Balloon valvuloplasty as destination therapy in elderly with severe aortic stenosis： a cardiac catheterization study

Background In the current era of transcatheter aortic valve replacement, there is renewed interest in balloon aortic valvuloplasty （BAV） and invasive hemodynarnic evaluation of aortic stenosis （AS）. The current report aimed to study the invasive hemodynamics of se- vere AS patients treated with BAV as destination therapy and to identify factors associated with better hemodynamic outcome and prognosis. Methods From 2009 to 2012, 63 high risk elderly patients were treated with BAV as destination therapy for symptomatic severe AS and were all prospectively included in the study. Their hemodynamies were invasively evaluated during catheterization, pre- and post-BAV at the same session. All Post-BAV patients were regularly followed-up. Results The patients （82 ~ 6 years, 52% male） had post-BAV aortic valve area index （AVAi） significantly increased and mean pressure gradient （MPG） significantly reduced. During the follow-up of 0.9 （maximum 3.3） years, those with post-BAV AVAi 〈 0.6 cm2/m2 compared with the AVAi _〉 0.6 cm2/m2 group had significantly higher mortality （60% vs. 28%, log-rank P = 0.02）, even after adjusting for age, gender, atrial fibrillation, chronic kidney disease, diabetes mellitus, coronary artery disease and EuroSCORE [HR： 5.58, 95% confidence interval （CI）： 1.62-19.20, P = 0.006]. The only independent predictor of moderate AS post-BAV was the pre-BAV AVAi increase by 0.1cm^2/m^2 （OR： 3.81, 95% CI： 1.33-10.89, P = 0.01）. Pre-BAV AVAi 〉 0.39 cm^2/m^2 could predict with sensitivity 84% and specificity 70% the post-BAV hemodynamic outcome. Conclusions BAV as destination therapy for severe AS offered immediate and significant hemodynamic improvement. The survival was significantly better when a moderate degree of AS was present.

This paper presents a microscopic traffic simulation-based method for urban traffic state estimation using Assisted Global Positioning System (A-GPS) mobile phones. In this approach, real-time location data are collected by A-GPS mobile phones to track vehicles traveling on urban roads. In addition, tracking data obtained from individual mobile probes are aggregated to provide estimations of average road link speeds along rolling time periods. Moreover, the estimated average speeds are classified to different traffic condition levels, which are prepared for displaying a real-time traffic map on mobile phones. Simulation results demonstrate the effectiveness of the proposed method, which are fundamental for the subsequent development of a system demonstrator.

近年来，国际建设行业得到了迅速发展，国际化也逐渐成为国际承包商的关键发展战略。承包商根据自身业务领域而开展的国际化、本地化或选择性的建筑战略，会对组织的成长和学习过程产生显著影响。基于空间分布来选择能够成功实施战略的领域是承包商的竞争优势和获得成功的关键。尽管承包商向国际工程投入了大量优势资源，但国际建设行业有其不同于传统建设或工程管理实施的独特性。很多学者意识到国际建设行业对不同方法的需求，却运用传统建设业和商业管理的工具来解读国际建设行业的独特特点。因此，本研究旨在表明国际建设行业的特殊范围和性质，并从工程科学的角度详细阐述国际建设和项目管理的独特性。在考虑其他关键商业、工程和建设管理战略的基础之上，国际建筑行业还需重视空间发展战略这一关键因素。运用于国际承包商的测量工具的发展，也使国际建设和项目管理较传统项目管理呈现出独特性。本研究揭示了空间发展战略对于承包商的重要意义。作为国际市场的潜在进入者和关注者，国际承包商需要认真考虑与组织潜在扩张相关的要素，并运用组织知识来加强国际战略的成功实施。研究表明，承包商国际化程度测量工具的发展是承包组织的关键工具，它揭示了承包商在动荡的全球经济中的依赖性、政策的可持续性、竞争对手和潜在进入者的表现，以及承包商的优势和劣势。这个工具有助于传统的管理方式向国际化和本地化战略的转变，能够为承包商创造竞争优势和巩固市场地位。在理论构建的基础之上，本研究对国际建设行业和中国海外承包商的典型案例进行了分析，结果具有显著意义。本研究证实了国际建设和项目管理的独特理论意义。虽然国际建设和项目管理的基础源于工程管理科学的其他领域，但是本文提出的具有针对性的国际建设和项目管理工具和方法的发展呈现出国际工程领域的独特性和典型意义。本文为未来国际建设和项目管理的独特发展创造了良好的平台，并为研究者和实践者发展自身国际建筑和项目管理的战略管理水平奠定了坚实的基础。

Impact of age on transcatheter aortic valve implantation outcomes：a comparison of patients aged≤ 80 years versus patients ＆gt; 80 years

ObjectiveTo investigate the procedural outcomes and the long-term survival of patients undergoing transcatheter aortic valve implantation （TAVI） and compare study results of patients≤ 80 years and patients ＆gt; 80 years *** total of 240 patients treated with TAVI were divided into two groups according to age≤ 80 years （n = 105; 43.8%） and ＆gt; 80 years （n= 135; 56.2%）. The baseline characteristics and the pro-cedural outcomes were compared between these two groups of *** the exception of peripheral artery disease and hypercho-lesterolemia, which were more frequently observed in the older age group, baseline characteristics were comparable between groups. Complica-tion rates did not differ significantly between patients≤ 80 years and patients ＆gt; 80 years. There were no differences in 30-day mortality rates be-tween patients aged≤ 80 years and patients ＆gt; 80 years old （9.5%vs. 7.4%, respectively;P = 0.557）. After a median follow-up of 28 months （in-terquartile range： 16-42 months）, 50 （47.6%） patients aged≤ 80 years died compared to 57 （42%） deaths in the group of patients ＆gt; 80 years old （P= 0.404）.ConclusionThe results of the present single center study showed that age did not significantly impact the outcomes of TAVI.

