OBj.CTIVE:To investigate the procedural outcomes and the long-term survival of patients undergoing transcatheter aortic valve implantation (TAVI) and compare study results of patients ≤ 80 years and patients > 80 years old.;METHODS:A total of 240 patients treated with TAVI were divided into two groups according to age ≤ 80 years (n = 105; 43.8%) and > 80 years (n = 135; 56.2%). The baseline characteristics and the procedural outcomes were compared between these two groups of patients.;RESULTS:With the exception of peripheral artery disease and hypercholesterolemia, which were more frequently observed in the older age group, baseline characteristics were comparable between groups. Complication rates did not differ significantly between patients ≤ 80 years and patients > 80 years. There were no differences in 30-day mortality rates between patients aged ≤ 80 years and patients > 80 years old (9.5% vs. 7.4%, respectively; P = 0.557). After a median follow-up of 28 months (interquartile range: 16-42 months), 50 (47.6%) patients aged ≤ 80 years died compared to 57 (42%) deaths in the group of patients > 80 years old (P = 0.404).;CONCLUSIONS:The results of the present single center study showed that age did not significantly impact the outcomes of TAVI.

Aim: We describe an approach to search for candidate genes for male infertility using the two human genome databases: the public University of California at Santa Cruz (UCSC) and private Celera databases which list known and predicted gene sequences and provide related information such as gene function, tissue expression, known mutations and single nucleotide polymorphisms (SNPs). Methods and Results: To demonstrate this in silico research, the following male infertility candidate genes were selected: (1) human BOULE, mutations of which may lead to germ cell arrest at the primary spermatocyte stage, (2) mutations of casein kinase 2 alpha genes which may cause globozoospermia, (3) DMR-N9 which is possibly involved in the spermatogenic defect of myotonic dystrophy and (4) several testes expressed genes at or near the breakpoints of a balanced translocation associated with hypospermatogenesis. We indicate how information derived from the human genome databases can be used to confirm these candidate genes may be pathogenic by studying RNA expression in tissue arrays using in situ hybridization and gene sequencing. Conclusion: The paper explains the new approach to discovering genetic causes of male infertility using information about the human genome.

Background:Cholera remains an important public health concern in developing countries including Kenya where 11,769 cases and 274 deaths were reported in 2009 according to the World Health Organization(WHO).This ecological study investigates the impact of various climatic,environmental,and demographic variables on the spatial distribution of cholera cases in ***:District-level data was gathered from Kenya’s Division of Disease Surveillance and Response,the Meteorological Department,and the National Bureau of *** data included the entire population of Kenya from 1999 to ***:Multivariate analyses showed that districts had an increased risk of cholera outbreaks when a greater proportion of the population lived more than five kilometers from a health facility(RR:1.025 per 1%increase;95%CI:1.010,1.039),bordered a body of water(RR:5.5;95%CI:2.472,12.404),experienced increased rainfall from October to December(RR:1.003 per 1 mm increase;95%CI:1.001,1.005),and experienced decreased rainfall from April to j.ne(RR:0.996 per 1 mm increase;95%CI:0.992,0.999).There was no detectable association between cholera and population density,poverty,availability of piped water,waste disposal methods,rainfall from j.nuary to March,or rainfall from j.ly to ***:Bordering a large body of water,lack of health facilities nearby,and changes in rainfall were significantly associated with an increased risk of cholera in Kenya.

A new approach for simulating the formation of a froth layer in a slurry bubble column is proposed. Froth is considered a separate phase, comprised of a mixture of gas, liquid, and solid. The simulation was carried out using commercial flow simulation software （FIRE v2014） for particle sizes of 60-150 μm at solid concentrations of 0-40vol%, and superficial gas velocities of 0.02-0.034m/s in a slurry bubble column with a hydraulic diameter of 0.2 m and height of 1.2 m. Modelling calculations were conducted using a Eulerian-Eulerian multiphase approach with k-ε turbulence. The population balance equations for bubble breakup, bubble coalescence rate, and the interfacial exchange of mass and momentum were included in the computational fluid dynamics code by writing subroutines in Fortran to track the number density of different bubble sizes. Flow structure, radial gas holdup, and Sauter mean bubble diameter distributions at different column heights were predicted in the pulp zone, while froth volume fraction and density were predicted in the froth zone. The model was validated using available experimental data, and the predicted and experimental results showed reasonable agreement. To demonstrate the effect of increasing solid concentration on the coalescence rate, a solid-effect multiplier in the coalescence effi- ciency equation was used. The solid-effect multiplier decreased with increasing slurry concentration, causing an increase in bubble coalescence efficiency. A slight decrease in the coalescence efficiency was also observed owing to increasing particle size, which led to a decrease in Sauter mean bubble diam- eter. The froth volume fraction increased with solid concentration. These results provide an improved understanding of the dynamics of slurry bubble reactors in the presence of hydrophilic particles.

