and gangrenous mastitis affected does. Animals were divided into two equal groups represented as clinically healthy （control） and diseased groups, respectively. The bacteriological examination of milk samples from diseased does revealed many types of bacterial infection. The isolated bacteria were Staphy/ococcus aureus （N=23/25）, Escherichia coil （N=11/25）, and Clostridium perfringens （N=4/25）. There was a significant increase in the levels of β-hydrexybutyrate, non-esterified free fatty acids, triglyceride, low density lipoprotein cholesterol （LDL-C）, aspartate aminotransferase, and alanine aminotransferase and a significant reduction in the levels of glucose, cholesterol, and high density lipoprotein cholesterol （HDL-C） in does with gangrenous mastitis compared to healthy does. Moreover, there was a significant increase in the levels of malondialdehyde and uric acid with a significant decrease in the levels of reduced glutathione, super oxide dismutase, and catalase in does with gangrenous mastitis compared to healthy does. In addition, there was a significant increase in the haptoglobin, serum amyloid A, fibrinogen, interleukin 6 （IL-6）, IL-1β, and tumor necrosis factor-a （TNF-a） in does with gangrenous mastitis compared to healthy ones. Conclusively, oxidative stress biomarkers, acute phase proteins, and proinflammatory cytokines play an essential task as biomarkers for gangrenous mastitis in does. Mastitis may be considered as one of the ketotic stressors in does after parturition.

Potential Benefits and Obstacles of the Use of Internet of Things in Saudi Universities: Empirical Study

Internet of Things (IoT) among of all the technology revolutions has been considered the next evolution of the internet. IoT has become a far more popular area in the computing world. IoT combined a huge number of things (devices) that can be connected through the internet. The purpose: this paper aims to explore the concept of the Internet of Things (IoT) generally and outline the main definitions of IoT. The paper also aims to examine and discuss the obstacles and potential benefits of IoT in Saudi universities. Methodology: the researchers reviewed the previous literature and focused on several databases to use the recent studies and research related to the IoT. Then, the researchers also used quantitative methodology to examine the factors affecting the obstacles and potential benefits of IoT. The data were collected by using a questionnaire distributed online among academic staff and a total of 150 participants completed the survey. Finding: the result of this study reveals there are twelve factors that affect the potential benefits of using IoT such as reducing human errors, increasing business income and worker’s productivity. It also shows the eighteen factors which affect obstacles the IoT use, for example sensors’ cost, data privacy, and data security. These factors have the most influence on using IoT in Saudi universities.

Botulinum toxin augmented bilateral lateral rectus recession versus three muscles surgery in large-angle intermittent exotropia

AIM: To report the surgical outcomes of correcting large angle intermittent exotropia in adult patients by bilateral lateral rectus muscle recession with intraoperative use of botulinum toxin and to compare the results with those of bilateral lateral rectus muscle recession and unilateral medial rectus resection. METHODS: The medical records of patients who underwent surgical correction of large angle intermittent exotropia [exotropia >50 prism dioptre(PD)] were retrospectively reviewed. Two groups of patients were identified;Group Ⅰ(21 patients) had bilateral lateral recti recession augmented with intraoperative botulinum toxin A(BTA) injection into the recessed muscles and group Ⅱ(30 patients) were treated by bilateral lateral recti recession with unilateral medial rectus muscle resection. Preoperative data were extracted for age, gender, refraction, type of exotropia, angle of stereopsis and angle of deviation. The main outcome measures were the postoperative angle of deviation and stereoacuity angle by Titmus test measured at the end of one year of postoperative follow up. RESULTS: By the end of the first postoperative year, 10 patients in group Ⅰ(47.6%) and 20 patients in group Ⅱ(66.7%) achieved esotropia/esophoria ely). Robust Hybrid Control for Ballistic Missile Longitudinal Autopilot

This paper investigates the boost phase＇s longitudinal autopilot of a ballistic missile equipped with thrust vector control. The existing longitudinal autopilot employs time-invariant passive resistor-inductor-capacitor （RLC） network compensator as a control strategy, which does not take into account the time-varying missile dynamics. This may cause the closed-loop system instability in the presence of large disturbance and dynamics uncertainty. Therefore, the existing controller should be redesigned to achieve more stable vehicle response. In this paper, based on gain-scheduling adaptive control strategy, two different types of optimal controllers are proposed. The first controller is gain-scheduled optimal tuning-proportional-integral-derivative （PID） with actuator constraints, which supplies better response but requires a priori knowledge of the system dynamics. Moreover, the controller has oscillatory response in the presence of dynamic uncertainty. Taking this into account, gain-scheduled optimal linear quadratic （LQ） in conjunction with optimal tuning-compensator offers the greatest scope for controller improvement in the presence of dynamic uncertainty and large disturbance. The latter controller is tested through various scenarios for the validated nonlinear dynamic flight model of the real ballistic missile system with autopilot exposed to external disturbances.

The outbreak of Covid-19 has taken the lives of many patients so *** symptoms of COVID-19 include muscle pains,loss of taste and smell,coughs,fever,and sore throat,which can lead to severe cases of breathing difficulties,organ failure,and ***,the early detection of the virus is very ***-19 can be detected using clinical tests,making us need to know the most important symptoms/features that can enhance the decision *** this work,we propose a modified multilayer perceptron(MLP)with feature selection(MLPFS)to predict the positive COVID-19 cases based on symptoms and features from patients’electronic medical records(EMR).MLPFS model includes a layer that identifies the most informative symptoms to minimize the number of symptoms base on their relative *** the model with only the highest informative symptoms can fasten the learning process and increase *** were conducted using three different COVID-19 datasets and eight different models,including the proposed *** show that MLPFS achieves the best feature reduction across all datasets compared to all other experimented ***,it outperforms the other models in classification results as well as time.
