Surgical treatment of non-functioning pancreatic neuroendocrine tumors:current controversies and challenges

There has been a rising trend in the incidence and prevalence of non-functioning pancreatic neuroendocrine tumors(NFPanNETs).While a significant number of the newly diagnosed NFPanNETs are asymptomatic,a majority of patients will present with liver metastasis(LM)at the time of *** resection remains the only curative treatment,especially for localized *** a majority of small NFPanNETs are indolent,some are *** heterogeneity in tumor biology presents the surgeon with the unique challenge of determining which patient will benefit from surgery,given the morbidity of pancreatic *** has been a recent push for a more aggressive approach to the care of these patients,given the emergence of data supporting such ***,the risk of over or under treatment has generated immense debate amongst experts in the *** heterogeneity of current practice guidelines and institutional practices around the world is a reflection of the disparate opinion on the management of *** this review,we set out to examine the evidence regarding some of the most controversial and challenging aspects of the surgical treatment of *** evaluate the following questions;should patients with small NFPanNETs≤2 cm in size be resected;should patients with metastatic NFPanNETs undergo surgical debulking,and should there be resection of the primary tumor in the setting of non-resectable metastatic disease?Although there are currently no Level 1 data to answer these questions conclusively,we believe that the current literature supports a more aggressive approach to the management of NFPanNET.

Pancreatic neuroendocrine tumors(PNETs)are a rare group of neoplasms originating from the endocrine *** are classified as functional or non-functional *** are more often diagnosed at a higher stage with distant metastases or advanced locoregional *** majority of individuals with hepatic metastases will ultimately die of liver failure;therefore,the treatment of liver tumor burden is critical to providing a survival *** surgical resection remains the only chance of cure for disease confined to the pancreas or for locoregional disease,the treatment of advanced or metastatic PNETs is more complex and often requires a multimodal *** review focuses on treatment options for well and moderately differentiated PNETs with metastatic disease to the *** include surgery,liver-directed therapies including ablative and intra-arterial therapies,and systemic therapies such as somatostatin analogues,targeted therapies,chemotherapy,and peptide receptor radionuclide *** an individualized treatment strategy requires careful assessment of liver tumor burden and predicted biological *** surgical resection of hepatic metastases secondary to PNET primary tumors is associated with improved survival in multiple retrospective *** goals of treatment for metastatic disease include prolonging overall survival and progression free survival,improving quality of life,and control of symptoms.

Synthesis of boron nitride nanorod and its performance as a metalfree catalyst for oxidative desulfurization of diesel fuel

In order to reduce the sulfur compounds in diesel fuel,boron nitride(BN)has been used as a novel metal-free catalyst in the present *** nanocatalyst was synthesized via template-free approach followed by heating treatment at 900℃ in nitrogen atmosphere that the characteristics of the sample were identified by the X-ray diffraction,Fourier-transform infrared spectroscopy,Raman spectroscopy,field emission scanning electron microscopy,transmission electron microscopy,atomic force microscopy,and N2 adsorption-desorption *** results of structural and morphological analysis represented that BN has been successfully *** efficacy of the main operating parameters on the process was studied by using response surface methodology based on the Box-Behnken design *** prepared catalyst showed high efficiency in oxidative desulfurization of diesel fuel with initial sulfur content of 8040 mg·kg^(-1)*** statistical analysis,a significant quadratic model was obtained to predict the sulfur removal as a function of efficient *** maximum efficiency of 72.4%was achieved under optimized conditions at oxidant/sulfur molar ratio of 10.2,temperature of 71℃,reaction time of 113 min,and catalyst dosage of 0.36 ***,the reusability of the BN was studied,and the result showed little reduction in activity of the catalyst after 10 times ***,a plausible mechanism was proposed for oxidation of sulfur compounds on the surface of the *** present study shows that BN materials can be selected as promising metal-free catalysts for desulfurization process.

Correlation between Atmospheric Temperature and Soil Temperature: A Case Study for Dhaka, Bangladesh

The main intention of this study was to substantiate the relation between atmospheric temperature and soil temperature within a system boundary. The study focused on coefficient of correlation that demonstrates the association between two variables. The earth surface temperature is anticipated to be affected by a set of meteorological parameters. Atmospheric temperature, humidity, precipitation and solar radiation directly influence the adjacent soil and the extent of impact must be varied at different part of the earth because of multiple factors. The primary step to validate the correlation was to collect two series of data for two variables i.e. atmospheric temperature and soil temperature of last ten years from 2003 to 2012. The coefficient of correlation was determined through Pearson’s distribution whereas atmospheric temperature was considered as independent and soil temperature was dependent variable. The coefficients were calculated distinctly taking the soil temperature not only at several depths but also at separate faces of a day commensurate to overlying air temperature. The results indicated strong positive correlation up to 20 cm depth of soil which blatantly validates the premise of the study. The procedure was concluded with the regression analyses through which dependent variable (soil temperature) is projected corresponding to independent variable (air temperature). All possible coefficients of correlation were gathered in order to make the final comparison and analyses over the variation of magnitudes. Anomalies at which two variables are unable to continue linearity were portrayed in figures of the paper. Finally, influence graph is made to illustrate relative influence at different nominated depth of soil based on the coefficient of correlation.

Review of intraoperative parathormone monitoring with the miami criterion: A 25-year experience

With the development of imaging and localization studies,focused parathyroidectomy with use of intraope-rative parathormone monitoring(IPM)is the mainstay of treatment for primary hyperparathyroidism at many health care centers both nationally and *** parathyroidectomy guided by IPM allows for surgical excision of the offending parathyroid gland through smaller *** Miami criterion is a protocol that uses a">50%parathormone(PTH)drop"from either the greatest pre-incision or pre-excision measurement of PTH in a blood sample taken 10 min following resection of hyperfunctioning *** removal of the hyperfunctioning parathyroid gland,a>50%PTH drop at 10 min indicates completion of parathyroidectomy,and predicts operative success at6 *** using the Miami criterion has demonstrated equal curative rates of>97%,which is comparable to the traditional bilateral neck *** focused approach,however,is associated with shorter recovery times,improved cosmesis,and lower risk of postoperative hypocalcemia.
