mutations of the bile salt export pump (BSEP) or the multidrug resistance P-glycoprotein 3 (mDR3) are linked to impaired bile salt homeostasis and lead to progressive familial intrahepatic cholestasis (PFIC)-2 and -3, respectively. The regulation of bile salt transporters in PFIC is not known. Expression of hepatobiliary transporters in livers of ten patients with a PFIC phenotype was studied by quantitative reverse transcription polymerase chain reaction, Western blotting, and immunofluorescence microscopy. PFIC was diagnosed by clinical and laboratory findings. All patients could be assigned to PFIC-2 or PFIC-3 by the use of BSEP-and mDR3-specific antibodies and by mDR3 gene-sequencing. Whereas in all PFIC-2 patients, BSEP immunoreactivity was absent from the canalicular membrane, in three PFIC-3 livers, canalicular mDR3 immunoreactivity was detectable. Serum bile salts were elevated to 276 ±233 and to 221 ±109 μmol/L in PFIC-2 and PFIC-3, respectively. Organic anion transporting polypeptide OATP1B1, OATP1B3, and mRP2 mRNA and protein levels were reduced, whereas sodium taurocholate cotransporting polypeptide (NTCP) was only reduced at the protein level, suggesting a posttranscriptional NTCP regulation. Whereas mRP3 mRNA and protein were not significantly altered, mRP4 messenger RNA and protein were significantly increased in PFIC. In conclusion, PFIC-2 may be reliably diagnosed by immunofluorescence, whereas the diagnosis of PFIC-3 requires gene-sequencing. Several mechanisms may contribute to elevated plasma bile salts in PFIC: reduced bile salt uptake via NTCP, OATP1B1, and OATP1B3, decreased BSEP-dependent secretion into bile, and increased transport back into plasma by mRP4. Upregulation of mRP4, but not of mRP3, might represent an important escape mechanism for bile salt extrusion in PFIC.

Phosphoribulokinase （PRK）, a nuclear-encoded plastid-localized enzyme unique to the photosynthetic carbon reduction （Calvin） cycle, was cloned and characterized from the stramenopile alga vaucheria litorea. This alga is the source of plastids for the mollusc （sea slug） Elysia chlorotica which enable the animal to survive for months solely by photoautotrophic CO2 fixation. The 1633-bp v. litorea prk gene was cloned and the coding region, found to be interrupted by four introns, encodes a 405-amino acid protein. This protein contains the typical bipartite target sequence expected of nuclearencoded proteins that are directed to complex （i.e. four membrane-bound） algal plastids. De novo synthesis of PRK and enzyme activity were detected in E. chlorotica in spite of having been starved of v. litorea for several months. Unlike the algal enzyme, PRK in the sea slug did not exhibit redox regulation. Two copies of partial PRK-encoding genes were isolated from both sea slug and aposymbiotic sea slug egg DNA using PCR. Each copy contains the nucleotide region spanning exon 1 and part of exon 2 of v litorea prk, including the bipartite targeting peptide. However, the larger prk fragment also includes intron 1. The exon and intron sequences of prk in E. chlorotica and v/itorea are nearly identical. These data suggest that PRK is differentially regulated in v. litorea and E. chlorotica and at least a portion of the v. litorea nuclear PRK gene is present in sea slugs that have been starved for several months.

Objective: To describe the clinical characteristics and clinical-anatomic correlations of homonymous hemianopia (HH). Background: Homonymous hemianopia impairs visual function and frequently precludes driving. most knowledge of HH is based on relatively few cases with clinical-anatomic correlations. methods: The authors reviewed medical records of all patients with HH seen in their service between 1989 and 2004. Demographic characteristics, characteristics of visual field defects, causes of visual field defects, neuroradiologic definition of lesion location, and associated neurologic deficits were recorded. Results: A total of 904 HH were found in 852 patients. A total of 340 HH (37.6%) were complete and 564 HH (62.4%) were incomplete. Homonymous quadrantanopia (264 HH, 29%) was the most common type of incomplete HH, followed by homonymous scotomatous defects (116 HH, 13.5%), partial HH (114 HH, 13%), and HH with macular sparing (66 HH, 7%). A total of 407 HH (45.0%) were isolated. Causes of HH included stroke (629 HH, 69.6%), trauma (123, 13.6%), tumor (102, 11.3%), brain surgery (22, 2.4%), demyelination (13, 1.4%), other rare causes (13, 1.4%), and unknown etiology (2, 0.2%). The lesions were most commonly located in the occipital lobes (45%) and the optic radiations (32.2%). Every type of HH, except for unilateral loss of temporal crescent and homonymous sectoranopia, was found in all lesion locations along the retrochiasmal visual pathways. Conclusion: Homonymous hemianopia is usually secondary to stroke, head trauma, and tumors. Although the characteristics of visual field defects can be helpful in lesion location, specific visual field defects do not always indicate specific brain locations.

