Quantitative Assessment of Liver Fibrosis by Elastography in Patients with Chronic Liver Disease: A Cross-Sectional Study in Lomé (Togo)

Aim: To describe the two-dimensional elastographic profile according to the Shearwave (2D-SWE) technique in patients with chronic liver disease in Lom. Materials and method: Cross-sectional, descriptive study conducted over seven month at the Autel dElie Clinic in Lom, from January to August 2022, on adult patients with chronic liver disease who underwent abdominal ultrasound coupled with two-dimensional elastography. Results: The sample size was 54 patients. The mean age of the patients was 33 12 years, with extremes of 18 and 66 years. Patients aged 30 years or less accounted for 48.1% (n = 26). All patients (n = 54) had at least one transaminase assay with a mean of 69.3 78.3 IU/l (AST) and 59.3 82.8 IU/l (ALT). There was no statistically significant association between the biological parameters and the presence of fibrosis. Viral liver disease was the main cause, accounting for 81.5% (n = 44) of cases, with no significant association with the degree of fibrosis. Ultrasound revealed a dysmorphic liver (57.4%;n = 31) and portal hypertension (18.5%, n = 10). Fibrosis stages F1, F2 and F4 accounted for (48.1%, n = 26), (24.1%, n = 13) and (13%, n = 7) of cases respectively. Liver dysmorphia was significantly associated with the presence of fibrosis (p = 0.012) and portal hypertension was significantly associated with the degree of fibrosis (p = 0.0063). Conclusion: Assessment of liver fibrosis in patients with chronic liver disease using 2D-SWE elastography is essential for patient follow-up.

One of the most important parameters in meteorology is the mean wind proﬁle in the tropical cyclone boundary *** vertical proﬁle of wind speed and wind direction were measured during the period of the Nisarga cyclone from May 31st,2020,to June 5th,2020,using the newly installed Phased Array Doppler Sodar system at the Center for Space and Atmospheric Science(CSAS),Sanjay Ghodawat University,Kolhapur(16.74◦N,74.37◦E;near India's western coast).Our analysis revealed that the maximum mean wind speed was 17 m/s on June 3,2020,at 10:00 *** also shows the change in wind direction from southwest to southeast on June 2 and 3,*** high-resolution reanalysis data in the domain,0–25◦N,65–110◦E,during the period from May 31st to June 5th,2020,revealed the variation of the atmospheric pressure of the Nisarga cyclone from 1000 to 1008 hPa,sea surface temperature(SST)between 30◦C and 31◦C,outgoing longwave radiation(OLR)between 100 and 240 Wm-2,wind speed between 3 and 15 m/s,and low values of vertical wind shear(VWS)were observed to the north of Nisarga *** observations may provide more insights for the study of boundary layer turbulence during cyclonic activities.

This study aims to investigate the abundance of AMF according to soil properties and altitudes in different cacao plantations of Cameroon. Physico-chemical analyses were made on soil samples collected from three agro-ecological zones. Soil samples were also used to evaluate directly the AMF abundance following the various altitudes and after trapping by sorghum plant. The results showed that soil properties, AMF spore abundances and colonization fluctuated significantly at different altitudes. The most represented texture was sandy loam. The bimodal zone presented a homogeneous texture (sandy loam) in all its localities. Cacao soil chemical characteristics showed that, the highest nitrogen rate (0.47%;p 0.05, Scott-Knott test) was recorded at Melong in a monomodal zone while Tonga in the Western highlands displayed the lowest rate (0.13%). Soil P concentration was significantly high in monomodal zones (Mbanga and Melong). Soil pH level indicated that the soil from Tonga in the Western highlands was neutral (pH = 6.67), and soils of other localities under study were acidic with the lowest (4.75) pH level recorded at Melong in a monomodal zone. In soil samples, the highest spore density (1.03 spores/g soil) was observed at Ntui in Bimodal zone, while the lowest spore density (0.26 spores/g soil) was observed at Bafang in the Western highlands. Root colonization showed that the sample from Bokito in a bimodal zone displayed the best frequency of mycorrhization (86.11%) while the sample from Bafang in the Western highlands recorded the lowest (27.11%). The PCA analysis highlighted that available phosphorus, pH and altitude all strongly correlated with AMF root colonization ability and can be used as a predictor of AMF colonization ability in cacao rhizosphere.

The transformation of uterine fibroids is common in relation to their development. Giant forms of cystic degeneration are rare. They raise diagnostic difficulties with other pelvic tumors, such as ovarian tumors and leiomyosarcomas. Magnetic resonance imaging specifies the original organ, the volume and the main relationships of fibromyoma with adjacent structures. The diagnosis of certainty is based on laparotomy coupled with histology. The authors illustrate these difficulties by observing a giant cystic degenerative fibroma in a 26-year-old G1P1 woman in the postpartum period.

