Quantitative Assessment of Liver Fibrosis by Elastography in Patients with Chronic Liver Disease: A Cross-Sectional Study in Lomé (Togo)

Aim: To describe the two-dimensional elastographic profile according to the shearwave (2D-SWE) technique in patients with chronic liver disease in Lom. Materials and method: Cross-sectional, descriptive study conducted over seven month at the Autel dElie Clinic in Lom, from January to August 2022, on adult patients with chronic liver disease who underwent abdominal ultrasound coupled with two-dimensional elastography. Results: The sample size was 54 patients. The mean age of the patients was 33 12 years, with extremes of 18 and 66 years. Patients aged 30 years or less accounted for 48.1% (n = 26). All patients (n = 54) had at least one transaminase assay with a mean of 69.3 78.3 IU/l (AST) and 59.3 82.8 IU/l (ALT). There was no statistically significant association between the biological parameters and the presence of fibrosis. Viral liver disease was the main cause, accounting for 81.5% (n = 44) of cases, with no significant association with the degree of fibrosis. Ultrasound revealed a dysmorphic liver (57.4%;n = 31) and portal hypertension (18.5%, n = 10). Fibrosis stages F1, F2 and F4 accounted for (48.1%, n = 26), (24.1%, n = 13) and (13%, n = 7) of cases respectively. Liver dysmorphia was significantly associated with the presence of fibrosis (p = 0.012) and portal hypertension was significantly associated with the degree of fibrosis (p = 0.0063). Conclusion: Assessment of liver fibrosis in patients with chronic liver disease using 2D-SWE elastography is essential for patient follow-up.

Ballistic performance of titanium-based layered composites made using blended elemental powder metallurgy and hot isostatic pressing

Metal matrix composites tiles based on Ti-6Al-4V(Ti64) alloy, reinforced with 10, 20, and 40(vol%) of either TiC or TiB particles were made using press-and-sinter blended elemental powder metallurgy(BEPM) and then bonded together into 3-layer laminated plates using hot isostatic pressing(HIP). The laminates were ballistically tested and demonstrated superior performance. The microstructure and properties of the laminates were analyzed to determine the effect of the BEPM and HIP processing on the ballistic properties of the layered plates. The effect of porosity in sintered composites on further diffusion bonding of the plates during HIP is analyzed to understand the bonding features at the interfaces between different adjacent layers in the laminate. Exceptional ballistic performance of fabricated structures was explained by a significant reduction in the residual porosity of the BEPM products by their additional processing using HIP, which provides an unprecedented increase in the hardness of the layered composites. It is argued that the combination of the used two technologies, BEPM and HIP is principally complimentary for the materials in question with the abilities to solve the essential problems of each used individually.

Conformity Assessment of Lead Aprons Used in Conventional Radiology: A Multi-Centre Survey in Togo

Aim: Lead aprons are used to protect against scattered radiation from the patient during interventional procedures and certain special conventional radiological examinations. Given the importance of the role lead aprons are supposed to play in radiation protection, we propose to assess their conformity in medical imaging departments in public and religious hospitals in Togo. Materials and method: A multi-centre survey conducted from 26 november to 06 December 2021 in the radiology departments of public and religious health facilities in Togo. All aprons in use were included. The evaluation criteria were physical (visual), quantitative (radiographic) and qualitative (dosimetric). Results: We had registred 43 aprons among wich 27 (62.79%) leaded aprons were labelled non-compliant and 16 (37.21%) were labelled compliant. Of the aprons judged to be non-compliant, 70.37% were more than 10 years old and 96.30% showed defects on the radiographic images. The most common defects were vampire marks (18.64%), multiple folds (16.96%), cracks (16.96%), multiple cracks (15.25%), tears (8.47%), absence of lead (5.08%), holes (3.39%) and lead corrosion (1.69%). Defective aprons (62.96%) had at least two defects. The defects were of thoraco-abdomino-pelvic (74.07%), thoracic (14.82%) and abdomino-pelvic (11.11%) topography. For indirect exposure at 50 and 70 kilovolts, all the aprons had an attenuation factor greater than 90%. After dosimetric measurement, 13.95% of aprons had attenuation factors below 90% for indirect exposure at 100 kilovolts. Conclusion: The compliance of the leaded decks is trifactorial (physical, radiographic and dosimetric). However, there is no significant difference in X-ray attenuation capacity between defective and normal decks.

