Femoral Access with Ultrasound-Guided Puncture and Z-Stitch Hemostasis for Adults with Congenital Heart Diseases Undergoing Electrophysiological Procedures

Aims:Although the application of ultrasound-guided vascular puncture and Z-stitch hemostasis to manage femoral access has been widely utilized,there is limited data on this combined application in adult congenital heart disease(ACHD)patients undergoing electrophysiological(EP)*** sought to evaluate the safety and efﬁcacy of ultrasound-guided puncture and postprocedural Z-stitch hemostasis for ACHD patients under-going EP *** and Results:The population of ACHD patients undergoing transfemoral EP pro-cedures at the University of Zurich Heart Center between January 2019 and December 2022 was observed and *** the study period,femoral access(left/right,arterial/venous)was performed under real-time ultrasound *** the end of the procedure,a single Z-stitch was performed at the puncture *** eval-uated the incidence of in-hospital complications associated with femoral access puncture in this *** 101 patients who had a total of 147 previous ipsilateral vascular punctures(mean 1.5 per person),100 patients underwent successful femoral vascular access for EP *** median age of the patients was 47±15 years and 34(34%)were male.Z-stitches were performed after the procedure in 100 patients with 303 femoral vascular accesses(mean 3 punctures per person).No patient developed vascular puncture relevant inguinal hematoma,pseudo aneurysm,arteriovenousﬁstula,venous or arterial ***:In ACHD patients undergoing EP procedures,optimal femoral access management can be achieved with ultra-sound-guided puncture and postprocedural Z-stitch hemostasis.

Adverse events related to physical restraint use in intensive care units: A review of the literature

Physical restraints are widely used and accepted as protective measures during treatment in intensive care unit (ICU). This review of the literature summarizes the adverse events and outcomes associated with physical restraint use, and the risk factors associated with their use during treatment in the ICU. The PubMed, Scopus, and Google Scholar databases were screened using predefined search terms to identify studies pertaining to adverse events and/or outcomes associated with physical restraint use, and the factors associated with their use in adult patients admitted to the ICU. A total of 24 articles (including 6126 patients) that were published between 2006 and 2022 were identified. The described adverse events associated with physical restraint use included skin injuries, subsequent delirium, neurofunctional impairment, and a higher rate of post-traumatic stress disorder. Subsequent delirium was the most frequent adverse event to be reported. No alternative measures to physical restraints were discussed, and only one study reported a standardized protocol for their use. Although physical restraint use has been reported to be associated with adverse events (including neurofunctional impairment) in the literature, the available evidence is limited. Although causality cannot be confirmed, a definite association appears to exist. Our findings suggest that it is essential to improve awareness regarding their adverse impact and optimize approaches for their detection, management, and prevention using protocols or checklists.

The impact of salps(Salpa thompsoni) on the Antarctic krill population(Euphausia superba): an individual-based modelling study

Background Krill（Euphausia superba） and salps（Salpa thompsoni） are key macrozooplankton grazers in the Southern Ocean ecosystem. However, due to differing habitat requirements, both species previously exhibited little spatial overlap. With ongoing climate change-induced seawater temperature increase and regional sea ice loss, salps can now extend their spatial distribution into historically krill-dominated areas and increase rapidly due to asexual reproduction when environmental conditions are favorable. Understanding the potential effects on krill is crucial, since krill is a species of exceptional trophic significance in the Southern Ocean food web. Negative impacts on krill could trigger cascading effects on its predators and prey. To address this question, we combined two individual-based models on salps and krill, which describe the whole life cycle of salp individuals and the dynamic energy budget of individual krill. The resulting new model PEKRIS（PErformance of KRIll vs. Salps） simulates a krill population for 100 years under varying chlorophyll-a concentrations in the presence or absence of *** All of the investigated krill population properties（abundance, mean length, and yearly egg production） were significantly impacted by the presence of salps. On the other hand, salp density was not impacted if krill were present. The article examines the existing infrastructure of open common spaces within two New Belgrade mass housing blocks(Blocks 23 and 70a)through a typo-morphological *** spaces between the buildings,although the most neglected,underused,and deteriorated components of mass housing neighbourhoods,are at the same time crucial to the quality,vitality and integrated governance of these *** represent the primary tangible commons in cities and *** question of urban commons is increasingly present in scientific literature,urban and architectural ***,approaches exploring the spatiality of the urban commons are scarce,leading to insufficient understanding of the spatial aspect and potentials of the already existing ***,this study includes(1)identification,typological decoding and classification of the common spaces,focusing on the case of New Belgrade blocks,followed by(2)analysis of the spatial patterns and integration of the identified spaces within the *** study confirms the complexity and diverse typology of the common *** finds that the in-between,common spaces contribute to higher integration of different segments of the *** open common spaces have an essential role in humanisation of the blocks,and thus the quality of life in the blocks as integrated *** findings indicate that the spatial setting of the open common spaces in New Belgrade blocks allows for(re)emergence of collective practices,leading to inclusive and integrated rehabilitation of the neighbourhoods.

Battery energy storage systems(BESSs)need to comply with grid code and fault ride through(FRT)requirements during disturbances whether they are in charging or discharging *** literature has shown that constant charging current control of BESSs in charging mode can prevent BESSs from complying with emerging grid codes such as the German grid code under stringent unbalanced fault *** address this challenge,this paper proposes a new FRTactivated dual control strategy that consists of switching from constant battery current control to constant DC-link voltage control through a positive droop *** results show that the strategy ensures proper DC-link voltage and current management as well as adequate control of the positive-and negative-sequence active and reactive currents according to the grid code *** is also shown that the proposed FRT control strategy is tolerant to initial operating conditions of BESS plant,grid code requirements,and fault severity.

The catalysis of the Hajos-Parrish reaction by cis-and trans-4,5-ethano-proline was explored experimentally and computationally with DFT(ωB97X-D and MN15)and DLPNO-CCSD(T).Both of the new catalysts are as active as proline or cis-and trans-4,5-methano-proline,with the trans-ethano-proline being as enantioselective as proline.A thorough theoretical analysis of the electronic factors influencing catalysis is reported.

BACKGROUND Giant hepatic cavernous hemangioma with multiple satellite nodules is a rare subtype of hepatic cavernous hemangioma,the most common vascular liver *** report on a tumor with unusual histologic features:(1)Finger-like infiltration pattern;(2)lack of encapsulation;(3)blurred tumor/liver interface;and(4)massive satellitosis-referring to the article“Hepatic cavernous hemangioma:underrecognized associated histologic features”.CASE SUMMARY A 60-year-old man presented with increasing uncharacteristic abdominal discomfort and mildly elevated blood parameters of acute *** revealed an unclear,giant liver tumor of the left liver lobe.A massive vascular tumor with extensive satellitosis broadly infiltrating the adjacent liver parenchyma was resected via hemihepatectomy of segmentsⅡ/Ⅲ.Histopathological diagnosis was giant hepatic cavernous hemangioma with multiple satellite nodules,featuring unusual characteristics hardly portrayed in the ***,this particular morphology can explain the difficult pre-and perioperative diagnosis of a vascular liver tumor that is usually readily identifiable by modern imaging *** This case emphasizes the exact histological workup of tumor and tumor-induced parenchyma changes in radiologically unclassifiable liver tumors.