Proximate analyses,in vitro antioxidant activity and GC-MS analyses of fermented Pentaclethra macrophylla seeds

The present study assessed the proximate composition,in vitro antioxidant activity and GC-MS analyses of fermented Pentaclethra macrophylla seeds extract(FPMSE).The in vitro antioxidant activity was assessed using 2,2-diphenyl-1-picrylhydrazl(DPPH)while Gas Chromatography-Mass Spectrophotometry(GC-MS)technique was used to identify the volatile compounds in *** revealed a dose dependent percentage inhibition of the extract on DPPH *** composition of FPMSE constitutes respectively;ash(0.33±0.12%),protein(2.19±0.08%),fats and oil(80.60±0.00%),carbohydrate(16.88±0.00%)while moisture and fibre were not ***,the pulverised sample constitute respectively;ash(1.27±0.58%),protein(10.51±0.60%),moisture(8.27±0.12%),fats and oil(32.33±0.12%),fibre(7.27±0.12%)and carbohydrate(40.35±0.00%).GC-MS analyses revealed fourteen(14)fatty acids including Octadecanoic acid(1.95%),Benzyloxymethylimine(43.89%),2-Allylpent-4-enoic acid benzyl ester(26.25%),Hexadecanoic acid,methyl ester(1.09%),Oleic Acid(0.25%),9,12-Octadecadienoic acid,methyl ester(Omega-6-fatty acid)(1.52%).The bioactive compounds identified in FPMSE could contribute to the pharmacological properties of *** seeds and hence,could be of considerable interest for the development of new drugs.

Obesity is a significant and escalating health issue both in Brazil and globally, with over 650 million overweight adults worldwide. The treatment of obesity can be performed clinically, endoscopically or surgically;surgical treatment proves to be safe and more effective in terms of weight loss and long-term maintenance. Objective: This study aimed to monitor the progress of weight loss and comorbidity control in patients undergoing sleeve gastrectomy with duodenal bipartition. Methods: This pilot project involved 8 patients divided into 2 arms. In the first arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenoileal transit bipartition (S-RYDITB), while in the second arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenojejunal transit bipartition (S-RYDJTB). Both procedures involved Roux-en-Y reconstruction without duodenal exclusion. In S-RYDITB, the duodenal-ileal anastomosis was performed 300 cm from the ileocecal valve (ICV), creating a 250 cm common channel and a 50 cm alimentary channel. In S-RYDJTB, a biliopancreatic loop was created 200 cm from the angle of Treitz, with a 1 m alimentary channel. Results: Five patients underwent the procedures, with one undergoing S-RYDITB and four undergoing S-RYDJTB. No adverse events such as hospitalizations, readmissions, reoperations, fistulas, bleeding, pulmonary embolism, diarrhea, dumping syndrome, or hypoglycemia occurred during the study period. The mean length of hospital stay was 2 days. The average BMI decreased from 37.27 kg/m2 preoperatively to 29.48 kg/m2 after 6 months. The significant percentage of weight loss was 21.22%, with excess weight loss of 63.6%. Ninety-five percent remission of comorbidities, including hypercholesterolemia, hypertriglyceridemia, diabetes, hypertension, steatosis, and pre-diabetes. L-Arginine Supplementation Mitigates Dichlorvos-Induced Haematocardiotoxicity, and Oxidative Stress in Male Wistar Rats

Due to its toxicity, dichlorvos—a common organophosphate pesticide—poses significant risks to human health. This study utilized male Wistar rats to explore the potential protective effects of L-arginine supplementation against dichlorvos-induced toxicity, focusing on cardiotoxicity, haematotoxicity and oxidative stress. The rats were divided into four groups: Control, L-arginine (L), Dichlorvos (D), and L-arginine + Dichlorvos (L + D). Dichlorvos was administered to the D group, L-arginine (100 mg/kg) to the L group, and both L-arginine and dichlorvos to the L + D group. The study evaluated various parameters, including cardiovascular, oxidative stress markers, and haematological indices. Significant changes in haematological parameters such as haemoglobin (Hb), haematocrit (HCT), and red blood cell count (RBC) indicated haematotoxicity after dichlorvos administration. Additionally, elevated cardiac markers, including lactate dehydrogenase (LDH) and creatine kinase-MB (CK-MB), suggested cardiotoxic effects. Exposure to dichlorvos also resulted in decreased antioxidant enzyme levels and increased oxidative stress indicators like malondialdehyde (MDA). Remarkably, L-arginine supplementation mitigated the damage caused by dichlorvos. It normalized the altered haematological parameters, demonstrating its protective effect against haematotoxicity. The rise in cardiac markers was reduced with L-arginine supplementation, indicating protection against cardiotoxicity. Moreover, L-arginine significantly decreased oxidative stress, as evidenced by lower MDA levels and restored antioxidant enzyme activity. In conclusion, L-arginine supplementation in male Wistar rats showed promising protective effects against dichlorvos-induced cardiotoxicity, haematotoxicity and oxidative stress. This suggests that L-arginine may offer a beneficial intervention to mitigate the adverse effects of dichlorvos on blood and heart health, paving the way for potential treatments for pesticide poison

Congenital radio-ulnar synostosis (CRUS) is a rare skeletal malformation of the upper limb and the most common congenital functional disorder of the elbow joint, causing limitation in forearm rotational movements, which may lead to difficulties with some activities of daily living. We reported a 4-year-old girl with congenital bilateral proximal radio-ulnar synostosis who presented with functional discomfort due to limitation of the prono-supination movements of the forearms and abnormal gestures when handling objects. She has clinical and radiological features of congenital radio-ulnar synostosis (CRUS). However, the parents declined corrective surgery because of wrong perception of the condition to be normal. This case highlighted the poor health seeking behavior and wrong illness perception prevalent in the developing countries.

An Appraisal of the Major National Dialogue as a Panacea to the Anglophone Crisis in Cameroon
