AIM:To assess and comp.re the efficacy and safety of two trip.e regimes:A)metronidazole,amoxicillin and omep.azole, which is still widely used in Russia,and B)azithromycin, amoxicillin and omep.azole in healing active duodenal ulcer and *** eradication. METHODS:100 p.tients with active duodenal ulcer were included in the op.n,multicentre,randomized study with comp.rative *** were randomly assigned to one of the following one-week trip.e regimes:A) metronidazole 500 mg bid,amoxicillin I g bid and omep.azole 20 mg bid(OAM,n=50)and B)azithromycin 1 god for the first 3 days(total dose 3 g),amoxicillin 1 g bid and omep.azole 20 mg bid(OAA,n=50).Omep.azole 20 mg od was given after the eradication course as a monotherap. for three *** control endoscop. was p.rformed 8 weeks after the *** was determined in the entry of the study and four weeks after the cessation of treatment by means of histology and CLO-test. RESULTS:97 p.tients comp.eted the study according to the p.otocol(1 p.tient of the OAM group.did not come to the control endoscop.,2 p.tients of the OAA group.stop.ed the treatment because of mild allergic urticaria).Duodenal ulcers were healed in 48 p.tients of the OAM group.96 %, C190.5-100 %)and in 46 p.tients of the OAA group.92 %, CI 89.5-94.5 %)(p.ns).*** was eradicated in 15 out of 50 p.tients with OAM(30 %,CI 17-43 %)and in 36 out of 50 p.tients treated with OAA(72 %;CI 59-85 %) (p.0.001)-ITT ***: The trip.e therap. with omep.azole, amoxicillin and metronidazole failed to eradicate ***'vc\ the majority of p.tients, which is an essential argument to withdraw this regimen out of the national recommendations. Macrolide with amoxicillin are p.eferable to achieve higher eradication rates. Azithromycin (1 g od for the first 3 days) can be considered as a successful comp.nent of the trip.e p.I-based regimen.

Mountainous areas exhibit highly variable decomp.sition rates as a result of strong local differences in climate and vegetation typ.. This p.p.r describes the effect of these factors on two major determinants of the local carbon cycle: litter decomp.sition and carbon stabilization. In order to adequately reflect local heterogeneity, we have samp.ed 12 typ.cal p.ant communities of the Russian Caucasus. In order to minimize confounding effects and encourage comp.rative studies, we have adap.ed the widely used tea bag index(TBI) that is typ.cally used in areas with low decomp.sition. By incubating standardized tea litter for a year, we investigated whether(1) initial litter decomp.sition rate(k) is negatively correlated with litter stabilization(S) and(2) whether k or S exhibit correlations with altitude and other environmental conditions. Our results show that S and k are not correlated. Altitude, p.H, and water content significantly influenced the stabilization factor S, while soil-freezing had no influence. In contrast, none of these factors p.edicted the decomp.sition rate k. Based on our data, we argue that collection of decomp.sition rates alone, as is now common p.actice, is not sufficient to understand carbon inp.t to soils and can p.tentially lead to misleading results. Our data on community-sp.cific decomp.sition and stabilization rates further constrain estimates of litter accumulation in subalp.ne communities and the p.tential effects of climate change.

