Introduction: Metabolic syndrome is one of the main risk factors of cardiovascular disease among hemodialysis patients. Objective: The main objective of this study was to determine the prevalence and factors associated with metabolic syndrome among hemodialysis patients in Cotonou in 2015. Patients and methods: It was a cross-sectional, descriptive and analytical study conducted from 05th October to 02nd november 2015 at the national Teaching Hospital, Hubert Koutoucou Maga of Cotonou. All patients aged 18 years and above, regularly under hemodialysis for the past 3 months and who gave their informed consent were included in the study. Those excluded were: currently hospitalized hemodialysis patients, hemodialysis patients hospitalized in the last three months, hemodialysis patients whose general condition deteriorated or unable to answer the questionnaire. Metabolic syndrome was defined according to the International Diabetes Federation’s criteria. Factors associated with metabolic syndrome were sought using logistic regression in univariate analysis. Confidence intervals were calculated at 95% and alpha significance threshold at 5%. Outcomes: In total 165 patients were included in the study. Male predominance was observed, with 1.27 sex-ratios. Average age was 49.3 ± 12.9 years with extremes ranging from 18 to 78 years. Metabolic syndrome is observed among 46 patients undergoing hemodialysis or 27.9% prevalence rate. Factors associated with the metabolic syndrome in univariate analysis were: gender (p 0.001), age (p = 0.007), body mass index (p = 0.029) and prior diabetes history (p = 0.011). Conclusion: Metabolic syndrome is common among hemodialysis patients. Early screening and fighting against associated risk factors are very important.

Knowledge Attitude and Practice of General Physicians for Early Detection of Diabetic nephropathy in Cotonou

Introduction: General physicians can play an important role in the early detection of diabetic nephropathy (Dn). Purpose: To assess the levels of general physicians’ knowledge, attitude and practice in terms of early detection of Dn in Cotonou. Method: It was a cross-sectional, analytical and descriptive study which was conducted from 1st March 2015 to 30th September 2015. Every general physician working in a health structure in Cotonou who consented to participate in the study was included. We did not included medical specialists and general physicians working in nephrology department. Data were collected through a survey form designed with a score to assess the various items such as: knowledge, attitude and practice. The significance threshold is set to below 0.05. Results: In total, 202 general physicians were included. The average age was 30.9 ± 6.9 years ranging from 24 to 68 years. A male predominance was observed with 2.2 sex ratio. The majority of respondent medical physicians had poor knowledge in 76.2% cases, bad attitudes (61%) and bad practices (64.9%) in terms of early detection of diabetic nephropathy. There was positive impact of continuing medical training focused on diabetic nephropathy on attitudes (p = 0.016) and practices (p = 0.001) of these physicians. Conclusion: Diabetic nephropathy requires particular attention. General physicians’ continuous training is a principal solution.

Profile of Metabolic Syndrome at CnHU HKM Cotonou

The metabolic syndrome combines morphological, physiological and biochemical abnormalities that evolve over time, predisposing the affected person to atherosclerosis and its complications. The purpose of our work was to determine the clinical and biological profile of metabolic syndrome in patients received for medical examination according to WHO criteria. This was a retrospective study over a 1-year period: from January 1st, 2018 to December 31st, 2018. The information collected was clinical (sociodemographic characteristics, anthropometric data, initial blood pressure, cardiovascular history) and biological data (fasting blood glucose level, lipid test). Two thousand six hundred and two patients visited during the study period, including 291 patients (11.18%) for a reason that may evoke the metabolic syndrome but 115 files were retained for this study. The age of the patients was 58.39 ± 10.02 years with extremes ranging from 32 to 81 years old. Metabolic syndrome was found in 77 patients, or 66.9% of patients who had consulted for a reason that might suggest this diagnosis. 44.3% had 3 criteria for definition;19.1% had 4 criteria and 3.5% had 5 criteria. All patients have benefited a mono, bi or antihypertensive therapy associated with antidiabetic or hypolipemic treatment. Metabolic syndrome better predicts cardiovascular risk than analysis of individual risk factors. Its prevention involves diet which must be hyposodium, hypolipidic, without fast sugar and daily physical activities.

