Diagnosis of neurocysticercosis among patients with seizures in northern coastal districts of Andhra Pradesh, India

Objective: To report cases of neurocysticercosis(NCC) from three neighboring districts of Andhra Pradesh state in India where NCC burden was never explored ***: A total of 160 patients presenting with recent onset seizures were recruited from neurology, general medicine, and pediatric outpatient clinics of a local major tertiary care teaching hospital serving above districts during the period 2011–2014. Brain imaging was performed in all the above cases. A commercial immunoglobulin G-ELISA kit(sensitivity = 85%; specificity = 94%) was employed for the serological diagnosis of ***: The recruited patients presented with generalized, simple partial, and complex partial seizures(55%, 31.25% and 13.75% respectively). NCC was diagnosed in 44 of160(27.5%) seizure cases based on imaging characteristics, and a positive serum antibody ELISA. No association was detected between seropositivity with the number and location of the lesion(s) in the ***: The possible potentiality of NCC could be identified as an underlying cause of the recent onset of seizures in this region as explored in the present study. It is recommended that NCC should be suspected as one of the major differential in every recent onset seizure with or without a radio imaging supportive diagnosis, especially in areas endemic for taeniasis/cysticercosis.

A Cognitive Approach to the Trauma Narrative in Toni Morrison's A Mercy

This paper studies the trauma narrative of Toni Morrison's novel, A Mercy(2008), using a cognitive narratological framework. The authors, in this paper, describe the response the novel elicits from the reader. And the paper argues that multiple focalization, polychronic narration, and representation of inconsistent information enable Morrison to depict effectively the devastating effects of trauma—whether sexual, socioeconomic, or racial—on individual personality. Various characters of multi-ethnic origins narrate the novel and certain events are told repeatedly from different vantage points. Thus, the narrative calls forth continuous efforts on the part of the reader to process the complex and bewildering information emerging from the novel's storyworld. A cognitive approach to the study of the novel provides an understanding of the behavior of the traumatized and the impact of slavery on black people's consciousness and identity. Moreover, the approach gives insight into the manner in which the narrative engages the readers cognitively in such a way that it enables their comprehension of the relationship between individual trauma and social forces of injustice and oppression.

High performance of low voltage controlled ring oscillator with reverse body bias technology

In complementary metal oxide semiconductor （CMOS） nanoscalc technology, power dissipation is becoming important metric. In this work low leakage voltage controlled ring oscillator circuit system was proposed for critical communication systems with high oscillation frequency. An ideal approach has been presented with substrate biasing technique for reduction of power consumption. The simulation have been completed using cadence virtuoso 45 nm standard CMOS technology at room temperature 27~C with supply voltage Vc^d = 0.7 V. The simulation results suggest that voltage controlled ring oscillator has characterized with efficient low power voltage controlled oscillator （VCO） in term of minimum leakage power （1.23 nW） and maximum oscilla- tion frequency （4.76 GHz） with joint positive channel metal oxide semiconductor and negative channel metal oxide semiconductor （PMOS and NMOS） reverse sub- strate bias technique. PMOS, NMOS and joint reverse body bias techniques have been compared in the presented work.

Is There an Improvement in Patient Survival/Code Blue Activation after Training Based on Simulation (Basic Life Support—BLS) Based Practice of Cardiopulmonary Resuscitation?

Background and Aim: The only way to survive a sudden cardiac arrest is when the CPR is performed immediately after the arrest. The focus of the present research study is to assess the effectiveness of a pre- and post-simulation-based BLS training (BLS) and the outcome was measured on the basis of patient survival after the cardiac arrest. Study Design: This pre- and post-training BLS/CPR training study enrolled all nursing staff, all hospital residents, internees, throughout the hospital in a simulation-based BLS training as per the standards of American Heart Association (AHA), to make them respond to immediate resuscitation and code blue activation during the cardiac arrest within the hospital premises including ED, wards, ICUs, MRI, CT and all miscellaneous areas. The providers completed self-efficacy questionnaires as per the AHA protocol before being certified and were evaluated during the emergency in hospital cardiopulmonary arrest. Results: 296 nursing staff, 206 non-healthcare professionals, 143 residents, 212 internees, and 98 medical staff grade doctors completed the BLS training (total 955 hospital staff—providers) were graded for the response by pre- and post-training testing. In the course of pre-BLS training period out of the 250 cardiac arrest patients, 68 patients (27.2%) had ROSC, while after instituting the BLS training period, 143 individuals (40.86%) of the 350 patients who had cardiac arrest had ROSC (p 0.05). Conclusion: A simulation-based CPR and BLS training curriculum greatly improves patient outcome by reducing mortality and morbidity with improved subjectivity, self-efficiency along with the objective assessment of the performance scores during acute cardiac arrest in Emergency Cardiovascular Care (ECC).

Economics of Yield and Production of Alkaloid of <i>Withania somnifera</i>(L.) Dunal

The yield parameters and cost economics of Withania somnifera were studied using Integrated Nutrient Management (INM) treatments. Withania somnifera is cultivated in around 10,780 ha with a production of 8429 tones in India. The annual demand of this herb increased from 7028 tones (2001-2002) to 9127 tones (2004-2005). The field experiment was conducted at the research farm of Department of Plant Physiology, College of Agriculture, Jabalpur during Kharif season of 2008-2009 (1st year) and 2009-2010 (2nd year), whereas the laboratory work was carried out in Quality Laboratory, College of Agriculture, Jabalpur. The soil of experimental field was sandy loam in texture with pH 7.5, EC 0.18 dsm-1 having good drainage. Soil analysis revealed that available nitrogen was low (202.0 kg·ha-1) whereas available phosphorus (16.25 kg·ha-1) and potassium (236.0 kg·ha-1) were in the medium range. The present paper shows how to determine the economics of varying INM treatments. Cultivation of W. somnifera in India is gaining popularity among farmers;however, due to poor soil fertility, and costly chemical fertilizers and pesticides its production is not economical or profitable.

