Laparoscopic management of remnant gall bladder with stones: Lessons from a tertiary care centre's experience

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone *** its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this *** objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant ***:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in South *** patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were *** following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative ***:In total,36 patients were included and *** majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 *** most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open *** fistula to the colon was observed in one *** was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients ***:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and *** who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain *** study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder

Influence of solar eclipse on seawater

Eclipse induced changes in solar radiation is a common interest of scientists all over the world. The disturbance of the heat balance along the supersonic travel of the trajectory of the Moon’s shadow could generate gravity waves during solar eclipse, which results a reduction in the concentration of ozone layer in the stratosphere. We, in this context, conducted some experiments to detect the possible radiations reaching the surface of the earth and the impact of such radiation in seawater during the recent total and annular solar eclipses. This is the first time that the variation in pH value of seawater during solar eclipse is examined, and the experimental data demonstrated that the solar eclipse phenomenon affects the pH value of seawater due to the shorter wavelength radiations received on the surface of the earth. The reduction is around 20% and 40% of the difference between ordinary water and seawater during total and annular eclipses respectively. The multidisciplinary influences of these findings are addressed.

Investigation of FOXP3 (rs3761548) polymorphism with the risk of preeclampsia and recurrent spontaneous abortion: A systemic review and meta-analysis

Objective:To investigate the association between forkhead box P3(FOXP3)(rs3761548)polymorphism and the risk of preeclampsia and recurrent spontaneous ***:Literature on the association of FOXP3 gene polymorphisms and susceptibility to preeclampsia and unexplained recurrent spontaneous abortion was retrieved by searching databases such as PubMed,Science Direct,Google Scholar and Embase from 2000 to *** association measure was analyzed using an odds ratio(OR)and 95%confidence interval(CI).All the statistical analyses were executed using RevMan 5.4 ***:In the present meta-analysis,11 articles were *** pooled results showed no association between FOXP3 gene polymorphism(rs3761548)and preeclampsia risk in allelic,recessive,dominant and over dominant contrast ***3 gene polymorphism(rs3761548)showed an association with recurrent abortion in allelic,recessive and dominant models(OR 1.85,CI 1.59-2.14;OR 2.02,95%CI 1.56-2.62;OR 2.69,95%CI 1.50-4.83,respectively),while no association in the over dominant contrast model(OR 1.35,CI 0.87-2.10).Conclusions:In the present study,FOXP3 gene(rs3761548)polymorphism is associated with risk of recurrent spontaneous abortion but not ***,larger sample size and multiracial studies are needed in the future to confirm the findings.

Nutritional Composition of Staple Food Bananas of Three Cultivars in India

Banana (Musa spp.) is an important ingredient of several dishes and its nutritional and other biochemical composition at different stages is yet to be scientifically studied. In the present study the most popularly cultivated species of Nendran, Nijali poovan and Robusta banana in Southern India are selected. variations in the nutritional and biochemical compositions associated with ripened and unripened stages of banana fruits were studied. Proximate composition, mineral and phytochemical compositions of ripened and unripened banana flours were analyzed and the total soluble sugars in unripened banana range from 1.70 to 2.15 mg/100g of the samples and that of ripened banana range from 37.5 to 43.8 mg/100g of the samples. Mineral compositions show that they are rich sources of calcium, phosphorus and iron. In addition to this they are rich sources of antioxidant potential phytochemicals such as polyphenols, flavanoids, vitamin C and lesser in quantity of anti nutritional factors such as phytates and oxalates. The current study revealed the variations of biochemical compositions of three varieties in unripened and ripened stages. This will be useful for the exploitation of these crops to obtain and formulate the value added products.

Analysis of Nanof luids as Cutting Fluid in Grinding EN-31 Steel

Grinding requires high specific energy which develops high temperatures at wheel work piece interface. High temperatures impair work piece quality by inducing tensile residual stress, burn, and micro cracks. Control of grinding temperature is achieved by providing effective cooling and lubrication. Conventional flood cooling is often ineffective due to enormous heat generation and improper heat dissipation. This paper deals with an investigation on using TRIM E709 emulsifier with Al_2O_3 nanoparticles to reduce the heat generated at grinding zone. An experimental setup has been developed for this and detailed comparison has been done with dry, TRIM E709 emulsifier and TRIM E709 emulsifier with Al_2O_3 nanoparticles in grinding EN-31 steel in terms of temperature distribution and surface finish. Results shows that surface roughness and heat penetration were decreased with addition of Al_2O_3 nanoparticles.

Renewable energy based microgrid system sizing and energy management for green buildings

The objective of this paper is to model a hybrid power system for buildings,which is technically feasible and economically *** a view to promote renewable energy sources,photovoltaics and wind turbines are integrated with the grid connected *** system is modeled and the optimal system configuration is estimated with the help of hybrid optimization model for electric renewables(HOMER).The logic is illustrated with a case study based on the practical data of a building located in southern *** building is associated with 3.4 MWh/day priority load(peak load as 422 kW),as well as 3.3 MWh/day deferrable load(peak load as 500 kW).Sensitivity analysis is performed to deal with uncertainties such as the increase in electricity consumption and grid tariff,environmental changes,*** the simulation result,it is observed that the designed system is cost effective and environment friendly,which leads to 6.18%annual cost savings and reduces CO_(2) emissions by 38.3%.Sensitivity results indicate that the system is optimal and adaptable in a certain range of unanticipated variances with respect to best estimated ***,an energy management strategy is developed for the optimal system to ensure reliable power during contingency and *** green and hybrid power system designed can be adaptable to any critical and large consumers of urban buildings.

Calcitonin receptor-like receptor(CRLR)is a human protein,that produces a calcitonin gene-related peptide receptor(CGRP)when associates with human receptor activity-modifying protein-1(HRAMP1).CGRP is believed to be involved in triggering of *** strategies were employed to design antimigraine drug using various CGRP antagonist/ligands but most of them have failed due to their inability to reach the target“CGRP receptor”as they get metabolized before conferring their pharmacological action and exhibit toxic effect to the *** present study,evaluated the binding of 13 phyto-chemical compounds of“Abrus precatorius”identified through GCMS analysis against the protein CGRP using“Discovery Studio software”.The molecular docking study and ADME/T properties prediction were performed with the compounds using C-DOCKER *** results showed that five lead compounds of *** may act as good inhibitors for migraine headache and the compounds can be re-designed and synthesized for better antimigraine activity.
