Evaluation of the genotoxic and cytotoxic potentials is a part of the analysis process where plant products with therapeutic properties must be submitted for being safely employed, mainly through long time points, doannesia princeps, also known as ＂cotieira＂ has been used on popular medicine as laxative and several diseases treatment. Aiming to analyze genotoxic potential ofd. princeps leaves extract, Allium cepa assay and micronucleus test were employed. No genotoxic activity presented J. princeps leaves extract; however, extracts cytotoxic activity over A. cepa meristematic cells and mice marrow cells was observed. J. princeps leaves extract presented antiproliferative activity at concentrations and systems employed indicating their therapeutic potential for cellular cycle inhibition. Moreover, results showed that in regard to its mutagenic effect, usage of J. princeps leaves tea on popular medicine has been a safe procedure, as well.

Immunization with rPb27 Protects Mice from the Disruption of VEGF Signaling in Paracoccidioides brasiliensis Infection

VEGF （Vascular endothelial growth factor） signaling is critical for endothelial cell survival and maintenance of the vasculature. Deregulation of VEGF signaling contributes to the physiopathology of many diseases. However, the ways in which infection with Paracoccidioides brasiliensis affects VEGF signaling and the influence of immunization with rPb27 （recombinant protein Pb27） on VEGF signaling have not yet been studied. Animals were immunized with rPb27 and subsequently infected with a virulent strain of P. brasiliensis. The fungal load was evaluated by measuring CFU （colony-forming unit） and histology was performed to evaluate the inflammatory reaction. At the two time points analyzed, the PC （positive control） and TREAT （treated） animals had decreased levels of pulmonary VEGF compared to basal levels. However, in the immunized （Pb27） and treated mice （Pb27 ＋ TREAT）, VEGF expression remained unchanged after infection. In the case of VEGFR-2, the Pb27 and Pb27 ＋ TREAT groups showed increased levels of expression. Regarding the levels of the eNOS enzyme, only the Pb27 group did not reduce the expression levels relative to baseline. The immunization with rPb27 kept VEGF signaling, NO production and increased VEGFR-2 expression, after infection with P. brasiliensis. Thus, the authors infer that immunization with rPb27 protects mice from the disruption of VEGF signaling in Paracoccidioides brasiliensis infection.

Decommissioning of Mature Oil Fields and Artisanal Fisheries： The Case of Todosos santos Bay, Brazil

Consequences of decommissioning oil fields on artisanal fishing activities are still little known in the literature. This paper is intended to shed some light on a process of dismantling and sinking of oil and gas structures in shallow waters, with severe disturbing impacts on low income artisanal fishing activities. From a socio-economic perspective, the relationship of oil industry with local communities is described, with the main perceived problems pointed out in local fishermen leadership perspective. The notions of ＂damages＂ and ＂mitigation＂ used by the oil industry are discussed in connection to the expansion and dismantling of oil installations during the past 20 yrs. A comparative view of oil fields decommissioning in Europe and Brazil during the late 1990s suggests the need to review transparency and social commitment standards which have been far less prominent in this Brazilian case. The authors believe that the Brazilian oil industry has acquired a social and environmental debt towards the whole society, as far as it has been unable to establish a clear and effective process for decommissioning their oil installations within the artisanal fishing areas of the Todosos santos Bay. Furthermore, the discussion of fair and specific compensations has been avoided, which otherwise would be instrumental to regain local economic conditions found among fishermen just few decades ago.