Post Episiotomy Morbidity among Parturient in Alex Ekwueme Federal University Teaching Hospital Abakaliki, Ebonyi State

Background: Episiotomy is a very common obstetric surgery, and it could be associated with serious complications. However, these complications largely are not noticed due to the shift of attention from mother to baby, after a successful delivery. Objective: To identify the morbidities associated with episiotomies and factors associated with such morbidities. Method: This was a questionnaire based cross sectional descriptive study among women who attended the Obstetric Department of Alex Ekwueme Federal University Teaching Hospital Abakaliki Ebonyi State, between 1st July and 31st nov, 2023. The study population consisted of parturients who had episiotomies in their previous confinements, attending either the antenatal clinic or the postnatal clinic at Alex Ekwueme Federal University Teaching Hospital Abakaliki Ebonyi State. Results: Macrosomia was the most common indication of episiotomy at a rate of 31.6%. The majority of episiotomies were performed on the parturients in their first confinement at a rate of 64.5% and the majority of repairs were performed by the Registrar at a rate of 65.0%. Informed consent was obtained from 45.79% of parturients while 54.21% were not informed before the episiotomy was administered. Only 55.3% of the parturient received analgesia before episiotomy was administered. The majority of the parturients had their episiotomy repaired between 10 - 15 minutes at a rate of 40.3%. Postoperative pain at the rate of 44.5% was the most common complication and dyspareunia as a form of sexual complication was the commonest at a rate of 31.3%. Conclusion: The parturient needs to be properly counselled before administration of episiotomy and adequate analgesia should be given, as episiotomy is a surgical procedure. Proper training of health workers on both the technique of administering and repairing episiotomy is important. Restrictive use of routine episiotomy in primigravidae is advised to reduce the rate of episiotomy.

Interpretation of Geological and Gravity Data from the Bamiléké Plateau (West-Cameroon): Implication for the Understanding of Its Underground Lithotectonic Geometry

The Bamiléké Plateau represents a key domain in the understanding of the geodynamics associated with the central Cameroon shear. The study aimed to highlight the subsurface architecture of the plateaus basement setting with focus on high potential areas for hydrogeological and mining development projects. To this end, geological field observations were carried out. Since the structures sought were near-surface, a separation approach based on the upward continuation method was applied to the Bouguer anomaly grid. A set of processing techniques, including vertical derivative or DZ, analytical signal or SA and categorization of gravity signatures, was applied to generate the residual map. The synthesis geological model, obtained from analysis and interpretation of the various transformed maps and 2.5D modeling of two gravity profiles P1 and P2 highlights the following features: 1) intrusions of steep-sided granitic batholiths from Dschang to Bandjoun (profile P1), increasing in width from nW (Dschang) to SE (Bandjoun);2) larger volume batholiths with moderate sides located at Bafang and Bangangté (profile P1). These plutonic massifs were weakened by brittle deformation, which favored the emplacement of phonolite or anorthosite dykes within them. The emplacement of these dykes was accompanied by compressional faults with high dip between Dschang and Bandjoun and extensional faults with medium dip between Bafang and Bangangté. These fault zones (trending n85E to n95E) are ideal for hydrogeological investigations in a basement setting, as well as a series of dyke networks that could potentially be preferred zones for the circulation and accumulation of useful substances. The resulting geological sections P1 and P2 highlight the influence of granitic intrusions in the geological system of the study area, as well as the structural control associated with the various dyke intrusions. This work presents a different approach to twin primes, an approach from the perspective of the Tesla numbers and gives a refresh and new observation of twin primes that could lead us to an answer to the Twin Prime Conjecture problem. We expose a peculiar relation between twin primes and the generation of prime numbers with Tesla numbers. Tesla numbers seem to be present in so many domains like time, vibration and frequency [1], and the space between twin primes is not the exception. Let us say that twin primes are more than just prime numbers plus 2 or minus 2, and Tesla numbers are more involved with twin primes than we think, and hopefully, this approach give us a better understanding of the distribution of the twin pairs.
