Neonatal hyperinsulinism with an ABCC8 mutation:A case report

BACKGROUND Neonatal hyperinsulinism can result from perinatal stress,genetic disorders,or syndromes,which can lead to persistent or intractable hypoglycemia in newborns.Mutations in the ABCC8 gene result in abnormal functioning of potassium channel proteins in pancreaticβ-cells,leading to an overproduction of insulin and congenital hyperinsulinemia.CASE SUMMARY We report a case of a high-birth-weight infant with postnatal hypoglycemia and hyperinsulinemia,whose mother had pregestational diabetes mellitus with poor glycemic control and whose sister had a similar history at birth.Whole-exome sequencing revealed a new mutation in the ABCC8 gene in exon 8(c.1257T>G),which also occurred in his sister and mother;thus,the patient was diagnosed with neonatal hyperinsulinism with an ABCC8 mutation.With oral diazoxide treatment,the child’s blood glucose returned to normal,and the pediatrician gradually discontinued treatment because of the child’s good growth and development.CONCLUSION We report a new mutation locus in the ABCC8 gene.This mutation locus warrants attention for genetic disorders and long-term prognoses of hypoglycemic children.

Zn-ion batteries(ZIBs)have a broad application prospect because of their advantages of high power,large capacity,and high energy density.However,the development of high-capacity,long-lifespan ZIBs is challenging because of the faster dendrite growth and the occurrence of the hydrogen evolution reaction.Laser-induced graphene(LIG)is a material with many defects and heteroatoms.Because of these characteristics,it plays an important role in improving nucleation.A simple and effective method for preparing LIG was proposed in this paper,and the LIG was covered on the surface of Zn foil to form a composite structure.This structure substantially reduces the nucleation overpotential of Zn and slows down the dendrite growth of Zn by improving the nucleation behavior of Zn^(2+).Simultaneously,the three-dimensional porous structure increases the specific surface area of the electrode,so the battery has a larger specific capacity.Compared with the bare Zn electrode,the composite electrode possesses lower overpotential and longer cycle life.In addition,the full battery using activated carbon as the active material exhibits great rate and cycle performance.This facile and scalable approach may solve the problem of Zn dendrite growth,which is crucial for the large-scale application of ZIBs.

An in situ groundwater remediation process, termed EGCW, was developed in this study by integrating in-well groundwater electrolysis into groundwater circulation well. Groundwater circulation carries electrolytically generated O2 and H2 into the impacted aquifer for in situ biodegradation of contaminants. In a two-dimensional tank filled with field sandy sediments,simulated trichloroethylene(TCE)-contaminated groundwater was circulated between an injection well with electrodes inside and a pumping well. Results from a 50-day EGCW experiment show that in-well electrolysis oxygenated most region near the injection well, and 10 mg/L TCE was mainly biodegraded aerobically to about 2.7 mg/L(73% removal) by the indigenous microbes. Aerobic TCE degradation was enhanced by the pulsed addition of acetate. Together with the proofs of stable carbon isotope fractionation(enrichment factor:-0.57‰–-1.53‰) and microbial community variation after EGCW treatment, aerobic cometabolism was proposed to be the most likely mechanism for TCE degradation. It is interesting to find that the intrinsic organic carbon in aquifer matrix could fuel the aerobic TCE degradation, particularly at low TCE concentrations. EGCW treatment is advantageous in terms of supplying appropriate dosages of electron acceptor(O_(2)) and donor(H_(2)) for in situ bioremediation because groundwater electrolysis and circulation are expedient and controllable.

A 3-year-old boy presented with bluish patch and scattered blue spots on the left side of his face.After several sessions of laser treatment,the azury patch in the periorbital area became even darker.Histopathology showed many bipolar,pigment-laden dendritic cells scattered in the papillary and upper reticular dermis.Immunohistochemically,these cells were positive for S100,SOX-10,melan-A,P16,and HMB-45.The positive rate of Ki-67 was less than 5%.Finally,the lesion was diagnosed with nevus of Ota concurrent with common blue nevus.Therefore,for cases of the nevus of Ota with poor response to laser treatment,the possible coexisting diseases should be suspected.

Adsorptive separation of propylene(C_(3)H_(6))from propylene/propane(C_(3)H_(8))is desired to replace energy-intensive cryogenic distillation for its energy efficiency,economical viability,and environment friendship.In this work,we report a cupper-based robust microporous metal-organic framework,Cu-Hmpba,constructed by the hard soft acid-base principle using the bifunctional pyridylcarboxylate ligand featuring methyl group,for high-efficient separation of the propylene/propane mixture.Under the synergistic effect of the protective group and hard soft acid base principle,Cu-Hmpba possesses good solvents stability,especially water stability.Cu-Hmpba exhibits the adsorption capacity of C_(3)H_(6)with 2.10 mmol g^(-1)and good equimolar C_(3)H_(6)/C_(3)H_(8)selectivity with 2.24 at ambient conditions.The gas adsorption experiments,IAST selectivity calculations and theoretical calculations comprehensively support the selective adsorption toward C_(3)H_(6)among C_(3)H_(6)/C_(3)H_(8)mixture.Furthermore,it could maintain stable under moisture environment,suggesting its potential for the industrial separation of C_(3)H_(6)/C_(3)H_(8)mixture.The results of experiments and simulations demonstrate that the Cu-Hmpba would be a candidate adsorbent for the separation and purification of light hydrocarbons,and this work provides the insight of syn-thesizing stable MOF materials for separating high-value chemicals.

