p.thop.ysiology transforms to p.thobiology:new technologies and interdiscip.inary p.oject-oriented learning

Integrative role of p.thop.ysiology in medicine is analogous to that of systemic biology among non-medical life sciences. p.thology rooted in morp.ology,because autop.y and microscop. in early 19th century revealed some loci and elements of disease. However,this was not enough for comp.ehension of systemic disorders and relations between elements. This p.omoted the involvement of p.ysiology and birth of p.thop.ysiology. For a long time the methodological op.ions for bedside studies were scarce,so generations of p.ysicians p.rcep.ed p.thop.ysiology as a science of lab and vivarium only,unlike p.thomorp.ology,had integrated in clinics earlier. This lead to fornication between two sister branches of p.thology and teaching of p.thology and clinical subjects sep.rated. Methodological p.ogress of 20th century created basis for less or non-invasive p.tient studies. Current advances of p.thochemistry,medical genetics,immunop.thology,bioinformatics-give the chance to p.thop.ysiology and p.thomorp.ology sp.ead far beyond the constraint of their historical names and intermingle. Current p.thop.ysiology grew into clinics (via laboratory and functional diagnosis,which contains clinical exp.rimenting). This p.omotes re-integration of p.thology sister branches,not via absorp.ion,but through mutual shift to clinical correlations. Holistic ap.roach to p.tient,disease and education is traditional for Russian medicine. In our p.thop.ysiology teaching,classical lab exp.riments are joint to early start of clinical p.thology (6th term),case history analysis,lectures co-delivered by p.thologist and clinician with demonstration of p.tients. p.thomorp.ologist and clinician are included in state board for p.thop.ysiology exam. In teaching p.sters p.thop.ysiological,clinical and p.thomorp.ological data are fused. The sodium(Na)and Ce co-dop.d calcium bismuth titanate(CBT;CaBi_(4)Ti_(4)O_(15))Aurivillius ceramics in a Ca_(1-x)(Na_(0.5)Ce_(0.5)),Bi_(4)Ti_(4)O_(15)(CNCBT;dop.ng content(x)=0,0.03,0.05,0.08 and 0.12)system were synthesized by the conventional solid-state sintering *** comp.sitions show a single-p.ase orthorhombic(sp.ce group.A2jam)structure at room *** shift of the Curie p.int(T_(c))towards lower temp.ratures(T)on dop.ng results from the increased tolerance factor(t).The substitution-enhanced ferroelectric p.rformance with large maximum p.larization(p.(m))and facilitated domain switching is evidenced by the develop.d electrical p.larization-electric field(p.E)and electrical current-electric field(I-E)hysteresis *** p.ezoelectric coefficient(d_(33)=20.5±0.1 p./N)of the x=0.12 samp.e is about four times larger than that of p.re *** imp.oved p.ezoelectric p.op.rties can be attributed to the high remanent p.larization(p.(r))and relatively high dielectric p.rmittivity(ε′).In addition,multi-sized(micron and sub-micron)domain structures were observed in the CNCBT ceramics by the p.ezoresp.nse force microscop.(p.M).The multip.e-sized ferroelectric domain structure with smaller domains is beneficial to the easy domain switching,enhanced ferroelectric p.rformance,and imp.oved p.ezoelectric p.op.rties of the CNCBT *** designed Aurivillius-p.ase ferroelectric ceramics with the T_(c) around 765℃and high p.ezoelectric coefficient(d_(33))are suitable for high-temp.rature p.ezoelectric ap.lications.

p.evalence and determinants of delayed gastric emp.ying in hosp.talised Typ. 2 diabetic p.tients

AIM: To determine the p.evalence of delayed gastric emp.ying (GE) in older p.tients with Typ. 2 diabetes mellitus. METHODS: One hundred and forty seven p.tients with Typ. 2 diabetes, of whom 140 had been hosp.talised, mean age 62.3 ± 8.0 years, HbA1c 9.1% ± 1.9%, treated with either oral hyp.glycemic drugs or insulin were studied. GE of a solid meal (scintigrap.y), autonomic nerve function, up.er gastrointestinal symp.oms, acute and chronic glycemic control were evaluated. Gastric emp.ying results were comp.red to a control range of hosp.talised p.tients who did not have diabetes. RESULTS: Gastric emp.ying was delayed (T50 > 85 min) in 17.7% p.tients. Mean gastric emp.ying was slower in females (T50 72.1 ± 72.1 min vs 56.9 ± 68.1 min, p.= 0.02) and in those rep.rting nausea (112.3 ± 67.3 vs 62.7 ± 70.0 min, p.p.= 0.02). There was no correlation between GE with age, body weight, duration of diabetes, neurop.thy, current glycemia or the total score for up.er gastrointestinal symp.oms. CONCLUSION: p.olonged GE occurs in about 20% of hosp.talised elderly p.tients with Typ. 2 diabetes when comp.red to hosp.talised p.tients who do not have diabetes. Female gender, nausea and early satiety areassociated with higher p.obability of delayed GE.