Disulfiram in the management of alcohol dependence: A comprehensive clinical review

Disulfiram remains a viable option as a treatment for alcohol dependence and has been shown in recent studies to be successful in treating patients with alcohol dependence in a manner that is superior to both naltrexone and acamprosate. It is also useful in dual diagnosis patients and those with co-morbid cocaine and alcohol dependence. Although disulfiram’s mechanism of action in alcohol dependence was long thought to be its effects as a psychological deterrent, more recent studies have uncovered potential anticraving effects as well. Recent reviews exhort to the importance of supervised disulfiram therapy in highlighting many of the potential and unique benefits of disulfiram. The present article will review the major clinical trials of disulfiram spanning nearly 60 years. It also discusses the usage of disulfiram across diverse populations along with monitoring for compliance and various adverse effects that may be encountered. The paper also reviews certain studies on long acting disulfiram therapy, recent comparative trials of disulfiram and its use in alcohol dependence. The review concludes with the role of disulfiram in the present day and long-term pharmacotherapy of alcohol dependence along with future research needs in this area.

A Comparative Study on Rat Intestinal Epithelial Cells and Resident Gut Bacteria (ii) Effect of Arsenite

Objective In order to use facultative gut bacteria as an alternate to animals for the initial gastrointestinal toxicity screening of heavy metals, a comparative study on rat intestinal epithelial cells and resident gut bacteria was undertaken. Methods in vitro growth rate of four gut bacteria, dehydrogenase （DHA） and esterase （EA） activity test, intestinal epithelial and bacterial cell membrane enzymes and in situ effect of arsenite were analysed. Results Growth profile of mixed resident population of gut bacteria and pure isolates of Escherichia coli, Pseudomonas sp., Lactobacillus sp., and Staphylococcus sp. revealed an arsenite （2-20 ppm） concentration-dependent inhibition. The viability pattern of epithelial cells also showed similar changes. DHA and EA tests revealed significant inhibition （40%-72%） with arsenite exposure of 5 and 10 ppm in isolated gut bacteria and epithelial cells. Decrease in membrane alkaline phosphatase and Ca^2＋-Mg^2＋-ATPase activities was in the range of 33%-55% in four bacteria at the arsenite exposure of 10 ppm, whereas it was 60%-65% in intestinal epithelial villus cells, in situ incubation of arsenite using intestinal loops also showed more or less similar changes in membrane enzymes of resident gut bacterial population and epithelial cells. Conclusion The results indicate that facultative gut bacteria can be used as suitable in vitro model for the preliminary screening of arsenical gastrointestinal cytotoxic effects.

In this paper,effects of lttice strain on dielectric properties of Sr_(1-x)Ca_(x)Bi_(2)Nb_(2)O_(9)(x=0.0-0.5 in steps of 0.1)ceramics are *** frequency dipolar polarization is observed to increase for 10%calcium addition and suppresses thereafter on higher calcium *** dielectric loss values for all doped samples were lower compared to undoped one at frequencies beyond 1 *** sample with x=0.1 shows room temperature dielectric constant in the range(130±12)over a wide frequency range from 50Hz to 1 MHz also minimum dielectric loss among all other sample for the same frequeny *** increase in Curie temperature(T_(c))by 50℃ is recorded in the present work by increasing 10%calcium content in SBN *** higher doped samples indicate increase in T_(c) by further higher *** addition,dc conductivity of all samples is measured for all samples and increase in calcium content is observed to generate more insulating compositions compared to undoped SBN with higher intrinsic activation energies.

Resilience Improves Neurocognition and Treatment Outcomes in Schizophrenia: A Hypothesis

There has been a revolutionary advance in the treatment and management of schizophrenia from a clinical aspect yet the social and functional outcomes remain poor. Cognitive function is impaired in schizophrenia and shows various domains of dysfunction like verbal memory, processing speed and working memory. It is also known to be a factor associated with poor outcome in schizophrenia. Resilience is a new concept psychobiological concept which is defined as individual’s ability to adapt swiftly to adverse life events and bounces back to normalcy. Resilience has genetic, neurobiological, neurochemical and psychological underpinnings. It is the ability to effectively deal with psychosocial stressors and appears to be one of the many factors associated with favourable outcomes in schizophrenia. Besides several neurobiological abnormalities associated with resilience, neucognitive functions are of particular interest. Persistent psychosocial stressors also lead to significant neurobiological changes which may be synergetic to poor outcome due to cognitive changes. Though there has been extensive research in the field of cognitive function in schizophrenia, the trajectory of its pathway of poor outcome remains undetermined. Resilience being a protective factor may be one of the psychobiological functions which modulate the effect of neurocognition on the outcome of schizophrenia. There has been some success with interventions aimed at improving cognitive function in schizophrenia whether pharmacological or non pharmacological. In this paper, we discuss a hypothesis that resilience may be a “linkage” between cognition and outcome. There is a need for interventions aimed at increasing resilience in patients with schizophrenia and we hypothesize giving evidence that this may in turn improve outcome and neurocognitive functioning in schizophrenia.
