BACKGROUND Monomorphic epitheliotropic intestinal T-cell lymphoma(MEITL)is a rare primary intestinal T-cell lymphoma,previously known as enteropathy-associated T-cell lymphoma type *** is an aggressive T-cell lymphoma with a poor prognosis and high mortality *** known major complications of MEITL are intestinal perforation and ***,we present a case of MEITL that was diagnosed following upper gastrointestinal bleeding from an ulcerative duodenal lesion,with recurrence-free survival for 5 *** SUMMARY A 68-year-old female was admitted to our hospital with melena and mild *** urgent esophagogastroduodenoscopy(EGD)revealed bleeding from an ulcerative lesion in the transverse part of the duodenum,for which hemostatic treatment was *** was diagnosed following repeated biopsies of the lesion,and cyclophosphamide,doxorubicin,vincristine,and prednisone(CHOP)chemotherapy was *** achieved complete remission after eight full cycles of CHOP *** the last follow-up examination,EGD revealed a scarred ulcer and 18Fluorodeoxyglucose(18FDG)positron emission tomography/computed tomography showed no abnormal FDG *** patient has been in complete remission for 68 mo after initial *** To rule out MEITL,it is important to carefully perform histological examination when bleeding from a duodenal ulcer is observed.

Natural Convection Melting in a Rectangular Heat Storage Tank of Carbon Nanotube Dispersed Latent Heat Storage Material

The pinning of quantized magnetic vortices in superconducting YBa_(2)Cu_(3)O_(7-δ)(YBCO or Y123)thin films with Y_(2)BaCuO_(5)(Y211)nanoinclusions have been investigated over wide temperature range(4.2-77 K).The concentration of Y211 nanoinclusions has been systematically varied inside YBCO thin films prepared by laser ablation technique using surface modified target *** pinning force density values(Fp∼0.5 TNm^(−3)at 4.2 K,9 T)have been observed for the YBCO film with moderate concentration of Y211 nanoinclusions(3.6 area%on ablation target).In addition,uniform enhancement in critical current density(J_(c))was observed in the angular dependent J_(c)measurement of YBCO+Y211 nanocomposite films.Y211 nanoinclusions have been found to be very efficient in pinning the quantized vortices thereby enhancing the in‐field J_(c)values over a wide range of *** the concentration of Y211 secondary phase into Y123 film matrix results into agglomeration of Y211 phase and observed as increased Y211 nanoparticle *** larger secondary phase nanoparticles are not as efficient pinning centers at lower temperatures as they are at higher temperatures due to substantial reduction of the coherence length at lower *** of the temperature dependence of J_(c)for YBCO+Y211 nanocomposite films has been conducted and possible vortex pinning mechanism in these nanocomposite films has been discussed.

进入21世纪以来,日本、韩国和我国台湾地区等东亚经济体相继进入乡村发展的新阶段。笔者从城乡联系、农业发展、人居环境、乡村治理四个视角,系统梳理东亚乡村发展和规划的新近实践和研究进展,以期为当下我国内地的乡村发展和规划提供有益借鉴。在城乡联系方面,东亚经济体已普遍进入城镇化发展的后期阶段,注重促进城乡要素的双向流动和乡村社会福利水平提升;在农业发展方面,提倡农业的生态化和智慧化发展,在保持小农基础上不断促进经营主体的集中,持续推进三次产业深度融合,并重视引入和培育新型专业化农民;在人居环境方面,强调设施配置的城乡公平性、闲置空间资源的再利用、应对灾害的韧性和文化景观的保护传承;在乡村治理方面,充分认识到上下共治的社会治理重要性,并不断完善乡村空间治理体系。最后,笔者探讨了东亚经济体的相关经验对我国内地乡村发展和规划研究的若干启迪。

Obstructive and secretory complications of diverting ileostomy

This review aimed to highlight the etiology,diagnosis,treatment,and prevention of obstructive and secretory complications associated with diverting ileostomy(DI).Obstructive complications at the stoma site are termed stoma outlet obstruction(SOO)or stoma-related obstruction(SRO).The incidence of SOO/SRO is 5.4%-27.3%,and the risk factors are multifactorial;however,the configuration of the stoma limb and the thickness of the rectus abdominis muscle(RAM)may be of particular ***-stomal tube decompression is initially attempted with a success rate of 33%-86%.A thick RAM may carry the risk of *** refinement,including a wider incision of the anterior sheath and adequate stoma limb length,avoids tension and immobility and may decrease SOO/*** complications of DI are termed high output stoma(HOS).Persistent HOS lead to water and sodium depletion,and secondary hyperaldosteronism,resulting in electrolyte imbalances,such as *** incidence of HOS is 14%-24%,with an output of 1000-2000 mL/d lasting up to three *** of HOS is commenced after excluding postoperative complications or enteritis and includes fluid intake restriction,antimotility and antisecretory drug therapies,and magnesium *** monitoring and surveillance programs have been successful in decreasing readmissions for dehydration.

OBJECTIVE:To examine whether specific stimulation of Shenshu(BL23)affects sympathetic nervous activity(SNA)-associated plasma renin concentration(PRC).METHODS:Eight healthy volunteers participated in three pattern conditions in random order:control(Cont),stimulation of Shenshu(BL23),and stimulation of sham point(Sham).All participants were initially in the supine position for>60 min,and then remained in the standing position during the experimental procedure to increase *** electrocardiogram was used to calculate low frequency/high frequency(LF/HF)ratio;blood was collected to analyze ***:The LF/HF ratio was significantly increased in the standing position when compared with the supine position(P<0.01).There was no difference in LF/HF ratio during or after stimulation of Shenshu(BL23)in the standing position when compared with before the stimulation in the supine position;however,the LF/HF ratio was significantly increased in Cont and Sham conditions(P<0.01).There was no difference in PRC after stimulation of Shenshu(BL23)in the standing position when compared with before the stimulation in the supine position;however,there was a significant increase in PRC in the Cont and Sham conditions(Cont P<0.05,Sham P<0.01).CONCLUSION:Our results demonstrated that specific acupuncture stimulation of Shenshu(BL23)in the standing position decreased SNA-associated PRC,which was not observed during acupuncture stimulation of the sham point.

Tracing sediment transport history using mineralogical fingerprinting in a river basin with dams utilizing sediment sluicing

Sediment causes a serious problem in relation to dam function. A cooperative sediment sluicing operation has been under way since 2017 to prevent sediment from accumulating in dams in the Mimi River,Miyazaki, Japan. To achieve a smooth and stable operation, it is very important to determine the sediment source and a sediment transport system to maintain the dam’s function. In the current study, the source and transport of sediment from the Mimi River basin have been analyzed with X-ray diffraction(XRD) to reveal the peaks of mineral species. The sediment samples were collected in the Mimi River basin from the Tsukabaru Dam to the sea in an area including 4 dams and 5 tributaries. In addition, the minerals in samples collected in 2014, before the start of the sediment sluicing operation, and from 2018 to 2020, after the start of the operation, were analyzed. An evaluation of the similarity of the sediment at each sampling point based on the X-ray diffraction peaks of mineral species showed that sediment distributed upstream was transported downstream in the year when the sediment sluicing was *** result indicates that the sediment sluicing operation at dams ensured the continuity of the sediment distribution in the Mimi River basin. In addition, an investigation done in 2020 showed that sediment particles very similar to those of the upstream tributaries were deposited downstream because of extensive flooding caused by a large typhoon. The management of sediment transport has the greatest importance in a river basin where a dam is to be constructed. It is possible to trace the history of the sediment distribution and movement resulting from the operation of sediment sluicing by using sediment mineral analysis.