Earth is the only habitable planet in the solar system and beyond in interstellar space for a distance that would take us at least 80,000 years to traverse at the speed of Voyager 1. Thus our home planet is “This Island Earth”. Here we use our Simple (engineering-type) Climate Model to calculate the change in global-mean near-surface air temperature from 1765 through the third millennium for historical emissions and two scenarios of future emissions of greenhouse gases: (1) a Reference scenario of unabated emissions, and (2) our Fair Plan scenario wherein emissions are phased out to zero from 2020 to 2100. The temperature change for the Reference cases increases to 5.2°C (9.4°F) in 2225 and remains there for at least 40 human generations. By design, the temperature change for the Fair Plan increases only to 2°C (3.6°F)—the limit adopted by the UN Framework Convention on Climate Change “to prevent dangerous anthropogenic interference with the climate system”—in 2082 and thereafter decreases through the remainder of the millennium. Accordingly, we need to adopt the Fair Plan to safeguard the climate of “This Island Earth”.

In this work, Bioactive-functionalized interpenetrating network (IPNs) hydrogel (BIOF-INPs) were prepared and investigated in vitro for the free radical detection/defense, therapeutic release as well as shear bond strength to dentine, ability to re-mineralize surface of the dentin after application of these bio-inspired materials using a biologically inspired mineralization process in vitro as well as investigating antimicrobial properties of the BIOF-INPs against S. aureous. The aim of this investigation was to evaluate the suitability and flexibility of the designer materials to act as an “in vitro” probe to gain insights into molecular origin of TMD and associated disorders.

Restorative materials in the new era aim to be “bioactive” and long-lasting. The purpose of the study was to design and evaluate novel chitosan hydrogels containing melatonin and/or propolis (antioxidant containing material), nystatin (antifungal), naproxen (pain relieve medication) and combinations thereof (chitosan-H-melatonin, chitosan-H-melatonin-naproxen, chitosan-H-propolis, chitosan-H-propolisnaproxen, chitosan-H-naproxen-propolis-melatonin, Chitosan/Propolis/Nystatin, Chitosan/Melatonin/Propolis/Nystatin, Chitosan/Propolis/BSA/Nystatin, Chitosan/Melatonin/BSA/Nystatin) as functional additive prototypes for further development of “dual function restorative materials”, to determine their effect on the dentin bond strength of a composite, to evaluate stability of the encapsulated antioxidants as well as evaluate antimicrobial properties of the selected group of “designer” functional materials. Materials and Methods: The above mentioned hydrogels were prepared by dispersion of the corresponding component in glycerol and acetic acid with the addition of chitosan gelling agent. The surface morphology (SEM), drug-polymer solid state interaction (FT-IR spectroscopy), released behaviours (physiological pH and also in acidic conditions), stability of the therapeutic agent-antioxidant-chitosan and the effect of the hydrogels on the shear bond strength of dentin were also evaluated. Results: The release of naproxen confers the added benefit of synergistic action of a functional therapeutic delivery when comparing the newly designed chitosan-based hydrogel restorative materials to the commercially available products alone. Neither the release of naproxen or the antioxidant stability was affected by storage over a 6-month period. The hydrogel formulations have a uniform distribution of drug content, homogenous texture and yellow colour (SEM study). All chitosan dentin treated hydrogels gave significantly

UNASSIGNED:To assess the characteristics and quality of cost utility analyses (CUA) related to otolaryngology within the CEA registry and to summarize their collective results.;UNASSIGNED:All cost-utility analyses published between 1976 and 2011 contained in the Cost-Effectiveness Analysis Registry (CEA Registry) were evaluated. Topics that fall within the care of an otolaryngologist were included in the review regardless of the presence of an otolaryngologist author. Potential associations between various study characteristics and CEA registry quality scores were evaluated using the Pearson product moment correlation coefficient.;UNASSIGNED:Sixty-one of 2913 (2.1%) total CUA publications screened were related to otolaryngology. Eighteen of 61 (29.5%) publications included an otolaryngologist as an author. Fourteen studies agreed on the cost effectiveness of at least unilateral cochlear implantation and six of seven (85.7%) studies demonstrated the cost effectiveness of continuous positive airway pressure (CPAP) for obstructive sleep apnea (OSA). Forty-six percent (28 of 61) of all manuscripts were published between 2008 and 2011. A more recent publication year was associated with a higher CEA registry quality score while the presence of an otolaryngologist author and j.urnal impact factor had no significant correlation with the quality of the CUA.;UNASSIGNED:Based on current evidence in the CEA registry, unilateral cochlear implantation for hearing loss and CPAP for OSA are both cost-effective therapeutic interventions. Although CUAs in otolaryngology have increased in quantity and improved in quality in more recent years, there is a relative lack of CUAs in otolaryngology in comparison to other subspecialties.
