AIM:To investigate the role of 18 F-fluorodeoxyglucose positron emission tomography(FDG-PET) in the diagnosis of small pancreatic cancer. METHODS:This study involved 31 patients with proven invasive ductal cancer of the *** patients were divided into 3 groups according to the maximum diameter of the tumor:TS1(maximum tumor size≤2.0 cm) ,TS2(>2.0 cm and≤4.0 cm) or TS3-4(>4.0 cm) .The relationships between the TS and various diagnostic tools,including FDG-PET with dual time point evaluation,were analyzed. RESULTS:The tumors ranged from 1.3 to 11.0 cm in *** of the 31 patients(97%) had a positive FDG-PET *** were 5 patients classified as TS1,15 as TS2 and 11 as *** sensitivity of FDG-PET,computed tomography(CT) and magnetic resonanceimaging(MRI) were 100%,40%,0%in TS1,93%,93%,89%in TS2 and 100%,100%,100%in TS3-4. The sensitivity of FDG-PET was significantly higher in comparison to CT and MRI in patients with TS1(P< 0.032) .The mean standardized uptake values(SUVs) did not show a significant difference in relation to the TS(TS1:5.8±4.5,TS2:5.7±2.2,TS3-4:8.2±3.9) ,*** the TS1 tumors(from 13 to 20 mm) showed higher SUVs in FDG-PET with dual time point evaluation in the delayed phase compared with the early phase,which suggested the lesions were malignant. CONCLUSION:These results indicate that FDG-PET with dual time point evaluation is a useful modality for the detection of small pancreatic cancers with a diameter of less than 20 mm.

Clinicopathological Analysis of Patients with Nonmuscle-Invasive Bladder Cancer Who Underwent Radical Cystectomy

Objectives: Surgical specimens obtained at the time of the last transurethral resection of bladder tumor (TURBT) for patients with nonmuscle-invasive bladder cancer (NMIBC) who underwent radical cystectomy were retrospectively evaluated in order to investigate the relationship between pathological variation and upstaging of NMIBC. Methods and Materials: Twenty patients (19 men, 1 woman;aged 69.4 ± 12.1 (mean ± SD) years) diagnosed with NMIBC underwent radical cystectomy during follow-up. Results: Five of the 20 patients (25%) had pathological upstaging in the radical cystectomy specimens. There was a statistical association between pathological upstaging and cancer death (p = 0.002). There were three patterns of pathological variation in the upstaged specimens: 1) in patients with BCG-resistant NMIBC, urothelial carcinoma invaded through the lamina propria;2) urothelial carcinoma showed diffuse invasion beyond the deep lamina propria, and the cancer cells had infiltrated as single cells and formed nodules;3) TURBT specimens showed a micropapillary variant. Conclusions: Since these pathological variations correlated with pathological upstaging, they may provide an indication for cystectomy in NMIBC patients.

Characteristics of transonic flow over an airfoil are determined by a shock wave standing on the suction surface. In this case, the shock wave/boundary layer interaction becomes complex because an adverse pressure gradient is imposed by the shock wave on the boundary layer. Several types of passive control techniques have been applied to shock wave/boundary layer interaction in the transonic flow. Furthermore, possibilities for the control of flow fields due to non-equilibrium condensation have been shown so far and in this flow field, non-equilibrium condensation occurs across the passage of the nozzle and it causes the total pressure loss in the flow field. However, local occurrence of non-equilibrium condensation in the flow field may change the characteristics of total pressure loss compared with that by non-equilibrium condensation across the passage of flow field and there are few for researches of locally occurred non-equilibrium condensation in a transonic flow field. The purpose of this study is to clarify the effect of locally occurred non-equilibrium condensation on the shock strength and total pressure loss on a transonic internal flow field with circular bump. As a result, it was found that shock strength in case with local occurrence of non-equilibrium condensation is reduced compared with that of no condensation. Further, the amount of increase in the total pressure loss in case with local occurrence of non-equilibrium condensation was also reduced compared with that by non-equilibrium condensation across the passage of flow field.

