Comparison of incidence of acute kidney injury after robot-assisted radical prostatectomy with that after open retropubic and extraperitoneal laparoscopic radical prostatectomies in patients with prostate cancer

Background: We retrospectively evaluated the postoperative renal function in patients who had undergone radical prostatectomy to compare the incidences of postoperative acute kidney injury (AKI) among the patients who had undergone robot-assisted radical prostatectomy (RARP), retropubic radical prostatectomy (RRP), and extraperitoneal laparoscopic radical prostatectomy (exLRP). Materials and methods: Patients with prostate cancer who had undergone radical prostatectomy at our institution between 2008 and 2014 were included. Robot-assisted radical prostatectomy was performed using an intraperitoneal approach in a 25-degree Trendelenburg position, whereas other procedures were performed with the patient in the supine position. We evaluated the serum creatinine levels and estimated glomerular filtration rates immediately after surgery and on postoperative day 1. We evaluated the incidence of AKI after prostatectomy using the Acute Kidney Injury Network criteria of the Kidney Disease: Improving Global Outcomes guidelines. Results: A total of 150 consecutive patients were included, with each of the 3 groups (RARP, RRP, and exLRP) comprising 50 patients. Postoperative AKI was observed in 15 (30.0%), 1 (2.0%), and 3 (6.0%) patients in the RARP, RRP, and exLRP groups, respectively. Stage 1 AKI was observed in all the patients except one. The incidence of AKI in RARP group was significantly higher than that in the other groups (p < 0.001). In the RARP group, the serum creatinine level was significantly elevated immediately after the surgery; however, it returned to baseline on postoperative day 1. Surgical procedures were the only independent factor associated with AKI incidence. Conclusions: This study suggest that compared with RRP and exLRP, RARP is associated with a higher incidence of postoperative AKI, although most patients recover rapidly. Intra-abdominal pneumoperitoneum may contribute to AKI onset.

BackgroundWe clarified the predictive factors for changes in the status of medications for lower urinary tract symptoms (LUTS) 2 years after local radiotherapy for nonmetastatic prostate cancer. Materials and methodsWe retrospectively included patients who underwent local external radiotherapy for nonmetastatic prostate cancer in 8 institutions between April 2001 and March 2016. Patients were divided into the medication and no-medication group based on the use of drugs for LUTS before radiotherapy. We defined improvement of LUTS as when the patient did not require medication for LUTS at 24 months after radiotherapy in the medication group and as deterioration when medication was required in the no-medication group. Logistic regression analysis was used to evaluate predictive factors for changes in medication status. ResultsAltogether, 505 patients were divided into a no-medication group (n = 352) and a medication group (n = 153). The number of patients with deterioration and improvement in LUTS was 49 (14%) and 36 (23%), respectively. In the multivariate analysis, the predictive variables for deterioration were the International Prostate Symptom Score (>= 8; odds ratio [OR], 2.21; p = 0.014) and the biopsy Gleason score (= 50%; OR, 2.530; p = 0.027) in the medication group. ConclusionsThese predictive factors for changes in the status of medications for LUTS at 2 years after external radiotherapy may help determine the definitive therapy for nonmetastatic prostate cancer.

Kostka functions associated to complex reflection groups and a conjecture of Finkelberg-Ionov

Kostka functions K_(λ,μ)~±(t), indexed by r-partitions λ and μ of n, are a generalization of Kostka polynomials K_(λ,μ)(t) indexed by partitions λ,μ of n. It is known that Kostka polynomials have an interpretation in terms of Lusztig's partition function. Finkelberg and Ionov(2016) defined alternate functions K_(λ,μ)(t) by using an analogue of Lusztig's partition function, and showed that K_(λ,μ)(t) ∈Z≥0[t] for generic μ by making use of a coherent realization. They conjectured that K_(λ,μ)(t) coincide with K_(λ,μ)^-(t). In this paper, we show that their conjecture holds. We also discuss the multi-variable version, namely, r-variable Kostka functions K_(λ,μ)~±(t_1,…,t_r).

In the past decade, the tidal energies have caused worldwide concern as it can provide regular and predictable re- newable energy resource for power generation. The majority of technologies for exploiting the tidal stream energy are based on the concept of the horizontal axis propellers, which can be derived from the design and operation of wind turbines. However, there are some peculiar features such as the propeller working in the seawater with free surface and the possible occurrence of cavitation as compared with wind turbines. Especially, for a coun- ter-rotating type tidal stream power turbine, it is difficult to accurately predict the interaction between the front and rear blades at the design stage by blade element momentum theory. As a result, CFD shows its advantage to predict the performance of counter-rotating type propellers of the tidal stream turbi^le. In order to improve the accuracy of CFD predictions, the predicted results must be verified with experimental values. In this paper, a CFD model using block-structured grid was set up and experimental test was performed in a water tunnel for a tidal stream turbine with counter-rotating type propellers. The comparison between CFD predictions and experimental data shows quite good agreement on the power coefficients, which provides an evidence of validation of the CFD model. Such results offer the necessary confidence in the accuracy of the set up CFD model for the coun- ter-rotating type tidal stream turbine.

