Three-dimensional analysis of the physiological foramen geometry of maxillary and mandibular molars by means of micro-CT

The aim of this study was to investigate the physiological foramen diameter, shape and distance between physiological and anatomical apex of maxillary and mandibular first and second molars. Accurate knowledge of the physiological foramina morphology; thus, inherent mechanical shaping technical hindrances, is decisive when taking the corresponding root canal final preparation decision. The morphological dimensions of a total of 1 727 physiological foramina were investigated by means of micro-computed tomography. Mean narrow and wide （to a high number, oval） diameters of the physiological foramen were 0.24, 0.22 and 0.33 mm and 0.33, 0.31 and 0.42 mm in mesiobuccal （MB）, distobuccal （DB） and palatal （P） roots in maxillary first molars; 0.24, 0.22 and 0.33 mm and 0.41, 0.33 and 0.44 in MB, DB, and P roots in maxillary second molars. Mandibular first molars showed mean narrow and wide diameters of 0.24 and 0.30 mm and of 0.39 and 0.46 mm in mesial （M） and distal （D） roots; second mandibular molars showed 0.25 and 0.31 mm and 0.47 mm in M and D roots. The mean distance between the physiological foramina and anatomical apex was 0.82, 0.81 and 1.02 mm and 0.54, 0.43 and 0.63 mm in MB, DB and P roots of the maxillary first and second molars, respectively. A mean distance of 0.95 mm （M） and 1.05 mm （D） in the first and 0.78 mm （M） and 0.81 mm （D） in the second mandibular molars was observed. Based on the results obtained, assumable recommendations for final preparation size of the physiological foramen were calculated. However, when taking into consideration, the resulting standard deviations of marginal errors must be cautiously considered when taking a final decision in clinical endodontic treatment.

Root canal morphology and configuration of 123 maxillary second molars by means of micro-CT

The aim of this study was to investigate the root canal configuration, accessory canals and number of main foramina of 123 maxillary second molars by means of micro-computed tomography. The teeth were scanned and reproduced with 3D software imaging. The root canal configuration and number of main foramina were evaluated by means of a four-digit system. The morphological complexity of human maxillary second molars is depicted by the number of accessory and connecting canals. The most frequently observed root canal configurations in the mesiobuccal root were 2-2-2/2 （19.5%）, 2-2-1/1 （14.6%） and 2-1-1/1 （13.0%）. A 1-1-1/1 configuration was observed in 93.5% and in 96.7% in the distobuccal and palatal roots, respectively. The MB1 mot canal had one accessory canal （18.7%）, and 8.9% of the MB2 root canal had one or two accessory canals. The distobuccal （11.3%） and palatal （14.6%） root canals had at least one accessory canal, and connecting canals were observed in 16.3% of mesiobuccal roots. The MB1, MB2, distobuccal and palatal root canals had one main foramen in 99.2%, 43.1%, 98.4% and 99.2% of samples, respectively. In the mesiobuccal root, one accessory foramen was detected in 14.6%, two were detected in 7.3%, and three were detected in 5.7%. The distobuccal root showed one or two accessory foramina in 9.1% of samples. The root canal configuration of maxillary second molars is quite heterogeneous; the mesiobuccal root has predominantly two root canal entrances （58.4%, 1 in 41.1%） with one main foramen （54.4%）. Two main foramina were observed in 43.0%. Morphological variations, connecting and accessory canals were observed in all apical thirds.

Recording neural signals from delicate autonomic nerves is a challenging task that requires the development of a lowinvasive neural interface with highly selective,micrometer-sized *** paper reports on the development of a three-dimensional(3D)protruding thin-film microelectrode array(MEA),which is intended to be used for recording low-amplitude neural signals from pelvic nervous structures by penetrating the nerves transversely to reduce the distance to the *** gold pillars(O 20 or 50μm,~60μm height)were fabricated on a micromachined polyimide substrate in an electroplating *** sidewalls were insulated with parylene C,and their tips were optionally modified by wet etching and/or the application of a titanium nitride(TiN)*** microelectrodes modified by these combined techniques exhibited low impedances(~7 kΩ at 1 kHz for ∅ 50μm microelectrode with the exposed surface area of~5000μm^(2))and low intrinsic noise *** functionalities were evaluated in an ex vivo pilot study with mouse retinae,in which spontaneous neuronal spikes were recorded with amplitudes of up to 66μ*** novel process strategy for fabricating flexible,3D neural interfaces with low-impedance microelectrodes has the potential to selectively record neural signals from not only delicate structures such as retinal cells but also autonomic nerves with improved signal quality to study neural circuits and develop stimulation strategies in bioelectronic medicine,e.g.,for the control of vital digestive functions.

Serum bile acid profiling reflects enterohepatic detoxification state and intestinal barrier function in inlammatory bowel disease

AIM:To determine free and conjugated serum bile acid (BA) levels in in? ammatory bowel disease (IBD) subgroups with defi ned clinical ***: Comprehensive serum BA profiling was performed in 358 IBD patients and 310 healthy controls by liquid chromatography coupled to electrospray ionization tandem mass ***: Serum levels of hyodeoxycholic acid, the CYP3A4-mediated detoxification product of the secondary BA lithocholic acid (LCA), was increased significantly in Crohn's disease (CD) and ulcerative colitis (UC), while most other serum BA species were decreased significantly. Total BA, total BA conjugate, and total BA glycoconjugate levels were decreased only in CD, whereas total unconjugated BA levels were decreased only in UC. In UC patients with hepatobiliary manifestations, the conjugated primary BAs glycocholic acid, taurocholic acid, and glycochenodeoxycholic acid were as significantly increased as the secondary BAs LCA, ursodeoxycholic acid, and tauroursodeoxycholic acid compared to UC patients without hepatobiliary manifestations. Finally, we found that in ileocecal resected CD patients, the unconjugated primary BAs, cholic acid and chenode-oxycholic acid, were increased significantly compared to controls and patients without surgical ***: Serum BA profiling in IBD patients that indicates impaired intestinal barrier function and increased detoxification is suitable for advanced diagnostic characterization and differentiation of IBD subgroups with defined clinical manifestations.