Objective::Prenatal infection with the Zika virus (ZIKV) can lead to congenital Zika syndrome (CZS), characterized by microcephaly and brain injury. However, there are questions regarding the prevalence of microcephaly/CZS after the ZIKV outbreak in defined geographic areas. This study aimed to identify adverse outcomes in live births of fetuses exposed in utero to the ZIKV, compared to unexposed births, as well as maternal sociodemographic, delivery, and birth ***::Here, we conducted a cross-sectional observational study to investigate the characteristics of all live births in the city of Tangará da Serra, Mato Grosso, Brazil, in 2016, after the outbreak of ZIKV infection in late 2015. All live births of children to women residing in the municipality of Tangará da Serra between January 1 and December 31, 2016, were evaluated, and head circumference was measured at birth and after 24 hours. Children born with microcephaly or a maternal history of confirmed or suspected prenatal ZIKV infection were evaluated by a multidisciplinary team. The outcomes of the exposed and non-exposed children were compared. Prevalence ratios and their respective 95% confidence intervals were calculated for sociodemographic, delivery, and live birth ***::Of 1,441 live births, 106 (7.3%) were from mothers with confirmed or highly probable exposure to ZIKV. The prevalence of severe congenital microcephaly (41.7/10,000) in Tangará da Serra in 2016 was ten-fold higher than that in Latin America before ***::This study may serve as a model to investigate possible outbreaks of infections in a defined geographical space in the future.

The Lipid Accumulation Product (LAP) is a clinical marker of visceral obesity and has been proposed as a simple, inexpensive, and accurate tool to estimate cardiovascular risk and mortality. The aim of this study was to verify the association of LAP with anthropometric, biochemical, visceral adiposity index and IR in adults and the elderly. This single cross-section center clinical study, with experimental, analytical, primary, and observational design, included 210 participants. Anthropometric (Body Mass Index (BMI), Waist Circumference (WC), and Neck Circumference (NC)), LAP, Visceral Adipose Index (VAI), and biochemical parameters (fasting glycemia, insulinemia (to calculate the Homa-IR index), total cholesterol, LDL-c, HDL-c, and triglycerides) were evaluated. The results showed that by separating the sample into three groups (adequate BMI and WC, adequate BMI and elevated WC, and elevated BMI and WC), the group with high BMI and WC showed a high value of LAP and VAI compared to the other groups, with a significant difference. Still, the data show a positive and significant correlation when relating the LAP with VAI, HOMA-IR, BMI, WC, NC, total cholesterol, triglycerides, and Diastolic Blood Pressure. Intensive care units’ acquired muscle weakness is present in approximately 50% of the patients. Although active muscle training can attenuate weakness, a large proportion of critical patients cannot participate in any active mobilization. Neuromuscular electrical stimulation may be an alternative strategy to reverse muscle weakness. The objective of the study was to review the scientific publications on the use of neuromuscular electrical stimulation and its parameters and the main results in patients hospitalized in intensive care units. This is an integrative review surveying studies in online databases. The studies were selected from the following descriptors: neuromuscular electrical stimulation AND parameters AND intensive care units AND muscle weakness. The inclusion criteria included articles that addressed the topic of neuromuscular electrical stimulation and the parameters used in patients admitted to intensive care units, aged 18 years or older. Exclusion criteria were studies involving animals, case reports, letters to the editor and book chapters. The search comprised articles in the Portuguese, English and Spanish languages from January 2013 to March 2019. Of the 185 articles identified, nine met the eligibility criteria. The studies were evaluated assessing the level of evidence, and the relevant information was presented in the table and discussed. The parameters of the neuromuscular electrical stimulation employed in the studies showed positive results for the maintenance of strength and muscle mass. There was evidence of benefits in the local and systemic microcirculation, potentially mobilizing endothelial stem cells, to prevent atrophy, to reduce mechanical ventilation time and stay in intensive care unit;and when incorporated into the usual physiotherapy care, proved to be more effective than usual care. Its use is safe and viable in critically ill patients.

