Monogenic f.atures of.urolithiasis: A comprehensive review

Objective: Urolithiasis f.rmation has been attributed to environmental and dietary f.ctors. However, evidence is accumulating that genetic background can contribute to urolithiasis f.rmation. Advancements in the identif.cation of.monogenic causes using high-throughput sequencing technologies have shown that urolithiasis has a strong heritable ***: This review describes monogenic f.ctors implicated in a genetic predisposition to urolithiasis. Peer-reviewed journals were evaluated by a PubMed search until July 2023 to summarize disorders associated with monogenic traits, and discuss clinical implications of.identif.cation of.patients genetically susceptible to urolithiasis ***: Given that more than 80% of.urolithiases cases are associated with calcium accumulation, studies have f.cused mainly on monogenetic contributors to hypercalciuric urolithiases, leading to the identif.cation of.receptors, channels, and transporters involved in the regulation of.calcium renal tubular reabsorption. Nevertheless, available candidate genes and linkage methods have a low resolution f.r evaluation of.the ef.ects of.genetic components versus those of.environmental, dietary, and hormonal f.ctors, and genotypes remain undetermined in the majority of.urolithiasis ***: The pathophysiology underlying urolithiasis f.rmation is complex and multif.ctorial, but evidence strongly suggests the existence of.numerous monogenic causes of.urolithiasis in humans.

Dif.erentiation of.wild boar and domestic pig populations based on the f.equency of.chromosomes carrying endogenous retroviruses

Analysis of.the f.equencies of.chromosomes carrying various classes of.porcine endogenous retroviruses (PERVs) and combinations of.these classes was perf.rmed in the swine species Sus scrof. L. by using maps constructed in two principal component coordinates. f.ur population clusters can be recognized in the maps. Cluster 1 is f.rmed by wild boars,cluster 2 by domestic meat breeds, cluster 3 by meat-and-lard (universal) breeds, and cluster 4 by miniature pigs. The maps indicate that modern domesticated swine meat breeds are the closest to the wild type. Meat-and-lard domestic swine breeds are more distant f.om wild boars, and miniature pigs are diverged the most. The maps showed that microevolution processes associated with PERV carriership f.equency had two basic dimensions, or vectors: the vector of.f.t deposition variation and the “minus” selection vector (determination of.commercial traits). Thus, PERVs may cause variation in pig physiology.

The Burden of.Undiagnosed Hypertension and Associated Risk f.ctors among Adults in a Rural Community in Imo Sate, South-East Nigeria

Background: Cardiovascular diseases are the leading cause of.death globally and hypertension is a major contributor to this burden. Many people with hypertension have poorly controlled blood pressure and up to half.of.the adults with hypertension are unaware of.their hypertensive status due to f.ctors that bother on poor management and poor screening approaches. The implication is that people who have poor access to healthcare especially those in the rural communities are at increased risk of.cardiovascular complications and all-cause mortality. Unf.rtunately, not much has been done to ascertain the burden of.undiagnosed hypertension and associated risk f.ctors in rural communities in Nigeria. Methods: We conducted a community-based cross-sectional study in a rural community in Imo State, Nigeria, on burden of.undiagnosed hypertension with participants recruited via a multi-stage sampling method. An interviewer-administered questionnaire was used, and standardized instruments were applied to obtain, process and analyze the data. Tests of.association between the independent variables and outcome were conducted using logistic regression. P-value of.Results: A total of.380 adults participated in the study. The mean age was 44.2 years. The prevalence of.undiagnosed hypertension was 35.8%. Logistic regression revealed that age, with the respondents in the age groups 26 - 35 years (OR = 10.647, 1.910 - 59.345, p-value = 0.007), 36 - 45 (OR = 3.680, 1.263 - 10.723, p-value = 0.017), 46 - 55 years (OR = 2.737, 1.114 - 6.727, p-value = 0.039), 56 - 65 years old (OR = 3.384, 1.610 - 7.115, p-value = 0.001);and being married (OR = 3.846, 1.118 - 13.233, p-value = 0.033), were independent risk f.ctors f.r undiagnosed hypertension. Conclusion: The prevalence of.undiagnosed hypertension in the rural population of.South-East Nigeria is high. Younger age (26 - 35 years) had the highest odds of.risk f.r occurrence of.hypertension. This research work relates the surf.ce of.a square and the area circumscribed by a circle, resulting in a value called Nikola Tesla constant. This constant starts with the calculation of.the areas of.the square and the inscribed circle, giving a ratio of.9/40 and f.om which a residual area of.the area proportions of.the geometric f.gures described is obtained. Plotting smooth curves, particularly those in round shapes, can be represented ef.iciently with the use of.Nikola Tesla constant, reducing complex mathematical calculus.

