Seismic Isolation of Reactor Assembly for a Fixed Base Accelerator Driven System Reactor Building

In the SILER （Seismic-Initiated events risk mitigation in LEad-cooled Reactors） Project, it is interesting to apply seismic isolation technology for the reactor assembly of the fixed base reactor building for ADS （Acceleration Driven System） heavy liquid reactor MYRRHA （Multipurpose Hybrid Research Reactor for High-Tech Application） which contains the most critical safety related components, such as reactor vessel, safe shutdown and control rod mechanisms, primary heat exchangers, primary pumps, spoliation target assembly and fuel assemblies, etc. The purpose of this paper is to investigate the possibility of an application of a partial seismic isolation to the safety critical components only, here, the reactor assembly. This paper presents the preliminary analysis results of the isolated reactor assembly and compares these with those of seismic isolated ADS reactor building. The analysis results show the reduction of the seismic acceleration response but the increase of the relative displacement for the reactor assembly. Some safety issues, especially, coolant＇s incapable covering the reactor core make difficult to apply for the partial seismic isolation of the ADS reactor assembly due to large relative displacement occurring the partial isolation system. Further study on the partial seismic isolation application of the critical safety components are also discussed.

Background: Blood cardioplegia is a technique with many variations in its use. Intermittent cross clamping with Lidoflazine has proven to deliver good cardioprotection in our center. Question: Is tepid (32°C) antegrade intermittent blood cardioplegia an efficient, safe and easy-to-use alternative to intermittent cross-clamping with Lidoflazine in elective isolated CABG in low-risk patients? Primary outcomes are heart enzymes (cTnI, CK-MB). Secondary outcomes are operation times, length of hospital stay, major complications and in-hospital mortality. Myocardial Protection during Cardiac Surgery: Warm Blood versus Crystalloid Cardioplegia

Purpose: Prevention of myocardial injury is essential during cardiac surgery. Both crystalloid and blood cardioplegia are popular methods for myocardial protection. Most experimental studies have been in favor of blood cardioplegia. The objective of this study is to determine whether the use of warm blood cardioplegia (BCP) is superior to crystalloid cardioplegia (CCP) by means of myocardial injury markers and clinical outcome parameters. Materials and Methods: In a consecutive series of 293 patients, the first 150 received crystalloid cardioplegia, whereas the next 143 patients received blood cardioplegia. Postoperative myocardial injury was assessed by CTnI and CK-MB. Perioperative morbidity and mortality and clinical outcome parameters (need for inotropic support, ICU and hospital stay) were recorded. An unpaired student t-test was performed to analyse continuous postoperative variables relating to myocardial damage. The presence of possible confounders influencing the CTnI or CK-MB concentrations was tested using a student t-test for continuous variables, for categorical variables ANOVA was used. A final longitudinal model was created for CTnI and CK-MB. CTnI was analyzed by a mixed model with random intercept and slope. For all tests performed, statistical significance was 5%. Results: Both groups were well matched with respect to preoperative variables. No significant difference could be found in maximum postoperative levels of CTnI (8.8 ± 18.4 μg/l in BCP vs 9.6 ± 16.5 μg/l in CCP, p = 0.6455) or CK-MB (19.2 ± 31.0 μg/l in BCP vs 26.4 ± 41.5 μg/l in CCP, p = 0.1209). Nor was there any significant difference in other postoperative variables. Testing treatment effect over time proved only significant influence of the surgical intervention type on CTnI levels in time (p < 0.001). Conclusion: This study could not show significantly higher myocardial injury in the group of patients receiving crystalloid cardioplegia versus warm blood cardioplegia. This suggests th

Background:Renal venous thrombosis (RVT) is the most common form of venous thrombosis in neonates,causing both acute and long term kidney dysfunction. Historical predisposing factors include dehydration,maternal diabetes,and umbilical catheters,but recent reports highlight associations with prothrombotic abnormalities. Study:Twenty three patients with neonatal RVT were analysed over 15 years. Predisposing factors,presentation,and procoagulant status were compared with renal outcome using multilevel modelling. Results:Median presentation was on day 1:19/23 (83%) had pre/perinatal problems,including fetal distress (14),intrauterine growth retardation(five),and pre-identified renal abnormalities (two);8/18 (44%) had procoagulant abnormalities,particularly factor V Leiden mutations (4/18). Long term abnormalities were detected in 28/34 (82%) affected kidneys; mean glomerular filtration rate was 93.6 versus 70.2 ml/min/1.73 m2 in unilateral versus bilateral cases (differen- ce 23.4; 95%confidence interval 6.4 to 40.4; p=0.01 ). No correlation was observed between procoagulant tendencies and outcome,but presenting renal length had a significant negative correlation:mean fall in estimated single kidney glomerular filtration rate was 3 ml/min/1.73 m2 (95%confidence interval 3.7 to -2.2; p =0.001) per 1 mm increase,and kidneys larger than 6 cm at presentation never had a normal outcome. Conclusions:This subgroup of neonatal RVT would be better termed perinatal RVT to reflect antenatal and birth related antecedents. Prothrombotic defects should be considered in all patients with perinatal RVT. Kidney length at presentation correlated negatively with renal outcome. The latter,novel observation raises the question of whether larger organs should be treated more aggressively in future.