Purpose: To clarify which patients need careful neurourological management after abdominal radical hysterectomy (RH) by investigating the effects of adjuvant radiotherapy and reversibility of neurogenic bladder (NB) on the storage function as well as the effects of urethral resistance on the emptying function. Methods: Data from sixty-two patients referred to our NB clinic after RH were retrospectively reviewed. Findings of urodynamic studies performed at 3 (UDS-1) and 12 (UDS-2) months after treatment were compared, and logistic analysis was used to calculate the odds ratio (OR) of the effects of radiotherapy and irreversible NB on decreased bladder capacity and decreased compliance. Irreversible NB was defined as the need for clean intermittent catheterization at the last follow-up. Results: At the median follow-up period of 41 months, 60% of the patients continued to require clean intermittent catheterization. Of patients with irreversible NB and radiotherapy, 80% had decreased bladder capacity and decreased compliance at UDS-2. For decreased bladder capacity and decreased compliance, ORs of adjuvant radiotherapy at UDS-2 were 38.42 (p Conclusions: Careful neurourological follow-up after RH is mandatory for patients who undergo adjuvant radiotherapy and have irreversible NB with impaired urethral relaxation.

Is Dose Adjustment Based on Several Factors of Body Size Descriptors Effective for Prevention of Thrombocytopenia by Linezolid Therapy in Hemodialysis Patients?

Previous study suggested that low body weight was one of the risk factors of thrombocytopenia induced by linezolid in non-hemodialysis patients. However, there have been little investigations for the linezolid-induced thrombocytopenia in hemodialysis patients. This study was to evaluate the association between several factors of body size descriptors and thrombocytopenia in hemodialysis-patients. No factor of body size descriptors showed significant correlation with linezolid-induced thrombocytopenia (patients with thrombocytopenia vs patients without thrombocytopenia: body weight;60.0 kg vs 55.3 kg, p = 0.82: body mass indices;21.1 kg/m2 vs 21.2 mg/m2, p = 0.44: ideal body weight;61.2 kg vs 59.5 kg, p = 0.21: lean body weight;50.1 kg vs 45.7 kg, p = 0.64: dosage amount;20.0 mg/kg vs 21.7 mg/kg, p = 0.74: body surface area;1.65 m2 vs 1.54 m2, p = 0.43). There were not significant differences in the body size descriptors between linezolid therapy for more than 14 days and for less than 13 days (more than 14 days vs less than 13 days: body weight;53.5 kg vs 56.8 kg, p = 0.75: body mass indices;20.9 kg/m2 vs 21.1 mg/m2, p = 0.47: ideal body weight;60.3 kg vs 59.9 kg, p = 0.17: lean body weight;44.3 kg vs 47.7 kg, p = 0.56: dosage amount;22.4 mg/kg vs 21.1 mg/kg, p = 0.67: body surface area;1.51 m2 vs 1.59 m2, p = 0.37). Our data suggested that dosage adjustment of linezolid based on body weight was not recommended in hemodialysis-patients.

Gastrointestinal duplications are uncommon congenital malformations that can occur anywhere along the gastrointestinal tract. Most cases are recognized before the age of 2 years, and those encountered in adults are rare. We describe here a case of ascending colon duplication in a 20-year-old male that caused intussusception and was treated laparoscopically. Although computed tomography revealed a cystic mass filled with stool-like material, the preoperative diagnosis was a submucosal tumor of the ascending colon. We performed a laparoscopic right colectomy, and the postoperative pathological diagnosis was duplication of the ascending colon, both cystic and tubular components. We conclude that gastrointestinal duplications, although rare, should be considered in the differential diagnosis of all abdominal and submucosal cystic lesions and that laparoscopy is a preferred approach for the surgical treatment of gastrointestinal duplications.
