Effect of a comprehensive geriatric assessment nursing intervention model on older patients with diabetes and hypertension

BACKGROUND The Comprehensive Geriatric Assessment(CGA)was introduced late in China and is primarily used for investigating and evaluating health problems in older adults in outpatient and community ***,there are few reports on its application in hospitalized patients,especially older patients with diabetes and *** To explore the nursing effect of CGA in hospitalized older patients with diabetes and *** we performed a retrospective single-center analysis of patients with comorbid diabetes mellitus and hypertension who were hospitalized and treated in the Jiangyin Hospital of Traditional Chinese Medicine between September 2020 and June *** the 80 patients included,40 received CGA nursing interventions(study group),while the remaining 40 received routine nursing care(control group).The study group's comprehensive approach included creating personalized CGA profiles,multidisciplinary assessments,and targeted inter-ventions in areas,such as nutrition,medication adherence,exercise,and mental ***,the control group received standard nursing care,including general and medical history collection,fall prevention measures,and regular patient *** 6 months of nursing care implementation,we evaluated the effectiveness of the interventions,including assessments of blood glucose levels fasting blood glucose,2-h postprandial blood glucose,and glycated hemoglobin,type A1c(HbA1c);blood pressure indicators such as diastolic blood pressure(DBP)and systolic blood pressure(SBP);quality of life as measured by the 36-item Short Form Survey(SF-36)questionnaire;and treatment *** After 6 months,the nursing outcomes indicated that patients who underwent CGA nursing interventions experienced a significant decrease in blood glucose indicators,such as fasting blood glucose,2-h postprandial blood glucose,and HbA1c,as well as blood pressure indicators,including DBP and SBP,compared with the control group(Pq

强往复荷载作用可能导致钢框架梁柱节点的焊接孔处发生超低周疲劳(ULCF)断裂,将数值模拟的应力应变状态代入循环孔洞扩张模型(CVGM)是预测ULCF断裂的普遍方法。将上述方法扩展至改进循环孔洞扩张模型(ICVGM),可用于各类焊接孔形节点的断裂预测,给出优化的焊接孔形。为此,基于既有试验数据提出ICVGM参数标定方法,确定q355钢的相关参数;将ICVGM预测结果与钢框架节点局部试件断裂试验结果进行对比,揭示二者的一致性,验证ICVGM较CVGM的准确性;同时,采用ICVGM获得不同焊接孔形节点的断裂倾向,提出优化建议及合理孔形。有限元参数分析表明:增加焊接孔长度可以降低节点的断裂倾向;采用半圆形及FEMA-350推荐孔形节点的断裂倾向最低;优化孔形使得断裂位置由热影响区转移至母材,从而提高节点延性。

The full array of the Large High Altitude Air Shower Observatory(LHAASO)has been in operation since July *** its kilometer-square array(KM2A),we optimized the selection criteria for very high and ultrahigh energyγ-rays using data collected from August 2021 to August 2022,resulting in an improvement in significance of the detection in the Crab Nebula of approximately 15%,compared with that of previous *** the implementation of these new selection criteria,the angular resolution was also significantly improved by approximately 10%at tens of *** aspects of the full KM2A array performance,such as the pointing error,were also calibrated using the Crab *** resulting energy spectrum of the Crab Nebula in the energy range of 10-1000 TeV are well fitted by a log-parabola model,which is consistent with the previous results from LHAASO and other experiments.

为了应对更加实际的增量式印刷电路板缺陷检测场景，本文将知识蒸馏与YOLOX相结合，提出了一种基于YOLOX的类增量印刷电路板缺陷检测方法。在只使用新训练数据的情况下，模型能够检测出所有学过的缺陷类型。通过对模型的输出特征和中间特征使用知识蒸馏来促进旧缺陷类别知识的传递，使得学生模型能够有效保留教师模型在旧缺陷类别上的检测性能。实验结果表明，本文方法能够显著缓解增量学习过程中的灾难性遗忘问题，在两阶段增量场景下，模型对所有缺陷的平均检测精度为88.5%，参数量为25.3×106，检测速度为39.8 f/s，便于工业设备部署的同时，可以满足增量式检测场景下印刷电路板(printed circuit board,PCB)质检的检测精度和检测速度要求。

Detection of false data injection attacks on power systems using graph edge-conditioned convolutional networks

State estimation plays a vital role in the stable operation of modern power systems,but it is vulnerable to cyber *** data injection attacks(FDIA),one of the most common cyber attacks,can tamper with measure-ment data and bypass the bad data detection(BDD)mechanism,leading to incorrect results of power system state estimation(PSSE).This paper presents a detection framework of FDIA for PSSE based on graph edge-conditioned convolutional networks(GECCN),which use topology information,node features and edge *** deep graph architecture,the correlation of sample data is effectively mined to establish the mapping relationship between the estimated values of measurements and the actual states of power *** addition,the edge-conditioned convolution operation allows processing data sets with different graph *** studies are undertaken on the IEEE 14-bus system under different attack intensities and degrees to evaluate the performance of *** results show that GECCN has better detection performance than convolutional neural networks,deep neural net-works and support vector ***,the satisfactory detection performance obtained with the data sets of the IEEE 14-bus,30-bus and 118-bus systems verifies the effective scalability of GECCN.