目的：本5年的前瞻性多中心研究通过比较其种植体成功率和存留率．种植体周围健康情况和软组织反应,牙槽嵴水平的稳定性以及并发症率,对上颌前牙区延期或即刻植入OsseoSpeed种植体行即刻负重的病例进行评价。材料和方法：选取单颗前牙缺失要求种植治疗的患者进行种植治疗并行即刻临时修复,12周后行全瓷冠永久修复。随访5年观察种植体存留率、骨水平、软组织水平以及种植体周围健康状况。结果：共计纳入130位患者，其中55位患者行即刻种植，58位患者行延期种植。5年后，45位即刻种植患者及49位延期种植患者仍在研究队列中。在第1年内，即刻种植组有3例失败（存留率946％），延期种植组有1例失败（存留率983％）,差异不具有统计学意义。5年后，牙槽间隔的骨水平距种植体参考点的平均距离分别为0.43±0.63mm和0.38±0.62mm（无显著统计学差异）。两组的龈乳头均随时间增加，永久冠修复后,种植体周围黏膜高度保持在稳定水平。延期种植组，与翻瓣手术相比，不翻瓣手术增加了种植体周围黏膜的量（平均0．78±1．34mmvs 0.19士0．79mm）。结论：5年后,即刻种植组和延期种植组相比，行即刻临时修复后，两组用于评价种植体成功率及种植体美学效果的软硬组织参数接近。整体的组织反应及种植体存留率支持在愈合后的牙槽骨及状态良好的拔牙窝种植行即刻临时修复。

OBJECTIVE: To identify risk factors and outcomes associated with a short umbilical cord. METHODS:We conducted a population- based case- control study using linked Washington State birth certificate- hospital discharge data for singleton live births from 1987 to 1998 to assess the association between maternal, pregnancy, delivery, and infant characteristics and short umbilical cord. Cases (n = 3,565) were infants diagnosed with a short umbilical cord. Controls (n = 14,260) were randomly selected from among births without a diagnosis of short umbilical cord. RESULTS: Case mothers were less likely to be overweight (body mass index 25 or more, odds ratio 0.7; 95% confi- dence interval 0.6, 0.8) and more likely to be primiparous (OR 1.4; 95% CI 1.3, 1.6). Case infants were more likely to be female (OR 1.3; 95% CI 1.2, 1.4), have a congenitalmalformation (OR1.6; 95% CI 1.4, 1.8), and be small for their gestational age (risk ratio 1.6; 95% CI 1.4, 1.9). A short cord was associated with increased risk for maternal labor and delivery complications, including retained placenta (RR 1.6; 95% CI 1.2, 2.3) and operative vaginal delivery (RR 1.4; 95% CI 1.3, 1.5). Adverse fetal and infant outcomes in cases included fetal distress (RR 1.8; 95% CI 1.6, 2.1) and death within the first year of life among term infants (RR 2.4; 95% CI 1.2, 4.6). CONCLUSION: Modifiable risk factors associated with the development of a short cord were not identified. Case mothers and infants are more likely to experience labor and delivery complications. Term case infants had a 2- fold increased risk of death, which suggests closer postpartum monitoring of these infants.

A search for the rare decays W^(+)→D_(s)^(+)γ and Z→D0γis performed using proton-proton collision data collected by the LHCb experiment at a centre-of-mass energy of 13TeV,corresponding to an integrated luminosity of 2.0fb−*** significant signal is observed for either decay mode and upper limits on their branching fractions are set using W^(+)→D_(s)^(+)γ and Z→μ+μ−decays as normalization *** upper limits are 6.5×10^(−4) and 2.1×10^(−3) at 95% confidence level for the W^(+)→D_(s)^(+)γ and Z→D^(0)γ decay modes,*** is the first reported search for the Z→D^(0)γ decay,while the upper limit on the W+→D+sγbranching fraction improves upon the previous best limit.

The first search for the doubly heavy Ωbc0baryon and a search for the Ξbc0baryon are performed using pp collision data collected via the LHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 TeV,corresponding to an integrated luminosity of 5.2 *** baryons are reconstructed via their decays to ∧c+π-andΞc+π-.No significant excess is found for invariant masses between 6700 and 7300 MeV/c2,in a rapidity range from2.0 to 4.5 and a transverse momentum range from 2 to 20 MeV/*** limits are set on the ratio of the Ωbc0andΞbc0production cross-section times the branching fraction to ∧c+π-（Ξc+π-） relative to that of the ∧b0（Ξb0） baryon,for different lifetime hypotheses,at 95% confidence *** upper limits range from 0.5×10-4to 2.5×10-4for theΩbc0→∧c+π-(Ξbc0→∧c+π-) decay,and from 1.4×10-3to 6.9×10-3for the Ωbc0→Ξc+π-(Ξbc0→Ξc+π-) decay,depending on the considered mass and lifetime of the Ωbc0(Ξbc0) baryon.
