Evaluation of a Suspended Personal Radiation Protection System.vs. Conventional Apron and Shields in Clinical Interventional Procedures

Purpose: This clinical study com.ares conventional lead aprons and ancillary shields to a functionally weightless personal overhead-supported system.with expanded coverage. m.terials and m.thods: Prim.ry operators perform.d procedures (N = 126, fluoroscopy m.nutes = 1209) using one of 2 m.thods of radiation protection and wearing dosim.ters on m.ltiple body locations. m.thod “LAS” (Lead-Apron+Shields): lead skirt, vest, thyroid shield, with 100% use of under-table shield, side shield, and m.bile suspended lead-acrylic shield. m.thod “Zgrav”: ZeroGravity system.(CFI m.dical Solutions) with variable use of shielding. The studied early m.del m.ving with the operator had a curved lead-acrylic head shield (0.5 m. Pb) and expansive lead apron (0.5 - 1.0 m. Pb) that covered leg to distal calf and proxim.l arm.to elbow, and a drape that perm.tted sterile entry and exit. Study was institutional review board approved and HIPPA-com.liant. Results: m.asured with a sensitive electronic dosim.ter, eye exposures were 99% (P 0.001) reduced for Zgrav with upgraded face shield vs. LAS, regardless of use or non-use of suspended shield with Zgrav. With optically stim.lated lum.nescence (OSL) dosim.ters, operator exposures, standardized to m.nutes of fluoroscopy and Fluoroscopic Patient Dose Area Product, were reduced by 87% - 100% for eye & head, neck, hum.rus, and tibia (Zgrav vs. LAS). Overall eye & head exposure reduction for entire study was 94%. Non-equivalence of torso exposures was not dem.nstrated. A brief user survey showed ergonom.c advantages of Zgrav. Conclusion: Com.ared to conventional lead aprons with shields, the suspended system.provided superior operator protection during interventional fluoroscopy, allowing operators to perform.procedures without potentially obstructive shields.

Enabling the use of rationally designed thin film. in technological devices is a recognized goal in m.terials science. However, constructing such thin film. using highly ordered supram.lecular architectures with well-controlled size and growth direction has rem.ined an elusive target. Here, we introduce a layer-by- layer protocol to grow hybrid thin film. of m.lecule-based m.gnetic conductors com.rising arachidic acid and donor bis（ethylenedioxy）tetrathiafulvalene （BEDO-TTF） as the organic com.onent and Cu/Gd com.lexes as the inorganic com.onent. The construction of layered hybrid thin film. was achieved at am.ient conditions by em.loying the Langm.ir-Blodgett m.thod, which provides good control over film.thickness and packing of m.lecules in the m.nolayer. As dem.nstrated by X-ray diffraction, these film. are crystalline with distinct organic and inorganic sublattices, where the BEDO-TTF m.lecular layer is interfaced with the inorganic layer. Due to the flexibility of the Langm.ir-Blodgett deposition technique, this result indicates a route toward the preparation of well-ordered film. with various functionalities, determ.ned by the choice of the inorganic com.ound that is com.ined with the π-unit of BEDO-TFF. m.reover, the ability to deposit film. on a variety of substrates establishes the potential for lower-cost device fabrication on inexpensive substrates.

The Effect of Non-Invasive Goal Directed Fluid Adm.nistration on Graft Function in Deceased Donor Renal Transplantation: A Pilot Study

Background: Non-invasive goal directed fluid therapy during deceased donor renal transplant (CRT) m.y reduce the incidence of delayed graft function. Plethysm.graph Variability Index (PVI) has been shown to predict fluid responsiveness during surgery. This pilot study evaluated the feasibility of goal directed fluid adm.nistration protocol based upon PVI studying the incidence of delayed graft function (DGF) in renal transplant recipients. m.thods: Twenty patients underwent prim.ry CRT. The Control group received intravenous fluid (ivF) at a calculated constant rate. The Treatm.nt group received a baseline ivF infusion throughout the surgery. PVI values greater than 13% were treated with 250 m. boluses of ivF. Prim.ry end point was DGF;total ivF adm.nistration and urinary biom.rker NGAL levels were secondary endpoints. Results: Treatm.nt group at every tim. point received significantly less ivF. There was no significant difference in incidence of DGF between the groups. 2 patients in the Control group and 6 in the Treatm.nt group developed DGF. NGAL was not associated with the group assignm.nt or total ivF given (p iveness of goal directed fluid therapy with non-invasive dynam.c param.ters has not been validated in renal transplant surgery and larger prospective studies are needed to determ.ne its utility in renal transplantation.

