Hepatitis C infected patients need vitamin D3 supplementation in the era of direct acting antivirals treatment

It has been reported that the serum level of vitamin D3(Vit D3) could affect the natural course of chronic hepatitis C(CH-C) and the response to treatment with pegylated interferon(Peg-IFN) and ribavirin. Although several mechanisms for the favorable effects of Vit D3 supplementation were reported, the total effect of Vit D3 supplementation remains unclear. Previously, we reported that supplementation with 1(OH)Vit D3 could enhance the Th1 response inducing not only a favorable immune response for viral eradication but also HCC control. Recently, the main treatment of CH-C should be direct acting antivirals(DAAs) without PegIFN. Peg-IFN is a strong immune-modulator. Therefore, an immunological analysis should be carried out to understand the effect of Vit D3 after treatment of DAAs without Peg-IFN. The induction of a favorable immune response by adding Vit D3 might be able to suppress the hepatocarcinogenesis after achieving SVR, especially in children and elderly patients with severe fibrosis lacking sufficient amounts of VitD 3.

In addition to surgical procedures, radiofrequency ablation is commonly used for the treatment of hepatocellular carcinomas(HCCs) of limited size and number. Transcatheter arterial chemoembolization(TACE), using iodized poppy seed oil, Lipiodol and anticancer drugs, has been actively performed for the treatment of unresectable HCC, particularly in Asian countries. Recently, Sorafenib become available for advanced HCCs when the liver is still sufficiently functional. Sorafenib is an oral multikinase inhibitor with antiproliferative and antiangiogenic effects. However, the effect of sorafenib seems to be inadequate to control the progression of HCC. Radiation therapy(RT)for HCC has a potential role across all stages of HCC. However, RT is generally not considered an option in HCC consensus documents or national guidelines, primarily because of insufficient supporting evidence. However, the method of RT has much improved because of advances in technology. Moreover, combined treatment of RT plus other treatments(TACE, sorafenib and chemotherapy etc.) has become one of the alternative therapies for HCC. Therefore, we should understand the various kinds of RT available for HCC. In this review, we focus on various kinds of external beam radiation therapy.

AIM To evaluate the comparative effectiveness of anagliptin and linagliptin on the clinical parameters in patients with type 2 diabetes mellitus(T2 DM). METHODS A 2-year retrospective cohort study was conducted in patients with T2 DM who received anagliptin and linagliptin. We enrolled 234 patients(anagliptin group, 117 patients; linagliptin group, 117 patients). RESULTS The glycemic control considerably improved 3, 6, 12, and 24 mo after the administration of both dipeptidyl peptidase-4(DPP-4) inhibitors. Following the administration of anagliptin, the diastolic blood pressure and serum total cholesterol levels decreased. However, serum high-density lipoprotein cholesterol levels increased and urinary albumin-creatinine ratio decreased following linagliptin administration. Furthermore, the liver function improved after the administration of *** These findings suggest that that the efficacy of DPP-4 inhibitors on the blood pressure, lipid profile, and liver function differs between anagliptin and linagliptin.

Major Outcomes through Recent ROSA/LSTF Experiments and Future Plans

Many experiments have been conducted on accidents and transients of pressurized water reactor (PWR) employing the rig of safety assessment/large-scale test facility (ROSA/LSTF). Recent research activities concerned with the OECD/NEA international joint projects included experimental investigation via the ROSA and ROSA-2 Projects, and counterpart testing with thermal-hydraulic integral test facilities under collaboration of the PKL-2, PKL-3, ATLAS, and ATLAS-2 Projects. Major results of the related integral effect tests (IETs) with the LSTF were reviewed to experimentally identify thermal-hydraulic phenomena involved, regarding the PWR accident sequences in accordance with the new regulatory requirements for the Japanese light-water nuclear power plants. Future separate effect test using the LSTF is planned to simulate loss of emergency core cooling system (ECCS) recirculation functions in a large-break loss-of-coolant accident (LOCA). Key results of the recent IETs utilizing the LSTF and future plans were presented relevant to multiple steam generator tube rupture accident with recovery operation, small-break LOCA with accident management measure on core exit temperature reliability, and small-break LOCA with thermal stratification under cold water injection from ECCS into cold legs. Also, main outcomes of the LSTF IETs were indicated for wide spectrum LOCA with core uncovery and anticipated transient without scram following small-break LOCA under totally failed high-pressure injection system.