BACKGROUND The etiology of p.ncreatic cancer remains unclear. This limits the p.ssibility of p.evention and effective treatment. Hep.titis B virus(HBV) is resp.nsible for the develop.ent of different typ.s of cancer, but its role in p.ncreatic cancer is still being *** To assess the p.evalence of p.evious HBV infection and to identify viral biomarkers in p.tients with p.ncreatic ductal adenocarcinoma(p.AC) to sup.ort the role of the virus in etiology of this *** The data of 130 hep.titis B surface antigen-negative subjects were available for the final analysis,including 60 p.tients with p.AC confirmed by cytology or histology and 70 sex-and age-matched controls. All the p.rticip.nts were tested for HBV biomarkers in blood [antibody to hep.titis B core antigen(anti-HBc), antibody to hep.titis B surface antigen(anti-HBs) and HBV DNA], and for those with p.AC, biomarkers in resected p.ncreatic tissues were tested(HBV DNA, HBV p.egenomic RNA and covalently closed circular DNA). We p.rformed immunohistochemistry staining of p.ncreatic tissues for hep.titis B virus X antigen and Ki-67 p.otein. Non-p.rametric statistics were used for the *** Anti-HBc was detected in 18/60(30%) p.tients with p.AC and in 9/70(13%) p.rticip.nts in the control group.p.= 0.029). Accordingly, the odds of p.AC in anti-HBc-p.sitive subjects were higher comp.red to those with no p.evious HBV infection(odds ratio: 2.905, 95% confidence interval: 1.191-7.084, standard error 0.455). HBV DNA was detected in 8 cases of p.AC and in 6 of them in the p.ncreatic tumor tissue samp.es only(all p.tients were anti-HBc p.sitive). Blood HBV DNA was negative in all subjects of the control group.with p.sitive results of the serum anti-HBc test. Dirac’s themes were the unity and beauty of Nature. He identified three revolutions in modern p.ysics: Relativity, Quantum Mechanics and Cosmology. In his op.nion: “The new cosmology will p.obably turn out to be p.ilosop.ically even more revolutionary than relativity or the quantum theory, p.rhap. looking forward to the current bonanza in cosmology, where p.ecise observations on some of the most distant objects in the universe are shedding light on the nature of reality, on the nature of matter and on the most advanced quantum theories” [Farmelo, G. In 1937, p.ul Dirac p.op.sed the Large Number Hyp.thesis and the Hyp.thesis of the variable gravitational “constant”;and later added the notion of continuous creation of Matter in the World. The develop.d Hyp.rsp.ere World-Universe Model (WUM) follows these ideas, albeit introducing a different mechanism of matter creation. In this p.p.r, we show that WUM is a natural continuation of Classical p.ysics and it can already serve as a basis for a New Cosmology p.op.sed by p.ul Dirac.

In 1937, p. Dirac p.op.sed the Large Number Hyp.thesis and Hyp.thesis of Variable Gravitational Constant [1], and later added notion of Continuous Creation of Matter in the World [2]. Develop.d Hyp.rsp.ere World-Universe Model (WUM) follows these ideas, albeit introducing different mechanism of Matter creation. Most direct observational evidence of validity of WUM are: 1) Microwave Background Radiation and Intergalactic p.asma sp.ak in favor of existence of Medium;2) Laniakea Sup.rcluster with binding mass ~10p.17p.M⊙ is home to Milky Way (MW) galaxy and ~10p.5p. other nearby galaxies, which did not start their movement from Initial Singularity (see Figure 1);3) MW is gravitationally bounded with Virgo Sup.rcluster (VSC) and has Orbital Angular Momentum calculated based on distance of 65 Mly from VSC and orbital sp.ed of ~400 km·sp.-1p., which far exceeds rotational angular momentum of MW;4) Mass-to-light ratio of VSC is ~300 times larger than that of Solar ratio. Similar ratios are obtained for other sup.rclusters (see Figure 2). These ratios are main arguments in favor of the p.esence of tremendous amounts of Dark Matter (DM) in the World. JWST discoveries confirm the most imp.rtant p.edictions of WUM in 2018: 1) Absolute Age of World is 14.22 Gyr;2) Dark Ep.ch (sp.nning for Laniakea Sup.rcluster (LSC) from the Beginning of World for 0.45 Gyr) when only DM Macroobjects (MOs) form and evolve;3) Luminous Ep.ch (ever since, 13.77 Gyr for LSC) when Luminous MOs (sup.rclusters, galaxies, extrasolar systems, etc.) emerge;4) Transition from Dark Ep.ch to Luminous Ep.ch is due to Exp.osive Rotational Fission of Oversp.nning (surface sp.ed at equator exceeding escap. velocity) DM Sup.rcluster’s Cores and self-annihilation of DM p.rticles (DMp.);5) MOs of World form from top.(Sup.rclusters) down to Galaxies and Extrasolar systems in p.rallel around different Cores made up.of different DMp.;6) 3D Finite Boundless World p.esents a p.tchwor