Objective: Despite cardiotoxicity overlap, the trastuzumab/pertuzumab and anthracycline combination remains crucial due to significant benefits. Pegylated liposomal doxorubicin(PLD), a less cardiotoxic anthracycline, was evaluated for efficacy and cardiac safety when combined with cyclophosphamide and followed by taxanes with trastuzumab/pertuzumab in human epidermal growth factor receptor-2(HER2)-positive early breast cancer(BC).Methods: In this multicenter, phase II study, patients with confirmed HER2-positive early BC received four cycles of PLD(30-35 mg/m^(2)) and cyclophosphamide(600 mg/m^(2)), followed by four cycles of taxanes(docetaxel,90-100 mg/m^(2) or nab-paclitaxel, 260 mg/m^(2)), concomitant with eight cycles of trastuzumab(8 mg/kg loading dose,then 6 mg/kg) and pertuzumab(840 mg loading dose, then 420 mg) every 3 weeks. The primary endpoint was total pathological complete response(tp CR, yp T0/is yp N0). Secondary endpoints included breast p CR(bp CR),objective response rate(ORR), disease control rate, rate of breast-conserving surgery(BCS), and safety(with a focus on cardiotoxicity).Results: Between May 27, 2020 and May 11, 2022, 78 patients were treated with surgery, 42(53.8%) of whom had BCS. After neoadjuvant therapy, 47 [60.3%, 95% confidence interval(95% CI), 48.5%-71.2%] patients achieved tp CR, and 49(62.8%) achieved bp CR. ORRs were 76.9%(95% CI, 66.0%-85.7%) and 93.6%(95% CI,85.7%-97.9%) after 4-cycle and 8-cycle neoadjuvant therapy, respectively. Nine(11.5%) patients experienced asymptomatic left ventricular ejection fraction(LVEF) reductions of ≥10% from baseline, all with a minimum value of >55%. Enhancing strength and ductility of AlSi10Mg fabricated by selective laser melting by TiB_(2)nanoparticles

In the metallic components fabricated by the emerging selective laser melting(SLM)technology,most strategies used for strengthening the materials sacrifice the ductility,leading to the so-called strengthductility *** the present study,we report that the strength and ductility of materials can be enhanced simultaneously by introducing nanoparticles,which can break the trade-off of the metallic *** the case of in-situ nano-TiB_(2)decorated AlSi10Mg composites,the introduced nanoparticles lead to columnar-to-equiaxed transition,grain refinement and texture *** increasing content of nanoparticles,the strength increases ***,the ductility first increases and then *** results show that the ductility is controlled by the competition between the crack-induced catastrophic fracture and ductile fracture associated with dislocation *** first increase of ductility is mainly attributed to the suppression of crack-induced catastrophic fracture when TiB_(2)nanoparticles *** the further increase of TiB_(2)nanoparticles,the subsequent decrease of ductility is mainly controlled by dislocation ***,the materials will exhibit the optimum strength and ductility combination in a certain range of TiB_(2)*** study clarifies the physical mechanism controlling ductility for nano-TiB_(2)decorated Al Si10Mg composites,which provides the insights for the design of structural materials.

Design of SiNridge-slot micro-ring resonator and generation of dual-comb based on orthogonally bicolor pumping

A novel high quality-factor （q） micro-ring resonator （MRR） structure based on the Si3N4ridge-slot waveguide is proposed， and the MRR is pumped by orthogonally polarized bicolor pumping to generate dual-comb. we optimized the structure of MRR by the finite element method and precise dispersion engineering， which finally obtained the suitable MRR geometry with negative dispersion characteristics at 1550 nm， having q of 1.7×107and the absorption loss as low as 2.6×10-5dB/cm. The simulation model of generating dual-comb is established as coupled Lugiato-Lefever equation （LLE）， which takes into account the higher order dispersion， cross-phase modulation （XPM）， multiphoton absorption， and external pumping. Solved by the split-step Fourier method （SSFM） and the fourth-order Runge-Kutta （RK4） method， the numerical results show that the generated dual-comb is periodically equally spaced distribution but with slightly different intensities in the time domain. In the frequency domain， there are 64 comb teeth with intensities higher than -100 dBm with a bandwidth of 120 nm. Particularly， in the case of bicolor orthogonal polarization pumping， a smaller amount of detuning does not greatly affect the bandwidth of the dual-comb.