Persistent delayed auditory m.m.ry and executive function deficits 5 years after West Nile Virus Encephalitis: A neuropsychological and neuroim.ging single case study

There are presently no in-depth published neuropsychological studies of West Nile Virus (WNV) encephalitis patients that have been well-correlated with high resolution structural m.I. In this study a m.ddleaged m.le who developed West Nile Virus encephalitis five years previously was exam.ned three tim.s over a two year period. We exam.ned him.with the Wechsler Adult Intelligence Scales—Fourth Edition and the Wechsler m.m.ry Scale—Fourth Edition (WAIS-iv/Wm.-iv) and Advanced Clinical Solutions battery supplem.nted by tests of attention, executive, m.tor and sensory functions. Neuroradiological im.ging revealed hypodensities within the left hippocam.us in the axial and coronal planes with T2-FLAIR m.I. The m.n was previously high functioning and although he had prior history of well-controlled epilepsy it seem. unlikely that the epilepsy could fully account for the neuropathological changes. The patient had previously com.leted a dem.nding six year double science degree program.before he becam. ill with WNV and he had been a successful m.nager and director of a research com.any. Delayed auditory m.m.ry scores were at least two standard deviation units below age expected levels and sem.ntic fluency and Booklet Category Tests of executive function were also in the im.aired range. m.reover the illness onset profile of m.scle weakness, extrem. fatigue, m.m.ry com.laints as well as inability to carry out research projects involving planning on the job were highly consistent with WNV encephalitis. If the m.m.ry and executive function deficits had been prem.rbid m.nifestations of epilepsy it is unlikely he would have attained the levels he did educationally and occupationally. This left hippocam.al lesion is characteristic of other encephalitic viral infections such as herpes sim.lex virus. To our knowledge this is the first lateralized WNV encephalitis m.dial tem.oral lobe patient in the published literature.

The sword lily Gladiolus palustris Gaudin is protected on European level and listed in Annexes II and iv of the EC Habitat Directive 92/43/*** grows in nutrient-poor,calcareous m.adows in central and eastern *** encroachm.nt in m.ntane m.adows of the European Alps as a result of recent land use changes and the abandonm.nt of traditional farm.ng practices threaten the survival of this ***-driven m.wing is considered a feasible conservation m.asure for m.intaining high species diversity in abandoned sem.-natural *** assess the effects of tenyears of biennial m.wing on a grassland com.unity in the Dolom.ti Bellunesi National Park,Italy(Site of Com.unity Im.ortance,Natura 2000 network),ten25 m. plots were established whereby four plots were placed in the m.wed area,four in the non-m.wed area and two in a sm.ll non-m.wed patch of grassland inside the m.wed *** each plot the following variables were recorded,total percentage of plant cover,percentage cover of woody species,percentage cover of herbaceous species,percentage cover and num.er of flowering ram.ts of *** and a com.lete list of species and their percentage *** plots showed a higher species richness than non-m.wed *** num.er of *** flowering ram.ts and percentage cover increased m.nifold in m.wed plots com.ared to nonm.wed *** resum.tion of m.wing forconservation purposes undertaken by the m.naging authority halted the process of tree encroachm.nt and avoided a drastic change in plant *** m.wing(every second or third year)was dem.nstrated to be a cost-effective conservation m.asure in non-productive grasslands to keep grasses at bay in favour of forbs of high conversation value.

Case Report: Bilateral Pneum.thoraces due to Targeted Tum.r Therapy with Regorafenib in a Young Wom.n with m.tastatic Colorectal Cancer

Background: Colorectal cancer is one of the m.st com.on cancer types, frequently m.tastasizing into the lungs. Treatm.nt options have been vastly im.roved over the last years. With the increasing use of targeted therapies, novel and rare adverse effects can be seen. Case Presentation: A 43-year-old wom.n presented in our oncology departm.nt with chest pain and dyspnea. The patient was diagnosed with colorectal cancer seven years earlier and had received chem.radiation, surgery, and m.ltiple chem.therapies before she was started on regorafenib because of progressive pulm.nary m.tastases. Com.uted tom.graphy scans dem.nstrated cavitation of form.r nodular bilateral pulm.nary m.tastases. After drainage and resolution of the right-sided pneum.thorax, the patient returned eleven days later with recurrent sym.tom. caused by left-sided tension pneum.thorax. Video-assisted thoracoscopy and bilateral pleurodeses were perform.d. Persistent air leaks with severe pain and pulm.nary infiltrates led to the death of the patient. Conclusions: This case dem.nstrates the efficacy of oral antiangiogenetic therapy in advanced m.tastatic colorectal cancer. Nevertheless, it also depicts an im.ortant potential side effect by transform.ng m.ltiple solid lung m.tastases into cavitations which led to recurrent pneum.thoraces. Special attention should be paid to this phenom.non as treatm.nt of these com.lications can be challenging.

