Ulcerative colitis patients in clinical remission demonstrate correlations between fecal immunochemical test results, mucosal healing, and risk of relapse

AIM: To assess the risk of relapse in ulcerative colitis (UC) patients in clinical remission using mucosal status and fecal immunochemical test (FIT) ***: The clinical outcomes of 194 UC patients in clinical remission who underwent colonoscopy were based on evaluations of Mayo endoscopic subscores (MESs) and FIT ***: Patients with an MES of 0 (n = 94, 48%) showed a ten-fold lower risk of relapse than those with an MES of 1-3 (n = 100, 52%) (HR = 0.10, 95%CI: 0.05-0.19). A negative FIT result (fecal hemoglobin concentrations ≤ 100 ng/mL) was predictive of patients with an MES of 0, with a sensitivity of 0.94 and a specific of 0.76. Moreover, patients with a negative FIT score had a six-fold lower risk of clinical relapse than those with a positive score (HR = 0.17, 95%CI: 0.10-0.28). Inclusion of the distinguishing parameter, sustaining clinical remission > 12 mo, resulted in an even stronger correlation between negative FIT results and an MES of 0 with respect to the risk of clinical relapse (HR = 0.11, 95%CI: 0.04-0.23).CONCLUSION: Negative FIT results one year or more after remission induction correlate with complete mucosal healing (MES 0) and better prognosis. Performing FIT one year after remission induction may be useful for evaluating relapse risk.

Helicobacter pylori infection in subjects negative for high titer serum antibody

AIM To investigate the clinicopathological features of the patients testing negative for high titer serum antiHelicobacter pylori(H. pylori) *** The antibody titers were measured using antigens derived from Japanese individuals. ^(13)C-urea breath test-positive individuals were defined as having H. pylori infection. We investigated the demographic characteristics, laboratory data, endoscopic findings including Kyoto classification of gastritis, and histology in negative-high titer patients without H. pylori eradication therapy. Kyoto classification consisted of scores for gastric atrophy, intestinal metaplasia, enlarged folds, nodularity, and *** Of the 136 subjects enrolled, 23(17%) had H. pylori infection. Kyoto classification had an excellent area under the receiver operating characteristics curve(0.886, 95% confidence interval: 0.803-0.968, P = 3.7 × 10^(-20)) for predicting H. pylori infection with a cutoff value of 2. Further, Kyoto classification, H. pylori density, and neutrophil activity had high accuracies(89.7%, 96.3%, and 94.1%, respectively). Kyoto classification was independent of the demographic and laboratory parameters in multivariate *** Endoscopic Kyoto classification of gastritis is a useful predictor of H. pylori infection in negative-high titer antibody patients.

Process of Human Resource Training for R＆D Projects Based on a Project Management Phronesis Acquisition Model

Research and development （R＆D） projects often employ management by objectives （MBO） as a way to make steady progress toward diffficult-to-reach targets. As a result, it is hard to motivate those project members who may be strongly oriented toward self-management. Furthermore, for R＆D projects that use the stage-gate approach, because there are multiple evaluation barriers, there may be distortions in the behavior or the training of R＆D personnel. For human resource training in R＆D projects, it is important to educate managers and project members in quality-cost-delivery or project-management-body-of-knowledge methods, but this is not enough. For successful R＆D, it is also important to consider approaches that will enable personnel to acquire high-level tacit knowledge related to project management （PM） and practical wisdom （i.e., phronesis）. If comprehensive human resource training that fosters the learning of PM-related knowledge and management by belief （MBB） is possible, this holds forth the prospect of effective human resource training methods for R＆D projects. The present research examines the human resource training process for R＆D projects by using a PM phronesis acquisition model. We confirmed the effectiveness of the PM phronesis acquisition model proposed in this research by testing in an actual R＆D project.

Enhanced Visible-Light-Sensitive Two-Step Overall Water-Splitting Based on Band Structure Controls of Titanium Dioxide and Strontium Titanate

Visible light-induced two-step overall water-splitting was achieved by combining two types of photocatalysts, which were prepared by introducing foreign elements into rutile titanium dioxide (TiO2) and strontium titanate (SrTiO3) with a controlled electronic band structure. Rutile TiO2 and SrTiO3 were doped with chromium and tantalum (Cr,Ta-TiO2) and with rhodium (Rh-SrTiO3), respectively, to introduce visible-light sensitivity. Under irradiation with only visible light from a 420-nm LED lamp, the simultaneous liberation of hydrogen and oxygen with a molar ratio of ~2:1 was achieved with these two types of photocatalysts in the presence of iodate ion/iodide ion as a redox mediator.

Limited resources of fossil energy require the search for new energy conversion processes and the increase of the efficiency of power plants to ensure the energy household in the future. Basically, changing the combustion mode from steady to pulse combustion makes an enormous reduction of fuel consumption possible. Active control systems for pulse combustors installed into the inlet further improve the efficiency and extend the operating range considerably. The control algorithm including fuel rate- air rate -pressure characteristics and control strategies as well are presented.

