Transjugular intrahepatic portosystemic shunt vs conservative treatment for recurrent ascites:A propensity score matched comparison

BACKGROUND Transjugular intrahepatic portosystemic shunt(TIPS) placement is an effective intervention for recurrent tense ascites. Some studies show an increased risk of acute on chronic liver failure(ACLF) associated with TIPS placement. It is not clear whether ACLF in this context is a consequence of TIPS or of the pre-existing liver *** To better understand the risks of TIPS in this challenging setting and to compare them with those of conservative *** Two hundred and fourteen patients undergoing their first TIPS placement for recurrent tense ascites at our tertiary-care center between 2007 and 2017 were identified(TIPS group). Three hundred and ninety-eight patients of the same time interval with liver cirrhosis and recurrent tense ascites not undergoing TIPS placement(No TIPS group) were analyzed as a control group. TIPS indication,diagnosis of recurrent ascites, further diagnoses and clinical findings were obtained from a database search and patient records. The in-hospital mortality and ACLF incidence of both groups were compared using 1:1 propensity score matching and multivariate logistic *** After propensity score matching, the TIPS and No TIPS groups were comparable in terms of laboratory values and ACLF incidence at hospital admission. There was no detectable difference in mortality(TIPS: 11/214, No TIPS 13/214). During the hospital stay, ACLF occurred more frequently in the TIPS group than in the No TIPS group(TIPS: 70/214, No TIPS: 57/214, P = 0.04). This effect was confined to patients with severely impaired liver function at hospital admission as indicated by a significant interaction term of Child score and TIPS placement in multivariate logistic regression. The TIPS group had a lower ACLF incidence at Child scores < 8 points and a higher ACLF incidence at ≥ 11 points. AIMTo test the effects of humic acids on innate microbial communities of the *** followed the effects of oral supplementation with humic acids (Activomin®) on concentrations and composition of colonic microbiome in 14 healthy volunteers for 45 d. 3 × 800 mg Activomin® were taken orally for 10 d followed by 3 × 400 mg for 35 d. Colonic microbiota were investigated using multicolor fluorescence in situ hybridization (FISH) of Carnoy fixated and paraffin embedded stool cylinders. Two stool samples were collected a week prior to therapy and one stool sample on days 10, 31 and 45. The effects of temperature and strain rate on the flow stress behavior of twin-roll cast, rolled and heat-treated AZ31 magnesium alloys were investigated under uniaxial tension. At high temperatures, dynamic recovery, continuous dynamic recrystallization, grain boundary sliding and the activation of additional slip systems lead to an improvement of the ductility of the alloys. The elongation to failure is nearly independent of the strain rate between 473 and 523 K at 10-2 s-1 and 10-1 s-1, which is related to the strain rate dependence of the critical resolved shear stress（CRSS） for nonbasal slip. Despite the high temperature, twins are even observed at 573 K and 10-3 s-1 because they have a low CRSS.

Mast cells were observed in autopsies from 11 females and 8 males. We confirm earlier observations that mast cells are more frequent in close vicinity to MS-plaques. In these plaque-border zone areas, defined as the area within 1 mm distance of the actual plaques, the average number of mast cells was 2.34/mm2 in males and 4.77/mm2 in females, which in average is appr. 10 times more than earlier observed in MS. The difference in number of mast cells between females and males is significant (p < 0.005) and is of interest since females are more inclined to developing MS than males. Mast cells were rare in areas without visible plaques. The mast cells were preferably located close to capillaries and venules. A mechanism for the probable role of mast cells, based on diet-factors and mast cell mediators in the pathogenesis of MS is discussed.

The hyperfine (hf) structure constants of three atomic niobium energy levels in the energy range around 23000 cm-1 (at 22936.90, 23010.58, and 23048.58 cm-1) are known with only limited accuracy, and the constants of combining levels are sometimes even unknown. Thus we performed laser spectroscopic investigations in the wavelength range between 5600 and 6500 Å, and we excited altogether 16 transitions in which these lower levels are involved. Beside a more precise determination of the hf structure constants of the three lower levels (which were determined on several lines sharing a common upper level), these experiments led to the knowledge of the hf constants of nine levels with previously unknown constants. Beside these results, also the hf constants of 13 further energy levels are reported. For six of these levels, the constants were previously unknown.

We study the time-dependent heat equation on its space-time domain that is discretised by a k-spacetree.k-spacetrees are a generalisation of the octree concept and are a discretisation paradigm yielding a multiscale representation of dynamically adaptive Cartesian grids with low memory *** paper presents a full approximation storage geometric multigrid implementation for this setting that combines the smoothing properties of multigrid for the equation’s elliptic operator with a multiscale solution propagation in *** the runtime and memory overhead for tackling the all-in-one space-time problem is bounded,the holistic approach promises to exhibit a better parallel scalability than classical time stepping,adaptive dynamic refinement in space and time fall naturally into place,as well as the treatment of periodic boundary conditions of steady cycle systems,on-time computational steering is eased as the algorithm delivers guesses for the solution’s long-term behaviour immediately,and,finally,backward problems arising from the adjoint equation benefit from the the solution being available for any point in space and time.

Background:The study aimed to investigate the 24-month safety and efficacy of implantation of two secondgeneration iStent inject trabecular micro-bypass stents with concomitant cataract ***:This consecutive case series included 164 eyes of 109 patients implanted with the iStent inject®device with concomitant cataract *** series was comprised of eyes with primary open-angle glaucoma(n=84),pseudoexfoliation glaucoma(n=42),normal-tension glaucoma(n=18),and ocular hypertension(n=20).All 164 eyes reached 9–14 months of follow-up(“12-month consistent cohort”),with a subset of 88 eyes reaching 21–26 months of follow-up(“24-month consistent cohort”).Performance outcome measures included intraocular pressure(IOP)and number of glaucoma *** outcomes included intra-or postoperative complications,the need for secondary procedures and corrected distance visual *** of change in continuous(e.g.,IOP)and categorical(e.g.,proportions of eyes on zero medications)measures between baseline and postoperative times were made with the paired t-test and McNemar’s chi-squared test,***:At 12 months postoperatively,IOP was reduced by 25.5%(from 20.0±5.5 mmHg to 14.9±2.0 mmHg;p<0.001);at 24 months postoperatively,IOP was reduced by 26.6%(from 20.3±6.1 mmHg to 14.9±1.9 mmHg;p<0.001).At 12 months postoperatively,mean number of glaucoma medications was reduced by 85.0%(from 2.0±1.0 to 0.3±0.8 medications;p<0.001);at 24 months postoperatively,mean number of medications was reduced by 81.0%(from 2.1±1.1 to 0.4±0.8 medications;p<0.001).After 12 months,96.3%of eyes had an IOP≤18 mmHg and 58.5%of eyes had an IOP≤15 mmHg,with 81.1%of eyes free of any medication,compared to 1.8%of eyes medication-free in the 12-month cohort at *** 24 months,98.9%of eyes had an IOP≤18 mmHg and 53.4%of eyes had an IOP≤15 mmHg,with 72.7%free of medication compared to 1.1%of eyes medication-free in the 24-month cohort at ***,a high safe
