Gastrointestinal stromal tumors in a cohort of Chinese patients in Hong Kong

AIM： To investigate the prevalence and clinical pattern of gastrointestinal stromal tumors （GISTs） in Hong Kong Chinese, and to assess the impact of introduction of CD117 on the disease incidence. METHODS： From the database of the Department of Pathology of Yan Chai Hospital, 47 patients, with GISTs from September 1995 to December 2003 were included in this study. Ten GISTs were diagnosed before the introduction of CD117. The clinical features, tumor characteristics, and treatment were analyzed. Factors predicting tumor related death or recurrence were studied with Cox proportional hazard model. RESULTS： The patients included 26 males and 21 females, with a mean age of 66.6 years （SD 13.1, range 29-87 years）. The estimated prevalence of GISTs was 13.4-15.6 per 100 000 people, with an annual inddence of 1.68-1.96 per 100 000 people. The annual incidence of GISTs before and after the introduction of CD117 was 1.1 per 100 000 people and 2.1 per 100 000 people respectively. Stomach （34 patients, 72.3%） was the most common location for the tumor, followed by the small intestine （8 patients, 17.0%）, esophagus （2 patients, 4.3%）, omentum （2 patients, 4.3%） and colon （1 patient, 2.1%）. Thirty-one patients （66%） had complete tumor resection. Eleven out of 16 deaths （23%） were tumor-related. The median survival time was 26 too. Five-year survival rate was 61.3%. The significant factors associated with tumor-related death or recurrence were incomplete resection, tumor size 5 cm or above, invasion to the adjacent organ or presence of metastasis, CONCLUSION： The incidence of GIST in Hong Kong is comparable to that in the United States but lower than that in Finland. The true incidence of GISTs could be underestimated before the introduction of CD117. Incomplete resection, tumor size 5 cm or above, invasion to the adjacent organ or presence of metastasis are factors predicting tumor-related death or recurrence.

A seed-directed approach to synthesizing Fe ZSM-22 zeolite without organic structure directing agent(OSDA)was developed by using Fe-rich diatomite as all aluminum and iron *** Fe ZSM-22zeolite with optimal crystallinity and purity can be obtained by systematically adjusting feed composition and synthesis *** show that Fe ZSM-22 zeolite synthesized with OSDA-free owns high crystallinity,obvious thin needle-shaped morphology and high Bronsted/Lewis acid ***,when used for n-octane hydroisomerization reaction,its derived catalyst exhibits the best catalytic performance reflected by the highest selectivity to C_(8)isomers compared to the two reference catalysts prepared based on a Fe-containing and a Fe-free ZSM-22 synthesized through an OSDA-directed route from natural diatomite and conventional chemicals,*** work provides an alternative route to sustainably synthesizing heteroatomic zeolites with high performance.

Outcomes of right-lobe and left-lobe living-donor liver transplantations using small-for-size grafts

AIM To analyze the outcomes of living-donor liver transplantation(LDLT) using left-lobe(LL) or right-lobe(RL) small-for-size(SFS) *** Prospectively collected data of adult patients who underwent LDLT at our hospital in the period from January 2003 to December 2013 were reviewed. The patients were divided into the RL-LDLT group and the LL-LDLT group. The two groups were compared in terms of short-and long-term outcomes, including incidence of postoperative complication, graft function, graft survival, and patient survival. A SFS graft was defined as a graft with a ratio of graft weight(GW) to recipient standard liver volume(RSLV)(GW/RSLV) of < 50%. The Urata formula was used to estimate *** Totally 218 patients were included for analysis, with 199 patients in the RL-LDLT group and 19 patients in the LL-LDLT group. The two groups were similar in terms of age(median, 53 years in the RL-LDLT group and 52 years in the LL-LDLT group, P = 0.997) but had significantly different ratios of men to women(165:34 in the RL-LDLT group and 8:11 in the LL-LDLT group, P < 0.0001). The two groups were also significantly different in GW(P < 0.0001), GW/RSLV(P < 0.0001), and graft cold ischemic time(P = 0.007). When it comes to postoperative complication, the groups were comparable(P = 0.105). Five patients died in hospital,4(2%) in the RL-LDLT group and 1(5.3%) in the LLLDLT group(P = 0.918). There were 38 graft losses, 33(16.6%) in the RL-LDLT group and 5(26.3%) in the LL-LDLT group(P = 0.452). The 5-year graft survival rate was significantly better in the RL-LDLT group(95.2% vs 89.5%, P = 0.049). The two groups had similar 5-year patient survival rates(RL-LDLT: 86.8%, LL-LDLT: 89.5%, P = 0.476).CONCLUSION The use of SFS graft in LDLT requires careful tailormade surgical planning and meticulous operation. Psychometrics of the chronic liver disease questionnaire for Southern Chinese patients with chronic hepatitis B virus infection