High-p.rmeance crosslinked p.MSp.thin-film comp.site membranes as sup.orts for CO_2 selective layer formation

In the develop.ent of the comp.site gas sep.ration membranes for p.st-combustion CO_2 cap.ure, little attention is focused on the op.imization of the membrane sup.orts, which satisfy the conditions of this technology. The p.imary requirements to the membrane sup.orts are concerned with their high CO_2 p.rmeance. In this work, the membrane sup.orts with desired characteristics were develop.d as high-p.rmeance gas sep.ration thin film comp.site(TFC) membranes with the thin defect-free layer from the crosslinked highly p.rmeable p.lymer, p.ly[1-(trimethylsilyl)-1-p.op.ne](p.MSp.. This layer is insoluble in chloroform and can be used as a gutter layer for the further dep.sition of the CO_2-selective materials from the organic solvents. Crosslinking of p.MSp.was p.rformed using p.lyethyleneimine(p.I) and p.ly(ethyleneglycol) diglycidyl ether(p.GDGE) as crosslinking agents. Op.imal concentrations of p.I in p.MSp.and p.GDGE in methanol were selected in order to diminish the undesirable effect on the final membrane gas transp.rt characteristics. The conditions of the kiss-coating technique for the dep.sition of the thin defect-free p.MSp.based layer, namely, comp.sition of the casting solution and the sp.ed of movement of the p.rous commercial microfiltration-grade sup.ort, were op.imized. The p.ocedure of p.st-treatment with alcohols and alcohol solutions was shown to be crucial for the imp.ovement of gas p.rmeance of the membranes with the crosslinked p.MSp.layer having thickness ranging within 1-2.5 μm. The claimed membranes showed the following characteristics: CO_2 p.rmeance is equal to 50—54 m^3(STp./(m^2 h bar)(18,500—20,000 Gp.), ideal CO_2/N_2 selectivity is 3.6-3.7, and their selective layers are insoluble in chloroform. Thus, the develop.d highp.rmeance TFC membranes are considered as a p.omising sup.orts for further modification by enhanced CO_2 selective layer formation.

Objective To evaluate the efficacy and safety of simultaneous administration of dutasteride,tadalafil and solifenacin in the treatment of benign p.ostatic hyp.rp.asia(Bp.)with overactive bladder symp.oms and lower urinary tract obstruction in p.eviously unsuccessfully treated *** p.tients in Group.A(n=97)received dutasteride 0.5 mg/day,tadalafil 2.5 mg/day,and solifenacin 2.5 mg/day;Group.B(n=95)received dutasteride 0.5 mg/day,tadalafil 5 mg/day,and solifenacin 5 mg/day;Group.C(n=103)received dutasteride 0.5 mg/day,tadalafil 20 mg/day,and solifenacin 10 mg/*** functional status of the lower urinary tract was assessed using the International p.ostate Symp.om Score(I-p.S),Overactive Bladder Questionnaire(OABq),International Index of Erectile Function(IIEF),and Male Sexual Health Questionnaire Ejaculatory Dysfunction(MSHQ-EjD)as well as *** The total score of the sexual function remained unchanged in Group.B of p.tients 81.3 p.ints vs.80.2 p.ints(p.0.05)according to MSHQ-EjD,61.4 p.ints vs.51.2 p.ints(p.0.05)according to IIEF *** total assessment of symp.oms of hyp.ractivity significantly decreased in Group.C according to OABq data after the 4th week of the study(17.5 p.ints vs.26.1 p.ints,p.0.05)and remained below the baseline until the end of the study(15.2 p.ints).Conclusions The simultaneous administration of standard doses of dutasteride,solifenacin,and tadalafil for 3 months is safe,effective,and can be recommended for p.tients with Bp. to reduce symp.oms of obstruction and hyp.ractivity of the bladder and maintain sexual function.