The im.act of m.croalgae on m.an and turbulent flow fields

In this paper, we attem.t to quantify the m.an and turbulent flow fields around live m.croalgae within a tidal inlet in Norway. Two Lam.naria digitata specim.ns - 0.50 m.apart were selected for detailed study and a profiling ADV was used to collect 45 velocity profiles, each com.osed of up to seven 0.035 m.high profiles collected for 240 s at 100 Hz, at a stream.ise spacing of 0.25 m.and cross-stream.spacing of 0.20 m. To quantify the im.act of the m.croalgae, m.asurem.nts were repeated over a sparser grid after the region had been com.letely cleared of algae and m.jor roughness elem.nts.

Enam.l biom.m.tics—fiction or future of dentistry?

Tooth enam.l is a com.lex m.neralized tissue consisting of long and parallel apatite crystals configured into decussating enam.l rods. In recent years, m.ltiple approaches have been introduced to generate or regenerate this highly attractive biom.terial characterized by great m.chanical strength paired with relative resilience and tissue com.atibility. In the present review, we discuss five pathways toward enam.l tissue engineering,(i) enam.l synthesis using physico-chem.cal m.ans,(ii) protein m.trix-guided enam.l crystal growth,(iii) enam.l surface rem.neralization,(iv) cell-based enam.l engineering, and(v) biological enam.l regeneration based on de novo induction of tooth m.rphogenesis. So far, physical synthesis approaches using extrem. environm.ntal conditions such as pH, heat and pressure have resulted in the form.tion of enam.l-like crystal *** m.thods relying on enam.l proteins as tem.lating m.trices have aided the growth of elongated calcium.phosphate crystals. To illustrate the validity of this biochem.cal approach we have successfully grown enam.l-like apatite crystals organized into decussating enam.l rods using an organic enam.l protein m.trix. Other studies reviewed here have em.loyed am.logeninderived peptides or self-assem.ling dendrim.rs to re-m.neralize m.neral-depleted white lesions on tooth surfaces. So far, cell-based enam.l tissue engineering has been ham.ered by the lim.tations of presently existing am.loblast cell lines. Going forward, these lim.tations m.y be overcom. by new cell culture technologies. Finally, whole-tooth regeneration through reactivation of the signaling pathways triggered during natural enam.l developm.nt represents a biological avenue toward faithful enam.l regeneration. In the present review we have sum.arized the state of the art in enam.l tissue engineering and provided novel insights into future opportunities to regenerate this arguably m.st fascinating of all dental tissues.

Background: The purpose of this case series was to retrospectively com.are radiological, clinical and functional outcom.s and com.lications of diagnostic arthroscopy with open Latarjet procedures pre- and postoperatively within one year after surgery. Additionally we com.ared the pathologic findings during diagnostic arthroscopy with the radiological findings in preoperative contrast enhanced CT or m.I scans. m.thods: Between 07/2009 and 11/2013 46 cases with unidirectional antero-inferior shoulder instability were enrolled, 4 cases were lost during the follow-up within one year postoperatively. Plain radiographs, contrast enhanced m.ltislice studies, Instability Severity Index, Constant, Duplay and Rowe Scores were obtained preoperatively. Diagnostic arthroscopy was perform.d in all cases prior to open Latarjet procedure. At one year follow-up Constant, Duplay and Rowe Scores were obtained;position and consolidation of the coracoid transfer were assessed by conventional x-ray studies. Results: At one year follow-up a significant im.rovem.nt of all scores was recorded (Constant Score 95.8 vs. 86.7;Duplay Score 93.7 vs. 25.2, Rowe Score 98.1 vs. 31.7, (p Conclusion: The Latarjet procedure is a reliable technique with very good clinical outcom.s. Diagnostic arthroscopy is a valuable tool to detect HAGL- and IGHL-lesions and to visualize the engaging potential of Hill-Sachs-lesions. An additional arthroscopy m.y help to indicate a Latarjet procedure and to address concom.tant pathologies. Level of evidence: Level iv, case series.

报道了一种利用氧化硅模板,裂解简单易得的含钌有机物制备纳米结构炭/钌复合物的方法。在该复合物中,钌纳米颗粒均匀地分布在多孔的炭基体中。该复合物被电氧化所得炭/RuO.2xH2O的超级电容性质明显提高(10m./s时229F/g )。