AIM： To test the psychometric properties of a Chinese [（Hong Kong） HK] translation of the chronic liver disease questionnaire （CLDQ）. METHODS： A Chinese （HK） translation of the CLDQ was developed by iterative translation and cognitive debriefing. It was then administered to 72 uncomplicated and 78 complicated chronic hepatitis B （CriB） patients in Hong Kong together with a structured questionnaire on service utilization, and the Chinese （HK） SF-36 Health Survey Version 2 （SF-36v2）. RESULTS： Scaling success was ≥ 80% for all but three items. A new factor assessing sleep was found and items of two （Fatigue and Systemic Symptoms） subscales tended to load on the same factor. Internal consistency and test-retest reliabilities ranged from 0.58-0.90 for different subscales. Construct validity was confirmed by the expected correlations between the SF-36v2 Health Survey and CLDQ scores. Mean scores of CLDQ were significantly lower in complicated compared with uncomplicated CHB, supporting sensitivity in detecting differences between groups.CONCLUSION： The Chinese （HK） CLDQ is valid, reliable and sensitive for patients with CHB. Some modifications to the scaling structure might further improve its psychometric properties,

Dietary analysis of the House Swift(Apus nipalensis)in Hong Kong using prey DNA in faecal samples

Ultra-small Pd clusters supported by chitin nanowires as highly efficient catalysts

For the first time, chitin microspheres woven from nanowires with multi-scale porous structures were used as an excellent support for a catalyst of ultra-small Pd clusters. The Pd species anchored on the precursor Pre-Pd@chitin were 0.6 nm in average size, while the reduced catalyst Red-Pd@chitin featured ultra-small particles of 1.3 nm in average size. X-ray absorption spectroscopy （XAS） and transmission electron microscopy （TEM） demonstrated that the Pd catalyst in both oxidative and reductive states retained good dispersity and ultra-small clusters. The catalyst was tested for the hydrogenation of p-nitroanisole, exhibiting an excellent initial rate （13× that of commercial Pd/C） and excellent turnover frequency reaching 52,000 h^-1. Furthermore, the catalyst could be recycled and used more than 10 times with no decay of the catalytic activity, suggesting potential industrial applications.

A multi-objective pigeon inspired optimization algorithm for fuzzy production scheduling problem considering mould maintenance

The fuzzy production scheduling problem considering mould maintenance(FPSP-MM) is studied. The processing time and the maintenance time are represented by triangular fuzzy numbers. When tasks are executed based on the sequence provided by the fuzzy schedule, the real duration of each task needs to be known so the posteriori solution with deterministic processing times can be obtained. Therefore, the concept of the schedule robustness needs to be considered for the fuzzy problem. The robustness is considered as the optimization objective except for the fuzzy makespan in this research. To optimize these two objective functions, a multi-objective pigeon inspired optimization(MOPIO) algorithm is developed. To extend the pigeon inspired optimization(PIO) algorithm from the single-objective case to the multi-objective case, nondominated solutions are used as candidates for the leader pigeon designation and a special crowding distance is used to ensure a good distribution of solutions in both the objective space and the corresponding decision space. Furthermore, an index-based ring topology is used to manage the convergence speed. Numerical experiments on a variety of simulated scenarios show the excellent efficiency and effectiveness of the proposed MOPIO algorithm by comparing it with other algorithms.

A network condition classification scheme for supporting video delivery over wireless Internet*

Real-time video transport over wireless Internet faces many challenges due to the heterogeneous environment including wireline and wireless networks. A robust network condition classification algorithm using multiple end-to-end metrics and Support Vector Machine (SVM) is proposed to classify different network events and model the transition pattern of network conditions. End-to-end Quality-of-Service (QoS) mechanisms like congestion control, error control, and power control can benefit from the network condition information and react to different network situations appropriately. The proposed network condition classifica- tion algorithm uses SVM as a classifier to cluster different end-to-end metrics such as end-to-end delay, delay jitter, throughput and packet loss-rate for the UDP traffic with TCP-friendly Rate Control (TFRC), which is used for video transport. The algorithm is also flexible for classifying different numbers of states representing different levels of network events such as wireline congestion and wireless channel loss. Simulation results using network simulator 2 (ns2) showed the effectiveness of the proposed scheme.

The liver is one of the most common sites for cancer metastasis,which leads to secondary hepatic *** all solid tumors,such as melanoma,breast cancer,gastric cancer,colorectal cancer,and lung cancer,metastasize to the liver.1,2 Overall,20–25%of colorectal cancer patients may eventually develop liver metastases(also known as CRC liver metastases).3 In addition to some common occurrence of liver metastasis in colorectal cancer,the liver is also one of the predominant metastatic sites for melanoma,including cutaneous and ocular melanoma.2 Thus,an understanding of the mechanism of liver metastasis is urgently needed for the development of novel strategies to prolong overall survival for patients with liver metastases.