High-Pressure Melting Experiments on Basalt-Peridotite Layered Source (KLB-1/N-MORB): Implications for Magma Genesis in Hawaii

In order to understand the melting processes that occur within recycled oceanic crust and mantle in a heterogeneous plume (e.g., that beneath the Hawaiian Islands), a series of high-pressure-high-temperature layered experiments were performed at 2.9 GPa, 5 GPa, and 8 GPa, from 1300°C to 1650°C, using a fertile peridotite KLB-1 and N-MORB. Our experiments at conditions below the dry peridotite solidus produced melt compositions that ranged from basaltic andesite to tholeiite. An Opx reaction band formed between eclogite and peridotite layers, likely via chemical reaction between a silica-rich eclogite-derived partial melt and olivine in the peridotite matrix. At temperatures at or above the dry peridotite solidus, substantial melting occurred in both basalt and peridotite layers, and fully molten basalt melt and melt pockets from the peridotite layer combined. In our layered experiments, major and minor element contents in reacted melts closely matched those of Hawaiian tholeiite and picrite, except for Fe. Partial melts of anhydrous run products had ~55 - 57 wt% SiO2 at low temperature (i.e., were andesitic) and had ~50 - 53 wt% SiO2 at high temperatures, slightly below the dry peridotite solidus (i.e., were tholeiitic, and similar to those that occur during the Hawaii shield-building stage). Based on the Fe- and LREE-enriched signature in Hawaiian tholeiites, we propose that recycled components in the Hawaiian plume are not modern N-MORB, but are Fe-rich tholeiite;a lithology that was common in the Archaean and early Proterozoic. We have demonstrated that the entire compositional spectrum of Hawaiian tholeiites (basalt to picrite) can be formed by basalt-peridotite reactive melting near the dry solidus of peridotite. Based on these results, we propose that the potential temperature of the sub-Hawaiian plume may be much lower than previously estimated.

Enhancement of Oxygen Evolution Activity of Ruddlesden-Popper-Type Strontium Ferrite by Stabilizing Fe4<sup>+</sup>

Development of active iron based water oxidation for designing an ideal artificial photosynthesis devices operating under benign neutral pH is highly demanded. We investigated the electrocatalytic activity of Ruddlesden-Pop-per-type strontium ferrite (Sr3Fe2O7) toward the oxygen evolution reaction (OER). Owing to the temperature-dependent efficiency of the charge disproportionation of Fe4+, the OER activity of Sr3Fe2O7 varied with the temperature, and the onset potential for the OER at a neutral pH underwent a negative shift of approximately 200 mV by increasing the temperature for the stabilization of Fe4+. When metal substitution was made to Sr3Fe2O7 for stabilizing Fe4+ at room temperature, the temperature dependence of the OER activity disappeared and the OER was driven at a small overpotential without increasing the temperature, indicating that the stabilization of Fe4+ is substantially important for achieving high OER activity.

Inhibition of gastric perception of mild distention by omeprazole in volunteers

AIM： To evaluate the effects of omeprazole on gastric mechanosensitivity in humans. METHODS： A double lumen polyvinyl tube with a plas- tic bag was introduced into the stomach of healthy volunteers under fluorography and connected to a barostat device. Subjects were then positioned so they were sitting comfortably, and the minimal distending pressure （MDP） was determined after a 30-rain adap- tation period. Isobaric distensions were performed in stepwise increments of 2 mmHg （2 min each） starting from the MDR Subjects were instructed to score feel- ings at the end of every step using a graphic rating scale： 0, no perception; 1, weak/vague; 2, weak but significant; 3, moderate/vague; 4, moderate but signifi- cant; 5, severe discomfort; and 6, unbearable pain. Af- ter this first test, subjects received omeprazole （20 mg, after dinner） once daily for 1 wk. A second test was performed on the last day of treatment. RESULTS： No adverse effects were observed. Mean MDP before and after treatment was 6.3 - 0.3 mmHg and 6.2：1：0.5 mmHg, respectively. One subject before and 2 after treatment did not reach a score of 6 at the maximum bag volume of 750 mL. After omeprazole, there was a significant increase in the distension pres- sure required to reach scores of 1 （P = 0.019） and 2 （P = 0.017） as compared to baseline. There were no changes in pressure required to reach the other scores after treatment. Two subjects before and one after omeprazole rated their abdominal feeling 〈 1 at MDP, and mean （±SE） abdominal discomfort scores at MDP were 0.13±0.09 and 0.04±0.04, respectively. Mean scores induced by each MDP ＋ 2, 4, 6, 8, 10, 12, 14, 16, 18 and 20 （mmHg） were 1.1±0.3, 2.0±0.4, 2.9±0.5, 3.3±0.4, 4.6±0.3, 5.2±0.3, 5.5±0.2, 5.5±0.3, 5.7±0.3, and 5.4, respectively. After omepra- zole, abdominal feeling scores for the same incremen- tal pressures over MDP were 0.3±0.1, 0.8±0.1, 2.0±0.4, 2.8±0.4, 3.8±0.4, 4.6±0.4, 4.9±0.3, 5.4±0.4, 5.2±0.6, and 5.0±1.0, respectively.