Every individuals expose to the lisk of conventional immunodeficiency in daily life with both internal and externals. In a series of investigation, we have been assessed the various health promoting menu such as hot spring hydrotherapy, acupuncture & moxibution, light excurse, evidenced Control water western medicine technique. In this report, we try to investigate the regulatory effect of hydrotherapy by activated water molecule as our representative menu under the influence of hypothalamus system. We set up both animal and human model for assessment of hydrotherapy. Before starting experimental assessment, six males and females seven-week-old ddY mice, are used for the acute oral toxicity study. There is no sign for toxicity as judged by body weight, organ inspection by veterinary doctor. Next, young ddY mice are experimentally induced diabetes and immune-deficient status by administrating cancer chemotherapeutic agent, Streptzotosin. Charged Water was evident to control the blood sugar level compared to the Control Water group together with conventional control group. Both experimental group of mice wear statistically significant for recovery from blood sugar level and antibody secreting cell both IgM and IgG plaque-forming cell. We confirm also activated water group delivered the granulocyte that slow down the oxidative stress by super oxide.

AIM:To evaluate the efficacy of pegylated interferon α-2b(peg-IFNα-2b) plus ribavirin(RBV) therapy in Japanese patients with chronic hepatitis C(CHC) genotype Ib and a high viral ***:One hundred and twenty CHC patients(58.3% male) who received peg-IFNα-2b plus RBV therapy for 48 wk were *** virological response(SVR) and clinical parameters were ***:One hundred(83.3%) of 120 patients completed 48 wk of treatment.53 patients(44.3%) achieved *** virological response(EVR) and end of treatment response(ETR) rates were 50% and 73.3%,*** clinical parameters(SVR vs non-SVR) associated with SVR,ALT(108.4 IU/L vs 74.5 IU/L,P = 0.063),EVR(76.4% vs 16.4%,P 56 years),SVR in females was significantly lower than that in males(17% vs 50%,P = 0.0262).EVR and adherence to Peg-IFN were demonstrated to be the main factors associated with ***:Peg-IFNα-2b plus RBV combination therapy demonstrated good tolerability in Japanese patients with CHC and resulted in a SVR rate of 44.3%.Treatment of elderly female patients is still challenging and maintenance of adherence to peg-IFNα-2b is important in improving the SVR rate.

A Study on Improvement of RANS Analysis for Erosion of Density Stratified Layer of Multicomponent Gas by Buoyant Jet in a Containment Vessel

The analysis on a density stratification layer consisting of multiple gases in the reactor containment vessel is important for the safety assessment of sever accidents. The JAEA (Japan Atomic Energy Agency) has started the project on the containment thermal hydraulics. We carried out CFD (computational fluid dynamics) analyses in order to investigate the erosion of the density stratification layer by a vertical buoyant jet under this project. We used the RANS (Reynolds averaged numerical simulation) and LES (large eddy simulation) models to analyze the erosion of a density stratification layer by a vertical buoyant jet in a small vessel which represents a containment vessel. This numerical study calculates the turbulent mixing of a two-component (air and helium) gas mixture. The turbulence models used for the RANS analyses are two types of k-ε models. The first model is the low Reynolds number k-ε model developed by Launder and Sharma. The second model is modified from the first model in order to accurately consider the turbulent production and damping in a stratification layer. The results indicated while the erosion rate calculated by the low-Re k-ε model was much faster than that of the LES model, the modified k-ε model could calculate the erosion rate similar to the LES result.