AIM:To study the impact of an endoscopy-based long-term study on the quality of life in healthy volunteers(HV).METHODS:Ten HV were included into a long-term prospective endoscopy-based placebo-controlled trial with 15 endoscopic examinations per person in 5 different drug *** completed short form-36(SF-36) and visual analog scale-based questionnaires(VAS) for different abdominal symptoms at days 0,7 and 14 of each drug *** wereperformed according to short-and long-term changes and compared to the control ***:All HV completed the study with duration of more than 6 *** quality of life score was comparable to a general *** of the SF-36 questionnaires showed no significant changes in physical,mental and total scores,either in a short-term perspective due to different medications,or to potentially endoscopic procedure-associated long-term cumulative *** to SF-36,VAS revealed no significant changes in total scores for pathological abdominal symptoms and remained unchanged over the time course and when compared to the control ***:This study demonstrates that quality of life in HV is not significantly affected by a longterm endoscopy-based study with multiple endoscopic procedures.

The obesity pandemic has led to a significant increase in patients with metabolic dysfunction-associated fatty liver disease(MAFLD).While dyslipidemia,type 2 diabetes mellitus and cardiovascular diseases guide treatment in patients without signs of liver fibrosis,liver related morbidity and mortality becomes relevant for MAFLD’s progressive form,non-alcoholic steatohepatitis(NASH),and upon development of liver *** should be prescribed in patients without significant fibrosis despite concomitant liver diseases but are underutilized in the real-world *** surgery,especially Y-Roux bypass,has been proven to be superior to conservative and/or medical treatment for weight loss and resolution of obesity-associated diseases,but comes at a low but existent risk of surgical complications,reoperations and very rarely,paradoxical progression of *** end-stage liver disease develops,obese patients benefit from liver transplantation(LT),but may be at increased risk of perioperative infectious *** LT,metabolic comorbidities are commonly observed,irrespective of the underlying liver disease,but MAFLD/NASH patients are at even higher risk of disease *** studies with low patient numbers evaluated if,and when,bariatric surgery may be an option to avoid disease recurrence but more high-quality studies are needed to establish clear *** this review,we summarize the most recent literature on treatment options for MAFLD and NASH and highlight important considerations to tailor therapy to individual patient’s needs in light of their risk profile.

目的通过低剂量阿斯匹林的处理作为炎性模型,研究分析人胃和十二指肠黏膜分泌性白细胞蛋白酶抑制因子(SLPI)表达下调是否与幽门螺杆菌(Helicobacter prlori,Hp)定居或感染有关.方法选择20例健康志愿者分成Hp+和Hp-组,其中Hp+组成功进行了根除治疗.每组10人,每人口服阿司匹林100 mg/d.通过活检提取受试者不同部位胃黏膜的总RNA和蛋白,采用RT/realtime PCR检测SLPI的mRNA表达水平和单抗ELISA定量测定SLPI蛋白,统计分析SLPI基因表达的相关数据.结果与Hp-组比较,Hp+组胃窦黏膜SLPI表达水平显著降低,但Hp+组经抗生素根除治疗后其SLPI水平恢复至与Hp-组相当的水平.在低剂量阿司匹林处理下,所有受试者表现组织学观察到的活动性或慢性炎性征.各组在药物处理的不同时间虽有一定的差异,但与药物处理对照组(第0天)比较,SLPI的表达差异没有统计学意义[第1天:(77±880)pg/10?g蛋白;第3天:(941±149)pg/10?g蛋白;第7天:(763±363)pg/10?g蛋白].阿司匹林处理的1周内以胃窦为主的胃炎持续存在(活动性:1.5～1.7;慢性:1.2～2.1),但与Hp-和Hpe(根除组)比较,Hp+组的胃窦黏膜SLPI蛋白表达仍显著降低.结论低剂量阿司匹林处理所致炎性反应对人胃黏膜SLPI的表达没有影响,胃窦黏膜SLPI基因表达的下调与Hp感染有关.

Lithocholic acid induction of the FGF19 promoter in intestinal cells is mediated by PXR

AIM: To study the effect of the toxic secondary bile acid lithocholic acid (LCA) on the expression of fibroblast growth factor 19 (FGF19) in intestinal cells and to characterize the pregnane-X-receptor (PXR) response of the FGF19 promoter region. METHODS: The intestinal cell line LS174T was stimulated with various concentrations of chenodeoxy-cholic acid and lithocholic acid for several time points. FGF19 mRNA levels were determined with quantitative realtime RT-PCR. FGF19 deletion promoter constructs were generated and the LCA response was analzyed in reporter assays. Co-transfections with PXR and RXR were carried out to study FGF19 regulation by these factors. RESULTS: LCA and CDCA strongly up-regulate FGF19 mRNA expression in LS174T cells in a time and dose dependent manner. Using reporter gene assays with several deletion constructs we found that the LCA responsive element in the human FGF19 promoter maps to the proximal regulatory region containing two poten-tial binding sites for PXR. Overexpression of PXR and its dimerization partner retinoid X receptor (RXR) and stimulation with LCA or the potent PXR ligand rifampicin leads to a signifi cant induction of FGF19 promoter activ-ity in intestinal cells. CONCLUSION: LCA induced feedback inhibition of bile acid synthesis in the liver is likely to be regulated by PXR inducing intestinal FGF19 expression.