Saturation evaluation of microporous low resistivity carbonate oil pays in Rub Al Khali Basin in the Middle East

To solve the problem that it is difficult to identify carbonate low resistivity pays(LRPs) by conventional logging methods in the Rub Al Khali Basin, the Middle East, the variation of fluid distribution and rock conductivity during displacement were analyzed by displacement resistivity experiments simulating the process of reservoir formation and production, together with the data from thin sections, mercury injection and nuclear magnetic resonance experiments. In combination with geological understandings, the genetic mechanisms of LRPs were revealed, then the saturation interpretation model was selected, the variation laws and distribution range of the model parameters were defined, and finally an updated comprehensive saturation interpretation technique for the LRPs has been proposed. In the study area, the LRPs have resistivity values of less than 1 Ω·m, similar to or even slightly lower than that of the water layers. Geological research reveals that the LRPs were developed in low-energy depositional environment and their reservoir spaces are controlled by micro-scale pore throats, with an average radius of less than 0.7 μm, so they are typical microporous LRPs. Different from LRPs of sandstone and mudstone, they have less tortuous conductive paths than conventional reservoirs, and thus lower resistivity value under the same saturation. Archie’s formula is applicable to the saturation interpretation of LRPs with a cementation index value of 1.77-1.93 and a saturation index value of 1.82-2.03 that are 0.2-0.4 lower than conventional reservoirs respectively. By using interpretation parameters determined by classification statistics of petrophysical groups(PGs), oil saturations of the LRPs were calculated at bout 30%-50%,15% higher than the results by conventional methods, and basically consistent with the data of Dean Stark, RST, oil testing and production. Lead is a ubiquitous pollutant and Pb pollution is a global public health problem.Lead has been reported to induce multiple adverse effects,including reproductive toxicity,neurotoxicity,carcinogenicity,nephrotoxicity,immunotoxicity,and hypertension[1].The traditional medical treatment available for Pb poisoning is chelation,which can save lives in individuals with very high blood Pb levels.The commonly used chelating agents include CaNa2EDTA and meso-2,3-dimercaptosuccinic acid.However,chelation therapy has strong short-term effects on the overall long-term management of Pb exposure.

As one of the most violent activities in the solar atmosphere,white-light flares(WLFs)are generally known for their enhanced white-light(or continuum)emission,which primarily originates in the solar lower atmosphere.However,we know little about how white-light emission is produced.In this study,we aim to investigate the response of the continua at 3600Åand 4250Åand also the Hαand Lyαlines during WLFs modeled using radiative hydrodynamic simulations.We take non-thermal electron beams as the energy source for the WLFs in two different initial atmospheres and vary their parameters.Our results show that the model with non-thermal electron beam heating clearly shows enhancements in the continua at 3600Åand 4250Åas well as in the Hαand Lyαlines.A larger electron beam flux,a smaller spectral index,or an initial penumbral atmosphere leads to a stronger emission increase at 3600Å,4250Åand in the Hαline.The Lyαline,however,is more obviously enhanced in a quiet-Sun initial atmosphere with a larger electron beam spectral index.It is also notable that the continua at 3600Åand 4250Åand the Hαline exhibit a dimming at the start of heating and reach their peak emissions after the peak time of the heating function,while the Lyαline does not show such behaviors.These results can serve as a reference for the analysis of future WLF observations.

Early versus delayed intervention in necrotizing acute pancreatitis complicated by persistent organ failure

Background:Current guidelines for the treatment of patients with necrotizing acute pancreatitis(NAP)recommend that invasive intervention for pancreatic necrosis should be deferred to 4 or more weeks from disease onset to allow necrotic collections becoming“walled-off”.However,for patients showing signs of clinical deterioration,especially those with persistent organ failure(POF),it is controversial whether this delayed approach should always be adopted.In this study,we aimed to assess the impact of differently timed intervention on clinical outcomes in a group of NAP patients complicated by POF.Methods:All NAP patients admitted to our hospital from January 2013 to December 2017 were screened for potential inclusion.They were divided into two groups based on the timing of initial interven-tion(within 4 weeks and beyond 4 weeks).All the data were extracted from a prospectively collected database.Results:Overall,131 patients were included for analysis.Among them,100(76.3%)patients were in-tervened within 4 weeks and 31(23.7%)underwent delayed interventions.As for organ failure prior to intervention,the incidences of respiratory failure,renal failure and cardiovascular failure were not signifi-cantly different between the two groups(P>0.05).The mortality was not significantly different between the two groups(35.0%vs.32.3%,P=0.83).The incidences of new-onset multiple organ failure(8.0%vs.6.5%,P=1.00),gastrointestinal fistula(29.0%vs.12.9%,P=0.10)and bleeding(35.0%vs.35.5%,P=1.00),and length of ICU(30.0 vs.22.0 days,P=0.61)and hospital stay(42.5 vs.40.0 days,P=0.96)were com-parable between the two groups.Conclusion:Intervention within 4 weeks did not worsen the clinical outcomes in NAP patients compli-cated by POF.