Contact Precaution: Acceptance of Health Professionals to Teaching Hospital

The adoption by health professionals to the practice of patient isolation is a decision that reduces patients’ risks of occupational exposure to potentially contaminated biological material and prevents nosocomial infections. Verify the compliance of health professionals to the practices of contact precautions in patients colonized by microorganisms in a teaching hospital. This was a prospective observational cohort study in a special-sized hospital located in southeastern Brazil. The observation of health professionals in delivery of health care to patients with contact precaution was performed regarding hand hygiene, use of overcoat, glove, and mask. We carried out 1502 observations involving the following professional categories: nursing technicians and/or nursing assistants (n = 1028;68.4%), nurses (n = 200;13.3%), physical therapists (n = 185;12.3%), and physicians (n = 89;5.9%), totaling 971.8 hours. Regarding the level of compliance of professionals to contact precaution practices with hand hygiene after the procedures, the use of overcoats, gloves, and masks, all were statistically significant (p Klebsiella pneumoniae was the most positive pathogen identified in laboratory tests. This study provided the mapping of the compliance of health professionals to the practices of contact precautions in order to support a safer management of patient care reducing the risks of Healthcare-Associated Infections (HAI). This study also showed that health professionals are using practice management to deal with their behavior to protect their health.

Atrial fibrillation is a cardiac arrhythmia of high prevalence in the population, especially in the elderly. Its main electrical characteristics are the interval between two successive irregular R waves, absence of P waves and presence of f waves between QRS complexes. The most common symptoms of atrial fibrillation are irregular palpitations associated with dyspnea, dizziness, feeling tired, fatigue and general malaise, but not all patients have any symptoms. The present report presents the history of an elderly patient who arrived at the hospital’s emergency department with irregular heart rhythm and palpitations. The patient’s symptoms, associated with the electrocardiogram results, indicated paroxysmal atrial fibrillation. Electrical cardioversion was performed, and after, cardiac ablation via the femoral vein at the hospital’s cardiology service. There were no complications during the procedure. As a routine imaging exam after ablation, control esophagogastroduodenoscopy was requested to verify that there was no formation of atrio-esophageal fistula developed by the invasive ablation procedure and electrocardiogram, which showed normal sinus rhythm. The patient remained in the cardiac intensive care unit for observation for 24 hours. After the electrical cardioversion and catheter ablation procedures, the patient improved his clinical picture of atrial fibrillation and was discharged after 24 hours of hospitalization. He received treatment to perform at home, to reduce acid reflux into the esophagus and to prevent thrombosis. He did not present pulmonary thromboembolism after hospital discharge. It is believed, therefore, that this form of treatment and management of paroxysmal atrial fibrillation is effective for the solution of the proposed problem and can also serve as a reference for other professionals within the cardiology service.

Casearia sylvestris Improved Cutaneous Burn Repair in Diabetic Rats

Objectives： The burn repair associated with diabetes complications showed considerable changes in the healing process and the development of alternative therapies to favor the repair is important. This study investigated the efficacy of the Casearia sylvestris in the burn repair in diabetic and non-diabetic rats. Methods： The animals were divided into four groups （n = 20）：（C） non-diabetics treated with carbopol gel; （G） non-diabetic treated with C. sylvestris extract in carbopol gel; （DM-C） diabetics treated with carbopol gel; （DM-G） diabetics treated with C. sylvestris extract in carbopol gel. Burn was induced using a metal plate （2 cm diameter/120 °C/20 sec）. Samples were collected on the 3rd, 7th, 14th and 21st days after the injury for histomorphometric analysis （inflammatory infiltrate, fibroblasts, blood vessels and collagen）, MPO （Myeloperoxidase） and NAG （N-Acetilglicosaminidase） dosage, hydroxyproline, glycosaminoglycans, TGF-β1, VEGF, collagen I and III. Key findings： C. sylvestris decreased the inflammatory process during the experimental period in diabetic and non-diabetic animals which has been demonstrated by MPO, NAG quantification, and TGF-β1 expression. C. sylvestris also increased blood vessels and decreased VEGF expression during the studied period in non-diabetic animals. Fibroplasia and collagenase were increased in all experimental periods in the DM-G group by means of de hydroxyproline quantification, collagen I, collagen III and glycosaminoglycans. Conclusions： C. sylvestris application can modulate favorably the time of inflammation and collagenous process in the repair of burn injuries in diabetic rats.