This serial research proposes the hybrid power system combined the wind power unit with the counter-rotating type pump-turbine unit,to provide the constant output for the grid system,even at the suddenly fluctuating/turbulent *** this paper,the tandem impellers of the counter-rotating type pumping unit was operated at the turbine mode,and the performances and the flow conditions were investigated numerically and *** 3-D turbulent flows in the runners were simulated at the steady state condition by using the commercial CFD code of ANSYS-CFX ver.12 with the SST turbulence *** providing the pump unit for the turbine mode,the maximum hydraulic efficiency is close to one of the counter-rotating type hydroelectric unit designed exclu-sively for the turbine ***,the runner/impeller of the unit works evidently so as to coincide the angularmomentum change through the front runners/impellers with that through the rear runners/impellers,namely to take the axial flow at not only the inlet but also the outlet without the guide *** results show that this type of unit is effective to work at not only the pumping but also the turbine modes.

AIM: To compare the combinative and individual effect of acarbose and gymnemic acid (GA) on maltose absorption and hydrolysis in small intestine to determine whether nutrient control in diabetic care can be improved by combination of them. METHODS: The absorption and hydrolysis of maltose were studied by cyclic perfusion of intestinal loops in situ and motility of the intestine was recorded with the intestinal ring in vitro using Wistar rats. RESULTS: The total inhibitory rate of maltose absorption was improved by the combination of GA (0.1g/L-1.0 g/L) and acarbose (0.1 mmol/L-2.0 mmol/L) throughout their effective duration (P <0.05, U test of Mann-Whitney), although the improvement only could be seen at a low dosage during the first hour. With the combination, inhibitory duration of acarbose on maltose absorption was prolonged to 3h and the inhibitory effect onset of GA was fastened to 15 min. GA suppressed the intestinal mobility with a good correlation (r = 0.98) to the inhibitory effect of GA on maltose absorption and the inhibitory effect of 2 mmol/L (high dose) acarbose on maltose hydrolysis was dual modulated by 1g/L GA in vivo indicating that the combined effects involved the functional alteration of intestinal barriers. CONCLUSION: There are augmented effects of acarbose and GA,which involve pre-cellular and paracellular barriers. Diabetic care can be improved by employing the combination.

The Paleoproterozoic （-1.73 Ga） basement rocks from Maddhapara, Bangladesh show a large range of chemical variations including diorite, quartz diorite, monzodiorite, quartz monzonite and granite. These are composed of varying proportions of quartz＋plagioclase＋K-feldspar＋biotite＋ hornblende±epidote＋titanite＋magnetite＋apatite and zircon. Amphibole and biotite, dominant ferro- magnesian minerals, have been analyzed with an electron microprobe. The biotite, Mg-dominant trioc- tahedral micas, is classified as phlogopitic nature. Relatively high Mg （1.33-1.53 pfu）, Mg^# （0.52-0.59） and low AIw （0.13-0.25 pfu） contents in the biotite reflect slightly fractionated magma, which might be a relative indicator for the origin of the parental magma. Biotite is also a very good sensor of oxidation state of the parental magma. Oxygen fugacity of the studied biotites estimate within the QFM and HM buffers and equilibrate at about -12.35 and -12.46, which exhibit the source materials were relatively higher oxidation state during crystallization and related to arc magmatism. Whereas, calcic amphi- boles, a parental member of arc-related igneous suite, display consistent oxygen fugacity values （-11.7 to -12.3）, low Al^# （0.16-0.21） with H2Omelt （5.6 wt.%-9.5 wt.%） suggest their reliability with the typical values of calc-alkaline magma crystallization. The oxygen fugacity of magma is related to its source material, which in turn depends on tectonic setting. Discrimination diagrams and chemical indices of both biotite and amphibole of dioritic rocks reveal calc-alkaline orogenic complexes; mostly I-type suite formed within subduction-related environments. Moreover, igneous micas are used as metal- logenic indicator. The biotites with coexisting amphibole compositions show an apparent calc-alkaline trend of differentiation. The study suggests that the trend of oxidized magmas is commonly associated with compressive tectonic and convergent plate boundaries.

Improvement in growth yield of single-walled carbon nanotubes with narrow chirality distribution by pulse plasma CVD

A pulse plasma chemical vapor deposition (CVD) technique was developed for improving the growth yield of single-walled carbon nanotubes (SWNTs) with a narrow chirality distribution. The growth yield of the SWNTs could be improved by repetitive short duration pulse plasma CVD, while maintaining the initial narrow chirality distribution. Detailed growth dynamics is discussed based on a systematic investigation by changing the pulse parameters. The growth of SWNTs with a narrow chirality distribution could be controlled by the difference in the nucleation time required using catalysts comprising relatively small or large particles as the key factor. The nucleation can be controlled by adjusting the pulse on/ofF time ratio and the total processing time.