As the rapid growth of mobile social networks,mobile peer-to-peer(P2P)communications and mobile edge computing(mEC)have been developed to reduce the traffic load and improve the computation capacity of cellular ***,the stability of social network is largely ignored in the advances of P2P and mEC,which is related to the social relations between *** plays a vital role in improving the efficiency and reliability of traffic offloading *** this paper,we integrate an edge node and the nearby P2P users as a mobile P2P social network and introduce the problem of adaptive anchored(k,r)-core to maintain the stability of multiple mobile P2P *** aims to adaptively select and retain a set of critical users for each network,whose participation is critical to overall stability of the network,and allocate certain resource for them so that the maximum number of users of all networks will remain engaged and the traffic of cellular network can be *** called the retained users as anchor *** address it,we devise a peer-edge-cloud framework to achieve the adaptive allocation of *** also develop a similarity based onion layers anchored(k,r)-core(S-OLAK)algorithm to explore the anchor *** results based on a real large-scale mobile P2P data set demonstrate the effectiveness of our method.

Objective: The aim of this study was to determine if pregnancy outcome for women undergoing in vitro fertilization is correlated with pre-conception thyroid-stimulating hormone level. Study design: We performed a retrospective cohort study of in vitro fertilization cycles in our private practice with an initial positive serum human chorionic gonadotropin level and thyroid-stimulating hormone level available (n = 364). We examined whether or not birth outcome differed between cycles in which the thyroid-stimulating hormone was > 2.5 mIU/L compared with cycles with a thyroid-stimulating hormone level of ≤ 2.5 mIU/L. Logistic regression was used to determine the association between thyroid-stimulating hormone level and spontaneous abortion rate. Results: Delivery outcome was available for 195 cycles, 36% of which had a thyroid-stimulating hormone level > 2.5. The gestational age at delivery was higher in cycles with a thyroid-stimulating hormone ≤ 2.5 than for cycles with a thyroid-stimulating hormone > 2.5 (38.5 vs 38.0 weeks for singletons, 36.0 vs 34.6 weeks for twins, overall P = .012 for thyroid-stimulating hormone level). The mean birth weight for cycles with a thyroid-stimulating hormone ≤ 2.5 was higher than for cycles with a thyroid-stimulating hormone > 2.5 (7.33 vs 6.78 lbs for singletons, P = .024 and 5.36 vs 4.83 lbs for twins, P = .023). Restricting analysis to cycles where the woman was not taking thyroid replacement did not change the overall conclusions. There was a trend toward increasing risk of miscarriage with increasing thyroid-stimulating hormone level in nondonor cycles, controlling for age and day 3 follicle-stimulating hormone level, but this trend did not reach statistical significance. Conclusion: A pre-conce-ption thyroid-stimulating hormone level > 2.5 mIU/L is associated with a lower gestational age at delivery and lower birth weight in women undergoing in vitro fertilization.

Background and Study Aims: There have so far been no prospective studies on th e value of flexible endoscopy for removing foreign bodies in the upper gastroint estinal tract. This study presents a clinical analysis of accidents with foreign bodies and prospectively evaluates the effectiveness of flexible endoscopy for removing them. Patients and methods: Atotal of 105 cases of foreign-body ingest ion in the upper gastrointestinal tract were evaluated, 29 (27.6%) in children and 76 (72.4%) in adults. Thirty patients (28.5%) had esophageal strictures. R esults: Thirty-nine of the foreign bodies (37.1%) consisted of food and 66 (62 .9%) were not food-related. The success rate of foreign-body extraction using only a conventional flexible endoscope and accessories for treatment was 98.0% , and with only a polypectomy snare and rat-toothed forceps it was 91.2%. Comp lications at themoment of foreign-bodyremoval occurred in nine patients (8.6%) ; there was only one (1%) esophageal perforation. The incidence of complication s related to the duration of foreign-body impaction was six (10.5%) with forei gn bodies impacted for up to 24h, 13 (52.0%) for those impacted for 24-48h, and three (60.0%) for those impacted for 48-72h (P ve and safe device for removing foreign bodies from the upp er gastrointestinal tract, with a high success rate using only the polypectomy s nare and the rat-toothed forceps as accessories. If foreign-body impaction las ts for more than 24h, there is a significant increase in the incidence of compli cations.
