Evolution of heart transplant donor characteristics in the 21st century: A United States single center’s experience

Despite a record setting number of heart transplants performed annually,the national donor shortage continues to plague transplant teams across the United *** we describe the barriers to adaptation of numerous“non-traditional”orthotopic heart transplant donor characteristics including donors with hepatitis C virus,those meeting criteria for donation after cardiac death,donors with coronavirus disease 19 infection,donors with the human immunodeficiency virus,and grafts with left ventricular systolic *** center’s objective was to increase our transplant volume by expanding our donor pool from“traditional”donors to these“non-traditional”*** detail how medical advances such as certain laboratory studies,pharmacologic interventions,and organ care systems have allowed our center to expand the donor pool thereby increasing transplantation volume without adverse effects on outcomes.

Cardiovascular diseases(CVD)form a principal consideration in patients with end-stage liver disease(ESLD)undergoing evaluation for liver transplant(LT)with prognostic implications in the peri-and post-transplant *** the predominant etiology of ESLD continues to evolve,addressing CVD in these patients has become increasingly ***,as the number of LTs increase by the year,the proportion of older adults on the waiting list with competing comorbidities increase,and the demographics of LT candidates evolve with parallel increases in their CVD risk *** primary goal of cardiac risk assessment is to preemptively reduce the risk of cardiovascular morbidity and mortality that may arise from hemodynamic stress in the peri-and post-transplant *** complex hemodynamics shared by ESLD patients in the pre-transplant period with adverse cardiovascular events occurring in only some of these recipients continue to challenge currently available guidelines and their uniform *** review focusses on cardiac assessment of LT candidates in a stepwise manner with special emphasis on preoperative patient *** hope that this will reinforce the importance of cardiovascular optimization prior to LT,prevent futile LT in those with advanced CVD beyond the stage of optimization,and thereby use the finite resources prudently.

Experimental studies suggest that angiotensin-converting enzyme(ACE) inhibitors with high tissue affinity confer a greater degree of vascular renin-angiotensin system suppression than those with low tissue affinity despite similar suppression of the circulating renin-angiotensin system. To test this hypothesis in a clinical setting, we randomized subjects with chronic heart failure to receive the low tissue affinity ACE inhibitor enalapril or the high tissue affinity ACE inhibitor trandolapril, and assessed the degree of circulating and vascular renin-angiotensin system suppression. Vascular renin-angiotensin system suppression was determined by measuring the pressor response to intravenous injections of angiotensin I. Circulating reninangiotensin system suppression was determined by measuring plasma angiotensin II. Vascular and circulating renin-angiotensin system suppression, endothelial function(flow-mediated vasodilation), and maximal exercise capacity(peak oxygen uptake) were assessed after a 4-week run-in period on open-label enalapril 40 mg/day and after 8 weeks of randomized double-blind treatment with enalapril 40 mg/day or trandolapril 4 mg/day. Twenty-six men and 4 women(mean age 52±11 years; mean left ventricular ejection fraction 25±9%; New York Heart Association class II[n=16] and III[n=14]) were studied. After a 2-month randomized treatment period, vascular renin-angiotensin system suppression, circulating renin-angiotensin system suppression, endothelial function, and exercise capacity did not differ between subjects treated with enalapril and those treated with trandolapril. Despite substantial differences in the tissue affinity of enalapril and trandolapril, the degree of vascular renin-angiotensin system suppression achievedwith these agents did not differ in subjects with chronic heart failure during long-term therapy.

Objective. Coeliac disease (CD) may present in its clas sical form with diarrhoea and weight loss, but also with atypical symptoms th at are both related and unrelated to malabsorption. Osteomalacia or osteopenia f ollowing malabsorption of calcium and vitamin D is known to occur in patients wi th CD, and in such cases secondary hyperparathyroidism(SHp) caused by low serum calcium levels is frequently found. However, the prevalence of CD in subjects wi th SHp has not been reported. Material and methods. In the Troms study 2001, ser um parathyroid hormone (pTH) and calcium were measured in 7954 subjects of whom 6061 were eligible for follow- up. From this group, 97 subjects with SHp (serum pTH ≥ 6.5 pmol/l and serum calcium p group. At the re- examination, only 29 of the 97 subjects wi th SHp still had elevated serum pTH levels. Among these were 3 of the subjects w ith CD. When grouping the serological test results as negative, borderline or po sitive, there was a significant difference between the SHp group and the control s for anti- tissue transglutaminase and anti- gluten IgA (p p, at least when SHp is persistent, should be tested for CD.