Objective: This case report aimed to demonstrate a possible neuromuscular ef.ect of.Latarjet nerves transection or truncal vagotomy, in association with sleeve gastrectomy plus antrojejunostomy, in order to reproduce a Roux-en-Y gastric bypass (RYGB) mechanistic principles, in patients with previous Sleeve Gastrectomy (SG) who had had weight regain, with or without concomitant gastroesophageal ref.ux disease (GERD). Background: Sleeve gastrectomy (SG) is one of.the most f.equently perf.rmed bariatric operations worldwide. Nevertheless, weight regain and gastroesophageal ref.ux disease (GERD) have been consistently demonstrated, in association with this technique, which may require a revisional procedure. RYGB is an option in such a situation but, implies in gastrointestinal exclusions, which represents a shortcoming of.this revision. Surpassing this inconvenient would be of.great value f.r the patients. Methods: We describe herein two cases of.SG revision f.r weight regain and GERD, with a f.llow-up of.one year. Gastroesophageal ref.ux disease was evaluated by validated questionnaire, upper endoscopy, seriography, high resolution manometry (HRM) and impedance pHmetry (I-pHmetry), in the pre and postoperative periods. A re-Sleeve Gastrectomy with antrojejunal anastomosis was done in both cases, af.er inf.rmed consents. The Latarjet nerves were transected in one case, due to a bleeding in the lef. gastric vessels and a truncal vagotomy was required in the other, to appropriately treat an associated hiatal hernia. Results: In the postoperative evaluation it was observed a pyloric spasm in both cases, during seriography and endoscopy, kept until the one-year f.llow-up. There was unidirectional contrast f.ow to the gastrointestinal anastomosis, f.lling the jejunal limb, in radiologic contrast study. No contrast passed through the pylorus. Nonetheless, the duodenum was kept endoscopically accessible. In the one-year evaluation, weight loss was adequate and GERD resolution

Quantitative Assessment of.Liver f.brosis by Elastography in Patients with Chronic Liver Disease: A Cross-Sectional Study in Lomé (Togo)

Aim: To describe the two-dimensional elastographic prof.le according to the Shearwave (2D-SWE) technique in patients with chronic liver disease in Lom. Materials and method: Cross-sectional, descriptive study conducted over seven month at the Autel dElie Clinic in Lom, f.om January to August 2022, on adult patients with chronic liver disease who underwent abdominal ultrasound coupled with two-dimensional elastography. Results: The sample size was 54 patients. The mean age of.the patients was 33 12 years, with extremes of.18 and 66 years. Patients aged 30 years or less accounted f.r 48.1% (n = 26). All patients (n = 54) had at least one transaminase assay with a mean of.69.3 78.3 IU/l (AST) and 59.3 82.8 IU/l (ALT). There was no statistically signif.cant association between the biological parameters and the presence of.f.brosis. Viral liver disease was the main cause, accounting f.r 81.5% (n = 44) of.cases, with no signif.cant association with the degree of.f.brosis. Ultrasound revealed a dysmorphic liver (57.4%;n = 31) and portal hypertension (18.5%, n = 10). f.brosis stages f., f. and f. accounted f.r (48.1%, n = 26), (24.1%, n = 13) and (13%, n = 7) of.cases respectively. Liver dysmorphia was signif.cantly associated with the presence of.f.brosis (p = 0.012) and portal hypertension was signif.cantly associated with the degree of.f.brosis (p = 0.0063). Conclusion: Assessment of.liver f.brosis in patients with chronic liver disease using 2D-SWE elastography is essential f.r patient f.llow-up.
