Background Obesity is a multifactorial chronic disease with a high,increasing worldwide *** causes account for 7%of the cases in children with extreme *** sources This narrative review was conducted by searching for papers published in the PubMed/MEDLINE,Embase and SciELO databases and included 161 *** search used the following search terms:"obesity","obesity and genetics","leptin","Prader-Willi syndrome",and"melanocortins".The types of studies included were systematic reviews,clinical trials,prospective cohort studies,cross-sectional and prospective studies,narrative reviews,and case *** The leptin-melanocortin pathway is primarily responsible for the regulation of appetite and body ***,several important aspects of the pathophysiology of obesity remain *** causes of obesity can be grouped into syndromic,monogenic,and polygenic causes and should be assessed in children with extreme obesity before the age of 5 years,hyperphagia,or a family history of extreme obesity.A microarray study,an analysis of the melanocortin type 4 receptor gene mutations and leptin levels should be performed for this *** are three therapeutic levels:lifestyle modifications,pharmacological treatment,and bariatric *** Genetic study technologies are in constant development;however,we are still far from having a personalized approach to genetic causes of obesity.A significant proportion of the affected individuals are associated with genetic causes;however,there are still barriers to its approach,as it continues to be underdiagnosed.

针对空天地一体化网络信息物理系统模型复杂、很难获得网络拓扑先验知识和模型化假设的特点,研究其基于深度强化学习的垂直切换策略。首先,综合考虑系统稳定性、切换开销和网络使用成本约束,将垂直切换策略问题建模为约束马尔可夫决策过程(cMDP),并给出保证可行解存在的充分条件;其次,提出约束-近端策略优化(cPPO)算法解决该问题,并在基站侧引入分布式强化学习机制加速训练收敛。相较于基准策略,仿真验证了所提垂直切换策略的优越性和有效性。

One of the most important parameters in meteorology is the mean wind proﬁle in the tropical cyclone boundary *** vertical proﬁle of wind speed and wind direction were measured during the period of the Nisarga cyclone from May 31st,2020,to June 5th,2020,using the newly installed Phased Array Doppler Sodar system at the center for Space and Atmospheric Science(cSAS),Sanjay Ghodawat University,Kolhapur(16.74◦N,74.37◦E;near India's western coast).Our analysis revealed that the maximum mean wind speed was 17 m/s on June 3,2020,at 10:00 *** also shows the change in wind direction from southwest to southeast on June 2 and 3,*** high-resolution reanalysis data in the domain,0–25◦N,65–110◦E,during the period from May 31st to June 5th,2020,revealed the variation of the atmospheric pressure of the Nisarga cyclone from 1000 to 1008 hPa,sea surface temperature(SST)between 30◦c and 31◦c,outgoing longwave radiation(OLR)between 100 and 240 Wm-2,wind speed between 3 and 15 m/s,and low values of vertical wind shear(VWS)were observed to the north of Nisarga *** observations may provide more insights for the study of boundary layer turbulence during cyclonic activities.

The increasing global environmental awareness, evidenced by recent worldwide calls for control of climate change and greenhouse emissions, has placed significant new technical mandates for automotives to improve engine efficiency, which is directly related to the production of carbon dioxide, a major greenhouse gas. Reduction of parasitic losses of the vehicle, powertrain and the engine systems is a key component of energy conservation. For engine efficiency improvement, various approaches include improvements in advanced combustion systems, component system design and handling—such as down-sizing, boosting, and electrification—as well as waste heat recovery systems etc. Among these approaches, engine friction reduction is a key and relatively cost-effective approach, which has been receiving significant attention from tribologists and lubricant-lubrication engineers alike. In this paper, the fundamentals of friction specific to the environments of engine components tribology are reviewed, together with discussions on the impact of developing vehicle powertrain technologies, surface and material technologies, as well as lubricant and additive technologies on promises of continuing friction and wear reduction trends. The international accords on climate change require further gains in fuel efficiency and energy sustainability from all industry sectors including those in the automotive and the broader internal combustion engine industries, and the latter encompass off-highway, power generation, marine, and rail industries as well. This paper focsuses on friction reduction in mainly automotive engines, however. The paper starts with a clarification of the common descriptors of mechanical losses and friction in the engine, followed by the topic of lubrication fundamentals such as lubrication regimes. Then the lubrication of the contacting surfaces in each of the major engine subsystems is discussed in turn. These subsystems include the piston assembly: ring-pack/liner,

AIM： To examine the psychological impact of chronic hepatitis c （cHc） diagnosis in a large cohort of cHc patients as compared with other stressful life events and chronic diseases carrying a risk of life-threatening complications. METHODS： One hundred and eighty-five outpatients with compensated cHc were asked to self-grade, using a 100-mm visual analogue scale （VAS）, the degree of stress caused by the learning of cHc diagnosis and the perceived severity of their disease. Diagnosis-related stress was compared to four other stressful life events and perceived cHc severity was compared to four other common chronic diseases. RESULTS： Learning of cHc diagnosis was considered a major stressful event （mean ± SD scores： 72±25）, significantly less than death of a loved-one （89±13, P〈0.0001） and divorce （78 ± 23, P〈0.007）, but more than job dismissal （68 ± 30, P〈 0.04） and home removal （26±24, P〈 0.0001）. cHc was considered a severe disease （74± 19）, after AIDS （94±08, P〈 0.001） and cancer （91± 11, P〈 0.001）, but before diabetes （66±23, P〈0.001） and hypertension （62±20, P〈0.001）. Perceived cHc severity was not related to the actual severity of liver disease, assessed according to Metavir fibrosis score. In multivariate analysis, diagnosisrelated stress was related to perceived disease severity （P〈0.001）, trait anxiety （P〈 0.001） and infection through blood transfusion （P〈 0.001）. cONcLUSION： Our results show the considerable psychological and emotional burden that a diagnosis of cHc represents, even in the absence of significant liver disease. They should be taken into account when announcing a diagnosis of cHc in order to reduce its negative effects.

Network coding has been proved to be an effective technique in improving the performance of data broadcast systems because clients requesting different data items can be served simultaneously in one broadcast. Previous studies showed that its efficiency is highly related to the content of clients' cache. However, existing data broadcast systems do not take network coding information into account when making cache replacement decisions. In this paper, we propose two networks coding-aware cache replacement policies called DLRU and DLRU-cP to supplement network coding assisted data broadcast in on-demand broadcast environments. In DLRU, both data access and decoding contribution are taken into account to make replacement decisions. DLRU-cP is based on DLRU but allows clients to retrieve decodable data items that have not been requested yet. The performance gain of our proposed cache replacement policies over traditional cache replacement policy is shown in the simulation results, which demonstrate conclusively that the proposed policies can effectively reduce the overall response time.