Biochar(BC)has gained attention for removal of toxic elements(TEs)from aqueous media;however,pristine biochar often exhibits low adsorption ***,various modification strategies in BC have been developed to improve its removal capability against *** zero-valent iron(nZVI)and iron oxides(FeOx)have been used as sorbents for TE ***,these materials are prone to agglomeration and also expensive,which make their usage limited for large-scale *** nZVI technical demerits could be resolved by the development of BC-based composite sorbents through the loading of nZVI or FeOx onto BC *** zero-valent iron modified BC(nZVIBC),FeOx-modified BC(FeOxBC)have attracted attention for their capability in removing pollutants from the aqueous ***,a potential use of nZVIBC and FeOxBC for TE removal from aqueous environments has not been well-realized or *** such,this article reviews:(i)the preparation and characterization of nZVIBC and FeOxBC;(ii)the capacity of nZVIBC and FeOxBC for TE retention in line with their physicochemical properties,and(iii)TE removal mechanisms by nZVIBC and *** nZVI and FeOx in BC increases its sporptive capability of TEs due to surface modifications in morphology,functional groups,and elemental *** combined effects of BC and nZVI,FeOx or Fe salts on the sorption of TEs are complex because they are very specific to *** review identified significant opportunities for research and technol-ogy advancement of nZVIBC and FeOxBC as novel and effective sorbents for the remediation of TEs contaminated water.

Preoperative Management of Cataract Surgery Candidates: An Evaluation of Their Perception and Preferences

Purpose: To assess the primary visual disability in patients with cataract and to evaluate their preference regarding the visual outcome after surgery. Materials and Methods: 120 patients (66 males, mean age 76.3 years, mean best corrected visual acuity 5.2/10 binocularly) participated. Perception regarding the primary visual disability related to cataract and preference of visual outcome after surgery were assessed by a questionnaire. All study procedures adhered to the principles outlined in the Declaration of Helsinki for research involving human subjects, and all participants gave written informed consent before their participation. Results: 74 patients (61.6%) had difficulty in distant vision, 5 (4.2%) in near vision, 17 (14.2%) mentioned blurred vision, 14 (11.7%) difficulty in driving especially at night, 7 (5.8%) complained about monocular diplopia and 3 (2.5%) about anisometropia. All mentioned that they preferred to obtain clear distant and near vision after cataract surgery. Conclusions: There was an interesting variety in visual disability related to cataract. The difficulty in distant vision was the primary visual deficit in the majority of the patients and the preference for clear distant and near vision was the desired postoperative visual outcome.

This paper presents the analysis of Earth Observation data records collected between 1975 and 2014 for assessing the extent and temporal evolution of the built-up surface in the frame of the Global Human Settlement Layer *** scale of the information produced by the study enables the assessment of the whole continuum of human settlements from rural hamlets to *** study applies enhanced processing methods as compared to the first production of the GHSL baseline *** major improvements include the use of a more refined learning set on built-up areas derived from Sentinel-1 data which allowed testing the added-value of incremental learning in big data ***,the new features of the GHSL built-up grids and the methods are described and compared with the previous ones using a reference set of building footprints for 277 areas of *** results show a gradual improvement in the accuracy measures with a gain of 3.6% in the balanced accuracy,between the first production of the GHSL baseline and the latest GHSL multitemporal built-up grids.A validation of the multitemporal component is also conducted at the global scale establishing the reliability of the built-up layer across time.

Assessment of the Most Common Pterygium Symptoms Leading to the Decision for Its Surgical Removal

Purpose: Pterygium is a benign growth of conjunctival tissue frequently related to chronic sunlight exposure and hot and dry climate. Purpose of this study was to evaluate the predominant symptoms in patients with pterygium leading to its surgical removal decision. Methods: 31 patients (19 males, 12 females), mean age 48.75 years old (SD 8.5), with primary pterygium and no other eye disease participated in the study. The predominant symptoms caused by the pterygium were assessed using a questionnaire. Results: The predominant symptoms associated with the pterygium were: discomfort/feeling of foreign body in 31 patients (100%), redness in 23 (74%), increased lacrimation in 17 (55%), aesthetic reasons in 12 (39%), visual disturbance (astigmatism/extension of the pterygium to the optical axis) in 4 (13%). In one case, despite the absence of annoying symptoms, surgical excision was proposed due to the atypical appearance of pterygium. For younger patients, the predominant reason for choosing surgical removal was that the patients are not content with external appearance caused by pterygium and in contrast lacrimation was the predominant reason for the older. In most patients, visual disturbance was not a significant cause although astigmatism in eyes with pterygium was 3.51 D (SD 1.82) and 0.95 D (SD 0.78) in healthy eyes respectively. Discussion: The main symptom in patients with pterygium is foreign body sensation;meanwhile aesthetic consideration and increased lacrimation are the main reasons leading to surgical removal decision for younger and older patients, respectively.

An increased cardiovascular morbidity and mortality, including the risk of sudden cardiac death(SCD), has been shown in patients with rheumatoid arthritis(RA). Abnormalities in autonomic markers such as heart rate variability and ventricular repolarization parameters, such as QTc interval and QT dispersion, have been associated with sudden death in patients with RA. The interplay between these parameters and inflammation that is known to exist with RA is of growing interest. In this article, we review the prevalence and predictors of SCD in patients with RA and describe the potential underlying mechanisms, which may contribute to this. We also review the impact of biologic agents on arrhythmic risk as well as cardiovascular morbidity and mortality.