ng>Introduction:ng> Gastric cancer is not typically a surgical emergency. However, it can evolve into urgent complications such as a perforation. We report a case of a perforated gastric ulcer that turned out to be a cancer. ng>Observation:ng> A 52-year-old man was admitted to the emergency department of Hubert-Koutoukou-Maga national University Medical Center in Cotonou for generalized abdominal pain. He was diagnosed with acute generalized peritonitis with perforated gastric ulcer, establishing a surgical indication. An antral perforation was found and a simple closure was performed. Anatomopathological examination of the surgical piece revealed a gastric adenocarcinoma within the granulation tissue. Following the impact assessment, he underwent a second surgery where a distal gastrectomy was performed with D2 lymphadenectomy followed by gastrojejunostomy. He developed an anastomotic gastrointestinal fistula during the postoperative period but was successfully medically treated. The patient received adjuvant chemotherapy with Epirubicin, Cisplatin and 5-Fluorouracil. The patient is still alive, 3 years after the gastrectomy. ng>Conclusion:ng> When faced with a perforated gastric ulcer, one must also consider a neoplastic cause. The emergency surgical treatment depends on the general condition of the patient and the pre-existing co-morbidities, the choice being made between a one-stage versus two-stage gastrectomy.

ng>Background:ng> Gastric volvulus is a rare surgical emergency that causes an upper digestive obstruction due to a rotation of the stomach greater than 180°. ng>Observation:ng> We report the case of a 2-year-old patient referred from a health care facility for incoercible vomiting. On admission, the clinical examination revealed that the condition started about 1 month back with poor general and nutritional condition. Pyloric stenosis was suspected but this hypothesis was rejected based on abdominal ultrasound. Further investigations were done with an abdominal X-ray and an upper gastrointestinal barium study which led to the conclusion of an organo-axial gastric volvulus. The patient died in the course of treatment. ng>Conclusion:ng> Gastric volvulus is an often-misunderstood condition. Upper gastrointestinal barium study remains the gold standard of diagnosis. Its management is surgical.

Context: Recent publications on WHO recommended methods for estimating the survival of LLIns are good guidelines for assessing the performance of long-lasting insecticidal nets (LLIns). Thus, this field trial study was undertaken to evaluate the durability of the Yorkool? Ln mosquito net distributed during the 2017 campaign in Benin. Methods: The monitoring of Yorkool? Ln nets was carried out in two districts (Djougou III and Barienou) in Djougou, department of Donga, northern Benin from October 2017 to March 2019. A representative sample of 250 households that had received the Yorkool? Ln polyester LLIns during the 2017 campaign was selected in the rural and urban areas of each district and monitored for 6, 12 and 18 months. An evaluation of the survival of Yorkool? Ln nets was conducted based on the rate of loss and physical condition of the surviving nets as measured by the proportional hole index (pHI). Finally, the chemical efficacy of these LLIns during each period was determined using the WHO cone tests. Results: Survival of Yorkool? Ln nets was similar in both rural and urban areas, although there was a difference in survival between the 6-month (95.3%), 12-month (89.7%), and 18-month follow-up periods (74.4%). A difference in survival was also observed between the netCalc model (84%) compared to the Yorkool? Ln nets of this study (74.4%). The attrition rate was 29.6% for LLIns at 18 months. Surprisingly, the physical integrity of the LLIns was minimally affected in the municipality. Indeed, the proportion of mosquito nets in good condition without a hole was 51.8% compared to 56.8% with a hole after 18 months. Only 7.8% of the LLIns in the two districts were damaged compared to 2.6% which needed to be replaced. The washing frequency, location of the LLIns and the frequency of use are some factors contributing to the appearance of the holes in LLIns.
